Abstract:Atypical teratoid/rhabdoid tumor (AT/RT) is a very rare and highly malignant embryonal tumor in the central nervous system (CNS). Five patients (4 girls and 1 boy) with AT/RT were treated in our hospital. The clinical histories, symptoms, neuroimaging aspects, therapies, histological and immunohistochemical findings and follow-up information were reviewed. The patients ranged from 8 to 40 months with a mean age of 20.6 months. One tumor was located in the spinal cord, two in cerebellum and two in the pineal region. The imagings of the tumors resemble medulloblastomas. Pathological examinations showed that one patient had medulloblastoma differentiation, one had choroid plexus carcinoma differentiation, and one had mesenchymal components. Immunohistochemical staining showed that all of the tumors lost the nuclear expression of integrase interactor 1 (INI1), and were positive for Vimentin, S-100 protein and epithelial membrane antigen. One case with no recurrence after 24 months may have benefited from radical excision and postoperative radiotherapy. The other 4 patients died 8, 4, 1 and 1-month respectively after operation without radiotherapy. The diagnosis of AT/RT depends on full sampling, careful observation the morphological characteristics and INI1 examination, even when the tumor are presented in uncommon sites, such as the spinal cord and the pineal region.

Abstract:Atypical teratoid/rhabdoid tumor (AT/RT) of the central nervous system is a highly malignant tumor that mainly occurs in children under the age of 3 and has only been rarely described in adults. The fact that AT/RT patients have such a terrible prognosis is even more regrettable. Herein, we reported two special cases of AT/RT, both of which were under 3 years. Symptoms at presentation included increased intracranial pressure and cerebellar symptoms such as headache, altered gait, and ataxia. As for the tumor location, one was infratentorial in the posterior fossa, and the other was the right lateral ventricle. Preoperative magnetic resonance imaging scans showed calcification and heterogeneous contrast enhancement in the lesions. The mass was excised surgically for the progression of symptoms. Postoperative pathologies of the tumors, combined with immunohistochemistry, revealed AT/RT. AT/RTs are often misdiagnosed as other types of brain tumors due to the lack of specific radiological features and other key characteristics. To improve awareness of AT/RT on the differential diagnosis of intracranial lesions among clinicians, we present this report and briefly summarize previous cases.

Abstract:OBJECTIVE:To study the clinicopathologic features and differential diagnosis of atypical teratoid/rhabdoid tumor (AT/RT) occurring in the central nervous system.METHODS:Two cases of AT/RT were studied by hematoxylin-eosin, reticulin and immunohistochemical staining. The clinical and pathologic features were analyzed and the literatures reviewed.RESULTS:Histologically, AT/RT was characterized by the presence of rhabdoid cells associated with various degrees of primitive neuroectodermal, epithelial or mesenchymal differentiation. Abundant reticulin fibers and a complex immunophenotype were observed. The tumor cells were positive for vimentin, CD99, epithelial membrane antigen, cytokeratin, glial fibrillary acidic protein, S-100 protein, neurofilament, desmin and smooth muscle actin. They were negative for synaptophysin, MyoD1, placental alkaline phosphatase and HMB45.CONCLUSIONS:AT/RT is a highly malignant tumor occurring in the central nervous system. It manifests mainly in children and occasionally in adults. The tumor is characterized by a heterogeneous histologic and immunohistochemical phenotype. It needs to be distinguished from a number of central nervous system tumors, including medulloblastoma, primitive neuroectodermal tumor, germ cell neoplasm and rhabdoid meningioma.

Abstract:Primary AT/RT is a rare highly malignant tumor of the CNS, usually occurring in children younger than 5 years of age. The objective of this study was to characterize the CT and MR imaging findings in a series of 5 adult patients with pathologically proved AT/RT. All 5 AT/RTs were supratentorial. In 2 patients who underwent nonenhanced CT, the tumors appeared isoattenuated, and 1 of the 2 tumors contained calcifications. Solid portions of the tumors on MR imaging were isointense on T1-weighted, T2-weighted, and FLAIR images, and 1 case showed restricted diffusion on DWI. The tumors also demonstrated a bandlike rim of strong enhancement surrounding a central cystic area on contrast-enhanced T1-weighted imaging. One tumor was associated with destruction of the calvaria. Although AT/RTs can have nonspecific findings, the tumors in our series were large and isointense on T1-weighted, T2-weighted, and FLAIR images with central necrosis and prominent rim enhancement.

