Correlation between the factors of insulin resistance and coronary artery restenosis

Abstract:Aim: To explore the relationship between insulin resistance and coronary artery restenosis, and to study the correlative factors of the latter so as to seek the possibility of rehabilitation intervention. Methods: Eleven factors including age, diabetes mellitus, fasting insulin were chosen from 191 patients (163 males, 28 females) who underwent coronary stent (CS) and were performed with a follow-up angiogram, and the patients were grouped according to whether with restenosis and hyperinsulinism or not. The relations of clinical factors above-mentioned with restenosis and hyperinsulinism with restenosis degree after CS were analyzed with single factor analysis, and the effect of hyperinsulinism on the incidence rate (survival rate) of restenosis after CS was analyzed by Kaplan-Meier. Results: Totally 263 cradles were put into 217 vessels of the 191 selected cases. There were 68 target lesions occurred restenosis in 54 cases the restenosis rate of target lesions of 25.9% (68/263). There were 36.1% patients (69/191) whose fasting insulin > 15 mU/L. The percentages of hyperinsulinmia patients in restenosis group( 48.1%) was higher than that in the non-restenosis group (31.4%) (χ2= 4.7, P < 0.05). Kaplan-Meier analysis demonstrated that the non-restenosis rate (survival rate) was lower in the hyperinsulinemia group (62.3%) than in the serum insulin group (77%) (χ2 = 4.7, P < 0.05). Conclusion: Hyperinsulinemia may be the risk factor for coronary stent restenosis, and the high fasting insulin level may accelerate the process of post-stenting coronary restenosis.
