Abstract:Drug induced hepatitis is the injury caused by drugs or their products of metabolism. At the present, at least more than 600 drugs can cause hepatitis. The manifests of the hepatitis are cellular necrosis, cholestasis, chronic hepatitis and hepatocirrhosis, etc. The article analyzes the pathogeny of 28 cases of drug induced hepatitis, so as to deepen the understanding about the drug induced hepatitis.

Abstract:Adverse drug reactions are among the most frequent side effects of synthetic and complementary alternative drugs and represent the premier causes of license revocations and acute liver failure. Drug-induced liver injury can resemble literally any other genuine liver disease and usually responds well to drug dechallenge. However, in some cases autoimmune-like hepatitis can evolve, requiring short- and sometimes long-term immunosuppression. Here, we present the hitherto first case of autoimmune-like hepatitis following treatment with zoledronic acid.

Abstract:Backgrounds Drug induced liver injury (DILI) is sometimes similar to autoimmune hepatitis (AIH) in serology and histology. Clinicians empirically screened DILI with significant autoimmune characteristics to implement clinical intervention. We tried to characterize DILI with autoantibodies by metabolomics. Methods Untargeted metabolomics coupled with pattern recognition approaches were performed on sera samples including AIH (n = 59), DILI with autoantibodies (DILI Ab+ , n = 68), and DILI without autoantibodies (DILI Ab- , n = 75). The differential metabolites and fingerprint metabolites between AIH and DILI Ab- were screened by orthogonal partial least squares-discriminant analysis and hierarchical clustering respectively. Results Of the 388 annotated differential metabolites between AIH and DILI Ab- , 74 fingerprint metabolites were screened. The eigenmetabolite compressed from the fingerprint possessed high discrimination efficacy (AUC:0.891; 95 %CI, 0.838–0.944). In the fingerprint-based PCA model, AIH and DILI Ab- were separated into three regions: the "pure region" of AIH (Region 1), the "pure region" of DILI Ab- (Region 3), the mixture region of AIH and DILI Ab- (Region 2). After incorporated into the PCA model, DILI Ab+ samples were distributed into the three regions, indicating that DILI Ab+ samples had different etiological tendencies. Moreover, the fingerprint-based radar model verified the results of PCA model characterizing DILI Ab+ . Notably, the antibody titers of DILI Ab+ in the three regions did not differ significantly, while the response rates for glucocorticoids were obviously different. The metabolic difference among DILI Ab+ in different regions mainly lies in energy metabolism. Conclusions In terms of metabolic signature, DILI Ab+ may not be a community of same pathogenesis, including AIH-inclined parts. Which deserves further study.

Abstract:Idiosyncratic drug-induced liver injury (IDILI) is an adverse drug reaction that occurs only in a minority of the population.IDILI also has many characteristics such as unpredictable and low morbidity,its occurrence has often not been clearly correlated with the dose,route,or duration of drug administration.Several studies have shown that IDILI is a synergistic effect which was caused by body diathesis,environment and drugs.In addition,evidence also suggests that most IDILIs are mediated by immunity.Chemical medicines-related IDILIs have been extensively studied,and a variety of immunological mechanism hypotheses have also emerged to explain the pathogenesis and characteristics of chemical medicines-related IDILIs.However,the traditional Chinese medicine (TCM)-related IDILI has always been neglected due to the complexity and specificity of TCM.In recent years,TCM-related IDILI has been gradually confirmed by researchers,and formed a new hypothesis,a immunological stress-mediated tri-elements synergetic mechanism hypothesis,which can reveal the pathogenesis and clinical characteristics of TCM-related IDILI.This paper is prepared to summarize the immunological mechanism hypotheses of chemical medicine-related IDILI and TCM-related IDILI to provide a scientific basis for guiding IDILI research and establishing its clinical risk prevention and control measures.
Clinical and histopathologic features of drug-induced autoimmune hepatitis

