Abstract:Drug induced hepatitis is the injury caused by drugs or their products of metabolism. At the present, at least more than 600 drugs can cause hepatitis. The manifests of the hepatitis are cellular necrosis, cholestasis, chronic hepatitis and hepatocirrhosis, etc. The article analyzes the pathogeny of 28 cases of drug induced hepatitis, so as to deepen the understanding about the drug induced hepatitis.

Abstract:Idiosyncratic drug-induced liver injury (IDILI) is an adverse drug reaction that occurs only in a minority of the population.IDILI also has many characteristics such as unpredictable and low morbidity,its occurrence has often not been clearly correlated with the dose,route,or duration of drug administration.Several studies have shown that IDILI is a synergistic effect which was caused by body diathesis,environment and drugs.In addition,evidence also suggests that most IDILIs are mediated by immunity.Chemical medicines-related IDILIs have been extensively studied,and a variety of immunological mechanism hypotheses have also emerged to explain the pathogenesis and characteristics of chemical medicines-related IDILIs.However,the traditional Chinese medicine (TCM)-related IDILI has always been neglected due to the complexity and specificity of TCM.In recent years,TCM-related IDILI has been gradually confirmed by researchers,and formed a new hypothesis,a immunological stress-mediated tri-elements synergetic mechanism hypothesis,which can reveal the pathogenesis and clinical characteristics of TCM-related IDILI.This paper is prepared to summarize the immunological mechanism hypotheses of chemical medicine-related IDILI and TCM-related IDILI to provide a scientific basis for guiding IDILI research and establishing its clinical risk prevention and control measures.

Abstract:In recent years, with an increasing number of drug types and unreasonable drug use, the incidence and mortality of drug-induced liver injury (DILI) have been increasing. The pathogenesis of DILI is complex, and may involve liver injury caused by the direct effect of drugs, immune-mediated liver injury, mitochondrial injury, and bile duct injury, etc. This article investigates the pathogenesis and its important role in the prevention and treatment of DILI, and reviews the research advances in the pathogenesis of DILI.

Abstract:Chemicals (such as alcohol, drugs, and pollutants) may cause liver injuries, which could consequently develop into fatty liver, hepatitis, fibrosis, cirrhosis, and liver failure, or even cancers. Liver injuries have been a serious public health problem worldwide. Numerous natural products and their bioactive components have shown protective action for liver injuries, such as blueberry, cactus fruits, Pueraria lobate, betaine, and silymarin. The underlying mechanisms mainly include antioxidation, anti-inflammation, anti-apoptosis, anti-necrosis, repairing damaged DNA, regulating the metabolism of lipids, and modulating primary bile acid biosynthesis. This review summarizes the natural products and bioactive compounds with protective effects on liver injuries caused by chemicals, and special attention is paid to the mechanisms of action. This updated information can be helpful to prevent and treat liver-related diseases, especially chemical-induced liver injuries.

Abstract:Mitochondria are major organelles of energy generation and material oxidation in cell and play a fundamental role in energy metabolism, free radicals production, aging, and apoptotic regulation.The possible factors leading to mitochondrial dysfunction include respiratory chain defects, metabolic enzyme inactivation, structural changes, mutations, etc. All of which will affect the normal function of the cells, resulting in the occurance of diseases. Mitochondria are important targets of drug toxicity, and the liver is also a major target of drug damage because it is an important organ of drug metabolism. A variety of clinical drugs, such as antivirals, anti-cancer drugs and antibiotics have signifi cantly shown the inducible mitochondrial injury of liver.Drugs induced liver injury primarily by changing the activity of enzyme and the structure of mitochondrial and/or decreasing the synthesis of mt DNA, further undermining β-lipid oxidation and the oxidative of liver cells.This paper provides the overview of research progress on the role of mitochondrial damage in drug-induced liver injury and gives thoughts on the prediction and prevention of drug-induced liver injury.

