Abstract:AIMS/INTRODUCTION:Visfatin is a newly discovered adipocytokine hormone, which exerts an insulin-like effect by binding to the insulin receptor-1. However, the role of visfatin in human gestational diabetes mellitus (GDM) remains controversial. The purpose of the present study was to investigate the correlation between serum visfatin and metabolism of glucose and lipid in GDM.MATERIALS AND METHODS:This was a prospective study. A total of 38 GDM patients and 35 age- and body mass index-matched controls were studied between January 2012 and October 2013. Fasting serum levels of visfatin, fasting plasma glucose, hemoglobin A1c and lipid profile were measured. Two-tailed t-tests and Pearson's correlation coefficient were used to analyze the data.RESULTS:Perinatal visfatin levels were negatively correlated with fasting plasma glucose, insulin resistance index and triglycerides in controls (r = -0.47, -0.51, -0.57, respectively; P < 0.05), and positively correlated with high-density lipoprotein cholesterol (r = 0.32, P < 0.05). A positive correlation with visfatin level only appeared in weight gain and body mass index in women with GDM (r = 0.36, 0.45, respectively; P < 0.05).CONCLUSIONS:Visfatin appears to be involved in glucose and lipid metabolism regulation and insulin resistance, suggesting a role in GDM pathogenesis.
Elevated serum visfatin levels are associated with poor prognosis of hepatocellular carcinoma

Abstract:Visfatin is considered a pro-inflammatory adipocytokine, and it is commonly increased in obesity-related diseases. This study aimed to evaluate the levels of serum visfatin in patients with hepatocellular carcinoma (HCC) and its diagnostic and predictive value in detecting HCC. Fasting serum levels of visfatin of 135 HCC patients, 115 chronic hepatitis B (CHB) patients, 129 liver cirrhosis (LC) patients, and 149 healthy controls were determined via enzyme-linked immunosorbent assay. Meanwhile, serum alpha fetal protein (AFP) and interleukin-6 (IL-6) were also assayed. The median serum visfatin concentration in HCC patients was 1.113 ng/mL (range: 0.823-2.214 ng/mL), which was significant higher than those of healthy controls, CHB patients, and LC patients (P<0.05). The serum visfatin concentration in HCC patients was positively correlated with AFP (r=0.595, P<0.001) and IL-6 (r=0.261, P<0.015) and was also associated with tumor size and tumor node metastasis stage. Moreover, elevated levels of serum visfatin were associated with a higher HCC risk for CHB and LC patients. Multivariate Cox regression analysis had shown that HCC patients with high levels of serum visfatin had significantly shorter overall survival times than those with low serum visfatin levels (P<0.001). Using a cutoff visfatin level of 1.403 ng/mL, the receiver operating characteristic curve analysis showed unappealing sensitivity and specificity values (45.76% and 74.79%, respectively; AUC=0.626) regarding visfatin's use as a diagnostic marker for HCC. Our results indicate that increased serum visfatin levels are associated with poor prognosis of HCC. Visfatin may be a potential therapeutic target of HCC.

