Abstract:Biopsy-proven renal complications of Castleman's disease (CD) are rare and current knowledge is largely based on sporadic case reports. We reported two more cases, both of which were multicentric CD with hyaline-vascular pathological pattern and presented with chronic renal failure. Case 1 was multicentric CD with renal mesangial proliferative glomerulonephritis complications, and case 2 was multicentric CD with membranoproliferative glomerulonephritis-like complications. Although both were eventually administered corticosteroids combined with cytotoxic drugs, both behaved in an aggressive and relapsing manner. We then made an analysis of 75 cases of biopsy-proven renal complications of CD (including our two cases) which were reported in 51 English literatures from January 1954 to March 2011. We found that the clinical and histological findings of renal complications of CD were heterogeneous. Death was observed in 17% patients after a median follow-up time of 22 months (0-204 months) since histological diagnosis of renal complications. The estimated 5-year cumulative survival rate was 75%. Better understanding and therapeutic interventions are required in further investigations.

Abstract:Objective To investigate the elinicopathoiogical features of Castleman disease with kidney injury. Methods Clinicopathological data of 10 Castleman disease patients with kidney injury from Peking University First Hospital and China-Japan Friendship Hospital were analyzed retrospectively. All the cases received biopsies of lymph node and kidney. Their renal tissues were examined by light microscopy, immunofluorescence and electron microscopy. Results Ten patients were all male with mean age (493:14) years. They presented edema and proteinuria, with mean urinary protein at (2.79±3.56) g/24 h, including one nephrotie syndrome (NS). Hematuria occurred in 8 cases, acute renal insufficiency in 6 cases, hypertension in 4 cases. Most of the patients had fever, fatigue, anorexia, weight loss, increased ESR and CRP, hypergammaglobulinaemia and decreased complements. Other abnormalities included anemia, thrombocytopenia, pleural effusion, hepatomegaly, splenomegaly, hypothyroidism, etc. Two cases demonstrated POEMS syndrome, one presented Sjogren syndrome. The enlargement of multiple cervical, axillary and inguinal lymph nodes were identified in all the patients. The pathological patterns of lymph node were plasma cell type in 4 cases, hyaline-vascular type in 3 cases, and mixed type in 3 cases. Pathological examination of renal biopsy showed thrombotic microangiopathy in 5 cases, crescentic glomerulonephritis in 2 cases, renal amyloidosis, minimal change disease and chronic tubular interstitial nephropathy in 1 case respectively. After immunosupressive reagents or COP therapy, lymph nodes became smaller, systemic symptoms were alleviated, proteinuira was decreased or disappeared, and renal function was recovered in most of patients. Conclusions Castleman disease with kidney injury manifests various symptoms with high prevalence of renal insufficiency and multiple systemic damage. Renal lesions present many patterns of pathological change with a higher frequency of thrombotic microangiopathy. It is necessary to examine the lymph nodes by ultrasound, radiology or biopsy for the patients of renal diseases with multiple systemic symptoms.

Abstract:OBJECTIVE:To improve the recognition of thoracic involvement of Castleman' s disease.METHODS:Ten patients with thoracic involvement of Castleman' s disease were retrospectively studied by review of the clinical manifestations, radiology, pathology, differential diagnosis, therapy and prognosis.RESULTS:The 10 patients were all females. Five of the patients, aged from 29 to 49 years, presented with multicentric lesions, manifested by interstitial pneumonia and mediastinal lymphadenopathy on radiology, as well as extrathoracic involvement including peripheral lymphadenopathy, and multiple organ impairment. The pathology was the plasma cell type. Treatment with corticosteroids and chemotherapy achieved partial remission in 4 cases, but 1 died of cardiac and respiratory failure. Localized lesions were found in the other 5 patients, aged from 13 to 49 years. No specific manifestations were revealed in these patients. The major radiological sign was right mediastinal masses, 6-9 cm in diameter, uniformly enhanced by contrast radiology. The pathology revealed vascular hyaline degeneration. Early surgical resection achieved good prognosis. The 5 patients were all alive.CONCLUSIONS:Diagnosis of thoracic involvement of Castleman' s disease is not easy. Early, repeated and multiple biopsy of lymph nodes is recommended, especially when the disease is multicentric or when the lymph nodes are massive. Synchronous enhancement with the vessels on contrast radiology is suggestive of the diagnosis.
Antiglomerular basement membrane disease associated with Castleman disease.