Abstract:Drug-induced liver injury(DILI)is one of the frequent reasons of abnormal liver function.Many drugs can result in DILI.Clinical manifestation is multifarious,and has no specific symptom and uncertain relation to the dosages of medicines.There are no standard criteria for diagnosis of DILI and there is also a need to eliminate other causes damage to the liver.In addition to treatment of liver-protective and jaundice-relieved drugs,development of artificial liver and liver transplantation have saving part of serious patients recently.Progress in research on pharmacogenetics has broken a new path to prevent and treatment DILI,but advanced study and clinical evidence based medicine to confirm safety and efficacy are needed.

Abstract:Abstract Drug-induced liver injury (DILI) is a common adverse drug reaction (ADR) and a serious threat to health that affects disease treatments. At present, no targeted clinical drugs are available for DILI. Traditional natural medicines have been widely used as health products. Some natural medicines exert specific hepatoprotective effects, with few side effects and significant clinical efficacy. Thus, natural medicines may be a promising direction for DILI treatment. In this review, we summarize the current knowledge, common drugs and mechanisms of DILI, as well as the clinical trials of natural drugs and their bioactive components in anticipation of the future development of potential hepatoprotective drugs.

Abstract:Background: Drug overdose or chemical exposures are the main causes of acute liver injury (ALI). Severe liver injury can develop into liver failure that is an important cause of liver-related mortality in intensive care units in most countries. Pharmacological studies have utilized a variety of comprehensive chemical induction models that recapitulate the natural pathogenesis of acute liver injury. Their mechanism is always based on redox imbalance-induced direct hepatotoxicity and massive hepatocyte cell death, which can trigger immune cell activation and recruitment to the liver. However, the pathogenesis of these models has not been fully stated. Many studies showed that gut microbiota plays a crucial role in chemical-induced liver injury. Hepatotoxicity is likely induced by imbalanced microbiota homeostasis, gut mucosal barrier damage, systemic immune activation, microbial-associated molecular patterns, and bacterial metabolites. Meanwhile, many preclinical studies have shown that supplementation with probiotics can improve chemical-induced liver injury. In this review, we highlight the pathogenesis of gut microorganisms in chemical-induced acute liver injury animal models and explore the protective mechanism of exogenous microbial supplements on acute liver injury.

Abstract:Acute liver failure caused by drug overdose is a significant clinical problem in developed countries. Acetaminophen (APAP), a widely used analgesic and antipyretic drug, but its overdose can cause acute liver failure. In addition to APAP-induced direct hepatotoxicity, the intracellular signaling mechanisms of APAP-induced liver injury (AILI) including metabolic activation, mitochondrial oxidant stress and proinflammatory response further affect progression and severity of AILI. Liver inflammation is a result of multiple interactions of cell death molecules, immune cell-derived cytokines and chemokines, as well as damaged cell-released signals which orchestrate hepatic immune cell infiltration. The immunoregulatory interplay of these inflammatory mediators and switching of immune responses during AILI lead to different fate of liver pathology. Thus, better understanding the complex interplay of immune cell subsets in experimental models and defining their functional involvement in disease progression are essential to identify novel therapeutic targets for the treatment of AILI. Here, this present review aims to systematically elaborate on the underlying immunological mechanisms of AILI, its relevance to immune cells and their effector molecules, and briefly discuss great therapeutic potential based on inflammatory mediators.

Abstract:Toxic hepatic damage also called chemical hepatopathy or drug-induced hepatopathy,is gradually become a prominent threat to human health recent years.On the basis of the general introduction about the classic conception and pathogenesis of toxic hepatopathy,the two hot spots of study involved in the molecular mechanism of toxic hepatic damage were discussed in the paper.The first hot spot is endoplasmic reticulum stress(ERS).It is considered that the exploration on the concrete role of ERS in hepatic damage would be very helpful in revealing the earlier link and particular mechanism of pathogenesis of hepatic damage,thereby openning a new visual angle in the differential diagnosis between toxic hepatopathy and other hepatopathy especially the virous hepatitis,as well as in developing the pertinent intervention on enhancing its cellular protective effect.The second hot spot is cytochrome P450,which is the key-enzyme in oxidation and reduction of chemicals both endogenous and exogeneous.It is also considered that the penetrated research on the status during metabolism of various hepatotoxic chemicals would be quite valuable in clarifing the precise pathogenesis and protective strategy.Because there has been a huge obstacle existed in the differential diagnosis between infectious hepatitis and toxic hepatopathy,it should be an excited expectation that the deep exploration on these field may provide a new important theoretic basis for it.

