Clinical Value of N Staging for Oral Tongue Carcinoma

Abstract:OBJECTIVE:To evaluate the clinical index related to cervical lymph-node metastasis from squamous carcinoma of the oral tongue.METHODS:The coincidence of the diagnosis of clinical node stage and the pathological node stage was evaluated in 170 patients with tongue carcinoma,by analyzing the number,the size of the node and the location of the node in large size.RESULTS:The coincidence of the clinical diagnosis and surgical-pathological diagnosis was 60%,while the coincidence between clinical and surgical-pathological diagnosis in clinical N1-N3 patients was 17.6%(12/68).The rate of cervical lymph-node metastasis in the patienis with multiple lymph nodes was higher than that in those with a single.Clinically,the rate of node metastases in the patients with the biggest preoperational node above 2 cm in diameter was higher than that in the patients with the one below 2 cm;the deviation was significant in the rate in the same side between in theⅠ area and in the Ⅱ-Ⅲ area of the biggest lymph-node,P0.05.The more abnormal the imaging features of the nodes were,the higher the rate of metastasis was.CONCLUSIONS:The coincidence between clinical and surgical-pathological diagnosis about node metastases is low.If the preoperational patients with tongue carcinoma are found to have the following indexes,such as multiple lymph nodes,the big node with a diameter above 2 cm,or abnormal imaging features of lymph-node,the rate of node metastases may be higher than 50% post-operationally.
