Abstract:Objective To explore the value and availability of echocardiography in the diagnosis of interrupted aortic arch (IAA) with aberrant right subclavian artery (ARSCA) in children. Methods The echocardiographic images and clinical data of 6 patient s with IAA and ARSCA confirmed by operation were analyzed retrospectively. Results six cases of IAA (all were type B) was associated with ARSCA. Among them, 4 cases were misdiagnosed as IAA-C. In IAA-B associated with ARSCA right and left common carotid artery were originated from the aortic arch before interruption, and they appeared as a symmetrical ″Y″ shape with ascending aorta; left and right subclavian artery were originated from the aortic arch after interruption, and the blood vessel lines of ARSCA were variable but always extending to the upper right; the ratio of the diameter of pulmonary artery and aorta were increased obviously. Conclusions It is significant to carefully examine aberrant right subclavian artery, and it was help to the cardiac surgery. Interrupted aortic arch: from a singular forensic case to clinical and diagnostic cue

Abstract:Background: Abnormalities of aortic arch include variables related to structure, location, anatomical relationships and collateral vessels. This group includes the malformation known as "interrupted aortic arch". During fetal life, oxygenated blood is provided by materno-fetal circulation and own fetal circulation. Case report.: This work discuss of female fetus born without any vital signs at 38th week by 34-years-old primigravida affected by gestational diabetes and anemia gravidarum. Pregnancy was regular. Labor and cardiotocographic tracings were regular, natural delivery; however, at the time of umbilical cord severing the baby went into cardiocirculatory arrest. Autopsy findings were: - hypertrophied right ventricle; - no morphological changes in interventricular septum, - ascending aorta physiologically arose from the left ventricle; after the emergence of the left subclavian artery and the anonymous trunk, it suddenly stopped at cul de sac. Thoracic tract arose as a collateral branch of the pulmonary artery. In addition, Botallo duct was patent. Thus, the diagnosis of death was interruption of aortic arch resulting in cardiogenic shock and irreversible cardiovascular collapse. Conclusion: In filed case, newborn presented "type B" interruption. Interruption manifests itself between left common carotid artery and the left subclavian artery. Descending aorta arose from the pulmonary trunk. So that, in this case, right ventricle provided systemic circulation. At delivery, with the severing of the umbilical cord and physiological closure of the Botallo ductus, acute right heart failure and shock occurred. This work aims to be a cue for scientific community, in order to assist clinical and diagnostic knowledge using autopsy data with expost perspective.
Isolated interrupted of aortic arch diagnosed using CT angiography: A case report and literature review.

The majority of patients with IAA rarely reach adulthood. In most cases, IAA is associated with an intracardiac malformation, such as ventricular septal defect, bicuspidaortic valve, and patent ductus arteriosus. We reported a 70-year-old man with isolated IAA diagnosed using computed tomography angiography (CTA) without previous intervention.Patient concerns:The patient presented with edema of the lower extremities and productive cough.Diagnoses:CTA displayed an interruption of the aortic arch distal to the left common carotid artery (type B) and extensive collateralization.Interventions:The patient refused surgery and underwent conservative therapy with follow-up examinations because of the extensive collateral vascularization.Outcomes:The patient is still currently undergoing conservative therapy with follow-up examinations.Lessons:IAA can present in adulthood with different clinical symptoms. Careful physical examination of the lower and upper peripheral pulses in adults with a chief complaint of hypertension is of outmost importance. CTA is a useful noninvasive imaging modality for the morphologic evaluation of aortic interruption and collateral circulation.

Abstract:BACKGROUND:Interrupted aortic arch (IAA) is a rare congenital heart disease defined by an interruption of the lumen and anatomical continuity between the ascending and descending major arteries. It is usually found within a few hours or days of birth. Without surgery, the chances of survival are low. If IAA patients have an effective collateral circulation established, they can survive into adulthood. However, IAA in adults is extremely rare, with few reported cases.CASE SUMMARY:A 27-year-old woman presented with a 6-year history of progressively worsening shortness of breath and chest tightness on exertion. She had cyanotic lips and clubbing of the fingers. A transthoracic echocardiogram revealed an enlarged heart and dilation of the main pulmonary artery. There was an abnormal 9 mm passage between the descending aorta and pulmonary artery. The ventricular septal outflow tract had a 14 mm defect. Doppler ultrasound suggested a patent ductus arteriosus and computed tomographic angiography showed the absence of the aortic arch. The diagnoses were ventricular septal defect, patent ductus arteriosus, and definite interruption of the aortic arch. Although surgical correction was recommended, the patient declined due to the surgical risks and was treated with medications to reduce pulmonary artery pressure and treat heart failure. Her condition has been stable for 12 mo of follow-up.CONCLUSION:Although rare, IAA should be considered in adults with refractory hypertension or unexplained congestive heart failure.

