Abstract:A 29-year-old male patient presented with erythema and papules along with itching more than 2 months, and the symptoms aggravated for 3 days. He did not respond to regular antihistamines treatment, but clinical symptoms were relieved by intramuscular injection of compound betamethasone. Skin biopsy showed dermis infiltrated by masses of eosinophils and distinctive "flame figures". Then, diagnosis of eosinophilic cellulitis was made and resolved by glucocorticoid treatment.

Abstract:A 57-year-old woman presented with erythematous papules arranged in an annular on both arms,neck,legs and abdomen.Histopathologic analysis of a skin specimen revealed the granulomatous infiltrate and altered collagen in the upper dermis.Mucin accumulation in collagen fibers was obvious by an alcian blue stain.Mild elastic fiber fragmentation was observed by elastic stain.Diagnosis:generalized granuloma annulare.
Flame Figures Associated with Eosinophilic Dermatosis of Hematologic Malignancy: is It Possible to Distinguish the Condition from Eosinophilic Cellulitis in Patients with Hematoproliferative Disease?

Abstract:Eosinophilic dermatosis of hematologic malignancy is a multifaceted dermatosis with a wide morphological spectrum, presenting as pruritic, erythematous, papular and occasionally vesicular, urticarial, nodular eruptions. Histopathologically eosinophil infiltration in the super and deep dermis was found. We reported a case of eosinophilic dermatosis of hematologic malignancy presented as urticarial and vesicular lesions in a patient with chronic lymphocytic leukemia. A skin biopsy revealed a prominent subepidermal blister and a diffuse infiltrate of eosinophils with flame figures in the dermis and subcutaneous tissue. Although flame figures associated with eosinophilic dermatosis of hematologic malignancy is rarely reported, we believe that it would not seem unusual to find them in this skin disease. Eosinophilic cellulitis, which share clinical and histological features with eosinophilic dermatosis of hematologic malignancy, has also been described as showing an association with hematoproliferative diseases. In order to clearly describe eosinophilic dermatosis in patients with hematologic malignancies, the terminology eosinophilic dermatosis of hematologic malignancy, instead of eosinophilic cellulitis, would be a more suitable term in patients with eosinophilic dermatosis.

As the disease is often associated with conditions such as inflammatory bowel disease (IBD), rheumatoid arthritis, haematological disorders and other diseases, diagnosis and treatment often require cooperation with other medical departments. Accordingly, dissemination of information about the disease to doctors in departments other than dermatologists, especially paediatricians, can help in its early detection. Case Presentation: The 11-year-old pediatric patient in the case initially diagnosed with acute febrile neutrophilic dermatosis was eventually confirmed as pustular PG through histopathological examinations of skin and other relevant examinations. The medical condition is lessened after treatment with a combination of glucocorticoids and adalimumab. Conclusion: PG is relatively rare in clinical settings, particularly among pediatric patients exhibiting persistent high fever and signs of pustular pyoderma gangrenosum. This case underscores the importance of considering the potential diagnosis of pediatric pustular PG when confronted with a child presenting persistent high fever and pustules after trauma. Additionally, the proactive initiation of adalimumab emerges as a promising treatment option for pediatric IBD -associated pustular PG. Keywords: pustular pyoderma gangrenosum, acute febrile neutrophilic dermatosis, misdiagnosis, adalimumab Pustular PG represents a rare etiology of skin ulcers in pediatric population, accounting for approximately 4% of global cases. 1 The ulcerative (classic) subtypepredominates, typically addressed as the first-line treatment with systemic corticosteroids. However, the occurrence of the blistering variant of PG, especially in children, is relatively uncommon. The absence of well-defined diagnostic criteria, encompassing specific clinical, histopathological, and laboratory findings, contributes to the potential for misdiagnosis. 2 The most common underlying etiologies in pediatric PG are inflammatory bowel diseases, hematologic malignancies, immune deficiencies and vasculitis. Thorough clinical examination is necessary to clarify the diagnosis and select appropriate treatment methods. 3 Here, we present the case of an 11-year-old pediatric patient who was initially misdiagnosed with acute febrile neutrophilic dermatosis. Subsequent skin histopathology and relevant examinations confirmed the diagnosis of pustular PG. The timely initiation of a treatment combining glucocorticoids and Tumour necrosis factor antagonists (adalimumab) yielded notable improvement in systemic symptoms. An 11-year-old patient suffered a knee injury on May 6, 2023, followed by the onset of fever on May 9. Subsequently, on May 10, the patient developed pustules on the upper lip and sought medical attention at an external institution. The initial diagnosis was "bullous impetigo and ulcerative stomatitis", and the patient received treatment involving intravenous drip administration of metronidazole and levofloxacin, along with the use of chlorhexidine mouthwash. However, the condition progressively deteriorated, leading to ulceration of the oral mucosa and difficulties in eating. On May 13, erythematous patches appeared on the patient's back, accompanied by the development of pustules. The rash extended progressively to involve the face, limbs, anterior chest and buttocks. Persistent fever persisted throughout this period, reaching a maximum temperature of 39.8°C. Empirical treatment attempts with azithromycin, penicillin, and linezolid did not result in any improvement and the condition continued to worsen. On May 21, the patient was admitted to our department with the provisional diagnoses of "unexplained fever" and "acute febrile neutrophilic dermatosis". The patient also presented with ongoing diarrhea and had recently undergone an 8kg weight loss. There was no significant medical history, allergies, or pertinent family medical history in the patient's background. Physical examination (Figure 1): Erythema, erosion, and crust formation are evident on the upper lip. The surface of the tongue exhibits shallow, map-like ulcers covered by purulent coating. A Case of Extranodal NK/T Cell Lymphoma Associated with Diffused Erythema