Abstract:We described a 33-year-old man with gross hematuria and acute renal failure, who had suffered from fever for 6months, multiple cervical lymph node swelling, splenomegaly, left-sided pleural effusion. He also suffered from anemia, thrombocytopenia, hypergammaglobulinemia, and his serum interleukin-6 levels were markedly elevated. Antiglomerular basement membrane antibodies were positive in the patient’s serum. Lymph node biopsy results were compatible with Castleman disease of “plasma cell” variant. Renal biopsy revealed cellular crescents in most of the glomeruli. Immunofluorescence studies showed strong deposition of IgG in a linear pattern along the glomerular basement membrane. Pathologic features were compatible with crescentic glomerulonephritis because of antiglomerular basement membrane disease. With intensive plasmapheresis and monthly chemotherapy (cyclophosphamide, oncovin, prednisone regimen), the patient experienced clinical and biochemical remission. Although autoimmune phenomenon had been described frequently in Castleman disease, to the best of our knowledge, this was the first report that the patient with rapid progressive glomerulonephritis mediated by antiglomerular basement membrane antibodies, which might be associated with Castleman disease.

Abstract:Objective To discuss the clinicopathologic features,diagnosis,differential diagnosis and etiology of Castleman disease.Methods A retrospective analysis of the clinical features,pathological morphology,immunohistochemistry and Epstein-Barr virus was carried out in 31 cases of Castleman disease.Results There were 22 cases of unicenrtic Castleman disease and 9 cases of multicentric Castleman disease.Pathologically,there were 25 cases of hayline vascular types,3 cases cyic of plasmat type and 3 cases of mixed type in all Castleman disease.In this series,there were 11 cases which occurred in retroperitoneum,which was the main site of Castleman disease (35.5％).Immunohistochemistry can assist diagnosis and differential diagnosis.The results of EBER-ISH showed negative signal in 15 cases of Castleman disease.There were 27 cases with complete follow-up data,including 18 cases of unicenrtic type which were healed by surgical resection without recurrence;in all cases of multicentric Castleman disease;1 case recurred,other multicentric cases achieved sustained remission;1 case died of heart disease.Conclusions Castleman disease is a rare lymphoproliferative lesion,and identified diagnosis depends on pathological examination.The accuracy of clinical subtype and histopathogenic type is important for treatment and outcome in the patients.Complete surgical resection is curative for unicenrtic Castleman disease,leading to excellent long-term outcomes.Combination chemotherapy and immunological therapy are usually used in multicentric Castleman disease,and the prognosis of these patients is poor.The use of rituximab is effective on multicentric Castleman disease.

Abstract:The clinical, pathological treatment, and prognostic data of 55 patients with Castleman disease (CD) were collected retrospectively from a single medical center. Thirty-four cases were classified as uni-centric; the remaining 21 cases were diagnosed as multi-centric CD. Regarding pathological classification, 38 cases were hyaline vascular type, account for 69.1% of all patients. Nine cases were diagnosed as plasmacytic type and eight cases as mixed cellularity type. Several prominent clinical complications were noted in this group of patients with CD: the skin, internal organs, and hematopoietic system were involved individually or concurrently. Clinical complications were distinct between different clinical and pathological subtypes of CD. The unique clinical and laboratory features of patients with paraneoplastic pemphigus suggests this diagnosis should be characterized as an independent disease entity. The presence of clinical complications is an independent prognostic factor in all patients with CD. More effective initial therapy should be considered in patients with CD with complications to improve the overall survival.
Renal Involvement in a Large Cohort of Chinese Patients with Castleman Disease.