Abstract:A right aortic arch and aberrant subclavian artery result from an interruption in the remodeling of the pharyngeal arch arteries. We occasionally encounter this anatomical variation during angiography. Patients with disorders such as Down syndrome and congenital heart disease show a high incidence of an aberrant right subclavian artery, and this anomaly can cause symptomatic esophageal or tracheal compression. The root of the aberrant artery may show dilatation(referred to as a Kommerell diverticulum), dissection, intramural hematoma, or rupture necessitating cardiac intervention using a surgical or endovascular approach. Neurointerventionalists should have working knowledge of the anatomy to rapidly understand the anatomy and ensure a safe procedure. A left transradial approach should be considered if prior knowledge of the aberrant subclavian anatomy is available.

Abstract:Objective:To investigate the multi-slice CT characteristics of congenital aortic arch anomalies in children, in order to improve the understanding and diagnostic level of this disease.Methods:CT data of 83 cases with congenital aor-tic arch anomalies were analyzed retrospectively,and the images were reconstructed using multi-planar reconstruction (MPR),maximum intensity projection (MIP),minimum intensity projection (MinIP)and volume rendering (VR).Results:Obstructive congenital aortic arch anomalies were observed in 22 cases,including 17 cases of coarctation of aorta (COA) (combined with aberrant right subclavian artery in 4 cases),5 cases of interruption of aortic arch (IAA).Non-obstructive congenital aortic arch anomalies were observed in 61 cases,including double aortic arch (DAA)in 8 cases,right aortic arch and right descending aorta in 44 cases,right aortic arch with aberrant left subclavian artery in 7 cases,left aortic arch and right descending aorta in 2 cases (combined with aberrant right subclavian artery in one case).Conclusion:MSCT can clearly demonstrate the aortic arch anomalies and associated malformations and has important value for diagnosis of congenital aor-tic arch anomalies .

Abstract:Objective Interruption of the aortic arch (IAA) is defined as absence of luminal continuity between the ascending and the descending aorta.It is a rare but severe malformation,usually with a deteriorative congestive heart failure and high mortality.We analysed the clinical informations of IAA in order to make improvement in the diagnosis and management.Methods The records of 21 IAA patients,aged 12 days to 11 years (mean age 1.5 months) were reviewed.Results There were ten male and 11 female,accounting for 0.1% of the patients with congenital heart disease of the same period.The anatomic subtype included type A 18 (85.7%),type B 3 (14.3%),and no type C in the records.All cases were associated with large PDA and VSD,some also with ASD (II),bicuspid or unicuspid aortic valve,and subvalvular aortic stenosis.Diagnosis was made in 10 of the patients and suspected in 8 by echocardiography.However,echocardiography failed to make diagnosis in 3 (14.3%).Surgery repair was performed in 9 of the patients.Two older patients were successfully operated in two stages.Seven patients underwent total correction,of which 4 were successful,3 died of ventricular fibrillation,shock due to allergy to protamine sulpate and severe pneumonia during perioperative stage.Conclusion Echocardiography is the primary important method to diagnose IAA,but angiography is necessary for determining diagnosis in some cases.Mortality is relatively higher for totally correction,and the complications during perioperative stage are the important causes of death.

