Abstract:Pyoderma gangrenosum (PG) is a chronic and rare refractory skin disease characterized by inflammatory papules, pustules, and latent ulcers. [ 1 ] In the clinic, patients often are admitted due to symptoms of PG, such as obvious pustules, ulcers, and pain, and are easily misdiagnosed as infectious diseases. Early debridement and incision drainage are performed according to the principles of infectious disease treatment. This may trigger the homomorphic reaction of PG in the acute phase, further aggravate the skin lesions, increase the difficulty of diagnosis, and bring great pain to the patients. PG is characterized by neutrophil infiltration in histology. The etiology and pathogenesis of PG are currently unclear, and there is no clear diagnosis and treatment guide. The skin lesion morphology, laboratory examination, and histopathology have no obvious specificity, which is an exclusive diagnosis and is easy to misdiagnose. Clinically, it can be divided into 4 types: ulcer, pustule, blister, and proliferative type. Ulcerative PG belongs to the most common of the 4 types, also known as classic PG, which was first reported by Brunsting et al in 1930. It can rapidly develop from a nodule or superficial hemorrhagic pustule to a painful ulcer, with central liquefaction necrosis, and a raised edge with a purple-red color. Irregular cribriform scars are often formed after the skin lesions are healed and are often accompanied by systemic diseases. In this report, we described the clinical data of 2 young males with PG complicated with hematological malignancies and reviewed the literature.

Pyoderma Gangrenosum in a 75-Year-Old Male With Follicular Diffuse Large B-cell Lymphoma

Abstract:Pyoderma gangrenosum (PG) is a rare neutrophilic dermatosis that characteristically presents with progressive ulcerative lesions. The association of PG with hematological malignancies remains unclear due to its varied clinical presentation. Herein, we report the unusual case of PG in a 75-year-old male with stage III follicular diffuse large B-cell lymphoma. Seven days subsequent to his first dose of rituximab, cyclophosphamide, doxorubicin, vincristine, prednisolone (R-CHOP) therapy, he presented to the emergency department with generalized malaise, bilateral lower extremity edema, and ecchymoses with ulcerative wounds on the dorsal of his feet. Due to the rapid progression of the patient's dermatological manifestations and declining clinical status, he required serial surgical wound debridement and a biopsy, which revealed an occlusive vasculopathy with dermal and epidermal necrosis. These pathological findings, along with the patient's clinical presentation, led to the diagnosis of PG. The patient was treated with negative pressure wound therapy, steroids, and tacrolimus ointment, which led to a marked improvement in the appearance of the patient's dermatological features and clinical status.

Abstract:Background: Pyoderma gangrenosum (PG) is a neutrophilic skin disease characterized by recurrent painful cutaneous ulcers, often accompanied by inflammatory bowel disease, joint pain, and other systemic damage. This disease is relatively rare in clinical practice and its diagnosis and treatment are often delayed, leading to secondary infections in the skin lesions, prolonged disease course, and increased disease burden on patients. This study retrospectively analyzed the clinical characteristics and treatment strategies of patients with PG admitted to our hospital and conducted a literature review, in order to improve the understanding of the disease among clinical doctors, enable patients to receive better diagnosis and treatment, and ultimately improve patient prognosis. Methods: Clinical data of patients diagnosed with PG and hospitalized in Beijing Chaoyang Hospital, Capital Medical University from January 2014 to December 2022 were retrospectively collected. The clinical manifestations, treatment strategies, efficacy, and disease outcomes were analyzed. Results: A total of 14 patients, including 8 males and 6 females, aged 14 to 66 years, were included. Skin lesion types: 13 cases were ulcer-type, 1 case was pustule combined with ulcer-type, and the lower limbs were the most commonly affected areas. All the 14 patients presented with comorbidities. All patients were treated with glucocorticoids, with a daily dose equivalent to 20 to 100 mg prednisone and a median dose of 40 mg. Among them, 3 patients were treated with minocycline in combination, 1 patient was treated with mycophenolate mofetil 0.5 twice daily in combination, 1 patient was treated with cyclophosphamide 0.1 once daily in combination, and 1 patient was treated with thalidomide 0.1 every night in combination. Conclusion: PG is a relatively rare immune-related skin disease. Our small sample data analysis found that male PG is not uncommon in the Chinese population. Systemic glucocorticoids can quickly control the symptoms of PG in most patients with PG. In patients with poor efficacy or limited use of glucocorticoids, immunosuppressive drugs or novel targeted drugs such as biologics or small-molecule drugs should be used in combination as early as possible. Skin lesion care focuses on preventing infection, avoiding surgical debridement, and emphasizing pain management and the symptomatic treatment of comorbidities.
Proliferative pyoderma gangrenosum complicated by myelodysplasia syndrome: Two case report

