Abstract:We describe the case of a 37-year old male who presented with a schwannoma in his right palatine tonsil. The tumour and tonsil were surgically removed transorally. Tonsillar schwannomas are slow growing, and the main symptoms were a foreign body feeling in the throat, dysphagia and snoring. The presence of a round, well-defined mass lesion with heterogeneous density on computed tomography (CT) may aid in differentiation from other tonsillar tumours. The CT findings in the reported case were different to those of schwannomas in other locations; the particular CT manifestations of tonsillar schwannomas seem to relate to the various pathological changes seen within the tumour.

Abstract:Solitary fibroma of the lacrimal sac is an extremely rare disease.In this paper,we reported one case of solitary fibroma of the lacrimal sac due to the chief complaints of uplift neoplasm in her right lacrimal sac complicated with epiphora for 20 years.Dacryocystography combined with orbital CT angiography,contrast-enhanced MRI of bilateral eyes revealed an oval mass in her right lacrimal sac.Lacrimal system irrigation demonstrated that the right lacrimal duct was obstructed.The reflux of irrigation fluid was observed.No purulent or hemorrhagic secretions were noted.The possibility of lacrimal sac-derived benign mass was considered.The patient underwent complete resection of the mass via medial and lateral approach.She was pathologically diagnosed with solitary fibroma of the lacrimal sac and discharged after effective therapy.This case prompts that solitary fibroma of the lacrimal sac is extremely rare with atypical clinical manifestations.The symptoms after lacrimal system irrigation are similar to those of lacrimal sac cyst.Consequently,it is likely to make a misdiagnosis or miss the diagnosis by ophthalmologists.

Abstract:Only a limited number of documented low-grade papillary Schneiderian carcinoma (LGPSC) cases have been reported since its initial description in 2015. In this study, the authors present a case report involving a young female patient who presented with this rare condition localized in the nasal cavity. On the basis of histologic examination of a small tumor sample, the initial diagnosis pointed to squamous cell carcinoma. After the surgery, however, the final histologic diagnosis was confirmed as LGPSC. Finally, after surgical intervention, the histologic diagnosis was confirmed as LGPSC. Remarkably, over a period of 22 months, the patient showed no signs of recurrence or metastasis. To achieve an accurate diagnosis and implement appropriate treatment strategies for LGPSC, a comprehensive understanding of both its clinical and histologic characteristics is necessary.

Abstract:AIM: To study the clinical and pathological characteristics of the lacrimal sac lymphoma, which is rare but it is the major type of non-epithelial malignant tumor in the lacrimal sac region. METHODS: Sixty-four cases of malignant lacrimal sac tumors in our hospital from 1986 to 2020 were retrospectively reviewed. Eight cases of lacrimal sac lymphoma were carefully reviewed. RESULTS: There were five mucosal-associated lymphoid tissue (MALT) lymphomas, one diffused large B-cell lymphoma, one NK/T cell lymphoma, and one mantle cell lymphoma. All eight patients represented symptoms of epiphora with swelling in the lacrimal sac for a certain period of time and showed no signs of systemic involvement at the first time of clinical visits. They had received either chemotherapy or radiotherapy after surgery. Long-term follow-up (from 11 to 220mo) showed that, except one patient with MALT lymphoma died for unknown reasons at 104mo after surgery, the other 7 patients were all alive with no signs of local recurrence, neither in other organs. CONCLUSION: Non-epithelial malignant tumors of the lacrimal sac are rare and lymphoma is the major subtype.
Clinical Features and Imaging Findings of Early Lacrimal Sac Neoplasms

Abstract:Background To examine the clinical features and imaging findings of early-stage lacrimal sac neoplasms. Methods In this retrospective study, patients presenting with epiphora were diagnosed with either lacrimal sac neoplasms or mucoceles. Diagnostic assessments included computed tomography (CT), enhanced magnetic resonance imaging (MRI), and pathological analysis. Results The study comprised nine patients (2 male; 7 female) with lacrimal sac neoplasms, 44% of whom had malignancies while 56% exhibited benign conditions. There was no statistically significant difference in the mean width of the nasolacrimal canal between patients with neoplasms (6.49 ± 3.68 mm) and mucoceles (5.50 ± 1.88 mm, P = 0.44). However, the depth of the nasolacrimal canal was significantly greater in the neoplasm group (5.08 ± 3.21 mm) compared to the mucocele group (2.85 ± 0.96 mm, P = 0.05). Notably, lacrimal sac mass extended above the medial canthal tendon in eight neoplasm patients, unlike any in the mucocele group, demonstrating distinct pathological presentations. Additionally, the diseased eyes in the neoplasm group showed significantly greater nasolacrimal canal width (6.49 mm) compared to the contralateral eyes (3.01 mm, P = 0.01) and a slightly deeper depth (5.08 mm vs. 2.66 mm, P = 0.04). Conclusions Findings suggest that a lacrimal sac mass extending above the medial canthal tendon warrants a CT scan. Patients with a nasolacrimal canal wider than the contralateral side should undergo enhanced MRI for accurate diagnosis and treatment planning.
Clinicopathological Analysis of Low‐grade Papillary Schneiderian Carcinoma: Report of Five New Cases and Review of the Literature