Abstract:Objective To evaluate MRI in the diagnosis of congenital malformations of aortic arch.Materials and Methods MRI findings in 61 patients with congenital malformations of aortic arch were retrospectively analyzed. Of 61 cases, cardioangiography was performed in 37 and the diagnosis was confirmed by operation in 44.Results The malformations in this study included coarctation of aorta (n=25), dextroaortic arch (n=24), interruption of aortic arch (n=5), simple patent arterial duct (n=4), aberrant right subclavicular artery complicated with aortic coarctation (n=2), solitary left subclavicular artery complicated with dextroaortic arch (n=1), persistent fifth aortic arch complicated with interruption of aortic arch (n=1), aortic kink (n=2) and ascending aorta kink (n=1). Conclusion In the diagnosis of congenital malformations of aortic arch, MRI is as effective as cardioangiography.

Abstract:In this paper, we describe an incidentally discovered case of interrupted aortic arch in a 28-year-old male patient with a history of long-standing poorly controlled hypertension. The patient presented to the hospital with a complaint of upper respiratory complaints and long-standing chest pain. A plain chest radiograph was requested to exclude a diagnosis of pneumonia, and the radiography spotted an incidental finding of inferior rib notching. A subsequent CT angiography was done for further characterization, and a diagnosis of interrupted aortic arch was confirmed. Therefore, although rare, IAA should be considered in adults with refractory hypertension or unexplained congestive heart failure.

Abstract:Objectives: Interruption of aortic arch (IAA) is a rare congenital heart disease. This study aims to investigate echocardiographic features and pathological ultrastructural characteristics of fetal IAA and to further analyze its pathological evolution. Methods: A retrospective analysis was conducted on prenatal echocardiographic, post-surgical, or autopsy findings of fetuses prenatally diagnosed with IAA. Prenatal echocardiographic tracking was used to observe the internal diameters and Z-scores of different segments of the aortic arch and the changes in the narrowed section. These observations were combined with autopsy and pathological findings to explore the potential intrauterine evolution of IAA and its cytological basis. Results: The study included 34 fetuses with IAA, with 3, 3, and 28 fetuses prenatally diagnosed with aortic arch dysplasia (AAD), coarctation of aorta (CoA), and IAA, respectively. The 3 AAD and 3 CoA fetuses chose termination of pregnancy 1 to 2 weeks after prenatal ultrasound diagnosis, and autopsy confirmed IAA. Among the 28 fetuses prenatally diagnosed with IAA, 6 cases of CoA progressively worsened, eventually evolving into type A IAA as observed through echocardiographic follow-up. The remaining 22 cases were diagnosed as IAA on the first prenatal ultrasound. Postnatal surgery corrected 3 cases, while 27 cases opted for pregnancy termination, and 4 cases resulted in intrauterine death. Echocardiographic features of the fetal IAA included a significantly smaller left ventricle compared with the right or negligible difference on the four-chamber view, a significantly smaller aorta than the pulmonary artery on the three-vessel view, and a lack of connection between the aorta and the descending aorta on the three-vessel-trachea and aortic arch views. The aortic arch appears less curved and more rigid, losing the normal "V" shape between the aorta, ductus arteriosus, and descending aorta. Color Doppler ultrasound showed no continuous blood flow signal at the interruption site, with reversed blood flow visible in the ductus arteriosus. Transmission electron microscopy of 7 IAA fetuses revealed numerous disorganized smooth muscle cells between the elastic membranes near the aortic arch interruption site, significantly increased in number compared with the proximal ascending aorta. The elastic membranes were thicker and more twisted near the interruption site. The interruption area lacked normal endothelial cells and lumen, with only remnants of necrotic endothelial cells, disorganized short and thick elastic membranes, and randomly arranged smooth muscle cells. Conclusions: Prenatal echocardiography is the primary diagnostic tool for fetal IAA. Post-surgical follow-up and autopsy help identify complications and disease characteristics, enhancing diagnostic accuracy. Some fetal IAA may evolve from AAD or CoA, with potential pathogenesis related to ischemia, hypoxia, and migration of ductal constrictive components.

