Abstract:PURPOSE: The purpose of the study was to evaluate the clinical features, treatment, and outcome of patients with conjunctival papilloma.MATERIALS AND METHODS: Twenty-two patients (22 eyes) with biopsy-proven conjunctival papilloma between January 2005 and January 2015 in a tertiary medical center were retrospectively reviewed. Clinical profiles, treatment, outcome, and factors related to recurrence were evaluated.RESULTS: There were 16 males (73%) and 6 females (27%), with a mean age of 47 years. The most common location of conjunctival papilloma was the caruncle (43%), followed by palpebral conjunctiva (29%), bulbar conjunctiva (14%), and fornix (14%). Recurrence developed in five patients (22.7%). The risk of postoperative recurrence was significantly related to the presence of bulbar conjunctival papilloma with corneal involvement (P = 0.043) and surgical excision alone (P = 0.039). One case with multiple recurrences developed nonkeratinizing carcinoma. Two young females developed conjunctival papilloma even after receiving human papillomavirus vaccinations.CONCLUSION: The recurrence of conjunctival papilloma is not uncommon, especially for those patients underwent surgical excision alone. Surgical excision with adjunctive therapy and long-term follow-up is rational for the treatment of conjunctival papilloma.

Abstract:Conjunctival papilloma is an acquired benign squamous cell tumor that can present at any age, but most frequently in the third and fourth decades of life. Papillomas have been associated with human papilloma virus (HPV) infection, usually types 6 and 11.Although histopathological diagnosis remains the gold standard, the advent of newer non-invasive imaging modalities such as optical coherence tomography (OCT) is transforming the way we diagnose and treat ocular surface tumors, including conjunctival papilloma. Management of these lesions can prove a challenge to the treating physician since not all lesions respond to medical and/or surgical therapy and in fact may worsen after surgical manipulation.In this review, the epidemiology, pathophysiology, clinical characteristics, and diagnosis of conjunctival papilloma including the use of OCT are discussed. Indications, efficacy, and side effects of currently available management options are also reviewed to guide the selection of the best treatment approach.

Abstract:Extraovarian peritoneal serous papillary carcinoma is a kind of malignancy with its primary site in the peritoneum.The histological and cytological characteristics of the tumor resemble that of the ovarian papillary serous carcinoma.Its nidus is diffused in the perineum,but absent from the ovary or just superficially involved.The clinical manifestations of the patient are mainly non-specific symptoms of gastrointestinal tract,like the presentation of ovarian cancer in late stage.The definite diagnosis of extraovarian peritoneal serous papillary carcinoma is based on the pathologic manifestation and the events found in the surgery.The principle of treatment consists of cytoreductive surgery followed by cisplatin-based combined chemotherapy.In this review,we summarized the clinical manifestation,diagnosis and therapy of extraovarian peritoneal serous papillary carcinoma,to provide more evidence for future clinical application.

Abstract:Background Isolated inverting papilloma (IP) of the sphenoid sinus is rare, with about 50 patients reported in the English-language literature. With its rarity, scientists have paid less attention to the clinical and radiographic characteristics of isolated IPs arising from the sphenoid sinus. Methods We describe the clinical presentation, imaging manifestations, and surgical methods of isolated IPs from the sphenoid sinus in 3 patients and review the English-language literature from 1970 to 2011. Results In total, 59 cases of isolated sphenoid sinus IPs (including our cases) have been reported, 40 patients with computed tomography or magnetic resonance imaging details. The most common presentation is headache. Lesions were found in the sphenoethmoidal recess in 26 cases. In 8 patients who underwent enhanced magnetic resonance imaging, the tumor was obviously intensified. We found 26 patients with bony destruction, and 8 lesions revealed the correct tumor origin. Conclusions A polyp-like mass in the sphenoethmoidal recess strongly implied the diagnosis of an IP in the sphenoid sinus. Bony destruction on computed tomography is a common characteristic of this disease and had a close relationship with the original tumor site in our study. Malignant transformation of IPs in the sphenoid sinus is much scarcer.
The Study on Expression of Human Papillomavirus and Phosphotyrosione and Tyrosine Phosphatase 2 in Condyloma Acuminatum Tissue Remodeling