Abstract:BACKGROUND:The association of kidney disease with Castleman disease (CD) is uncommon. To date, most studies have been based on single-case reports. Here, we describe renal involvement in CD in a large Chinese cohort.METHODS:Seventy-six CD patients were identified in one clinical center. Clinical and pathological characteristics of patients with renal involvement were described, which were also compared with cases identified through a systematic literature review.RESULTS:Nineteen patients (25%) exhibited renal involvement. Patients with multicentric clinical type (59 versus 0%) or plasma cell (PC)/mixed cellularity histological variant (61.5 versus 6%) were more likely to have renal involvement (P < 0.001). Proteinuria (with 7/19 reaching nephrotic range) and acute renal failure (12/19, 63%) were the main clinical presentations. Kidney biopsy revealed various glomerular diseases (10/11) and interstitial nephritis (1/11), while with 'thrombotic microangiopathy-like' lesions were the most common pathological characteristics (6/11, 55%). This contrasted significantly with the literature in which amyloidosis was the most reported. Renal outcomes responded well to chemotherapy. Nine (9/12, 75%) patients with acute renal failure recovered completely, one recovered partially. Overall, only three (3/19, 16%) patients progressed to end-stage kidney disease. Renal involvement did not influence survival rate (log-rank test, P = 0.73) in the follow-up.CONCLUSIONS:CD with multicentric type and PC or mixed cellularity variant are often associated with renal complications. Thrombotic microangiopathy-like lesions are the most common pathological characteristics. Chemotherapy can reverse kidney damage in most cases.

Abstract:Castleman's disease is a disorder of the lymph nodes that encompasses heterogeneous clinical conditions and can be classified into two main types - hyaline vascular and plasma cell. The affected age group ranges widely from two to 80 years old at the time of diagnosis, with a variable clinical presentation. The etiology of Castleman's disease is not yet well-understood; however, a number of factors have been linked to its pathogenesis including certain cytokines, viral infections, autoimmunity, immunodeficiency, chronic inflammation, and Kaposi sarcoma. In this study, we present a case of a 52-year-old Saudi female with a history of pulmonary embolism and deep venous thrombosis, who was then found to have mild splenomegaly, mediastinal, bilateral hilar, supraclavicular, paraaortic, and right axillary lymphadenopathy on CT scan, to be later diagnosed as a multicentric Castleman's disease (MCD). Moreover, the clinical picture, pathogenesis, clinical and histological variants, as well as the treatment options of MCD are discussed.

Abstract:Objective Castleman's disease(CD)is a rare lymphoproliferative disorder characterized by various clinical mani-festations and pathological features .The study was to improve the recognition of Castleman's disease by summarizing its clinicopatholog-ic features. Methods A retrospective analysis was conducted on the clinical data , histopathological changes , treatment and progno-sis of 20 CD patients admitted in Jinling Hospital , Nanjing University School of Medicine from February 2010 to April 2017. Results Among the 20 CD patients, there were 11 males and 9 females, the median age was 41.5 (14~73) years, 9 cases were unicentric Cas-tleman's disease ( UCD) and 11 cases multicentric Castleman's disease ( MCD) .UCD tended to be found by physical examination or local palpable masses , characterized by enlarged lymph nodes with hyaline-vascular type in pathological form , treated mainly by resec-tion with favorable prognosis .MCD frequently appeared with constitutional symptoms and multiple system involvement , such as fever , edema, hyperglobulinemia and increasing CRP level , mostly presenting plasma cell type in pathological form with poor prognosis being mainly treated by comprehensive therapy . Conclusion The diagnosis of CD mainly depends on pathological examination .UCD ap-pears mild clinical features with favorable prognosis after surgery , while MCD is characterized by complex clinicial manifestation and should be treated by comprehensive therapy which results in poor prognosis .

Abstract:Castleman disease (CD) is a heterogeneous hematological condition characterized by distinctive histopathological features. The etiology remains unclear, and clinical symptoms are generally nonspecific. CD can occur in any location containing lymphatic tissue, with the mediastinum being the most common site, while adrenal involvement is rare. Adrenal CD is typically incidentally discovered during physical examination, commonly affecting one side. Imaging studies often make it challenging to differentiate from common adrenal tumors, necessitating pathological confirmation. Surgical intervention is the preferred treatment, and the prognosis is generally favorable. This paper presents a rare case of a left adrenal mass, which was diagnosed as Castleman disease following surgical resection.