Abstract:A 26-year-old female patient presented with recurrent fever,subcutaneous nodules and erythema six monthsago,and developed diffuse erythema accompanied with edema half month ago. Dermatopathology showed degeneration and necrosis of epidermal cells,basal liquefaction,infiltrations ofinflammatory cells in the upper derm,and degeneration of local subcutaneous fat. Further histological examination of nasal mucosal membrane showed dysplasia of mesenchymal lymphocytes,with large and distorted nucleus and positive staining of LCA,CD3,UCHL-1,CD8,CD56,TIA-1 and GranzymeB,and negative staining of CD4,and diagnosis of extranode NK/T cell lymphoma was made. We considered the symptoms of the widespread erythema,subcutaneous nodules should be an alert of potential malignant carcinoma,and the skin eruption was cutaneous paraneoplastic syndrome. Therefore,comprehensively examination would benefit to early diagnosis of potential malignant carcinoma.

Abstract:A 30-year-old man was admitted due to 2 days of persistent fever and generalized lesions. The patient presented with extensive erythema and vesicles throughout his body, accompanied by pruritus and fever (reaching 39°C), without any apparent trigger. He reported a 10-year history of atopic dermatitis (AD) and had intermittently used oral antihistamines and topical corticosteroids to manage his condition; recently, he developed AD lesions on his face, chest, and back. Apart from this, he had no other medical history. Examination revealed worsening lesions in the AD areas (Figure 1). Palpation detected enlarged lymph nodes in the submandibular and inguinal regions.

Abstract:A case of subcutaneous Sweet's syndrome is reported. A 58-year-old woman developed erythematous nodules on eyelid and extremities with fever for 1 week. Physical examination showed an erythematous nodule on eyelid about 2 cm in diameter with ridges edge. There were over ten painful erythematous nodules on extremities, especially on wrists, elbows, knees and ankles. Histopathology examination showed a normal epidermis with mild inflammatory changes in the dermis. The subcutaneous adipose lobular showed a dense, predominantly neutrophilic infiltration and leukocytoclasis was present. The diagnosis is subcutaneous Sweet's syndrome.

A Case of Allergic Granulomatosis Presented with Chronic Recurrent Cutaneous Vesicles

Abstract:A case of allergic granulomatosis( Churg-Strauss Syndrome) was reported. A 52-year-old woman presented with recurrent rash and numbness on both lower extremities for 9 months and got fever for 1 week. Her blood routine exams showed increased eosinophilic granulocytes in the past 9 months. She complained of asthma for 3 years. Physical examination revealed swollen erythemas on both lower limbs with fusion blisters which contained turbid fluid in it. The absolute white cell count came out to be 12.16×109/L with the EO% 67. 9%;Nerve EMG suggested neurogenic damage of lower limbs. Histopathological examination showed an infiltration of large amounts of eosinophils surrounding the vessels in the dermis. Diagnosis of Allergic granulomatosis(Churg-Stauss syndrome) was made finally. The patient was given oral prescription of prednisone 50mg qd and cyclophosphamide(CTX) 400mg intravenously once a week. The patient was still under follow-up.