Abstract:OBJECTIVE:To investigate the clinicopathological features of atypical teratoid/rhabdoid tumor(AT/RT) occurring in the central nervous system.METHODS:Nine cases of AT/RT of the central nervous system were studied by hematoxylin-eosin staining,cytochemical and the immunohistochemical staining with clinicopathologic features,imaging data and combining of the literature.RESULTS:In 9 patients,7 cases were older then 15(77.8%) and 2 cases were younger than 15(22.2%),average age was 6 years old;The clinical symptoms were headache,vomiting or the offside body numbness,etc.Eight cases were supratentorial(88.9%);Imaging features of the tumor displied a solidcystic mass with obviously edema around.One of the patients had obvious annular wall around formation;Histologically,AT/RT was characterized by the presence of rhabdoid cells associated with variable components of epithelial,primitive neuroectoderma,myxoid and glioma components and so on.Immunohistochemical study demonstrated a strong positivity for Vim,GFAP,EMA and CKpan,but negative for INI1,CD117 and PLAP,Ki-67 proliferation index was more than 40%.CONCLUSIONS: AT/RT is a highly malignant tumor but rarely occurs in the CNS.It is more likely seen in children;The clinical manifestations of the lesion intracranial related symptoms,the tumor cells have differentiation histopathology and immunohistochemical phenotype;It is important to use immunohistochemical studies in correct dignosis,surgery is the main treatment method with chemotherapy and peripheral stem cell therapy,which can prolong survival time,but the prognosis is very poor.

Abstract:AT/RT occurs in younger patients, has more aggressive clinical behavior and is associated with much worse outcomes than medulloblastoma. On MRI, AT/RT (G‐I) can be differentiated from medulloblastoma (A‐F) by a lower ADC value, more peritumoral invasion, and the absence of the "tumor central vein sign". Background Atypical teratoid rhabdoid tumor (AT/RT) occurs at a younger age and is associated with a worse prognosis than medulloblastoma; however, these two tumor entities are mostly indistinguishable on neuroimaging. The aim of our study was to differentiate AT/RT and medulloblastoma based on their clinical and MRI features to enhance treatment planning and outcome prediction. Methods From 2005–2021, we retrospectively enrolled 16 patients with histopathologically diagnosed AT/RT and 57 patients with medulloblastoma at our institute. Abstract:Objective To analyze the MRI features of intracranial atypical teratoid/rhabdold tumors in pediatrics.Methods The MRI findings of 12 children with CNS atypical teratoid/rhabdold tumors proved by operation and pathology were retrospectively analyzed.Results There were 8 males and 4 females(median age 20 months,age range 6 months to 9years).Tumors location at infratentorial in 6 and supratentorial in 4,infratentorial and supratentorial in 2.The biggest lesion was 8.3*6.8cm.The lesions located at supratentorial were usually larger than that at infratentorial.The tumors showed heterogeneous isointensity on T1 flair in 12 lesions,isointensity on T2 FSE in 10,hypreintensity on T2 FSE in 2 and hypreintensityon diffusion weighted image in 11 lesions.Hemorrhage and necrosis in tumors were noted in 10 cases,peri-cystic in 9 cases.The tumors showed significant enhancement in 9,mild enhancement in 1 and no enhancement in 2.Disseminated tumors in the leptomeninges was showed in 3 patients.Mild peritumoural odema was noted in 6 cases.Conclusion Atypical teratoid/rhabdoid tumor is a rare and highly malignant CNS tumor,predominantly occur in young children and located at infratentorial.The tumours are usually large size,isoinernsity on T1 and T2 weighted and hyperintensity on DWI images,various enhancement apperence,relatively leptomeninges dissemation.