Abstract:Objective To explore the expression of human papillomavirus (HPV) 6/11,16/18,31/33 group and tyrosine phosphatase 2 (SHP-2) in condyloma acuminatum (CA) tissue remodeling.Methods HPV 6/11,16/18,31/33 group and SHP-2 were detected in skin lesions of 36 patients with condyloma acuminatum, tumor tissues of 13 cases of cervical cancer and normal foreskin tissue of 10 normal controls by in situ hybridization and SP immunohistochemical stain.Results The positive rates of HPV 6/11,16/18,31/33 group in condyloma acuminatum and cervical cancer were 80.5%,38.9%,36.1% and 69.2%,38.4%,53.8% respectively.The results showed that the expression of HPV 6/11,16/18,31/33 group in condyloma acuminatum and cervical cancer were significantly higher than those in normal epidermis (P0.05).The positive expression rates of phosphorylated tyrosine and SHP-2 in condyloma acuminatum and eerviaal cancer were 97.2%,22.2% and 100%,0%,respectively.However the cytoplasm of normal foreskin showed negative expression. Conclusion There is an aberrant activation of phosphorylated tyrosine and SHP-2 in condyloma acuminatum and cervical cancer. SHP-2 may play an important role in the HPV infection.

Abstract:Objective: To analyze the clinical pathological and genetic mutation characteristics of conjunctival lymphoepithelial carcinoma. Methods: A retrospective case series study was conducted. Data from three patients diagnosed with conjunctival lymphoepithelial carcinoma and treated with tumor resection surgery at Tianjin Eye Hospital from January 2006 to December 2022 were collected. Four paraffin specimens (including one patient undergoing two surgeries) were subjected to immunohistochemical staining for epithelial antigen and lymphocytic antigen. Epstein-Barr virus (EBV)-encoded RNA (EBER) was detected using in situ hybridization, and whole-exome sequencing was performed on three specimens from two patients using next-generation sequencing methods. Results: All three patients were males aged over 65, with a disease duration ranging from 3 to 44 months. The tumors were unilateral, located on the bulbar or limbal conjunctiva, appearing red, with a maximum diameter of 4-20 mm. Imaging examinations revealed anterior location of the tumors with no involvement of the orbital bone, extraocular muscles, optic nerve, or paranasal sinuses. No local lymph node metastasis was observed in any patient. Pathological findings included undifferentiated carcinoma nests with significant reactive lymphocytic and plasma cell infiltration. Tumor cells were positive for pan-cytokeratin (CK-pan), epithelial membrane antigen (EMA), tumor protein 40 (p40), and tumor protein 63 (p63), with a cell proliferation index (Ki67) exceeding 80%. Cluster of differentiation 20 (CD20), CD3, and CD8 were positive for lymphocytes. In situ hybridization showed partial tumor cell expression of EBER in two specimens of one patient. Whole-exome sequencing revealed 58, 50, and 36 mutated genes in the three specimens, with enriched signaling pathways including melanoma signaling pathway, Notch1 signaling pathway, and RHOQ GTP cycle; enriched biochemical processes included amino acid starvation response, programmed cell death, regulation of lipid synthesis, sodium ion transport, and chromosome segregation. The common mutated gene in all three specimens was SZT2, and SZT2 was involved in the amino acid starvation response. One patient underwent a second complete resection surgery 40 months after partial excision, while the other two underwent complete resection surgery without recurrence. Two patients did not undergo radiation or chemotherapy, and one was lost to follow-up. Conclusions: Conjunctival lymphoepithelial carcinoma is associated with prominent lymphocytic and plasma cell infiltration, some cases are associated with EBV infection, and SZT2 mutations are present in conjunctival lymphoepithelial carcinoma.