Abstract:OBJECTIVE:To study the pathogenesis and the differential diagnosis of Castleman's disease.METHODS:Histopathology, immunohistochemical staining and clinical courses of 26 cases of Castleman's disease (CD) were studied with follow-up study of 16 cases.RESULTS:The present study included 6 cases of multicentric type, 20 cases of localized type in the clinical aspects and 19 cases with hyaline vascular type, 4 cases with plasma cell type, 3 cases with mixed type in the histologic aspect. The Multicentric type presented systemic lymphadenopathy, anemia, hyperglobulinemia, hepatosplenomegaly, skin changes, and lung disorder and kidney disfunction, of which 1 case died of respiratory and renal insufficiency. 13 of the 20 localized cases were of the hyaline vascular type, and with good prognosis. 7 of the 20 cases showed paraneoplastic pemphigus associated with hyperglobulinemia (4/7) and lung disease (5/7). The pathologic features composed of proliferation of the mantle zone B cell, follicular dendritic cell, plasma cell and small vessels. In immunohistochemical staining, kappa and lambda light chains were detected in each CD case.CONCLUSIONS:Many diseases are similar to CD clinicopathologically. It is important to make differential diagnosis through pathological study. Castleman's disease is a lymphoproliferative disorder. The pathogenesis of this multicentric disorder may be associated with autoimmune disease.

Abstract:Objective:Castleman's disease is a rare lymphoproliferative disorder that is often classified as hyaUniversity from 1977 Objective:To 2002.Results:Twelve patients with single mass had a good prognosis after operation.Only four of them had nonproductive cough and abdominal pain before operation.Six young patients were described with severe mucocutaneous ulceration and erosion.Tumors were found in the retroperiObjective:Toneum and mediastinum,and pathowas mixed type.Five of them complained of progressive dyspnea with fatal outcome.The clinical features,radiological findings and the pulmonary function tests were consistent with those of bronchiolitis obliterans(BO).Four patients with multicentric CD presented manifestation of multisystem involvement.Three of them died although they had received corticosteroids,chemotherapy and radiation therapy.Conclusions: SympObjective:Toms of CD are diverse.Patients with mucocutaneous erosion frequently had lung involvement.It is different from traditional categories and should be considered as an independent type.

Abstract:Objective Castleman′s disease ( CD) is a rare lymphoproliferative disorder, which has a very high misdiagnosis rate according to its lack of clinical and imaging specificity.The study was to improve the recognition, diagnosis and management of this disease by analyzing the clinicopathological characteristics and concluding diagnosis and treatment as well as literature review. Methods Retrospective analysis was conducted on clinical data of 31 CD patients (13 males and 18 females) who were hospitalized in the Affiliated Drum Tower Hosipital to Medical college of Nanjing University from January 2006 to Feburary 2014.Their mean age was (47.4 ±13.3) years.There were 24 cases with unicentric Castleman′s disease (UCD) and 7 cases with multicentric Castleman′s disease ( MCD) .UCD tended to be present in the form of an enlarged and painless mass which generally remained asymptomatic by accidental touch or regular physical examination.MCD was usually associated with constitutional symptoms.All the 24 patients with UCD underwent complete surgical resection.7 patients with MCD received different treatment methods, such as surgical resection, ste-riod and rituximab in combination with chemotherapy ( CHOP) . Results Postoperative pathology confirmed 23 cases were hyaline vascular type while 8 were plasma cell type.25 cases had been followed up (46.3 ±32.3) months after operation, among which 1 UCD case died of pancreatic head carcinoma 13 months after operation, 1 case with MCD plus pemphigus died 2 months after surgical resection of retroperitoneal lymphadenectasis, another MCD case died in 1 month without any treatment.The remaining 22 cases were alive. Conclusion CD is lack of clinical specificity and its diagnosis should be made by pathology.Each subtype differ greatly in prognosis, therefore different treatments should be taken after definitive subtype diagnosis.
Multicentric Castleman Disease As A Rare Cause Of Nephrotic Syndrome: A Case Report