Abstract:Objective To analyze the clinical and histological features of drug-induced autoimmune hepatitis (DI-AIH).Methods From January 2008 to June 2011,five patients with auto-immune hepatitis (AIH),having a definitive history of medicine taking prior to the onset of disease and accepted liver biopsy were retrospectively analyzed.The general information was collected,which included gender,age,onset of the disease,medication,clinical manifestations,treatment and follow-up.The laboratory findings were also collected,which included total bilirubin (TBil),direct bilirubin (DBil),alanine transaminase (ALT),aspartate transaminase (AST),alkaline phosphatase (ALP),gamma glutamyl transpeptidase (GGT),globulin,γ-globulin percentage,albumin (Alb),eosinophils,antinuclear antibodies,antimitochondrial antibodies,anti-smooth muscle antibodies and the type of live injury.The liver tissue sections of patients were stained with hematoxylin-eosinstaining (HE) and reticular fiber staining and then pathological changes were observed.Patients with DI-AIH were scored with AIH scoring scale.Results All five patients with DI-AIH were female,average age was 48.0--+7.5.Prior to the onset of disease,all patients had taken Chinese traditional medicine.Anorexia and fatigue were the most common clinical symptoms.Among laboratory findings,the level of ALT ((795.0+467.4) U/L),AST ((730.44-451.5) U/L),TBil ((80.3-+ 64.1) μmol/L) and DBil ((65.2 +_ 58.0) μmol/L) significantly increased.One patient was antinuclear antibody positive.One patient had drug-induced liver injury,pathological features were spotty necrosis of liver cells,liver tissue eosinophil infiltration and liver cell microbubble type degeneration.All the patients had interface hepatitis,periportal infiltration of lymphocytes or lymphocytes-plasma cells,liver cells adjacent to lesion showed rosette-like structure.All the patients received glucocorticoid treatment.After glucocorticoid withdrawal,the liver function was normal during the follow-up period.Conclusion There are no specificity of clinical manifestations,laboratory findings and histological features in patients with DI-AIH.

Abstract:Drug-induced liver injury is an important adverse drug reaction. Due to the lack of specificity in clinical symptoms and pathological features, there are still no reliable diagnostic biomarkers, so drug-induced liver injury is a diagnosis of exclusion. The article reviews the relevant advances in the association between novel human leukocyte antigen gene polymorphisms and drug-induced liver injury in order to identify potential biomarkers and provide a new method for the prediction and diagnosis of drug-induced liver injury. Henceforth, while studying the association between them, it will also need that the large sample and prospective studies to gain supporting evidence to implement translational application, so as to improve the safety and effectiveness of medication and achieve individualized treatment.
Features of Autoimmune Hepatitis in Patients with Drug-induced Liver Injury

Abstract:BACKGROUND & AIMS: Drug-induced liver injury (DILI) has features similar to those of other liver diseases including autoimmune hepatitis (AIH). We aimed to characterize the clinical and autoimmune features of liver injury caused by nitrofurantoin, minocycline, methyldopa, or hydralazine. METHODS: We analyzed data from 88 cases of DILI attributed to nitrofurantoin, minocycline, methyldopa, or hydralazine included in the Drug-Induced Liver Injury Network prospective study from 2004 through 2014. Sera were collected from patients at baseline and follow-up examination and tested for levels of immunoglobulin G (IgG), antibodies to nuclear antigen (ANA), smooth muscle (SMA), and soluble liver antigen (SLA). An autoimmune score was derived on the basis of increases in levels of IgG, ANA, SMA, and SLA (assigned values of 0, 1+, or 2+). AIH- associated HLA-DRB1*03:01 and HLA-DRB1*04:01 allele frequencies were compared with those of the general population (controls). RESULTS: Of the 88 cases, 80 were women (91%), 74% had hepatocellular injury, and 25% had severe injury. At the onset of DILI, 39% of cases had increased levels of IgG, 72% had increased levels of ANA, 60% had increased levels of SMA, and none had increases in SLA. A phenotype of autoimmunity (autoimmune score >= 2) was observed in 82% of cases attributed to nitrofurantoin and 73% of cases attributed to minocycline (73%) but only 55% of cases attributed to methyldopa and 43% of cases attributed to hydralazine (P = .16 for nitrofurantoin and minocycline vs methyldopa and hydralazine). We observed a decrease in numbers of serum samples positive for ANA (P = .01) or SMA (P <.001) and in autoimmune scores (P <.001) between DILI onset and follow-up. Similar percentages of patients with DILI had HLA-DRB1*03: 01 (15%) and HLA-DRB1*04: 01 (9%) as controls (12% and 9%, respectively). CONCLUSIONS: In analysis of data from the DILIN prospective study, we found that most cases of DILI attributed to nitrofurantoin or minocycline and about half of cases that were due to methyldopa and hydralazine have a phenotype of autoimmunity similar to AIH. These features decrease with recovery of the injury and are not associated with the typical HLA alleles found in patients with idiopathic AIH.