Abstract:Two cases of proliferative pyoderma gangrenosum complicated by myelodysplastic syndrome (MDS) are reported. Both patients were male, with 2 years and 3 months of disease course respectively. One patient died of MDS after 18 months of disease onset, and the another patient was still following up. Patients diagnosed as pyoderma gangrenosum should be examined carefully for systemic complications, especially need to perform the hematological tests. These patients have poor prognosis if complicated by myelodysplastic syndrome.
Clinical Analysis of Pyoderma Gangrenosum: a Report of 15 Cases

Abstract:Objective To explore the clinical characteristics and treatment strategy for pyoderma gangrenosum (PG). Methods Clinical data of 15 PG patients (from Jan. 1995 to Dec. 2011) in Peking University Third Hospital were retrospectively analyzed, and its clinical characteristics and therapy were studied. Results There were 15 PG patients, including 11(73.3%) males and 4(26.7%) females. In 15 patients, 8(53.3%) had the complication of systemic diseases, the ulcer area was 2.8%(0.5%-7.0%) of total body surface area (TBSA) before treatment and 1.2%(0.6%-2.0%) after treatment, and 7(46.7%) did not have the complication of systemic diseases, the ulcer area was 1.6%(1.0%-3.5%) TBSA before treatment and 0.4%(0.1%-0.9%) after treatment. The differences of ulcer area between patients with and without systemic diseases before and after treatment were both statistically significant (P<0.05). Two of them have the complication of myelodysplastic syndrome, whose ulcer areas were larger than 5% of their whole body areas. One of them has effective response to surgical intervention in his stable stage. Conclusions PG is more common in male, and is prone to have the complication of systemic disease, especially myelodysplastic syndrome, which is often accompanied by large ulcer area. The surgical intervention should be forbidden in its progressive stage. The prognosis is remarkably improved by surgical intervention of local ulcer surface on the basis of systemic treatment when the disease is in its stable period. Abstract:Pyoderma gangrenosum (PG) is a rare autoinflammatory skin disorder, which is characterised by rapidly developing and tender cutaneous ulcers. The treatment of PG is challenging. Palmoplantar pustulosis (PPP) is also an autoinflammatory dermatosis with sterile pustules on the palms and/or the soles. We demonstrated a 68-year-old patient with coexisting autoinflammatory diseases including PG, 1-year history of plaque psoriasis and PPP, recovered after treatment with adalimumab. We also reviewed published reports of PG-associated autoinflammatory syndromes with adalimumab.

Abstract:Pyoderma gangrenosum (PG) is a rare autoinflammatory ulcerative neutrophilic skin disease. Its clinical presentation is a rapidly progressing painful skin ulcer with ill-defined borders and surrounding erythema. The pathogenesis of PG is complex and not fully understood. Clinically, patients with PG often have various systemic diseases, the most common being inflammatory bowel disease (IBD) and arthritis. Due to the lack of specific biological markers, diagnosing PG remains difficult, which easily resulting in misdiagnosis. Some validated diagnostic criteria have been applied in clinical practice that facilitate its diagnosis. The treatment of PG currently consists mainly of immunosuppressive and immunomodulatory agents, especially biological agents, which have bright prospects for PG therapy. After the systemic inflammatory response is controlled, the problem of wounds becomes the main contradiction in PG treatment. Surgery is not controversial for PG, increasing evidence shows that with adequate systemic treatment, the benefits of reconstructive surgery for patients are increasing.

