Abstract:Objective:To repair compound defects in mandible tumor cases is the challenge work to oral maxillofacial surgeons.By now,the conventional method of reconstructing mandibular defects was autogenously bone flap transplantation,especially,the vascularized bone flap;according to defect of soft tissue,pedicle myocutaneous flap or free skin flap may be the most popular,free osseous myocutaneous flap of fibula was made to reconstruct the compound defects of mandible and soft tissue.But the patients should receive another operation in other region and another defect was caused at the same time.Complication was not seldom such as lame,large scar and so on..That Implants was placed in osseous myocutaneous flap simultaneously was also difficulty.Methods:The tumors and adjacent parts of the mandible were completely resected in 6 patients,remaining the lower edge of mandible.Functional neck dissections were performed,preserving sternocleidomastoid muscles,its blood supply and partial surface skin.Sternocleidomastoid muscle myocutaneous flap with half pieces of clavicle flap was made.The flap was rotated to repair defect,a micro-plate was fixed at both ends of the bone.Dental implants were placed in the clavicle simultaneously.Second stage operation was performed four months post operation,free palate mucosa was transplanted to reconstruct the gingiva surrounding implant.Distraction osteogenesis was used to restore vertical alveolar crest defect,the model of mandible was manufactured by CAD/CAM before operation to confirm the vector and location of distraction device.During operation the lingual side blood supply of distraction bone block must be protected.The height of distraction was 0.8mm every day post operation till the design vertical crest height was acquired.After three months fixation,the distraction device was removed and implants were placed simultaneously.4 months later,the prosthesis restorations were finished.Results:No death or recurrence was found in these patients during the follow-up,there were no necrosis of muscular pedicels and bone flaps,the clavicle flaps that reconstruct the mandibular defects survived and all the dental implants in the clavicle got good stability without loosening or significant shadow around in the panoramic view.All the patients had good pronunciation and masticatory function,the function and stability of neck and shoulder were not significantly destroyed.In distraction osteogenesis cases,implants were stability and restored crown has good function.Conclusion:The sternocleidomastoid muscle myocutaneous flap with half piece of clavicle,having good blood supply,is not only simple to perform,but also doesn't destroy the figure and function of the clavicle.It's suitable to reconstruct the medium-mini-size operative complex defects after oral carcinoma resection.Distraction osteogenesis was an effect method in treatment of alveolar vertical height defect after mandible block resection.These methods are reasonable for reconstruction of dento-mandible defect.
