Abstract:Population aging lifestyle shift has increased stroke prevalence in China since the 1980s. Stroke is becoming the leading cause of death in many parts of the country. The socioeconomic cost associated with stroke is climbing, and there is no evidence to show the trends will change in coming years. In contrast to the growing number of patients, prevention and management of stroke are far from satisfactory in China. The control ratios of modifiable risk factors for cardiovascular diseases, such as hypertension and diabetic mellitus are very low. Patients treated with thrombolysis and well-organized stroke units are very few compared with the huge patient population. Incidence and recurrence, as a result, are unacceptably high. Establishing a regular communicating platform to exchange experience and new knowledge is an effective way for narrowing the gap for stroke management among countries of different socioeconomic development. For this purpose, International Stroke Society, now merged with World Stroke Federation as World Stroke Organization (WSO), initiated a sequential conference, the International Stroke Summit in China in 2004. The first summit was held in Beijing in September 2004, which called together more than 2000 researchers and doctors from China and many other countries. The second summit was held in Nanjing in August 2006, which was cochaired by Geoffrey Donnan and Xinfeng Liu, and convened more than 1000 clinicians and researchers. The third summit was held in November 2007 in Wuhan. A total of 650 doctors and researchers attended the meeting. The Fourth International Stroke Summit was held in July 2008 in Nanjing, China. As in the previous meetings, the motif of narrowing the gap between developing and developed countries in stroke management permeated through the agenda of the conference. Several topics, such as acute thrombolysis after cerebral infarction, endovascular treatment for intracranial and extracranial atherosclerotic artery stenosis, prehospital management of stroke, and prevention and treatment of vascular cognitive impairment, garnered much interest. The Fourth International Stroke Summit was cochaired by Xinfeng Liu from Nanjing University, China, Werner Hacke from University of Heidelberg, Germany, and Michael Chopp from Henry Ford Hospital, USA. Professor Hacke opened the meeting with a keynote address in which he expressed greetings from WSO to the participants on behalf of Professor Geoffrey Donnan, the president of the WSO. Dr Hacke encouraged Chinese doctors to participate in more international stroke conferences and to publish their research results in the most important stroke journals, such as Stroke, Cerebrovascular Diseases, and International Journal of Stroke. Xinfeng Liu introduced the recent progress in endovascular procedures in treating atherosclerotic diseases. He mentioned the unique status of carotid angioplasty and stenting in China and other developing countries. Because of the shortage of skilled neurosurgeons and reluctance of patients to have a major operation in the neck region, carotid endarterectomy is unavailable in most Chinese hospitals. This situation left many patients with severe carotid stenosis treated with medications of undetermined results or left untreated completely. Liu said the emergence of endovascular techniques provide these patients a treatment alternative, which may change their outcome and improve their quality of life in the future. Werner Hacke spoke on the management of severe stroke, in which he discussed the rationales and indications of three major strategies for severe stroke, rtPA thrombolysis, decompressive surgery, and hypothermia. He also introduced sonolysis with microbubbles and mechanical retrieval devices which he believes can increase the recanalization rates in thrombolytic patients. Michael Chopp delivered a lecture on stimulating brain plasticity and recovery after stroke. He introduced the new theory of remodeling brain after stroke by combining strategies of angiogenesis, synaptogenesis, and neurogenesis. He also reported the new roles of statins in improving angiogenesis and neurogenesis after cerebral ischemia. Zhenguo Liu, a cardiologist from Ohio University, USA, discussed patent foramen ovale (PFO) as a potential risk for cerebral infarction. He introduced endovascular strategies for treating PFO. The prehospital management of stroke patients is another hot topic from the meeting. It was reported that Chinese patients had a longer prehospital delay after stroke onset than western patients. Poor transportation status and ignorance of stroke warning signs are among the main causes of this disadvantage.