Abstract:Background Hematologic malignancies (HMs) are well-known underlying comorbidities of pyoderma gangrenosum (PG). However, studies quantifying the likelihood of PG after HMs are yet to be performed. Objective To investigate the bidirectional association between PG and several HMs, namely acute leukemia, chronic leukemia, Hodgkin lymphoma, non-Hodgkin lymphoma, and multiple myeloma. Methods A population-based retrospective cohort study was conducted to study the risk of HMs in patients with PG ( n = 302) as compared to age-, sex-and ethnicity-matched control subjects ( n = 1,799). A case–control design was used to estimate the likelihood of PG in individuals with a preexisting history of HMs. Adjusted hazard ratios (HRs) and adjusted odds ratios (ORs) were estimated by Cox regression and logistic regression, respectively. Results The prevalence of preexisting HM was higher in patients with PG than in controls (6.7% vs. 0.9%, respectively). The likelihood of having PG was significantly greater among patients with a history of HM (adjusted OR, 7.88; 95% CI, 3.85–16.15; p < 0.001), particularly during the first year following the diagnosis. This association was significant for acute leukemia, chronic leukemia, non-Hodgkin lymphoma, and multiple myeloma but not for Hodgkin lymphoma. The incidence rate of HM was 3.3 (95% CI, 1.2–7.4) and 1.6 (95% CI, 0.9–2.6)/1,000 person-years among patients with PG and controls, respectively. Relative to controls, patients with PG were not more likely to develop subsequent HM (adjusted HR, 2.22; 95%CI, 0.77–6.45; p = 0.142). Compared to other patients with PG, those with HM-associated PG experienced an increased all-cause mortality rate (adjusted HR, 2.19; 95%CI, 1.09–4.40; p = 0.028). Conclusion HM, particularly acute leukemia and multiple myeloma, are associated with an elevated likelihood of provoking PG.

[Analysis of clinical features and treatment of pyoderma gangrenosum].

Abstract:Objective: To explore the clinical features and treatment of pyoderma gangrenosum (PG). Methods: A retrospective observational study was conducted. From January 2012 to July 2021, 25 patients with PG who met the inclusion criteria were admitted to Beijing Fucheng Hospital, including 16 males and 9 females, with the age of onset of disease being 14 to 75 years. Among them, the classification of PG identified 17 cases of ulcerative type, 6 cases of pustular type, 1 case of proliferative type, and 1 case of bullous type. Six patients were accompanied with systemic diseases, while 19 patients were not accompanied with systemic diseases. At the same time of systemic treatment with glucocorticoids, dressing changes or surgical skin grafting was performed on the wounds. The results of laboratory and histopathological examinations, the overall curative effects and follow-up of patients, the wound healing time of patients with negative and positive microbial culture results of wound secretion specimens, and the curative effects of patients with and without systemic diseases were analyzed. Results: The results of blood routine examination of 19 patients were abnormal, and all the immunological indexes were normal in all the patients; the microbial culture results of wound secretion specimens were positive in 14 patients; and the histopathological examination results of ulcer boundary tissue in 15 patients with rapid wound progress were mainly local tissue inflammatory changes. The wounds were cured in 17 patients, mostly healed in 7 patients, and not healed in 1 patient. After one-year's follow-up, the PG in 3 patients relapsed due to self-discontinuation of medication after discharge, and the wounds were healed gradually after adjustment of medication, while the remaining patients had no relapse. The days of wound healing in 14 patients with positive microbial culture results of wound secretion specimens were 21-55 days, and the days of wound healing in 11 patients with negative microbial culture results in wound secretion specimens were 20-54 days. In the 6 patients with systemic diseases, the wounds of 3 patients were cured, and the wounds of the other 3 patients were mostly healed. In the 19 patients without systemic diseases, the wounds of 14 patients were cured, the wounds of 4 patients were mostly healed, and the wound of 1 patient was not healed. Conclusions: The laboratory examination and pathological manifestations of patients with PG lacks characteristics, and their clinical manifestations are rich and diverse, thus PG can be easily misdiagnosed. The glucocorticoids combined with immunosuppressive therapy have good effects on PG. Surgical intervention can be performed on the wounds. Specifically, excessive debridement is not recommended in the acute phase, but skin grafting can be performed in the contraction phase.
[Pyoderma gangrenosum: a report of 3 cases].

Abstract:OBJECTIVE:To improve the surgeon's understanding of pyoderma gangrenosum (PG) and avoid misdiagnosis and incorrect treatment. METHODS:Three cases of PG managed in the department of general surgery during the past 10 years were retrospectively reviewed. RESULTS:All of the 3 cases mainly presented with necrosis of skin and soft tissues, followed by formation of painful and extensive creeping ulcer. They all had fever and were initially diagnosed and managed as "infection", but repeated wound exudates culture showed negative results and antimicrobial therapy was not effective. The ulcers were enlarged quickly despite of active debridement. Histopathology of ulcer biopsy suggested nonspecific inflammation. After the diagnosis of PG was established, systemic therapy with steroids and immunosuppressants were administered together with local wound care. The progress of the disease was controlled soon and fully resolved finally. CONCLUSIONS:Pyoderma gangrenosum is misdiagnosed frequently. For severe patients, systemic therapy with steroids and immunosuppressants should be used as early as possible and skin lesions would heal together with mild local wound care.