Abstract:Neutrophilic figurate erythema (NFE) is a rarely reported figurate erythema that clinically presents similarly to erythema annulare centrifugum (EAC) with neutrophil-predominant perivascular and interstitial infiltrate in the dermis on histopathology. We present the case of a 32-year-old active-duty military male who presented with a chronic treatment-resistant skin rash. The rash began on his thighs five years previously and was treated with topical and oral antifungals repeatedly without improvement. The patient was deployed overseas during the rash onset, but the rash persisted upon his return stateside. No triggers were identified. His persistent skin eruption consisted of erythematous polycyclic annular plaques with a "trailing edge" scale. Histologic examination revealed perivascular neutrophils and perivascular and interstitial eosinophils without signs of vasculitis or infection. With only 15 reported cases, it can be difficult to recognize leading to long delays in diagnosis and treatment. Despite having a clinical course similar to EAC, NFE may require anti-neutrophil therapy to resolve completely.

Abstract:A case of facial skin candidal granuloma was reported.A 14-year-old girl had a history of erosion around mouth for 10 years and interspersed plaques,scab on face for six years.Physical examination showed dark-red plaques with tan thick scab of various sizes on her face.Fungal microscope examination showed mass hyphae and that was identified as Candidas albicans.Histopathological examination revealed infiltration of diffuse lymphocytes,sparse neutrophils and multinucleated giant cells in upper dermis.Mass hyphae was found in cuticle and spores were found in multinucleated giant cells by PAS stain.The diagnosis was made as candidal granuloma.The lesions improved after treatment of two-month with Griseofulvin,six-month with BCG-PSN,seven-month with Itraconazole,most of the scab was dropped.The patient is still under treatment.

Abstract:Facial neutrophilic dermatosis is a relatively recent and rare entity. Herein, we report an original case with a clinical appearance of crusty pyoderma gangrenosum and limited face involvement, classifying it as neutrophilic dermatosis (ND) of the face. The latter is an infrequent entity and forms part of the ND spectrum. The few reported cases in the literature are characterized by a restricted distribution on the face, as in our case. Nevertheless, this entity remains controversial: some consider it a variant of Sweet's syndrome; others see it as a truly independent entity. Matthews et al. reported an association between ND of the face, as in the case of our patient with a crusty appearance, and ulcerative colitis. Our observation and that of Matthews et al. underline the wide and varied potential for the clinical presentation of this entity.

Abstract:Introduction: Pyoderma gangrenosum (PG) is a rare noninfectious neutrophilic skin disease. The diagnosis of PG is mainly based on clinical manifestations. Therefore, the clinical features of PG are important for confirming the diagnosis of this disease. Herein, the clinical data of 2 young males with PG complicated with hematological malignancies were reported, and the literature were reviewed. Case presentation: The first case was a 22-year-old male who was admitted due to a systemic rash, headache, and fever. Physical examination showed black scabs on the skins of the extremities, trunk, scalp, and face. Biopsy of the skin lesion showed epidermal edema, spongy formation, neutrophil infiltration, acute and chronic inflammatory cell infiltration in the dermis, showing purulent inflammation with epidermal erosion. The bone marrow biopsy showed obviously active proliferation of nucleated cells, granulocytes at various stages, abnormal morphological neutrophils, and occasionally observed young red blood cells. The diagnosis of PG and chronic myelomonocytic leukemia (CMML-0) was made. The second case was a 28-year-old male who presented a swollen, painful right calf following injury and then developed ulcers on skin and soft tissues. Bone marrow biopsy showed obviously active nucleated cell proliferation, suggesting a myeloid tumor. He was also diagnosed with PG and hematological malignancies. They both received hormone and antiinfection therapy. After treatment, their body temperature, infection, and skin lesions were improved. However, both of them were readmitted and had a poor prognosis. Conclusions: PG may be associated with hematological malignancies. For patients with typical skin lesions and obvious abnormal blood routines, it is necessary to investigate the possibility of PG with hematological malignancies.

Abstract:A 53-year-old male patient presented with a 20-year history of an asymptomatic papule on the neck.No dysfunction of thyroid was found. Histologically,there was localized accumulation of mucin in the upper and mid dermis with a loose dermal stroma. The alcian blue staining was positive. The diagnosis of cutaneous focal mucinosis was made.

Abstract:A 25-year-old male patient presented erythema with desquamation on the scalp for 1 year.He had a 1-month history of pruritic skin,lesion on trunk and limbs.Skin examination showed widespread annular lesions with polycyclic arrangement on the trunk and limbs with development of peripheral vesicular lesions.Nikosky′s sigh was negative.Skin biopsy showed intraepidermal vesicules containing eosinophils,no acantholytic cells,papillary oedema and dermal inflammatory infiltrate containing eosinophils,lymphocytes and histiocytes.Directimmunofluorescence staining showed the cell surface C3 deposits throughout the epidermis.
