Association of Matrix Metalloproteinase-9 Gene Polymorphism with Gastric Cancer in Guangdong Province

Abstract:Background: Matrix metalloproteinase-9 (MMP-9), which can degrade type IV collagen of extracellular matrix and basal membrane, is considered to play critical roles in cancer development and aggression. Aims: To investigate the association of nonsynonymous single nucleotide polymorphism at MMP-9 R279Q (Gln279Arg) with susceptibility to gastric cancer in people of Guangdong Province. Methods: A case-control study was conducted in 550 gastric cancer patients diagnosed by surgical specimen pathology and 550 matched healthy controls from Guangdong Province. Genotypes of R279Q were determined by PCR-RFLP. Logistic regression model, Kaplan-Meier survival curve and Cox regression model were used to evaluate the influences of R279Q polymorphism on gastric cancer risk, various clinicopathological subtypes of gastric cancer and prognosis. Results: Compared with the QQ genotype, the RR and RQ genotypes were associated with significantly elevated risk of gastric cancer (OR=2.86, 95% CI: 1.67-4.90, P<0.001; OR=1.88, 95% CI: 1.08-3.26, P=0.025). Further stratification analyses indicated that the effect of allele R on cancer risk was noteworthy in gastric cancer of intestinal type, non-cardiac and poorly differentiated, but not in gastric cancer of diffuse or mixed type, cardiac and moderate to well differentiated. No significant difference was found in survival rates between gastric cancer patients with different R279Q genotypes (P=0.716), and the R279Q genotype was excluded from the multivariate Cox regression model. Conclusions: Polymorphism of MMP-9 R279Q is correlated with the susceptibility to gastric cancer, especially intestinal type gastric cancer in people of Guangdong Province, but cannot serve as an independent prognostic factor.