Abstract:We report a case of myeloid neoplasm with the abnormality of PDGFRB gene secondary interstitial pneumonia. A 66-year-old male complained dry cough and shortness of breath after regular activities. Two years ago, high-resolution computerized tomography (HRCT) showed interstitial lung diseases, and no clear etiology was found. At that time, the diagnosis was idiopathic pulmonary fibrosis. In the period of hospitalization, the interstitial lung disease was significantly advanced, and the peripheral blood eosinophil was significantly increased. ETV6-PDGFRB fusion genes was positive detected by FISH. Myeloid neoplasm with the abnormality of PDGFRB gene was diagnosed. The patient was treated with the tyrosine kinase inhibitor-imatinib mesylate which is the first choice for this disease currently.Myeloid neoplasm with the abnormality of PDGFRB gene is a rare hematologic tumor characterized by myeloid dysplasia, eosinophilia, and PDGFR gene rearrangement. Most of this tumor occurs in male, often with splenomegaly, and a few have damages in liver, skin, heart, and other organ tissues.
Pyoderma Gangrenosum As the Initial Manifestation of Small Cell Lung Cancer with Dermatomyositis

Abstract:To the Editor: A 54-year-old man developed painful ulcers on the neck one month ago, followed by periorbital edema. The ulcers then spread to his trunk, back, and upper arms. He had muscle weakness, mild muscle soreness with activity, and dysphagia for foods for the past three weeks. He was diagnosed as drug eruption in a local hospital and received irregular corticosteroids therapy for two weeks. Thereafter, the ulcers started to heal. He had a 40 years’ history of smoking. The patient was emaciation, and physical examination revealed heliotrope sign, poikiloderma involving the face, “V” sign of the chest, and the upper arm, nail fold telangiectasia, and large well-defined ulcers on the abdomen, back, and upper arms, there was no Gottron sign [Figure 1]. It was hard for him to raise his arms and squat. The triceps and quadriceps were tender to palpation. Investigation revealed elevated levels of antinuclear antibodies (1:40; normal range, <1:40), creatin kinase (382 U/L; normal range, 56–244 U/L), lactic dehydrogenase (254 U/L; normal range, 109–245 U/L), white blood cell (19.71 × 109/L, normal range, 3.5–9.5 × 109/L), erythrocyte sedimentation rate (78 mm/h; normal range, 0–15 mm/h), C-reactive protein (117.52 mg/L; normal range, <8 mg/L), neurospecific enolase (25.03 ng/ml; normal range, 0–16.30 ng/ml), and electromyography showed muscle damage. Chest computed tomography plain and enhanced scan suggested possible malignant tumor in left upper lobe [Figure 1]. The biopsy of axillary lymph node showed “small cell lung cancer”. Histopathology of ulcer edge revealed ulceration of the epidermis with a dense neutrophils infiltration. The patient was treated with methylprednisolone 40 mg/d and minocycline 100 mg/d. A topical strong-potent corticosteroid and antibiotics were applied to the ulcers daily. After 4 weeks’ treatment, the weakness relieved and ulcers healed [Figure 1]. Now, the patient is receiving chemotherapy targeted to the lung cancer with methylprednisolone 40 mg/d and methotrexate 15 mg/week. DM and PG are both paraneoplastic dermatoses. DM-associated ulcers were generally caused by calcification,[1] vasculitis, and panniculitis.[2] The ulcers are usually small, deep, and resistant to drug therapy. Some ulcers occur at the site of Gottron sign, and some occur on the buttocks and thighs, calcinosis in adults with DM was associated with fingertip ulcers.[1] The pathogenesis of PG involves neutrophilic dysfunction, inflammatory mediators, and genetic predisposition. Interleukin-1β (IL-1β) and its receptor are found to increase in PG lesions, suggesting the role of autoinflammation through inflammasome creation. The inflammasome is critical to activating IL-1β to ward off pathogenic infection, dysfunction, or activation in the absence of infection can trigger an inflammatory cascade and the onset of PG.[3] Tumor-associated or tumor-elicited inflammation is dependent on cytokine and chemokine production by tumor cells and other cellular components of tumor microenvironment. Various tumors, such as lung and breast cancer were reported to overexpress IL-1β. IL-1β produced by tumor cell may lead to granulocyte colony-stimulating factor-dependent neutrophil expansion due to IL-17 expression by γδT cells.[4] At the early stage of lung cancer, recruited neutrophils can present antigens to anti-cancer T cell.[5] Hence, it can be probably suggested that the overactive immune response to the cancer results in PG, and abnormal immune response to cancer leads to DM. In conclusion, we reported a case of malignancy with concurrent DM and PG, and raise a hypothesis of pathogenesis, the patient responded well to corticosteroids combined with minocycline therapy. Abstract:At the Department of Laboratory Medicine at Yamagata University, our medical students are trained to have practical knowledge and the ability to plan laboratory examinations for the clinical diagnosis of patients. For 4th-year medical students, 31 clinical lectures are scheduled on a variety of subjects concerning laboratory findings. The 2-week practical training of 5th-year students includes an educational curriculum that allows them to master the various aspects of laboratory examinations, such as collecting blood samples, determining flow-volume curves, and conducting abdominal echograms. Finally, 6th-year medical students undergo a 5-week training period during which they learn the essentials of laboratory examinations. We are proud of our present clinical curriculum for medical students. However, within a couple of years, the clinical educational program for medical students will be changed for the purpose of better preparing medical doctors in Japan. The advisory organ of the Japanese Ministry of Health, Labor, and Welfare has recommended the induction of a newly developed model core curriculum that will be nationally standardized and will focus on problem-based learning for medical students. Therefore, the present educational system, even in the field of laboratory medicine, will have to be changed.