Abstract:Purpose: The purpose of this article is to explore the clinical and neuroradiologic properties of atypical teratoid/rhabdoid tumors. Methods: Data from 6 pediatric patients with atypical teratoid/rhabdoid tumors, which mainly contained the features of magnetic resonance imaging (MRI) and positron emission tomography (PET)/computed tomography (CT), was retrospectively analyzed. Follow-up was conducted in all patients through clinic services and/or telephone consultation. Results: The patients included 4 males and 2 females, aged from 3.2 to 83.1 months at the initial diagnosis. All patients had MRI scans. Two patients underwent 18F-fluorodeoxyglucose PET/CT scintigraphy preoperatively and 4 postoperatively. All primary lesions were located in the cranial cavity and the average diameter of lesions was 37.2 mm. Cerebrospinal fluid spread on enhanced T1-weighted images were found in 2 patients. Multiple metastases were found on MRI and PET/CT scans, which were located at cranial cavity, spinal cord, lung and lymph node. The primary and metastatic lesions showed evident uptake of 18F-fluorodeoxyglucose. Two patients underwent total tumor removal, and 4 patients underwent subtotal removal. None of the patients received shunt surgery. Follow-up was performed in all 6 patients. One patient survived event-free 38.4 months after resection. The mean overall survival of the remaining 5 patients was 5.1 months. Conclusion: We identified specific PET/CT and MRI features that can facilitate the recognition of atypical teratoid/rhabdoid tumors prior to biopsy.

Abstract:Atypical teratoid/rhabdoid tumours (AT/RTs) are rare, highly malignant tumours of the central nervous system (CNS) with poor prognosis that usually affect young children. The aim of this study was to assess the clinicopathological features and prognostic factors of AT/RTs. Here, we describe the clinicopathological and immunohistochemical characteristics, along with the treatments and outcomes, of 22 patients with AT/RTs treated in our hospital from 2010 to 2015. Morphologically, cytoplasmic vacuoles, the most common characteristic in our cases, were observed in 68% of the cases. Similarly, vesicular nuclei were detected in 68% of the cases. However, rhabdoid cells were found in only 59.1% of the cases and were not observed in 40.9% of the cases. Immunohistochemical analysis revealed loss of nuclear INI1 expression in all 22 cases. Age, surgical resection and adjuvant therapy, but not tumour location, were associated with AT/RTs patient prognosis. Our results showed that cells with cytoplasmic vacuoles or with vesicular nuclei are more common than rhabdoid cells in patients with AT/RTs and that a lack of INI1 protein expression is the most useful marker for the differential diagnosis of AT/RTs. Young age is a negative prognostic factor, whereas gross total surgical resection and adjuvant therapy are positive prognostic factors for AT/RT patients.

Abstract:Atypical teratoid/rhabdoid tumor of the central nervous system has aggressive growth with high potential CSF dissemination and frequently affects very young children.The tumor contains rhabdoid cells and primitive neuroectodermal,malignant mesenchymal and epithelial elements.Immunohistochemistry is helpful in differentiating atypical teratoid/rhabdoid tumor from primitive neuroectodermal tumor/medulloblastoma.Atypical teratoid/rhabdoid tumor is cytogenetically characterized by monosomy 22 or deletion of chromosome band 22q11.2.Intensified therapy combined with surgical resection has been suggested for its treatment.Due to the secondary cystic/necrotic changes and hemorrhage,the imaging features of the tumor are variable.