Abstract:Castleman disease (CD) is a benign lymphoproliferative disorder that is clinically divided into two types: multicentric Castleman disease (MCD) and unicentric Castleman disease (UCD). Herein is presented a case of MCD characterized by edema, proteinuria, anemia, and renal insufficiency. The patient did not respond well when initially treated as nephrotic syndrome. After systematic evaluation, a final diagnosis of MCD was established by lymph nodes biopsy. The patient received 4 cycles of rituximab and prednisone. The edema resolved significantly, and his condition improved with normalized renal function and hemoglobin. Based on the case of MCD mimicking nephrotic syndrome, the diagnostic dilemma MCD may present to clinicians is discussed, with an emphasis on definitive diagnosis of MCD.

Abstract:Castleman disease is a complex benign lymphoproliferative disorder characterized by the enlargement of a single lymph node or a group of lymph nodes. Its etiology is unclear, with the mechanism of action of IL-6 and HHV-8 implicated as possibly associated with the development of the disease. Diagnosis depends on the histopathological findings of the involved lymph nodes. Surgical resection can be curative, but a small number of cases may be unresectable and need radiation and chemotherapy with subsequent resection if possible.
Multicentric Castleman’s Disease in a Renal Allograft Recipient: a Case Report and Literature Review

Abstract:Giant lymph node hyperplasia is the main symptom of Castleman’s disease (CD), which is a rare and easily overlooked lymphoproliferative disease that mimics both benign and malignant lesions. Several cases of CD after renal transplantation have been reported in the literature. A 43-year-old man was admitted to our medical center with high serum globulin levels after receiving a living donor kidney transplant. A lymph node biopsy raised suspicion for multicentric CD. Because of the poor therapeutic effect of reduction of immunosuppression, the patient was treated with CHOP chemotherapy comprising four cycles of monthly cyclophosphamide (750 g/m2, day 1), vincristine (1.4 g/m2, day 1), doxorubicin (50 g/m2, day 1), and prednisone (60 mg/m2, daily). Following the treatment, his serum globulin levels decreased to the normal range. At the 2-year follow-up examination, the abdominal, axillary, and inguinal lymph nodes had significantly decreased in size. Without a pathological diagnosis, multicentric CD after renal transplantation can be easily ignored and misdiagnosed. If the clinical signs cannot be relieved by minimizing the dose of common immunosuppressants, the CHOP regimen may be a better option. Biological agents may be added in patients with positive immunohistochemistry staining and good economic conditions.
Idiopathic Multicentric Hyaline Vascular-Type Castleman Disease

Abstract:Castleman disease is a rare lymphoproliferative disorder presenting with localized or disseminated lymphadenopathy and systemic symptoms. It can be categorized clinically as unicentric or multicentric, histopathologically as hyaline vascular, plasma cell, or mixed variant, and etiologically, considering the subtypes based on causative viral agents and associated syndromes. The multicentric type can mimic other haematological malignancies, ranging from asymptomatic to multiple organ involvement. Although its pathophysiology is not well known, the current approved treatments are directed towards interleukin-6, CD-20, and viral agents. The authors present an 82-year-old leucodermic man presented with a 2-week history of constitutional symptoms. Examination revealed pallor, hepatosplenomegaly, and palpable left axillary lymphadenopathy. Investigation showed anaemia, thrombocytopenia, polyclonal hypergammaglobulinemia, hypoalbuminemia, and high acute phase reactants, with image study revealing multiple axillary, mediastinal, inguinal, and pelvic lymphadenopathies. The lymph node biopsy was consistent with hyaline vascular-type Castleman disease without human herpersvirus-8 markers. He started prednisolone with initial improvement evolved poorly on a short term. Castleman disease has a broad spectrum of clinical manifestations, associations, and complications that bring a diagnostic challenge, requiring a multidisciplinary approach. Clinicians should be familiar with its features because proper diagnosis and aggressive targeted treatment are the pillars of proper management of these patients.