Abstract:Results of therapy with thyroid hormones (LT4 and/or LT3) were evaluated in 104 patients (95 females and 9 males) with simple diffuse or nodular goiter. The mean interval of observation was 7.2 years. The reported results were analysed in relation to patient's age at the onset of the pathology, time elapsed since its appearance, place of origin and sex of the patient, results of laboratory studies, and clinical signs.

Abstract:Background Over the past few decades, advances in medical research and diagnostic tools have shed light on some aspects of pyoderma gangrenosum (PG). Nevertheless, the multifactorial etiology, pathogenesis, and optimal management strategies for PG need to be further investigated. To address these knowledge gaps and contribute to a better understanding of this complex dermatological disorder, we collected epidemiological, clinical, and therapeutic aspects of a case series of PG patients occurring in our department over the past 10 years. Methods We performed a single‐centered, retrospective, observational study analyzing all cases with a diagnosis of PG observed at the Dermatology clinic of the Fondazione Policlinico A. Gemelli IRCCS Catholic University from January 1, 2013, to January 1, 2023. For each case, we retrieved demographic data, the presence of other skin and systemic conditions, and the histopathological and clinical characteristics of PG, such as clinical variant, number of lesions, disease localization, previous therapy, response to treatment, and occurrence of relapse. Results We included 35 patients, 22 females and 13 males with a mean age of 40.0 years. Twenty patients (57.1%) had multiple localizations of disease, and the most commonly involved site was the lower limbs (85.7%). The lesions were mainly associated with inflammatory bowel diseases (51.4%) and hidradenitis suppurativa (37.1%). Clinical resolution with complete re‐epithelialization was achieved in 25 patients (71.4%) with an average time of 20.8 months. On average, patients who underwent therapy with biological drugs had better outcomes. Conclusions PG is a severe, rare, and pleomorphic disease associated with a broad spectrum of conditions. Corticosteroids remain the primary first‐line approach for severe forms, but using biological immunosuppressants is promising.

Abstract:A 55 old male patient was admitted to our hospital because of bilateral lower limb lesions. The lesions had lasted for over 10 years and aggravated for half a year which were manifested as scattered ulcers on both legs with clear demarcations and purple-red borders. Histopathology showed pauci-infiltration of lymphocytes around the vessels in the whole depth of dermis and a predominant infiltration of plasma cells. A combination of intravenous cyclophosphamide and oral DDX showed a better effect.

Abstract:A rare case of multiple myeloma complicated with pyoderma gangrenosum is reported. A 58-year-old Chinese woman presented multiple papules on the left lower leg, which developed to plaques and became ulceration during four months. A biopsy of the lesion demonstrated leucocytoclastic vasculitis. IgA lambda light-chain was highly detected, and bone marrow biopsy revealed with plasmacyte and juvenile plasmacyte hyperplasia, which indicated the diagnosis of multiple myeloma IgA λ type. IgA paraprotein may contribute to the pathogenesis of pyoderma gangrenosum.