Abstract:Objective. Visfatin, a newly discovered adipocytokine, is thought to play a role in the pathogenesis of metabolic-syndrome-related cancers. The aim of this study was to assess the clinical significance of serum levels and tissue expression of visfatin in relation to endometrial cancer (EC).Methods. A total of 234 EC patients were included in this study. Serum visfatin, metabolic and anthropometric parameters were measured in EC patients and controls. Serum visfatin levels were detected using ELISA. Tissue expression of visfatin was analyzed using immunohistochemistry in tissue microarrays. The correlation between clinicopathological variables and visfatin in EC tissues and the prognostic value of visfatin for overall survival was evaluated.Results. Serum levels of visfatin were significantly higher in EC patients than in controls (P<0.05). In univariate and multivariate logistic regression models, a positive association between EC and serum visfatin, BMI, waist-to-hip ratio, diabetes, and hypertension was evident (P<0.05). Visfatin expression was significantly higher in EC tissue than in normal endometrial tissue (P=0.001). Moreover, serum visfatin levels were significantly positively correlated with tissue expression of visfatin in EC patients (P<0.05). High visfatin expression in EC tissues was significantly associated with advanced FIGO stage (P=0.016) and myometrial invasion >= 1/2 (P=0.023). The overall survival rate of EC patients was significantly higher in the group with negative visfatin expression than with positive visfatin expression (P=0.035).Conclusions. Visfatin is a potential serum biomarker and prognostic factor for EC that may indicate high risk for EC and EC progression. It may also be a novel potential therapeutic target for EC. Abstract:To characterise the changes in serum visfatin levels in late normal pregnancy and pre-eclampsia.Twenty-seven women with pre-eclampsia were recruited. Twenty-eight women in the third trimester of normal pregnancy served as pregnant control and 28 healthy non-pregnant women as non-pregnant control. Serum levels of visfatin were measured with an enzyme-linked immunosorbent assay.The means of serum visfatin were 626.4+/-45.5 ng/ml (mean+/-SEM) in non-pregnant control, 695.9+/-92.5 ng/ml in pregnant control, and 308.3+/-80.0 ng/ml in pre-eclampsia, respectively, and were significantly different among the groups (p<0.001). Visfatin level was significantly lower in pre-eclampsia compared to non-pregnant control (p=0.004) and pregnant control (p<0.001). Women with severe pre-eclampsia had a significantly lower serum visfatin level than those with mild pre-eclampsia (114.6+/-80.9 versus 425.2+/-122.1 ng/ml, p=0.037).A decrease in visfatin level was demonstrated in pre-eclampsia, suggesting that visfatin and adipokine-associated metabolic abnormalities are involved in the pathogenesis of the disease.

Abstract:Visceral adipose tissue and skeletal muscle mass are associated with carcinogenesis and clinical outcomes in patients with cancer. The aim of the present study was to determine the influence of body composition parameters on postoperative survival in patients with gastric cancer. Demographic data and systemic inflammatory response data were obtained from patients with gastric cancer undergoing radical gastrectomy. The patient's skeletal muscle and visceral fat were assessed using computed tomography, and the corresponding skeletal muscle index (SMI) and visceral fat index (VFI) were calculated. Univariate and multivariate analyses were then performed. Of the 342 patients from whom information was collected, 125 of these patients eventually succumbed to the disease. A total of 271 (79.24%) of the patients were male and 71 (20.76%) were female. Regarding the entire cohort, the mean age was 64 years [interquartile range (IQR), 56‐74 years], while the mean body mass index collected was 21.53 (IQR, 19.27‐24.22). The median SMI and VFI of the patients were 47.73 (IQR, 41.67‐55.51) and 41.28 (IQR, 36.62‐45.36), respectively. It was concluded that a low SMI and VFI were associated with worse survival outcomes. However, the neutrophil‐to‐lymphocyte ratio and perioperative blood transfusion were not significantly associated with overall survival (OS). Among the indicators assessed, a low VFI was an independent risk factor associated with the worst OS time (hazard ratio 1.59; confidence interval, 1.03‐2.45; P=0.038). Finally, a prognostic nomogram was constructed which included the VFI to assist clinicians in making more informed decisions. In conclusion, after data collection and analysis, it was found that there was a significant correlation between a low VFI and a shorter OS time in patients with gastric cancer following gastrectomy, suggesting that VFI may be a promising therapeutic target for postoperative interventions to improve patient survival further.

