Abstract:The paper introduces the design of a novel insulin syringe based on MSP430 MCU. Controlled by an MSP430F4152 microprocessor, the syringe is designed to drive a stepper motor to complete each insulin injection with an exact preset dosage for diabetic patients. The product features small size, low power consumption, accuracy and easy manipulation, which is a best substitute for the traditional mechanical injector. After describing the hardware configuration, the paper then gives the idea of software design as well as the implementation of such major functions as motor rotation and LCD display.

Abstract:Based on the comparison between the features and properties of several kinds of existing needle-free injectors, a flexible needle-free injector which was adjustable in both dose and injecting depth was proposed to overcome the disadvantages of non-adjustable needle-free injectors. The original design was optimized by the application of adjustable pre-pressure spring structure for the purpose of regulating the potential energy stored by the spring in according with the actual application situation. A serial of ampoules were also designed to match different doses. Simulation experiments and analysis conducted on Fluent verify that this new needle-free injector is adjustable in a dose range of 0. 1~1 mL and a depth range of 6~20 mm.

Abstract:Insulin injection relies on strict blood glucose monitoring. However, existing techniques and algorithms for blood glucose monitoring cannot be completed in a timely way. In this study, we have developed a new intelligent glucose-sensitive insulin delivery system to stabilize blood glucose levels in the body. This system does not require real-time detection of blood glucose. First, we successfully synthesized a nanoscale material called PAM-PAspPBA-b-PEG by using chemical methods. We then conducted TEM, DLS, and H-1-NMR analyses to characterize the physicochemical properties, such as size, molecular composition, and configuration of the nanomaterial. We verified the glucose responsibility of the insulin loading nanoscale material in vitro and evaluated its safety and effect on mice in vivo. Results showed that insulin-loaded PAM-PAspPBA-b-PEG is glucose-sensitive, safer and more effective than regular insulin injection. This study provides a basis for future development of smart insulin delivery systems.

Abstract:While a typical way for diabetes therapy is discrete insulin infusion based on long-time interval measurement, in this paper, we design a closed-loop control system for continuous drug infusion to improve the traditional discrete methods and make diabetes therapy automatic in practice. By exploring the accumulative function of drug to insulin, a continuous injection model is proposed. Based on this model, proportional-integral-derivative (PID) and fuzzy logic controllers are designed to tackle a control problem of the resulting highly nonlinear plant. Even with serious disturbance of glucose, such as nutrition absorption at meal time, the proposed scheme can perform well in simulation experiments.

Abstract:To improve diabetic patients' compliance to multiple injection protocols, we developed and tested a new insulin jet injector, the Preci-Jet 50. The prototype has the following features: small size (14 × 2 cm) and weight (160 g), capability of mixing two types of insulin, accuracy and reliability of the ejected volume (dose), ease of use and sterilization, simplicity of design, and capacity of adjusting jet pressure to individual skin resistance. The ejected volume, evaluated by gravimetry, was more accurate and more reliable with the injectors (N = 18) than with 0.5-cc disposable plastic syringes (N = 18). The dead space of the injectors (N = 16), as evaluated by isotopic recuperation of radioactive insulin, was minimal, allowing mixed insulin injections. The human-device interface evaluation demonstrated that diabetic patients (N = 13) learned easily to manipulate the injector and that their ability to use it properly improved after 1 mo of use. We conclude that this injector may be a practical tool for insulindependent diabetic patients.

Abstract:Insulin is a commonly prescribed drug for the treatment of type I and type II diabetes. As for the efficiency in controlling blood glucose level, insulin therapy is one of the most effective treatments for diabetes. The current administration route of insulin is mainly through subcutaneous injection, which leads to many undesirable side effects such as pain, local tissue necrosis or infection, and nerve damage. Recently, various closed-loop and smart insulin delivery systems have been developed based on the emerging nanotechnologies. Recent progress in the construction of closed-loop and smart insulin delivery system, which mainly focuses on the response mechanism, different strategies for fabricating the carrier matrix, and the regulation principle of the smart insulin release is described. Advantages and drawbacks of the current insulin delivery systems are also discussed, along with the opportunities and challenges in future.

Abstract:Frequent insulin injections remain the primary method for controlling the blood glucose level of individuals with diabetes mellitus but are associated with low compliance. Accordingly, oral administration has been identified as a highly desirable alternative due to its non-invasive nature. However, the harsh gastrointestinal environment and physical intestinal barriers pose significant challenges to achieving optimal pharmacological bioavailability of insulin. As a result, researchers have developed a range of materials to improve the efficiency of oral insulin delivery over the past few decades. In this review, we summarize the latest advances in material design that aim to enhance insulin protection, permeability, and glucose-responsive release. We also explore the opportunities and challenges of using these materials for oral insulin delivery.