Abstract:Objective To investigate the treatment experience of atypical teratoid/rhabdoid tumors (AT/RT) of the central nervous system in infants in order to improve the level of awareness for the disease.Methods The treatment course of 12 infants with AT/RT diagnosed by pathology and immunohistochemistry at the Department of Neurosurgery,Children's Hospital of Shanghai Fudan University from October 2010 to March 2015 were analyzed retrospectively,and the correlation analysis and the literature review were performed.Results In 12 children,AT/RT of 10 were resected completely,and 2 were subtotally resected.Postoperative complications:2 patients had subdural effusion,2 had secondary epilepsy,and 1 had mutism.They were all improved after symptomatic treatment.All the children were not treated with adjuvant therapy after procedure,such as radiotherapy and chemotherapy.Two children were lost to follow up after procedure.Nine children were followed up and they all had recurrence,and 1 is still in follow-up.The shortest recurrence time after procedure was 2 months and the longest time was 9 months after procedure.The recurrence time was not more than 1 years.Three of them had recurrence in situ,6 had cerebrospinal fluid disseminations and intracranial multiple metastases.All the recurrent children were not treated with surgery again.Conclusions The clinical incidence of AT/RT is low and the number of cases is less,however,its malignant degree is high and had strong invasion.Although they treated with comprehensive treatment,such as total resection and postoperative chemoradiotherapy,often the treatment effect of infants is poor.Although there are certain characteristics on imaging,it is still hard to differentiate the medulloblastomas with the PNET and other tumors.The final diagnosis should be confirmed by the pathological diagnosis.

Abstract:Atypical teratoid/rhabdoid tumors (AT/RTs) are lethal central nervous system tumors, which are primarily diagnosed in infants. Current treatments for AT/RTs include surgery, radiotherapy, and chemotherapy; these treatments have poor prognoses and challenging side effects. The pivotal genetic event in AT/RT pathogenesis comprises the inactivation of SMARCB1 or SMARCA4. Recent epigenetic studies have demonstrated mutual and subtype-specific epigenetic derangements that drive tumorigenesis; the exploitation of these potential targets might improve the dismal treatment outcomes of AT/RTs. This review aims to summarize the literature concerning targeted molecular therapies for pediatric AT/RTs.

Abstract:Atypical teratoid/rhabdoid tumors (AT/RTs) are rare, highly malignant central nervous system tumors that predominantly occur in young children. A 22-year-old woman presented with a 4-year history of relapsing tinnitus and gradual hearing loss. Neuroimaging revealed an enhanced intrinsic left internal auditory canal mass. The patient underwent radiotherapy treatment. Three years later, the tumor size continued to increase, as observed by imaging, and ultimately evolved into the left cerebellopontine angle. As a consequence, a total tumor resection was performed, and a pathological diagnosis of AT/RT was made. Aggressive radiotherapy and chemotherapy treatment continued; however, the tumor recurred within 11 months after the total tumor resection. The patient died within 4 months of the second operation. Histopathologically, the tumor contained characteristic rhabdoid cells with areas that resembled a classical primitive neuroectodermal tumor. Immunostaining showed loss of INI1 protein expression in tumor cells, and fluorescence in situ hybridization showed a hemizygous deletion of the hSNF5/INI1 gene region on 22q11.2. This is the first report of an AT/RT that arised from the acoustic nerve in a young adult. Despite manifold diagnostic and therapeutic advances, the prognosis of patients with AT/RT remains poor.

Abstract:非典型畸胎瘤样/横纹肌样肿瘤(atypical teratoid/rhabdoid tumor,AT/RT)是一种小儿罕见的中枢神经系统恶性肿瘤,在 WHO(2007)神经系统肿瘤分类中,AT/RT被定义为中枢神经系统恶性胚胎性肿瘤(WHO Ⅳ级),占1～17岁儿童中枢神经系统原发肿瘤的1.3%,占0～2岁婴幼儿中枢神经系统肿瘤的6.7%[1].国内关于此瘤的报道大多涉及病理学[2],有关影像学方面报道非常少[3],且均为个案报道.笔者搜集3例AT/RT,并对其影像表现进行分析。

