DSA-guided Foam Sclerotherapy for the Treatment of Venous Malformations in Children: Clinical Observation

Abstract:Objective To evaluate the clinical efficacy of DSA-guided foam sclerotherapy in treating venous malformations in children.Methods During the period from June 2011 to Apr.2012,a total of 93child patients with DSA-proved venous malformations were treated with interventional sclerotherapy under general anesthesia by using sclerosant foam.The diagnostic criteria were as follows:the vascular lesion was present at birth or appeared after birth,the lesion's extent became larger with increasing age,the lesion exhibited a blue color when it was superficially located,and the posture test was positive.On MRI characteristic features could be found.Based on the velocity of the backflow,the venous malformations could be divided into hyper-backflow type(n = 39) and hypo-backflow type(n = 54).All patients were followed up for 6-24 months(mean 15 months).Results DSA-guided foam sclerotherapy was successfully accomplished in all 93 patients.The mean dose of sclerosant foam used in each patient was 6.5 ml.Of the 93patients,cure was achieved in 24,basic cure in 38 and effective response in 14,with a total effective rate of81.7%.The difference in the total effective rate between the two types was statistically significant(P 0.01).The adverse effects included transient visual disturbances(n = 4),swelling of tumor(n = 16) and cutaneous pigmentation(n = 6).Conclusion For the treatment of venous malformations in children,DSA-guided foam sclerotherapy is safe and effective with fewer complications.This technique is especially useful for thehypo-backflow type of venous malformations.