Abstract:Objective To introduce the experience of single stage repair of interrupted aortic arch with associated complex cardiac lesions. Methods From Jan.2000 to Dec.2005,13 patients were hospitalized with the mean age of(1.1±2.4) years and mean body weight of(7.3±7.2) kg at operation.The associated lesions included transposition of great arteries,truncus arteriosus,double outlet right ventricle,aortic origin of right pulmonary artery,persistence and stenosis of the fifth arch,and aberrant origin of right subclavian artery with mild left ventricular outflow tract obstruction.(Results)Single stage corrective operations were performed in 12 kids,and 1 kid underwent palliative correction.The hospital mortality was 23.1%(3 cases).Mild stenosis of aortic arch anastomosis recurred in 3 kids and 1 death occurred during follow-up. Conclusion Interrupted aortic arch is a rare congenital cardiac anomaly with high mortality in neonate without appropriate interventions during perioperative period.For most patients,repair should be undertaken at the time of diagnosis to reduce the risk of pulmonary vascular occlusive disease.Though early surgical mortality for primary single stage repair is gradually decreasing now,outcomes of interrupted aortic arch remains of concern,especially in patients with associated complex cardiac lesions.

Abstract:Aortic arch anomalies encompass a diverse spectrum of conditions. Elucidating the prevalence of these anomalies, their impact on patient wellbeing, and the most effective diagnostic tools are crucial steps in ensuring optimal patient care. This paper aims to explore the various presentations of aortic arch anomalies, emphasizing the remarkable utility of computed tomography (CT) angiography in their definitive diagnosis and characterization. We conducted a retrospective study on patients who were submitted to the CT angiography of the thoracic aorta or supra-aortic trunks, or the contrast-enhanced CT scans of the thorax and/or cervical region between January 2021 and February 2024 in our Hospital. Out of the total of 2350 patients, 18 were diagnosed with aortic arch anomalies, with an average age of approximately 55 years. The aortic arch anomalies identified in the study were as follows: left aortic arch with the aberrant origin of the right subclavian artery, right aortic arch (types I and II), double aortic arch, aortic coarctation, aortic pseudocoarctation, and ductus diverticulum. Although often asymptomatic, aortic arch anomalies require recognition and CT using advanced post-processing techniques is the optimal diagnostic method with the ability to also identify other associated cardiac or vascular malformations.

Abstract:BACKGROUND:High mortality and morbidity rates are associated with the conventional surgery for aortic arch aneurysm because of cardiopulmonary bypass and deep hypothermic circulatory arrest. In this report, we describe a "hybrid" treatment for aortic arch aneurysm that combines the surgical debranching procedure and the stenting technique.METHODS:A surgical bypass graft is created from the ascending aorta to every main branch of the aortic arch. Subsequently, an endovascular stent graft is deployed retrogradely through the femoral artery with the aid of digital subtraction angiography.RESULT:The patient was discharged on postoperative day 7. A computed tomography scan was routinely performed at 1 week postoperatively to confirm the good patency of all grafts and the stent.CONCLUSION:The feasibility and the benefit of the hybrid treatment for aortic arch aneurysm are confirmed.
Incidental finding of aberrant left common carotid artery during intracerebral hemorrhage examination

Abstract:We report a case involving an aortic arch anomaly that has not been previously documented. The patients were a 76-year-old female who was urgently transported to the hospital because of a sudden disturbance of consciousness. Neurological symptoms indicated impending brain herniation, and the patient was diagnosed with hypertensive intracerebral hemorrhage (left putamen-thalamus) and acute hydrocephalus, thereafter she died approximately 6 hours after arrival. Head and neck-to-chest computed tomography angiography revealed an atypical aortic arch branching pattern. From the proximal side, the left common carotid artery, right common carotid artery, right subclavian artery, and left subclavian artery originated sequentially, suggesting an aberrant left common carotid artery. In the context of acute stroke management, understanding such aortic arch variations is crucial for planning access routes and treatment strategies for neuroendovascular procedures.

Abstract:Objectives: The acute phase of aortic dissection is very dangerous. The identification of risk factors for disease is beneficial to the early diagnosis and treatment of dissection. The aim of this study was to explore the relationship between the different branches of aortic arch and the occurrence of aortic dissection. Methods: A total of 80 patients with Stanford B aortic dissection treated in our hospital were collected. Computed tomography angiography was performed in all the patients. The patients who were excluded from aortic dissection were selected as the control group. The data of the two groups were treated with three dimensional reconstruction of the post processing workstation. The related data of the branches of the aortic arch were measured and compared. Results: Compared with control group, the diameters of brachiocephalic trunk (BCT), left common carotid artery (LCA) and left subclavian artery (LSA) in aortic dissection patients were 16%, 25% and 20% higher. The straight distances from LCA and LSA to the aortic root were 17% and 21% longer. The angles between BCT, LCA, LSA and the long axis of aortic arch were 6%, 7% and 11% larger. There was no significant difference of adjacent distance branches of the aortic arch. Conclusion: Aortic arch morphology is related to the occurrence of Stanford B aortic dissection.