Abstract:Diabetes mellitus, a disorder of glucose regulation, is a global burden affecting 366 million people across the world. An artificial "closed-loop" system able to mimic pancreas activity and release insulin in response to glucose level changes has the potential to improve patient compliance and health. Herein we develop a glucose-mediated release strategy for the self-regulated delivery of insulin using an injectable and acid-degradable polymeric network. Formed by electrostatic interaction between oppositely charged dextran nanoparticles loaded with insulin and glucose-specific enzymes, the nanocomposite-based porous architecture can be dissociated and subsequently release insulin in a hyperglycemic state through the catalytic conversion of glucose into gluconic acid. In vitro insulin release can be modulated in a pulsatile profile in response to glucose concentrations. In vivo studies validated that these formulations provided improved glucose control in type 1 diabetic mice subcutaneously administered with a degradable nano-network. A single injection of the developed nano-network facilitated stabilization of the blood glucose levels in the normoglycemic state (<200 mg/dL) for up to 10 days.

Abstract:Diabetes is a chronic metabolic disease characterized by lack of insulin in the body leading to failure of blood glucose regulation. Diabetes patients usually need frequent insulin injections to maintain normal blood glucose levels, which is a painful administration manner. Long-term drug injection brings great physical and psychological burden to diabetic patients. In order to improve the adaptability of patients to use insulin and reduce the pain caused by injection, the development of oral insulin formulations is currently a hot and difficult topic in the field of medicine and pharmacy. Thus, oral insulin delivery is a promising and convenient administration method to relieve the patients. However, insulin as a peptide drug is prone to be degraded by digestive enzymes. In addition, insulin has strong hydrophilicity and large molecular weight and extremely low oral bioavailability. To solve these problems in clinical practice, the oral insulin delivery nanosystems were designed and constructed by rational combination of various nanomaterials and nanotechnology. Such oral nanosystems have the advantages of strong adaptability, small size, convenient processing, long-lasting pharmaceutical activity, and drug controlled-release, so it can effectively improve the oral bioavailability and efficacy of insulin. This review summarizes the basic principles and recent progress in oral delivery nanosystems for insulin, including physiological absorption barrier of oral insulin and the development of materials to nanostructures for oral insulin delivery nanosystems.

Abstract:Diabetes, as a global threat nowadays, causes the wide usage of exogenous insulin. Subcutaneous injection is the traditional insulin administration in the diabetes treatments and is inconvenient and painful, which results in potential side effects of hypoglycemia. Scientists have made great efforts to develop alternatives. As the promising solutions, smart insulin delivery systems fabricated with glucose- responsive compounds attract considerable attention in the field of diabetes treatments. This review discusses the classifications, properties, response mechanisms of the glucose-responsive compounds, glucose oxidase and concanavalin A and recent progress of glucose-responsive insulin delivery systems.
A novel multi-task software architecture applied in the intelligent insulin injector: An improved polling loop.

Abstract:It seems impossible for some kinds of microprocessors to run a perfect commercial or open source embedded OS, for their limited internal resources and computing power, As a result, it appears to be more difficult for them to deal with complex multiple tasks. In order to solve the design problem of multi-task software architecture in such processors, we proposed a novel method: improved polling loop, which was developed from the traditional polling loop, based on event-driven programming, running without any embedded OS, and successfully applied in the intelligent insulin injector[1]. This method overcomes the problem that may exist in the traditional polling loop method when responding to more than two tasks coming sequentially, which is caused by the fixed scanning order once the codes is written down. Our method dynamically allocates tasks and implements them strictly in the order of a specified priority. It has the feature of dynamic time allocation, high system stability and convenience of making changes in system functionality in the future.

Expert Opinion on Drug Delivery

Abstract:Introduction: The morbidity of diabetes mellitus is increasing, and subcutaneous injection of exogenous insulin is well established as an effective therapeutic strategy for reducing complications associated with the disease. However, the pain that accompanies repeated injections is an important drawback, and can detrimentally affect the adherence to therapy. Recently, there have been great improvements in injection devices and techniques, including the development of microneedle systems and quantitative injection technologies, which have increased the accuracy of injection, decreased leakage of insulin to the skin surface, and reduced pain. Areas covered: This review highlights some limitations of current techniques for the injection of insulin and its analogs, and describes new methodologies and strategies that have been developed in an attempt to overcome these limitations. Furthermore, novel technologies currently under development that are potential future prospects for insulin delivery are discussed. Expert opinion: New technologies have provided easier and well-tolerated treatment regimens for diabetes patients. However, to further improve patients' satisfaction, self-regulated insulin delivery, automatic adjustment of needle length, memory function to the injection device, use of novel materials could be introduced into insulin injection. Intelligent control of insulin delivery and soluble microneedle arrays may be important areas of future research.