Abstract:Low-grade papillary Schneiderian carcinoma (LGPSC) is a rare and newly described entity of the sinonasal tract. The aim of this study was to evaluate the clinicopathological and molecular characteristics in order to identify typical features for differential diagnosis.Of the 3000 cases of sinonasal tumour studied during a period of 6 years, five cases were reviewed and diagnosed as LGPSC. All five patients were female (mean age, 47.8 years; range, 18-64 years) and had undergone multiple surgeries (3-10 surgeries). Both the sinonasal tract and the middle ear were involved in four patients. Nodal metastasis occurred in two patients, and one patient developed a distant metastasis to the left lung. Histologically, tumours had branched and crowded papillae with pushing boundaries. Tumour epithelia were multilayered and arranged in an orderly pattern without cilia. No malignant cytological features were observed in any of the cases. Immunohistochemical findings revealed a scattered distribution of Ki67-positive cells and positive staining for epithelial membrane antigen, mainly in the outermost-layer cells. Human papillomavirus (HPV) DNA was found in two patients and genotyped as HPV type 16. Sanger sequencing did not reveal any epidermal growth factor receptor or Kirsten rat sarcoma viral oncogene homologue gene mutation in the five cases.We report on five new cases of LGPSC, and confirm LGPSC as a new sinonasal carcinoma that behaves aggressively with metastatic potential. The combination of clinical behaviour and typical histological features can distinguish LGPSC from sinonasal papilloma and other carcinomas.
Unveiling a distinctive presentation: Benign manifestation of primary ductal adenocarcinoma of the lacrimal gland

Abstract:Objective: To report the case of a man with well-demarcated cystic lesion in the right lacrimal gland, subsequently diagnosed as primary ductal adenocarcinoma of the lacrimal gland. Methods: We report the clinical presentation, histopathologic, immunohistochemical features and treatment of this unique primary ductal adenocarcinoma of the lacrimal gland. Additionally, we analyze the factors contributing to the tumor's presentation and review the current literature regarding this uncommon neoplasm. Results: The patient underwent total resection of the mass, revealing primary ductal adenocarcinoma of lacrimal gland. Histopathological examination revealed a solid nested, sieve-like (with observed acne-like necrosis), tubular, and small nested arrangement of the tumor. The tumor cells exhibited marked heterogeneity, and vascular tumor thrombi were observed in the stroma, along with evidence of neurological invasion. The immunophenotype analysis showed positivity for androgen receptor, GATA binding protein 3, gross cystic disease fluid protein 15 and cytokeratin-7, which is considered a ductal adenocarcinoma of lacrimal gland. Subsequently the patient underwent local radiotherapy. No local recurrences or metastases were reported during the 1-year follow-up. Conclusions: Primary ductal adenocarcinoma of lacrimal gland is a rare tumor of the lacrimal gland. The manifestation of ductal adenocarcinoma of the lacrimal gland may include well-demarcated cystic lesions, indicative of distinct subtypes. Various treatment options are adapted to different subtypes of ductal adenocarcinoma of the lacrimal gland.