Abstract:Obesity is a well-known factor that leads to many diseases including endometrial cancer. The adipose tissue is a heterogeneous organ of internal secretion. Visfatin is a newly discovered protein produced by fat tissues. The purpose of this work was to evaluate serum level concentrations of visfatin in patients with endometrial cancer based on clinical progression and histopathological tumor differentiation. The diagnostic capabilities of visfatin protein in high differentiation (FIGO III and IV) from a lower (FIGO I and II) clinical stage and prognostic degree of cell differentiation (G1 versus G2, G2 versus G3) on the basis of the analysis of the area under the ROC curve are as follows: 0.87, 0.81, and 0.86. Significantly higher concentrations of visfatin have been observed in patients with invasion of the blood vessels ( p = 0.02 ) and lymph node metastases ( p = 0.01 ) in reference to the depth of infiltration of the endometrium ( p = 0.004 ), as well as the size of the tumor ( p = 0.003 ). Visfatin serum concentrations did not differ due to the invasion of the lymphatic vessels only. Visfatin seems to be a good marker of endometrial cancer progress. High visfatin serum level predicts poor prognosis in endometrial cancer patients.

Vitisin B As a Novel Fatty Acid Synthase Inhibitor Induces Human Breast Cancer Cells Apoptosis.

Abstract:Breast cancer is one of the most common cancers and the second leading cause of cancer mortality in women worldwide. Novel therapies and chemo-therapeutic drugs are still in urgent need to be developed for the treatment of breast cancer. One of the most important metabolic hallmarks of breast cancer cells is enhanced lipogenesis. Increasing evidences suggest that fatty acid synthase (FAS) plays an important role in the development of human breast cancer, for the expression of FAS is significantly higher in breast cancer cells than in normal cells. In addition, FAS inhibitors, such as curcumin, ursolic acid, and resveratrol, have shown anti-cancer potential. In the present study, we discovered that vitisin B, a natural stilbene isolated from the seeds of Iris lactea Pall. var. chinensis (Fisch.), was a novel FAS inhibitor. We found that vitisin B could down-regulate FAS expression and inhibit intracellular FAS activity in MDA-MB-231 cells. Also, we reported for the first time that vitisin B exhibited apoptotic effect on human breast cancer cells. Given all of this, we proposed a hypothesis that vitisin B has an application potential in the chemoprevention and treatment of breast cancer.

Abstract:Objective:To investigate the prognostic value of survivin expression in breast cancer.Methods:Survivin expression was detected in 180 paraffin-embedded breast cancer samples with immunohistochemistry SP method and the relationships among survivin expression and clinical stage,ER,menstruation,axillary lymph node metastasis,pathological type,disease free survival and overall survival were aslo investigated.Results:The positive expression rate of survivin in breast cancer was 66.1%,and there was no relationship among survivin expression and clinical stage,ER,menstruation,axillary lymph node metastasis and pathological type;Univariate analysis(log-rank test) showed that survivin expression correlated with disease free survival and overall survival significantly,patients with positive survivin expression had poorer prognosis compared to those with negative survivin expression.And multivariate analysis with Cox regression showed that survivin is an independent biomarker of disease free survival,but not an independent biomarker of overall survival.Conclusion:Survivin can be used as an independent biomarker to predict prognosis of breast cancer,patients with positive expression of survivin have a poorer prognosis.

Abstract:Plasma visfatin concentration has been enhanced in ischemic stroke. The aim of the current investigation was to test whether determination of visfatin in plasma is associated with 6-month clinical outcomes including mortality and unfavorable outcome (modified Rankin Scale score >2) in the patients with ischemic stroke. Between July 2009 and January 2012, plasma visfatin concentrations of 186 patients and 100 healthy individuals were quantified by enzyme-linked immunosorbent assay. Plasma visfatin concentrations were higher in patients than in healthy individuals (108.5 +/- 41.1 ng/mL vs. 13.8 +/- 3.9 ng/mL, P < 0.001). A logistic regression analysis selected plasma visfatin concentration as an independent predictor for 6-month clinical outcomes (both P < 0.01). Using receiver operating characteristic curve analysis, plasma visfatin concentration was found to predict 6-month clinical outcomes with the high predictive performance. The predictive value of visfatin was in the range of National Institutes of Health Stroke Scale score (both P > 0.05). Combined use of visfatin and National Institutes of Health Stroke Scale score did not improve the predictive significance (both P > 0.05). Thus, visfatin may help in the prediction of long-term clinical outcomes in patients with ischemic stroke. (C) 2013 Elsevier Inc. All rights reserved.