Abstract:Drug-Induced Liver Injury (DILI) and herb Induced Liver Injury (HILI) continues to pose a substantial challenge in both clinical practice and drug development, representing a grave threat to patient well-being. This comprehensive review introduces a novel perspective on DILI and HILI by thoroughly exploring the intricate microenvironment of the liver. The dynamic interplay among hepatocytes, sinusoidal endothelial cells, Kupffer cells, hepatic stellate cells, cholangiocytes, and the intricate vascular network assumes a central role in drug metabolism and detoxification. Significantly, this microenvironment is emerging as a critical determinant of susceptibility to DILI and HILI. The review delves into the multifaceted interactions within the liver microenvironment, providing valuable insights into the complex mechanisms that underlie DILI and HILI. Furthermore, we discuss potential strategies for mitigating drug-induced liver injury by targeting these influential factors, emphasizing their clinical relevance. Abstract:At present, Drug -induced liver injury(DILI) has become the most common cause of acute liver injury in clinical practice .This article elaborates on the pathogenesis of DILI , new biomarkers, liver tolerance to drug toxicity, adaptability and susceptibility, and testing threshold of alanine aminotransferase .Meanwhile, we should understand the development history of diagnostic scale , develop unified standards and understanding, and attach importance to DILI induced by medicinal herbs and dietary supplements , and in -depth studies on the pathogenesis of DILI should be performed to prevent or reduce DILI .There are several problems in DILI studies in China and obvious gaps between China and foreign countries, with a lack of basic research and blanks in clinical aspects .Therefore, we must strive to keep up with international research so that drugs can better serve the human .

Abstract:Drug-induced liver injury(DILI)is a rare side effect of drugs caused by all kinds of prescription or over-the-counter chemi-cals,biological agents,traditional Chinese medicine(TCM),natu-ral medicine(NM),health products,dietary supplements and their metabolites,and even excipients,which can lead to jaundice,liver failure,or even death.Although it is rare in term of single drug,the occurrence of DILI in all liver injuries is not low due to the wide range of drugs and foods involved.Moreover,there was an increasing trend of incidence of DILI since 2010 worldwide,with Asian regions showing the highest incidence[1].

Abstract:Acetaminophen(AAP)-induced hepatic injury is one of the common causes of drug-induced hepatic injury.Up to date,the mechanisms of AAP-induced hepatic injury are still incompletely understood.Recent advances suggest that reactive metabolite formation,glutathione depletion,alkylation of proteins,especially mitochondrial proteins and peroxynitrite formation are critical initiating events for the toxicity.This review will focus on more recent advances in mitochondrial dysfunction after AAP overdose.Additional,oxidative stress and inflammatory mediators are also important for the overall outcome.

Abstract:Drug-induced liver injury (DILI) has become a major topic in the field of Hepatology and Gastroenterology. DILI can be clinically divided into three phenotypes: hepatocytic, cholestatic and mixed. Although the clinical manifestations of DILI are variable and the pathogenesis complicated, recent insights using improved preclinical models, have allowed a better understanding of the mechanisms that trigger liver damage. In this review, we will discuss the pathophysiological mechanisms underlying DILI. The toxicity of the drug eventually induces hepatocellular damage through multiple molecular pathways, including direct hepatic toxicity and innate and adaptive immune responses. Drugs or their metabolites, such as the common analgesic, acetaminophen, can cause direct hepatic toxicity through accumulation of reactive oxygen species and mitochondrial dysfunction. The innate and adaptive immune responses play also a very important role in the occurrence of idiosyncratic DILI. Furthermore, we examine common forms of hepatocyte death and their association with the activation of specific signaling pathways.

Abstract:Drug-induced liver injury (DILI) is a major concern in clinical studies as well as in postmarketing surveillance. It is necessary to establish an animal model of DILI for thorough investigation of mechanisms of DILI and searching for protective medications. This article reviews the current status and future perspective on establishment of DILI models based on different hepatotoxic drugs, as well as the underlying mechanisms of liver function damage induced by specific medicine. Therefore, information from this article can help researchers make a suitable selection of animal models for further study.
