Study on the efficacy and safety of Transurethral enucleation and resection of the prostate in treatment of IV grade Benign prostatic hyperplasia

Abstract:Objective: To study the efficacy and safety of transurethral enucleation and resection of the prostate (TUERP) in the treatment of IV grade Benign prostatic hyperplasia(BPH). Methods: Total of 35 cases of IV grade BPH underwent TUERP. Operative safety were evaluated according to the vital sign, definition of the operation visual field, operation time, postoperative hemoglobin and blood electrolyte, irritation symptoms of bladder, secondary bleeding, the time of lavage fluids became clear. All cases' emiction condition were followed up, whose therapeutic efficacy were evaluated according to I-PSS,QOL and Qmax 3 months later. Results: Operations in all 35 cases were successful. Vital sign was steady, the operation visual field was clear. The average Operation time was (102.3±25.6) min. The postoperative hemoglobin content was not significant decreased, the blood electrolyte content was normal and no transurethral resection syndrome (TURS) occurred. Irritation sign of bladder occurred in 7cases and secondary bleeding in 1 case. The average time of lavage fluids became clear was (28.5±9.7) hours. All the cases urinate unobstructedly after operation, But 5 cases had stress urinary incontinence who cured after expectant treatment. I-PSS was(5.6±1.8), QOL was (1.1±0.9) and Qmax was(21.3±4.8)ml/s 3 months later. No urethral stricture and bladder neck contracture occurred. Conclusion: TUERP is an modified method of TURP. It has the advantages of both TURP and open prostatectomy. It is safe and effective treatment for IV grade benign prostatic hyperplasia by undergoing TUERP.