Abstract:PurposeElevated plasma fibrinogen levels are associated with tumor progression and poor outcomes in different cancer patients. The objective of this study was to investigate the clinical and prognostic value of preoperative plasma fibrinogen levels in patients with operable breast cancer.MethodsTwo hundred and twenty-three patients diagnosed with breast cancer were retrospectively evaluated in this study. Plasma fibrinogen levels were examined before treatment and analyzed along with patient clinicopathological parameters, disease-free survival (DFS) and overall survival(OS). Both univariate and multivariate analyses were performed to identify the clinicopathological parameters associated with DFS and OS.ResultsElevated preoperative plasma fibrinogen levels were directly associated with age of diagnose (<= 47 vs. > 47, p<0.001), menopause (yes vs. no, p<0.001), tumor size (T1&T2 vs. T3&T4, p = 0.033), tumor stage (Ivs.IIvs.III, p = 0.034) and lymph node involvement (N = 0 vs. 1 <= N >= 3 vs. N >= 4, p<0.001), but not with histological grade, molecular type and other Immunohistochemical parameters(ER, PR, HER2 and Ki-67). In a univariate survival analysis, tumor stage, tumor size, lymph node involvement (p<0.001/p<0.001) and plasma fibrinogen (p<0.001/p<0.001) levels were associated with disease-free and overall survival, but just lymph nodes involvement (p<0.001, hazard ratio [HR] = 2.9, 95% confidence interval [CI] = 1.6-5.3/p = 0.006, HR = 3.2, 95% CI = 1.4-7.3) and plasma fibrinogen levels (p = 0.006, HR = 3.4, 95% CI = 1.4-8.3/p = 0.002, HR = 10.1, 95% CI = 2.3-44.6) were associated with disease-free and overall survival in a multivariate survival analysis, respectively.ConclusionsThis study demonstrates that elevated preoperative plasma fibrinogen levels are associated with breast cancer progression and are independently associated with a poor prognosis in patients with operable breast cancer.

Abstract:Increasing evidences suggested visfatin, a newly discovered obesity-induced adipocytokine, is involved in promotion of cancer malignancy and correlated with worse clinical prognosis. While its effects and mechanisms on progression of colorectal cancer (CRC) remain unclear. Our clinical data show that visfatin protein is over expressed, positive associated with lymph node metastasis, high-grade tumor, and poor prognosis in 87 CRC patients. The levels of plasma visfatin are significantly upregulated in Stage IV colon cancer. Visfatin can significantly promote the in vitro migration and invasion of CRC cells via induction epithelial mesenchymal transition (EMT). It can increase the expression and nuclear translocation of Snail, a key transcription factor in regulating EMT. While silencing of Snail attenuates visfatin induced EMT. Further studies reveal visfatin can inhibit the association of Snail with GSK-3β and subsequently suppress ubiquitylation of Snail. In addition, visfatin can increase the expression and nuclear translocation of β-catenin, elevate its binding with Snail promoter, and then increase the transcription of Snail. While inhibitor of PI3K/Akt, LY294002, abolishes visfatin induced up regulation of Snail, Vimentin (Vim), β-catenin, and phosphorylated GSK-3β. In summary, our data suggest that increased expression of visfatin are associated with a more aggressive phenotype of CRC patients. It can trigger the EMT of CRC cells via Akt/GSK-3β/β-catenin signals.
High Visceral Fat in Female Breast Cancer Patients Correlates with the Risk of Progression after Adjuvant Chemotherapy