Abstract:OBJECTIVE:To analyse the etiology, clinical features and prognosis of drug-induced liver disease (DILD), in order to draw more attention to this problem.METHODS:332 cases of DILD admitted to the First Affiliated Hospital of Zhejiang University from 2000-2006 were retrospectively analysed.RESULTS:A variety of drugs, including herbal medicines (27.1% of our total cases), anti-tuberculosis drugs (13.3%) and immunosuppressive drugs (10.8%) caused drug-induced liver damage. The hepatocellular type (43.1%) was the most common type seen clinically and the mixed type (32.5%) and cholestatic type (24.4%) were somewhat less.CONCLUSIONS:A variety of medicines may cause DILD; herbal medicines, anti-tuberculosis drugs and immunosuppressive drugs were frequently found to be responsible, the hepatocellular type was the most common, while the mixed type and cholestatic type were somewhat less. Serum albumin, total bile acids, direct bilirubin and blood prothrombin time are important predictors of the prognosis of DILD.

Abstract:Autoimmune liver disease(AILD) and non-alcoholic fatty liver disease(NAFLD) are two major types of chronic inflammatory liver diseases that lead to progressive decline of liver function.Both of them are lack of characteristic clinical symptoms and biochemical parameters.As the worldwide popularity of Western lifestyle and the global trend of obesity and NAFLD,autoimmune hepatitis(AIH) and primary biliary cirrhosis(PBC) would be co-existence with NAFLD, which makes the diagnosis more difficult.When the patients have definite NAFLD and unexplained liver function deterioration,the possibility of concurrent AILD should be taken into account.This article summarized the clinical and pathologic features ofNAFLD-AILD co-existenceand explored its possible mechanism.

Abstract:BACKGROUND Autoimmune phenomena can be used in some patients with nonalcoholic fatty liver disease (NAFLD) in the clinic, but these patients are not autoimmune hepatitis patients. AIM To determine whether autoimmunity is present in patients with NAFLD, this study was performed. METHODS A total of 104 patients with NAFLD diagnosed by liver biopsy at Tianjin Second People’s Hospital between 2019 and 2023 were enrolled. The patients were divided into three groups according to their biopsy results: The NAFL (n = 36), nonalcoholic steatohepatitis (n = 51), and liver cirrhosis groups (n = 17). RESULTS The differences in IgA, an immune marker, among the three groups of patients were statistically significant (P = 0.025). In all NAFLD patients, antinuclear antibody and anti-smooth muscle antibody were the most common autoantibodies. The antinuclear antibody detection rate was the highest at 48.1%. The cirrhosis group had the highest autoantibody positivity rate (64.7%). Portal enlargement is also common in NAFLD patients. The rates of positivity for portal lymphoplasmacytic infiltration, small bile duct hyperplasia and interfacial hepatitis were highest in the cirrhosis group; the differences between the cirrhosis group and the other two groups were significant (P < 0.05). Hepatocellular rosettes were identified only in the cirrhosis group (11.8%). CONCLUSION Autoimmune phenomena occur in NAFLD patients, especially in patients with NAFLD-related cirrhosis, in whom this phenomenon may be more pronounced.

Abstract:Drug-induced liver injury (DILI) is a rare yet potentially life-threatening disease. Besides intrinsic DILI, which is mainly caused by paracetamol overdosing and which is dose-dependent and predictable, there is idiosyncratic DILI-an unpredictable and dose-independent injury of the liver caused by certain medications that only occurs in a minority of patients taking this drug. The reason why some patients react with DILI towards a specific drug is still unknown. However, the immune system plays a central role, which is underlined by the association of certain human leukocyte antigen (HLA) polymorphisms and DILI caused by specific drug classes. Due to the immunological processes that lead to the liver injury in DILI, there are great overlaps regarding laboratory and histological parameters between DILI and autoimmune hepatitis (AIH). Differentiating DILI and AIH can therefore be challenging, especially at the time of presentation. In addition, there are other immunologically mediated DILI phenotypes, in particular the newly defined drug-induced autoimmune-like hepatitis (DI-ALH) and liver injuries caused by checkpoint inhibitors (CPI). DI-ALH is characterized by autoimmune features and good responses towards corticosteroids, with the difference that DI-ALH mostly does not relapse after discontinuation of corticosteroids. CPI-induced liver injury has become more frequent with the rising use of those drugs and is characterized by a distinct histopathological pattern with granulomatous hepatitis and infiltration dominated by cytotoxic T cells. Similarly, the recently recognized liver injury following vaccinations also shows an autoimmune phenotype; however, in contrast to AIH, cytotoxic T cells seem to dominate the inflammatory infiltrates in the liver.