Abstract:Objective To investigate the clinical and pathological features of pediatric CNS atypical teratoid/rhabdoid tumor(AT/RT)and evaluate the role of INI1 antibody on the differential diagnosis and prognosis of AT/RT.Methods The clinical data and pathological features of 12-year-old boy with AT/RT were concluded.The protein expression of INI1 was detected by immunohistochemistry;follow-up was performed and relative documents of that disease were compared;the above data were retrospectively analyzed.Results Mioroanatomy showed typical teratoid cells and primitive mesenchymal cells in the tumor.Both of these 2 tumor components displayed positive reactions of vimentin and CD99.The SMA and EMA positive expressions were noted in the teratoid cells.Immunohistochemical staining demonstrated that INI1 antibody was nuclear negative in tumor cells,but positive within endothelial cells of the blood.Follow-up information showed the patient was dead 2 months after the operation.Conclusions CNS atypical AT/RT is highly aggressive tumor.The appearance of rhabdoid cells is its important morphologic features and its main differential diagnosis evidence.Mesenchymal components and the various results of immunohistochemical staining indicate that AT/RT tumor cells,having the character of the embryonal tumors,have various differentiation potential.The expression of INI1 antibody suggests that a mutation or deletion in the INI1 gene occurs in the tumor,which has important value in diagnosing and differentiating the tumor. Abstract:Objective:To investigate the clinical features and prognostic factors of central nervous system (CNS) atypical teratoid/rhabdoid tumor (AT/RT).Methods:Thirty-one AT/RT patients diagnosed by postoperative pathological examination and surgically treated at Department of Neurosurgery, West China Hospital, Sichuan University from January 2007 to August 2019 were retrospectively analyzed. Kaplan-Meier method was used to analyze the postoperative survival of the children.Results:Among the 31 children, 17 were males and 14 were females. The median age of onset was 22 months (2 months to 15 years). There were 21 cases in the ≤3-year-old group and 10 cases in the >3-year-old group. Of the 31 patients, 17 were in the supratentorial region, 13 in the infratentorial region, and 1 across the supratentorial and infratentorial regions. The maximum diameter of the tumor was 2.5-9.1 (5.2±1.7) cm. Twenty-five patients had total tumor resection and 6 had subtotal resection. Compared with the > 3-year-old group, the proportion of men in the≤3-year-old group was slightly higher (3/10 vs. 14/21), and the proportion of tumors located in the supratentorial region was slightly lower (8/10 vs. 9/21), and the rate of peritumoral edema was slightly lower (4/9 vs. 2/20), while the differences were not statistically significant (all P>0.05). Followed up for 1 to 60 months, the median survival time was 5 (1-60) months. The 1-year survival rate was 16.1% (5/31) and the 2-year survival rate was 3.2% (1/31). Kaplan-Meier analysis showed that the median survival time in the ≤3-year-old group was 4.0 (1.0, 6.5) months, and that in the >3-year-old group was 9.5 (6.0, 21.0) months. The difference was statistically significant ( P=0.002). The median survival time of 9 children with adjuvant radiotherapy and/or chemotherapy was 7.0 (4, 18.5) months, and the median survival time of 22 children who underwent surgery only was 4.0 months (1.0, 7.0), the difference was statistically significant ( P=0.037). Conclusion:The overall prognosis of AT/RT seems poor. In addition, the proportion of young children ≤3 years old is relatively higher and their survival time seems shorter.Adjuvant radiotherapy and/or chemotherapy may help prolong the survival of children.

Abstract:Atypical teratoid/rhabdoid tumours (AT/RTs) are rare central nervous system neoplasms that frequently occur in infants and children and have a very poor prognosis. In recent years, molecular analysis of AT/RTs has shown that biallelic inactivation of SMARCB1 (INI1, SNF5, BAF47) or SMARCA4 (BRG1) frequently occurs. Here, we present a case of basal ganglia AT/RT with SMARCB1 gene deficiency and CDK6 gene amplification in a 5-year-old child. A 5-year-old boy was hospitalized due to a 1-week history of frontal and parietal headache. Magnetic resonance imaging (MRI) demonstrated a 3 cm × 2 cm × 1.5 cm heterogeneous enhanced mass located at the right basal ganglia that partially protruded into the right lateral ventricle. The lesion was successfully resected under electrophysiological monitoring and neuronavigation. The postoperative pathological examination implied an AT/RT diagnosis, with loss of SMARCB1 protein, SMARCB1 gene deficiency and CDK6 gene amplification. Unfortunately, the patient died due to respiratory and circulatory failure at 5 weeks after the operation. To date, standard regimens have not yet been established due to the lack of large-scale prospective studies for AT/RT. The p16-RB signalling pathway should be considered as a potential target for AT/RT treatment modalities. Apart from traditional regimens, targeted therapies, especially CDK4/6 inhibitors, are likely a promising therapeutic option for AT/RT treatment.