Abstract:RATIONALE: Clinical and epidemiological evidence indicate that obesity is associated with the risk and progression of breast cancer. Body mass index (BMI) as a measure of adiposity does not precisely describe individual body composition and adipose tissue distribution. We aimed to investigate the association between body composition and the efficiency of adjuvant chemotherapy as well as post-treatment progress among female breast cancer patients.METHODS: Participants included 199 females with stage I-III breast cancer. Body composition, including body fat mass, visceral fat level, and skeletal muscle mass, was assessed based on the bioelectrical impedance analysis (BIA). The Kaplan-Meier survival curves, log-rank test, and Cox proportional-hazards model were used to estimate the effects of body composition as prognostic factors on survival.RESULTS: Postmenopausal women had a higher proportion of visceral fat compared to premenopausal women (64% vs. 33.87%, P < 0.001). Compared with those with normal visceral fat level, patients with high visceral fat level were older (P < 0.001), had higher body fat mass (p < 0.001), skeletal muscle mass (P = 0.013), minerals (P = 0.011), protein (P = 0.036), triglycerides (P = 0.038), cholesterol (P = 0.022), and low-density lipoprotein cholesterol (LDL-C) (P = 0.015). A more prolonged disease-free survival (DFS) was noted in patients with a normal visceral fat level as compared to patients with a high visceral fat level (hazard ratio [HR] 1.9, 95% CI 1-3.5; adjusted HR 1.77, 95% CI 0.932-3.36).CONCLUSIONS: A high visceral fat level in female patients with breast cancer is associated with a shorter DFS after adjuvant chemotherapy. Body composition alongside BIA provides a quick and noninvasive approach to identify breast cancer patients with a higher risk of cancer progression.

Methods: hTe expressions of visfatin and mutant p53 in 68 tissue specimens of gastric cancer and their adjacent normal gastric mucosa were examined by immunohistochemical staining, the relations of visfatin expression with the clinicopathologic factors and p53 expression were analyzed, and the inlfuences of visfatin and p53 expression on the survival of the patients were also analyzed. hTe visfatin expression in different human gastric cell lines and normal gastric mucosal cells was detected by immunolfuorescent assay and Western blot analysis respectively, and the alterations in proliferation and colony-forming ability as well as p53 expression were determined in gastric cancer cells atfer exposure to the visfatin speciifc inhibitor FK886. Results: hTe positive expression rate of either visfatin or p53 expression in gastric cancer tissue was signiifcantly higher than that in normal gastric mucosa (bothP<0.05), and both expressions were significantly associated with invasion depth, lymph node metastasis and TNM stage (allP<0.05), moreover, there was a significant positive correlation between visfatin and p53 expressions in gastric cancer tissue (r=0.404,P=0.001); the median survival time in patients with visfatin or p53 positive expression was signiifcantly shortened compared with those with their opposite condition (bothP<0.05), and the combination analysis of visfatin and p53 showed that the median survival time decreased in a descending order in patients with both negative expression, one of the two positive expression and both positive expression (χ2=15.83,P=0.000). Both results from immunolfuorescence and Western blot showed that the visfatin expression levels in all studied gastric cell lines were signiifcantly higher than that in normal gastric mucosal cells with varying degrees (allP<0.05), which was highest in BGC823 cells;atfer FK866 treatment, the proliferation and colony-forming ability were signiifcantly decreased, and p53 protein expression was signiifcantly up-regulated in BGC823 cells (allP<0.05). Conclusion: hTe visfatin expression is increased in gastric cancer tissue, which may synergistically interact with p53 to contribute to the progression of gastric cancer.
Association of Plasma Concentration of Visfatin with Obesity and Metabolic Parameters in Chinese Han Adults