Abstract:Insulin is essential for type 1 and advanced type 2 diabetics to maintain blood glucose levels and prolong lives. The traditional administration requires frequent subcutaneous insulin injections that are associated with poor patient compliance, including pain, local tissue necrosis, infection, and nerve damage. Taking advantage of emerging micro- and nanotechnologies, numerous alternative strategies integrated with chemical approaches for insulin delivery have been investigated. This review outlines recent developments in the controlled delivery of insulin, including oral, nasal, pulmonary, transdermal, subcutaneous and closed-loop insulin delivery. Perspectives from new materials, formulations and devices at the micro- or nano-scales are specifically surveyed. Advantages and limitations of current delivery methods, as well as future opportunities and challenges are also discussed.

Abstract:Objective: To introduce the characteristics of insulin preparations and its related delivery systems and make the clinicians and pharmacists familiar with the insulin preparations in order to use them rationally. Methods: By searching the relevant literatures, the classifications of insulin preparations, the development of delivery systems and the new progress of non-injections was introduced systematically. Results: There are many insulin preparations in clinical use, such as regular insulin, rapid analogues, NPH, lente, ultralente, gargine, premixed ones and so on. Insulin delivery systems include insulin pen or pump. Nowadays,the study of non-injections is another popular research topic. Conclusion: Different preparations and administration of insulin can meet the needs of individual treatment.

Abstract:The construction of an Arduino Uno based syringe pump with the ability to measure the patient's body temperature during injection and detect the contrast material bubble has not been done so far. A syringe pump is a device that controls the volume and flow of milliliters to microliters for a certain period of time with very high precision, which gradually pushes the injection piston. This use of syringe pump for drug administration has been done for years. The working principle of this system consists of installing two steel rods to push the syringe piston at a set speed, which works through a coupling system. The advancement of technology has made it possible to use a motor as the driving force of the syringe filled with liquid released through the needle, and the electronic system in this tool is used as a controller. The design using a stepper motor based on Arduino Uno is chosen to adjust the flow rate; the research includes two stages of software and hardware construction. The hardware is implemented with the components needed to build a syringe pump with an Arduino Uno as the brain of the system and a stepper motor as a piston pusher.The flow rate in the syringe pump can be adjusted with the keyboard and displayed on the screen. It is also possible to see the patient's body temperature on the screen, and to detect bubbles in the contrast liquid during injection, we also put a sensor in this device that alerts the operator.

Abstract:Severely diabetic patients need insulin input to maintain the body's glycemic balance. However, traditional injection methods are often associated with poor adherence and an increased risk of hypoglycemia. Microneedle technology offers a promising solution by minimizing pain and trauma during insulin administration. Nonetheless, achieving prolonged glycemic control by microneedle with high insulin loading remains a significant challenge. Herein, we introduce an innovative microneedle patch that draws inspiration from the elegant lightinduced blooming of water lily petals. The patch features a glucose-responsive hydrogel network crafted from two modified polysaccharide polymers, which enables the delivery of long-acting insulin without depending on glucose oxidase. By incorporating phenylboronic acid-modified sodium alginate, quaternary ammonium chitosan, and polyvinyl alcohol into a hydrogel matrix, we have created a microneedle system that harbors dynamic borate ester linkages and electrostatic attractions, resulting in heightened sensitivity to blood glucose levels. The electrostatic interaction acts as a relatively stable crosslinking point, balancing the dynamic reproducibility response based on the borate ester bond. This self-adaptive hydrogel can regulate insulin-controlled release by responding to changes in glucose concentration. Herein, we achieved massive insulin loading (20 IU) with long lasting glycaemic control (48 h) in a single treatment of diabetic SD rats.
A Novel Artificial Pancreas: Energy Efficient Valveless Piezoelectric Actuated Closed-Loop Insulin Pump for T1DM

Abstract:The objective of this work is to develop a closed-loop controlled insulin pump to keep the blood glucose level of Type 1 diabetes mellitus (T1DM) patients in the desired range. In contrast to the existing artificial pancreas systems with syringe pumps, an energy-efficient, valveless piezoelectric pump is designed and simulated with different types of controllers and glucose-insulin models. COMSOL Multiphysics is used for piezoelectric-fluid-structural coupled 3D finite element simulations of the pump. Then, a reduced-order model (ROM) is simulated in MATLAB/Simulink together with optimal and proportional-integral-derivative (PID) controllers and glucose–insulin models of Ackerman, Bergman, and Sorensen. Divergence angle, nozzle/diffuser diameters, lengths, chamber height, excitation voltage, and frequency are optimized with dimensional constraints to achieve a high net flow rate and low power consumption. A prototype is manufactured and experimented with different excitation frequencies. It is shown that the proposed system successfully controls the delivered insulin for all three glucose–insulin models.

Abstract:Here, a continuous two-step glass-capillary microfluidic technique to produce a multistage oral delivery system is reported. Insulin is successfully encapsulated into liposomes, which are coated with chitosan to improve their mucoadhesion. The encapsulation in an enteric polymer offers protection from the harsh gastric conditions. Insulin permeability is enhanced across an intestinal monolayer.
