Abstract:Castleman's disease (CD) is a benign lymphoid neoplasm characterized by massive proliferation of the lymphoid tissue due to an uncertain cause.1 Two clinical types (localized, multicentric) have been described with three histological variants (hyaline-vascular, plasma cell and mixed type). The expected localization is mediastinum and rarely pararenal retroperitoneum.2 We reported two cases of CD in the pararenal retroperitoneum, one was complicated with autoimmune hemolytic anemia. CASE REPORT Case 1 A 33-year-old asymptomatic woman was incidentally found to have a well-circumscribed hypoechoic mass at the right pararenal area on ultrasonography. Physical examination revealed no peripheral lymphoadenopathy and no significant abnormality. Leukopenia (3.0×109/L) was observed and improved (3.7×109/L) spontaneously four days later. Routine blood biochemistry was normal. Computerized tomography (CT) scan revealed a solid wedge shaped mass (4.1 cm × 2.6 cm) at the right pararenal localization mimicking an adrenal neoplasma (Figure 1), homogenous enhancement (lower than renal parenchyma) could be observed after intravenous contrast injection.Figure 1: . A 5-mm transverse computed tomographic section shows a well-circumscribed mass (arrow) of right pararenal retroperitoneum displacing inferior vena cava anteriorly and right adrenal gland laterally.The lesion was found to be hormonally inactive after an endocrinologic survey that included determination of urinary normetanephrine, metanephrine and serumal aldosterone, hydrocortisone, renin, angiotensin. Certain histopathologic diagnosis was essential and exploration was performed laparoscopically through a retroperitoneal way. The tumor was resected completely with ultrasound knife, though it adhered closely to the inferior vena cava and the right adrenal. The mass (4.0 cm × 4.0 cm × 3.0 cm) appeared light brown and well-capsulated. Pathological examination demonstrated characteristics of the hyaline-vascular type of CD (Figure 2). The patient got discharged uneventfully at postoperative day 2 and remained free of the disease after one year.Figure 2: . Histological examination reveals germinal centers pierced by vessels and cuffed by small lymphocytes arranged in a concentric pattern (HE staining, original magnification ×100).Case 2 A 43-year-old woman was hospitalized for 3-week fever (oral 37.3°C&-;38.3°C), dizziness, debility and chest distress. Physical examination revealed anemic face, no enlarged lymphonode was palpated. Complete blood count revealed hypochromic microcytic anemia (hemoglobin 69 g/L) and leukopenia (3.3×109/L). Bone marrow examination suggested iron deficiency anemia and hemolysis. Direct Coombs test and direct anti-IgG test are both positive, which suggesting autoimmune hemolytic anemia. Nothing special was found in chest X-ray film. Ultrasonography showed a pararenal hypoechoic mass with obscure boundary to right kidney, in which arterial flow could be detected by Doppler (Figure 3A). On CT film, an irregular mass (about 13 cm × 6 cm × 5 cm) occupying renal sinus was revealed, homogenous enhancement (lower than renal parenchyma) could be observed after intravenous contrast injection. Magnetic resonance imaging (MRI) demonstrated a mass with homogeneous and isohypointense signal on T1-weighted image and with homogeneous and hypointense signal on T2-weighed image to that of renal cortex respectively (Figure 3B and 3C). Mild hydronephrosis was detected by both CT and MRI examinations. Enlarged lymphonodes around vena cava could be observed in all above images.Figure 3: . Mild hydronephrosis and compressed inferior vena cava could be observed in MRI.Retroperitoneal malignant tumor complicating secondary autoimmune hemolytic anemia was firstly took into account and exploration was performed laparoscopically. The mass occupying right renal sinus adhered closely to right kidney and inferior vena cava and enlarged lymphonodes around vena cava was detected, which suggested a malignant tumor. Lymphonode biopsy was performed and pathological examination demonstrated the characteristics of the plasma cell type of CD (Figure 4). The patient resumed physical activity and oral intake within 36 hours; then we began her further treatment in time.Figure 4: . B: Original magnification ×400.DISCUSSION CD is clinically classified into localized (or unicentric) subtype and multicentric subtype. Localized disease manifests as a solitary mass, which may be well circumscribed or infiltrative. It is associated with lymphadenopathy confined to one lymph node or nodal area, and usually follows a benign course. Multicentric disease carries a worse prognosis, and subsequent infection or malignancy may lead to death.3 Histologically, CD is divided into the hyaline-vascular form and a plasma cell variant, the former is more common in localized disease and the latter is more common in multicentric disease.2,4 The hyaline-vascular subtype contains numerous regressively transformed follicles and associated vascular proliferation. The plasma cell type of CD contains hyperplastic follicles and marked plasma cell proliferation in the interfollicular region; the mixed form exists with both subtypes.2 Although CD can arise wherever lymphoid tissue is found, the most frequently involved site is the mediastinum and tumors in the retroperitoneum (7%) and pararenal area (2%) are uncommon.5 It can locate supra to the kidney mimicking an adrenal tumor6,7 like case 1, lateral to the kidney occupying renal sinus4 like case 2, or lateral to the kiney without infiltrating other organ.5,8 Diagnostic image sometimes mimic malignancy but can not identify CD because of the lack of tumor-specific signs.7,8 The CT and MRI findings of our cases are consistent with those in previous reports, while heterogeneous signals within the tumor attributing to calcificaiton, fibrous septa or vessels have also been refered.4,6 Moreover, mild hydronephrosis detected in case 2 was observed in all lesions of 5 masses occupying renal sinus, suggesting differential diagnosis of CD should be refered in such a mass.4 Doppler scan is very helpful in demonstrating the marked degree of tumor vascularity8 as case 2. Angiography confirms its retroperitoneal location and feeding vessels and enables preoperative embolization.5,8 Absence of gallium uptake was found in both unicentric and multicentric CD, which is valuable for distinguishing CD from other lymphoproliferative disorders.9 Localized diseases are usually asymptomatic while multicentric disease is a systemic illness with disseminated lymphadenopathy manifesting constitutional symptoms and laboratory abnormalities, such as anemia, hypoalbuminemia, hypergammaglobulinemia, and elevated erythrocyte sedimentation rates.2,3 CD also can be associated with autoimmune disease such as autoimmune hemolytic anemia (like case 2) and paraneoplastic pemphigus.6,10 The immune dysregulation of interleukin-6 has become the leading theory explaining complicated autoimmune disease, constitutional symptoms and laboratory abnormalities.10 Complete surgical resection has been reported to be curative and remains the treatment of choice for localized disease,3,5–8,11 while chemotherapy and immunotherapy are effective for multicentric one which histologically confirmed by lymphonode biopsy.2 There are several reports that localized CD in abdominal cavity were treated laparoscopically.12,13 Some masses are well-vascularized and adjacent to great vessels,8,12 laparoscope can offer magnified images to faciliate and secure dissection. Lots of multicentric CD manifest with palpable lymphadenectasis2 and were not found in case 2, which made the diagnosis difficult. Moreover, histological diagnosis of CD is difficult and is essentially based on cell architecture, thus requiring study of the entire surgical specimen. Fine-needle biopsy can not offer enough information for correct diagnose and is potentially dangerous for a hypervascular tumor.6,13 Therefore, laparoscopic approach and lymphonode biopsy might be more helpful in such situation. CD should be included in the differential diagnosis of retroperitoneal tumors. The laparoscopic approach enables the resection of the lesion and histological diagnosis with the advantage of magnified images, minimal invasiveness and rapid recovery.