Abstract:Objective To investigate the plasma concentration of visfatin in obesity patients and non-obesity controls and its correlation with the metabolic characteristics of obesity.Methods The height,body weight,waistline,and blood pressure of 171 healthy subjects were measured,and the BMI and WHR were calculated.The serum levels of FPG,Fins,TC,TG,LDL-c and HDL-c were also measured.The serum level of Visfatin was measured with ELISA assay,and its correlation with BMI,WHR and metabolic indexes was analyzed.Results(1)The MI,WHR,FPG,Fins and UA were significantly higher and the HDL-c level was significantly lower in obesity group than non-obesity group.(2)Plasma levels of visfatin in obesity group was a little higher than non-obesity group,but the difference had no statistical significance(2.88±0.86 vs.2.69±0.89,P=0.180).(3)In obesity group,fasting plasma visfatin was correlated positively with hip circumference and diastolic blood pressure.Multiple regression analysis showed hip circumference was an independent factor influencing plasma visfatin level.Conclusions There is no significant difference in the plasma levels of visfatin between obesity and non-obesity people.In obesity patients,hip circumference is the main factor influencing plasma visfatin level.

Abstract:Background Concurrent chemoradiotherapy is the preferred treatment for stage IVB cervical cancer; however, some patients experience a poor prognosis. The prognostic significance of body composition indicators, including visceral obesity, has been extensively investigated in patients with cancer. This study aimed to assess the impact of body composition indicators, specifically pretreatment fat content, on the survival outcomes of patients with stage IVB cervical cancer. Methods We retrospectively analyzed clinical information from patients diagnosed with stage IVB cervical cancer between 2010 and 2018. We measured visceral obesity (visceral-to-subcutaneous adipose tissue area ratio [VSR]) and skeletal muscle index (SMI) on pretreatment computed tomography (CT) images. We evaluated the impact of these body composition parameters on the prognosis of patients with cervical cancer. Results Overall, 116 patients were included, 81 of whom had complete clinical and imaging information. Based on the cut-off values from X-tile analysis, we categorized patients into high and low VSR and SMI groups. The overall survival (OS) rate of patients with a high VSR was significantly higher than that of patients with a low VSR (P = 0.022). Multivariate Cox regression analysis showed that a low VSR was an independent risk factor for the prognosis of patients with stage IVB cervical cancer. Conclusion Visceral obesity before radiotherapy and chemotherapy has a protective effect on the prognosis of patients with stage IVB cervical cancer, while low muscle index and VSR are associated with poor prognosis.

Abstract:5-Fluorouracil (5-FU) is the preferred chemotherapeutic drug used in the treatment of colon cancer; however, drug resistance affects its clinical efficacy. Visfatin, an adipokine that promotes tumour development, has the potential to increase resistance to chemotherapy. The present study aimed to verify the effects of visfatin on the sensitivity of colon cancer cells to 5-FU and to elucidate the potential mechanisms involved. Tissue microarrays (TMAs) were used to analyse visfatin differential expression in normal colon and colon cancer tissues, and the data were further validated in vitro. Cell Counting Kit-8, clone formation, caspase-3/7 activity assays, as well as other analyses were used to verify the effects of visfatin on sensitivity to 5-FU. TMA and correlation analyses were used to predict and verify the correlation between visfatin and stromal cell-derived factor-1 (SDF-1). Rescue experiments and PI3K/Akt inhibitors were used to verify the role of the visfatin/SDF-1/Akt axis in the sensitivity of colon cancer cells to 5-FU. Visfatin was found to be highly expressed in colon cancer tissues and cell lines. Moreover, visfatin knockdown increased apoptosis, reduced cell proliferation and enhanced the chemosensitivity of DLD-1 and SW48 cells to 5-FU. A positive correlation between visfatin and SDF-1 was observed, with the knockdown of visfatin enhancing cell sensitivity to 5-FU chemotherapy by targeting the SDF-1/C-X-C chemokine receptor type 4 (CXCR4) axis. Furthermore, the Akt signalling pathway downstream of SDF-1/CXCR4 proved to be critical in the decreased sensitisation of colon cancer cells to 5-FU induced by visfatin. On the whole, the present study demonstrates that visatin can potentially decrease colon cancer cell apoptosis, promote proliferation and decrease colon cancer cell sensitivity to 5-FU via the visfatin/SDF-1/Akt axis.