Abstract:Atypical teratoid/rhabdoid tumors (AT/RTs) are rare malignant neoplasms of the CNS that preferentially affect young children. We herein report an adult case of AT/RT surviving for more than 5 years with the residual tumor. The patient, a 24-year-old man at onset, presented with a contrast-enhancing mass lesion in the left occipital lobe, and underwent partial tumor resection. Histologically, the tumor was predominantly composed of long spindle cells exhibiting nuclear palisading and perivascular pseudorosettes, which appeared to mimic mesenchymal, ependymal and Schwann cell tumors. A small number of isolated rhabdoid cells did not initially attract attention, and a tentative pathological diagnosis of a malignant mesenchymal tumor was made. In a later examination focusing on the small areas of rhabdoid cells, the extensive loss of the nuclear expression of INI1 was detected in all areas. Diffuse staining with vimentin and focal immunoreactivity for epithelial membrane antigen and alpha smooth muscle actin were observed not only in AT/RT foci, but also in spindle cell areas. Thus, polyphenotypic immunoreactivity was evident. Fluorescence in situ hybridization revealed a homozygous deletion of chromosome 22q covering the INI1 locus. Histopathological differences between infant and adult AT/RTs currently remain unclear. In the case of a malignant adult brain tumor showing a hardly classifiable morphology and immunophenotypic diversity, an analysis of the INI1 status may contribute to an accurate diagnosis.

Abstract:Abstract BACKGROUND Atypical teratoid/rhabdoid tumor (AT/RT) is a rare and highly aggressive tumor that occurs most commonly in infants and young children. The prognosis for patients with AT/RT is generally poor, with a five-year survival rate of less than 30%. METHODS We retrospectively analyzed the clinical data of patients with AT/RT in the past 10 years at the Guangdong Sanjiu Brain Hospital RESULTS A total of 18 cases (male/female = 9:9; median age at diagnosis, 3.5 years (range 1-58 years old)) were diagnosed with AT/RT between 2017 and 2023. AT/RT had higher prevalence in children < 36 months (55.6%), and at infratentorial (66.7%). The median follow-up time was 15.5 months, with a range of 2-62 months. Survival analyses demonstrated that patients > 3 years of age (n = 9 (50%)) had median survival time 28months and patients ≤ 3 years of age (n = 9 (50%)) had median- survival time 15 months (p =0.034) The average survival time for disseminated tumor was 15 months, and for patients with localizedtumors was 44 months. (P=0.012) CONCLUSION Older age, localized tumor improves the survival of patients with AT/RT.

Abstract:Malignant rhabdoid tumor is a rare tumor that arises predominantly in infants and young children.According to different anatomic origins,this tumor mainly can be divided into malignant rhabdoid tumor of the kidney (MRTK),atypical teratoid rhabdoid tumor (ATRT) and extrarenal extracranial rhabdoid tumor (EERT).The mutation or deletion of the tumor suppressor gene SMARCB1 on chromosome 21q plays an important role in the development of tumor.Morphologically,tumor cells are like typical rhabdoid cells,characterized by classic eosinophilic cytoplasmic inclusions.The diagnosis relies on combined histological and immunohistochemical analysis.Rhabdoid tumor is a highly malignant tumor with poor prognosis.Multiple therapeutic strategies include surgery,radiotherapy and systemic chemotherapy,but the survival rate is still very gloomy.This paper reviewed the epidemiology,clinical pathology,immunohistochemistry,diagnosis,historical treatment and discovery of targeted novel therapy.