Abstract:Drug-induced liver injury(DILI)is a rare side effect of drugs caused by all kinds of prescription or over-the-counter chemi-cals,biological agents,traditional Chinese medicine(TCM),natu-ral medicine(NM),health products,dietary supplements and their metabolites,and even excipients,which can lead to jaundice,liver failure,or even death.Although it is rare in term of single drug,the occurrence of DILI in all liver injuries is not low due to the wide range of drugs and foods involved.Moreover,there was an increasing trend of incidence of DILI since 2010 worldwide,with Asian regions showing the highest incidence[1].

Abstract:Objective To explore the etiology and clinical features of acute drug-induced liver disease and promote the level of clinical diagnosis and therapy.Method The clinical data of 51 patients meeted with the diagnostic criteria of acute durg-induced liver diseases were reviewed and analyzed.Results The drugs involved were Chinese herb medicine in 33.3%,antibiotics in 19.6%,antitumor drugs in 11.8%,antituberculosis drugs in 7.8%,antithyroid drugs in 7.8%,cardiovascular drugs in 5.9%,hypoglycemic drugs in 3.9%,psychotropic drugs in 3.9%,non-steroidal antiinflammatory drugs in 2.0%,others in 0.5%.Among all these cases,there were 22 cases hepatocyte damage,24 cases cholestaric,mixed damage 5 cases.Conclusion Many drugs can lead to acute drug-induced liver disease.We should strengthen the mornitoring of liver function in order to diagnose and treat it timely.
Clinical Analysis of Acute Drug-induced Liver Disease in 275 Cases

Abstract:Objective In order to improve the treatment and early diagnosis of acute drug-induced liver disease,we analyzed the causative drugs,clinical manifestation and pathological characteristics of the disease.Methods From January 2000 to December 2005,275 cases confirmed acute drug-induced liver disease according to Maria Criterion and hospitalized in Zhongshan hospital Fudan university were retrospectively reviewed.Each was examined by drug history,clinical symptom and signs,laboratory tests and therapeutic effect.In 41 cases,hepatic biopsy was carried out to help diagnosis.Results The incidence of acute drug-induced liver disease among all of the acute liver injury was annually increased.The most common drugs which induced acute liver injury were traditional Chinese medicine(23.3%,64/275cases),antineoplastics(15.3%,42/275),hormones and other immunosuppressant agents(13.8%,38/275),antihypertensive drugs and other cardiovascular drugs(10.2%,28/275),NSAIDs(8.7%,24/275).Hepatocellular injury was the predominant type in these cases(48%,132cases).The principal clinical manifestation included nausea(54.8%),fatigue(50.2%),jaundice(35.6%) and 27.9% patients had no symptoms.Most patients were cured with good prognosis.The total effect was 94.2% after treatment.Conclusion The clinicians should draw attention to the prevention,diagnosis and therapy of drug-induced liver disease.
The Clinical And Histological Characteristics And Outcomes Of Drug Induced Chronic Liver Injury After Removing Causative Drugs

Abstract:Background/Aims: To investigate characteristics and outcomes of drug induced chronic liver injury after removing causative drugs.Methodology: Between Apr 2001 and Mar 2010, patients diagnosed as drug induced liver injury and with chronic courses, were observed. Chronic liver injury was defined as persistent biochemical abnormality for more than 6 months after drugs withdrawal.Results: Forty patients were observed with mean age 41 years and female 28 (70%). Of the 40 patients, 32 (80%) cases showed hepatocellular injury at the onset of liver damage, 4 (10%) cases showed cholestatic injury, and 4 (10%) cases showed mixed injury. Of 32 patients with hepatocellular injury, 24(75%) cases resemble acute icteric hepatitis at the onset of liver damage. Of 27 patients with hepatocellular injury who underwent liver biopsy, 14 (51.9%) cases showed chronic hepatitis with a mean follow up of 17 months. Of total 40 patients, 15 (37.5%) patients resolved; 18 (45%) cases remained persistent liver injury, 4 (10%) cases developed cirrhosis, 3 (7.5%) cases died. Herbs (45.5%) are the commonest drugs implicated in chronic liver injury.Conclusions: Most cases with drug induced chronic liver injury after removing causative drugs resemble acute icteric hepatitis at the onset of liver damage. Some patients can resolve, some may progress to cirrhosis. Herbs are important drugs for drug induced chronic liver injury.