Therapie Der Patient unterzog sich einem endoskopischen Eingriff mit einer Dakryozystorhinostomie. Schlussfolgerung Der hier berichtete Fallbericht zeigt eine atypische Lokalisation eines weltbekannten Tumors mit einem verdächtigen Risikofaktor, damit jeder HNO ein IP des Tränenapparats unter irgendwelchen Anschwellungen der lateralen Nasenwand ins Auge fasst. History A chemical trauma was followed by a painless, growing mass in the medial canthal area associated with epiphora. Findings Rhinoendoscopy, Computed Tomography and Magnetic Resonance showed a well-defined mass in left lacrimal fossa extended to the nasolacrimal duct. Treatment An endoscopic resection and a dacryocystorhinostomy were performed. Conclusion The aim of the present study is to show an atypical presentation of a well-known tumor along with a suspicious risk factor and to lead otolaryngologists to consider an inverted papilloma of the lacrimal system among lesions causing lateral nasal swelling. Abstract:Spitz nevus (SN) is a benign melanocytic lesion with cytologic and architectural atypia. It is sometimes difficult to distinguish SNs from atypical Spitz tumor (AST), Spitz melanoma, or conventional melanoma. SNs frequently develop in Caucasians and appear on the skin of the head and lower extremities. Lesions on the ear in Asian populations are rare. Here, we report a “red Spitz tumor” on the ear of a Chinese 18year-old boy. Dermoscopic examination revealed possibly malignant features presented as polymorphous vessels along with central white area, pseudo-network depigmentation and atypical peripheral globular pattern. The results of histopathological examination strongly suggested that the neoplasm was a compound SN and no recurrences or metastases occurred during 1-year follow-up post-surgery. Further, we review the literature on 4 previously reported cases of SN on the ear and summarize themain points of SN diagnosis and differential diagnosis with atypical Spitz tumors and melanoma.
Sialadenoma Papilliferum :A Clinicopathologic Study of 6 Cases

Abstract:PURPOSE:To study the clinicopathologic feature,nature,biological behavior and histogenesis of sialadeno-ma papilliferum(SP).METHODS:The clinicopathological features of6cases of SP were analyzed.Immunohistochemical staining was used in4cases,and the antibody used was AE1/AE3,CK8,S-100,SMA,Vim,GFAP.RESULTS:The average age of the patients was64years old,and the ratio of male to female was1:1.All of them were asymptomatic.Of the6cas-es,3on the margin of the tongue,2on the palate and1in the base of the tongue.The average diameter of the tumor was1cm.Microscopically,the tumor was lack of capsule.Superficially the squamous cells of oral mucosa proliferated to form multiple papillary extensions.Immediately subjacent to the papillary mucosa,the tumor was composed of cysts or duct-like structures with different sizes.The luminal surface of the ducts exhibited papilla projections.The ducts and their pap-illary folds were lined with a double layer of cells.Following-up information was obtained in4patients.No cases had re-currence and metastases.The Immunohistochemical results were AE1/AE3and CK8being positive in the cells of surface layer,and S-100,AE1/AE3,SMA,Vim and GFAP being positive in the cells of basal layer.CONCLUSIONS:Sialadeno-ma papilliferum was a benign neoplasm.Histologically,it is characterized as multiple papillary projections in cysts and duct-like structures.The prognosis of SP was good.It was most likely that SP arised from the duct cell resembling secre-tory ducts.
Space-occupying Lesions of the Lacrimal Gland at One Tertiary Eye Center in China: a Retrospective Clinical Study of 95 Patients.

Abstract:AIM:To investigate the treatment status and prognosis of space-occupying lacrimal gland lesions at one tertiary eye center in China.METHODS:A retrospective clinical study was performed on 95 patients with space-occupying lesions of the lacrimal gland surgically treated at the Eye & ENT Hospital of Fudan University from 2003 to 2007. The reviewed clinical data included age, gender, side of the lesion, duration of signs and symptoms, histopathological diagnosis, treatment modality, recurrence (local, regional, and distant metastasis) and survival.RESULTS:Of the 95 cases (99 eyes), pleomorphic adenomas were the most common lesions (43 cases), followed by lymphoid disorders (14), inflammatory pseudotumors (11), carcinoma ex-pleomorphic adenomas (11), and adenoid cystic carcinomas (ACC, 6). There were 8 patients with relapsed pleomorphic adenomas. Five of these 8 cases had malignant pathological changes. All patients with ACC had metastasis and three of them died during their follow-up.CONCLUSION:Our study indicated that the most common lacrimal gland lesions were pleomorphic adenomas. Multiple recurrence and surgical procedures may increase the risk of tumor progression. ACC had a high incidence of tumor metastasis and a poor prognosis.

Abstract:BACKGROUND Here we present a rare case of localized amyloidosis involving the nasolacrimal duct and lacrimal sac which was managed by endoscopic surgery.CASE SUMMARY A 50-year-old man whose medical history included bilateral ventricular fold and vocal cord amyloidosis complained of bilateral epiphora. Magnetic resonance imaging revealed a neoplasm within the nasolacrimal sac. Characteristic positivity for Congo red staining and birefringence under a polarized microscope proved the diagnosis of amyloidosis. Dacryocystorhinostomy via an endoscope obtained a favorable result. A one-year follow-up found no recurrence.CONCLUSION There are few reports on amyloidosis involving the lacrimal outflow system, and management and outcome are not clear. Endoscopic dacryocystorhinostomy can be a choice to relieve symptoms. Regular follow-up and monitoring of systemic diseases are highly recommended.
Rare case of mantle cell lymphoma of the lacrimal sac