Abstract:Background Cine angiography and echocardiography have been utilized to diagnose congenital aortic arch anomalies. However, the visualization of great vessels by echocardiography is limited, while cine angiography requires cardiac catheterization with ionizing radiation. Contrast-enhanced magnetic resonance angiography (MRA) is a noninvasive modality suitable for visualization of congenital aortic arch anomalies. Objective To evaluate the utility of contrast-enhanced MRA in the diagnosis of persistent fifth aortic arch, a rare congenital aortic arch anomaly, and to compare the diagnostic accuracy of MRA with that of echocardiography and cine angiography. Materials and methods In four pediatric patients, contrast-enhanced MRA studies were performed for diagnosing persistent fifth aortic arch. The findings of MRA were compared with echocardiographic findings and confirmed by cine angiography and operation. Results Transthoracic surface echocardiography noted an aberrant vessel arising from the ascending aorta in two of four patients; the etiology of this vessel was uncertain. In the other two patients a diagnosis of coarctation was made. Of the four patients, only one was diagnosed with interruption of the aortic arch. Contrast-enhanced MRA clarified uncertain echocardiographic findings, enabling the correct diagnosis of persistent fifth aortic arch with fourth aortic arch interruption in all four patients. Conclusion Contrast-enhanced MRA is a safe, accurate, and fast imaging technique for the evaluation of persistent fifth aortic arch and may obviate the need for conventional cine angiography. Cardiac catheterization may be reserved for some types of complicated congenital heart disease and for obtaining hemodynamic information.
Single-Stage Correction for Taussig–Bing Anomaly Associated with Aortic Arch Obstruction

Abstract:Taussig–Bing anomaly and aortic arch obstruction are two types of complex congenital cardiac malformations. Almost 50% of patients with Taussig–Bing anomaly have aortic arch obstruction. This report assesses the surgical outcomes of single-stage correction in neonates with both defects. Between November 2006 and November 2015, 39 neonates with Taussig–Bing anomaly and aortic arch obstruction (28 patients with coarctation of the aorta and 11 patients with interrupted aortic arch) underwent a one-stage arterial switch operation and aortic reconstruction. There were three in-hospital deaths and one late death (8 months after the surgery). The short-term survival rate was 92.3% (36/39), and the mid-term survival rate was 89.7% (35/39). Follow-up data were available for all patients who survived the operation (range 6–92 months). Echocardiology showed six cases of recoarctation, three cases of left ventricular outflow tract obstruction, three cases of right ventricular outflow tract obstruction, four cases of pulmonary artery stenosis, five cases of aortic regurgitation, and eight cases of pulmonary regurgitation. Eight patients required a reoperation during the follow-up period with no mortality. All survivors remained in good condition (New York Heart association functional class I or II). Single-stage correction of Taussig–Bing anomaly with aortic arch obstruction in neonates had favorable short- and mid-term outcomes in terms of mobility and reoperation rate. The optimal operative procedure should be chosen according to the position of the coronary arteries and the type of aortic anomaly.
Usefulness of three vessel-trachea view and parasagittal plan for prenatal diagnosis of interrupted aortic arch

Abstract:Background: Interrupted aortic arch (IAA) is a rare congenital heart disease characterized by loss of continuity between the ascending and the descending aorta. Prenatal diagnosis of IAA by echocardiography is challenging but nonetheless can be accomplished via a systematization of cardiac fetal evaluation. Case presentation: We report a case of fetal IAA type A prenatally diagnosed through two-dimensional echocardiography using both a three vessel-trachea view and a sagittal view. Conclusion: Prenatal counseling regarding the diagnosis and prognosis about this anomaly is still challenging nowadays due to associated anomalies/chromosomal abnormalities who may impact the prognosis. Fetal autopsy in all cases of pregnancy termination after abnormal ultrasound findings is important in order to make a full diagnosis and characterize the anomaly.