Abstract:Hepatitis C virus (HCV) infection and interferon (IFN) treatment may induce the occurrence of a series of autoantibodies and even induce autoimmune disease.The autoimmune disease will not relieve completely even IFN treatment has been stopped.Relevant treatment including immunosuppressants may be needed.To clarify the clinical characteristics of autoimmune hepatitis (AIH) induced by IFN,1626 cases of hepatitis C treated with IFN in the Division of Gastroenterology, Hepatology and Nutrition,Kurashiki Medical Center,Japan from 1992 to 2008 were analyzed.Among them,only one female patient developed AIH.The patient recovered after steroid therapy.HCV RNA infection may induce the producing of non-organ specific autoantibodies,and IFN treatment itself also can induce the appearance of autoantibodies.Such an associated autoimmune phenomenon is not an absolute contraindication to IFN therapy.However,IFN should be used cautiously when the titers of autoantibodies are very high.
A Case of Autoimmune Hepatitis Initially Manifesting As Hepatic Encephalopathy

Abstract:Autoimmune hepatitis (AIH) is a T-cell-mediated liver disease characterized by elevated transaminases, circulating autoantibodies, hypergammaglobulinemia, and interface hepatitis. A 66-year-old female patient visited our department due to recurrent episodes of altered consciousness, sleep-wake inversion, and asterixis, indicating hepatic encephalopathy (HE). Her liver biopsy results clearly demonstrated interface hepatitis. The patient's severe HE does not parallel her relatively stable liver function and was attributed to a wide retroperitoneal collateral vein shunting blood directly into the inferior vena cava, bypassing the liver, and allowing excess neurotoxins to enter the central nervous system. Due to the unfavorable benefit-risk ratio of embolization and the patient's stable liver function, non-invasive treatments were adopted, and prednisolone was discontinued. The patient experienced no further episodes of HE thereafter. To the best of our knowledge, this is the first AIH case with a spontaneous portosystemic shunt directly shunting blood into the inferior vena cava. A crucial lesson from this case is that when HE cannot be fully explained by liver dysfunction, abdominal CT scans should be carefully inspected for possible anatomical variations. This case also underscores the importance of a multidisciplinary approach in managing AIH in elderly patients, who may benefit more from a tailored treatment regimen rather than strictly following standard treatment guidelines.

Abstract:Drug-induced liver injury has become one of the main factors of global fatality, but it seems to be difficult to reach an unified diagnostic criteria for it. After more than 20 years’ development, a world of progression has been obtained in the exactitude and manipuility of the clinical judgment of drug-induced liver injury. And now, some diagnostic criteria for druginduced liver injury have appeared and is being widely used, such as ICM, Maria and so on.

Abstract:Objective To investigate the etiology, clinical features, treatment and its efifcacy of drug-induced liver injury in order to improve clinical doctors understanding of this problem. Methods Seventy four cases of hospital patients with drug-induced liver injury were divided into UDCA group and without UDCA group, whose etiology, liver function and treatment outcome were retrospectively analyzed and whose liver function were assessed two weeks after treatment. Results A wide variety of drugs can cause liver damage, the most were traditional Chinese medicine (59.46%), followed by anti-inlfammatory drugs (18.92%), and then were anti-TB drugs (8.11%) and Mongolian drug (4.05). Conventional liver protecting treatment was effectively, and UDCA group showed better efficacy. Conclusions Drug-induced liver injury was closely related to regional differences and drug variety. The efifcacy of UDCA group in the treatment of drug-induced liver injury were signiifcant.

Abstract:We present a patient with a drug-induced liver injury with autoimmune features as a result of infliximab therapy for ulcerative colitis. This is a rare but serious side effect in patients receiving this treatment which clinicians should consider in the event of liver function test derangement.