Abstract:Mantle cell lymphoma of the ocular and periorbital regions is extremely rare but should be considered in the differential diagnosis of lesions affecting the periorbital tissues. In this study, we present a rare case of mantle cell lymphoma of the lacrimal sac in a 65-year-old male presenting with a mass in the lacrimal sac region and epiphora. After clinical examinations and imaging studies, the mucocele was misdiagnosed. Considering the unexpected findings during external dacryocystorhinostomy, a frozen biopsy was performed, which confirmed the diagnosis of lymphoma.

Abstract:Purpose: To explore the endoscopic and histopathologic characteristics of the lacrimal sac (LS) and the nasolacrimal duct in cases with punctal agenesis presenting with LS swellings. Methods: This is a prospective, interventional study of 13 LSs of 8 patients presenting with punctal agenesis and LS swellings over a 5-year study period (June 2018–July 2023). Complete ophthalmic examination was performed along with CT scans of the orbits. The LS was exposed with an anterior lacrimal crest incision and explored. The exterior and interior of the sac swelling were examined and LS flaps were sent for histopathological analysis. Results: The mean age of the patients was 23 years with a male:female ratio of 5:3. All patients presented with swelling in the LS region. Occasional epiphora was the presenting feature in 6 patients. All patients had both upper and lower punctal agenesis on the affected side with associated LS swelling. CT scans showed a hypodense cystic swelling with expansion of the LS fossa and bony nasolacrimal duct ending abruptly short of the inferior meatus. Intraoperatively, the LS had thin, translucent walls with mucoid secretion and an absence of common canalicular opening. Histopathological analysis showed epithelium consistent with LS with less robust development of the sac walls and poorly structured lacrimal drainage-associated lymphoid tissue with scanty stromal inflammation. Conclusions: The development of LS and the nasolacrimal duct is structurally affected in patients with punctal agenesis. Retrograde approaches for epiphora resolution are not logical in such cases but conjunctivodacryocystorhinostomy with Jones tube can be a feasible option.

Abstract:A 78-year-old Caucasian female presented with a painless mass in the right orbit that had progressively enlarged over several months. Computed tomography scan of the orbits showed a right lacrimal gland mass with no bony erosion. Histopathologic analysis of the biopsy specimen revealed invasive squamous cell carcinoma positive via in-situ hybridization for high-risk human papillomavirus. The patient underwent successful removal of the right lacrimal gland tumor en bloc, followed by adjuvant radiotherapy. This is an extremely rare case of primary squamous cell carcinoma of the lacrimal gland and the first report describing human papillomavirus positivity in this tumor location.
Scrna-Seq: First Atlas and Cellular Landscape of Lacrimal Sac: Implications in Primary Acquired Nasolacrimal Duct Obstruction Pathogenesis.

Abstract:Purpose:The aim of this study was to describe the transcriptional changes of individual cellular components in the lacrimal sac in patients with primary acquired nasolacrimal duct obstruction (PANDO) and attempt to construct the first lacrimal sac cellular atlas to elucidate the potential mechanisms that may drive the disease pathogenesis.Methods:Lacrimal sac samples were obtained intra-operatively during the endoscopic dacryocystorhinostomy (EnDCR) procedure from five patients. Single-cell RNA sequencing was performed to analyze each individual cell population including epithelial and immune cells during the early inflammatory and late inflammatory phases of the disease.Results:Eleven cell types were identified among 25,791 cells. T cells and B cells were the cell populations with the greatest variation in cell numbers between the two phases and were involved in immune response and epithelium migration-related pathways. The present study showed that epithelial cells highly expressed the genes of senescence-associated secretory phenotype (SASP) and were involved in influencing the inflammation, neutrophil chemotaxis, and migration during the late inflammatory stage. Enhanced activity of CXCLs-CXCRs between the epithelial cells and neutrophils was noted by the cell-cell communication analysis and is suspected to play a role in inflammation by recruiting more neutrophils.Conclusions:The study presents a comprehensive single-cell landscape of the lacrimal sac cells in different phases of PANDO. The contribution of T cells, B cells, and epithelial cells to the inflammatory response, and construction of the intercellular signaling networks between the cells within the lacrimal sac has further enhanced the present understanding of the PANDO pathogenesis.
Primary Lacrimal Sac Tumors with Extension into Vicinity: Outcomes with Endoscopy-Assisted Modified Weber-Ferguson's Approach