Abstract:Ciliated muconodular papillary tumor (CMPT) is a rare type of pulmonary tumor with mucoid features. Only a few cases confirmed by surgery have been reported worldwide. We report a case of CMPT following sublobectomy, with no recurrence detected in 10 months of follow-up. The accumulation of similar cases is essential for pathologists and surgeons to improve their understanding of such tumors.

Abstract:Mixed squamous cell and glandular papilloma (mixed papilloma) is a rare, benign, pulmonary neoplasm and most commonly found in senile patients with male predilection. There were only 18 cases reported so far in the English literature. We herein present one case of mixed papilloma in a 61-year-old male patient. Contrast-enhanced computed tomography scanning revealed a solitary nodule in the inferior lobe of left lung with the obstruction of basal segmental bronchus. Histologically, the glandular component of the mixed papilloma was characterized by the prominent " Medusa head-like" micropapillae and occupied more than 70% of the whole lesion. In the current case, these micropapillary patterns were so worrisome and should be distinguished with primary or metastatic micropapillary adenocarcinoma, especially in frozen slides or if the specimen is limited. By reporting this case, we aim to remind the pathologist with this uncommon histological structure in mixed papilloma.

Abstract:Squamous papilloma involving the middle ear as a primary lesion is an extremely rare occurrence. The aims of the present study were to investigate the presence of human papilloma virus (HPV) and Epstein-Barr virus (EBV) infections in primary middle ear squamous papilloma and to describe the clinical and pathological features of the disease along with therapeutic strategies. A retrospective review was conducted of four patients with clinical and pathological diagnoses of middle ear squamous papilloma. In situ hybridization (ISH) for a wide range of HPV DNA subtypes and EBV-encoded RNA was performed in the tissue samples obtained from these patients. Only two cases of primary squamous papilloma in the middle ear have been previously reported in the English literature. These papillomas developed in males of ~60-years of age and otorrhea was the most frequent complaint. Premalignant changes were observed in two of the present cases and ISH of HPV and EBV was negative in all four cases. The results of the present study indicated that chronic inflammatory stimulation, not HPV and EBV infection, is involved in the occurrence of middle ear squamous papilloma and its malignant transformation. Radical surgery and long-term postoperative follow-up are recommended due to its malignant and recurrent potential. Further genetic investigations with additional new cases are required to clarify the pathogenesis of squamous papilloma involving the middle ear.

The aim of this study was to evaluate the clinicopathological and molecular characteristics in order to identify typical features for differential diagnosis.Of the 3000 cases of sinonasal tumour studied during a period of 6 years, five cases were reviewed and diagnosed as LGPSC. All five patients were female (mean age, 47.8 years; range, 18-64 years) and had undergone multiple surgeries (3-10 surgeries). Both the sinonasal tract and the middle ear were involved in four patients. Nodal metastasis occurred in two patients, and one patient developed a distant metastasis to the left lung. Histologically, tumours had branched and crowded papillae with pushing boundaries. Tumour epithelia were multilayered and arranged in an orderly pattern without cilia. No malignant cytological features were observed in any of the cases. Immunohistochemical findings revealed a scattered distribution of Ki67-positive cells and positive staining for epithelial membrane antigen, mainly in the outermost-layer cells. Human papillomavirus (HPV) DNA was found in two patients and genotyped as HPV type 16. Sanger sequencing did not reveal any epidermal growth factor receptor or Kirsten rat sarcoma viral oncogene homologue gene mutation in the five cases.We report on five new cases of LGPSC, and confirm LGPSC as a new sinonasal carcinoma that behaves aggressively with metastatic potential. The combination of clinical behaviour and typical histological features can distinguish LGPSC from sinonasal papilloma and other carcinomas.
