Abstract:Background and purpose:To date,surgery still plays a crucial role in the treatment of rectal cancer.The accurate preoperative staging provides helpful information to choose operative procedures for rectal cancer.This study aimed to compare multi-slice computer tomography with transrectal ultrasound in the role of preoperative assessments of patients with middle and lower rectal cancer in order to determine their value in making surgery decisions.Methods:170 patients were prospectively enrolled into the study who were diagnosed as rectal cancer with tumor's inferior border from the dentate line of ≤10 cm.Participants were randomly assigned into MSCT group(n=85) and TRUS group(n=85),which then received preoperative MSCT examination or transrectal ultrasound respectively.We compared the outcomes of preoperative staging and predictive surgery programs with postoperative pathologic staging and fi nal surgery programs respectively.Results:163 patients were available for analysis.There was no statistical difference between MSCT group(83 cases) and TRUS group(80 cases) in the baseline characteristics.The accuracy of preoperative T staging,N staging and surgical procedures prediction in MSCT group were 69.88%,67.47% and 77.11%.The corresponding rates in TRUS group were 86.25%,61.25% and 92.50%.In T staging and surgical procedures prediction,signifi cant statistical differences were observed between the two groups with P=0.012 and P=0.003 respectively.However the analysis showed no signifi cant differences in N staging between two groups(P=0.407).When analyzing the relationship between multiple clinicopathologic factors and the operative procedures,there were statistical differences in the pathological T staging(P=0.002),M staging(P<0.001),TNM staging(P<0.001),and the distance of tumor to the dentate line(P=0.001).Conclusion:TRUS has a higher accuracy of preoperative staging in middle and lower rectal cancer and provides better evidence to make surgery decisions than MSCT.
