Abstract:Objective To study the subtypes and developing features of post -hemorrhagic stroke dementia.Methods Eligible cases were collected by following-up and neuropsychological tests,and their subtypes were divided into possible AD and VaD in terms of NINCDS-ADRDA and NINDS-AIREN respectively.The incidence time between the two subtypes,as well as between the operation group and non -operation group were then analyzed.Results According to the Kaplan-meier analysis,the mean incidence time of possible AD was(17.19± 2.20) months,95%CI(12.89～21.50);and that of possible VaD was(27.53±2.09)months,95%CI(23.43～31.62).The difference between the two subtypes was significant(χ2=6.71,P<0.05).Conclusions The presentation of post-hemorrhagic stroke dementia varies in terms of neuropsychological test significantly shifting from possible AD-type to VaD-type.The average developmental time of AD-type is earlier than that of VaD-type.Operation doesn't affect the development of post-hemorrhagic stroke dementia.

Abstract:Vascular cognitive impairment (VCI) is a major contributor to age-related dementing illnesses which imposes a tremendous burden on families and society. It is a heterogeneous group of brain disorders. However, cerebral small vessel disease (CSVD) accounts for about 50%-70% of VCI, which represented a more homogeneous subtype of VCI. Advanced multimodal neuroimaging techniques like brain network connectome analyses are currently applied to explore the underlying mechanism of VCI. Some progress in the field of structural and functional brain network researches on a poststroke longitudinal CSVD cohort (Renji CSVD Cohort Study) was reported. Global and regional brain network characters were compared between patients with CSVD and healthy control. It suggested that distributed brain structural network disruption may play a pivot role in the cognitive decline. The results showed that brain structural network characters have distinctive differentiating capacity on the cognition of patients with CSVD.
The MRI semi-volumetric analysis of subcortical vascular cognitive impairment

Abstract:Objective To explore the neuroimaging markers of subcortical vascular cognitive impairment ( s-VCI) . Methods Based on the detailed neuropsychological tests and clinical interview , 92 subjects with subcortical ischemic vascular disease ( SIVD ) were recruited and classified into three groups according to their cognitive state, 29 patients with no cognitive impairment (NCI), 39 patients with vascular mild cognitive impairment ( VaMCI ) , and 24 patients with vascular dementia ( VaD ) . Their demographic , vascular risk factors and neuroimaging data were compared . Results The differences between the three groups in the number of subcortical white matter lacunar infarcts ( LI) as well as the scores of the white matter lesions ( WML) , medial temporal atrophy (MTA) and cerebral atrophy (CA) were statistically significant .Pairwised analysis showed that the numbers of LI within subcortical white matter in the VaMCI ( P<0.01 ) or VaD group ( P<0.05 ) were significantly higher than that in the NCI group .The scores of WML in the VaD group were significantly higher than that in the VaMCI or NCI group ( all P<0.01 ) .The scores of MTA in the VaD group were also significantly higher than that in the NCI group ( P<0.01 ) .The scores of CA in both groups of VaMCI and VaD were significantly higher than that in the NCI group (all P<0.05).The scores of WML, MTA, and CA, as well as the numbers of LI within subcortical white matter ( all P<0.05 ) and total LI numbers ( P<0.05 ) in patients with VCI were significantly higher than that in patients with NCI .Logistic regression analysis showed that low level of education , diabetes mellitus , the numbers of LI within subcortical white matter , the scores of MTA and CA were independent risk factors for s-VCI . Conclusion s-VCI is independently related to low level of education , diabetes mellitus , subcortical white matter injuries , as well as cortical and medial temporal atrophy .
Correlation Analysis of the Risk Factors for Subcortical Vascular Cognitive Impairment

Abstract:Objective Subcortical ischemic vascular disease (SIVD), characterised by multiple lacunar infarcts and white matter lesion (WML), has been considered as a major cause of subcortical vascular cognitive impairment (SVCI). The objective of the study is to identify the risk factors for SVCI in a group of SIVD patients who were registered in a stroke secondary prevention clinic. Methods Based on the detailed neuropsychological investigation and clinical interview, 61 cases that met the criteria of SIVD were recruited and classified as 3 groups of different cognitive state: no cognitive impairment (NCI), vascular cognitive impairment no dementia (VCI⁃ND) and vascular dementia (VaD). The latter two were further grouped as vascular cognitive impairment (VCI). Their demographic data, vascular risk factors and neuroimaging information were documented. Results 1) The total prevalence of VCI (VCI⁃ND and VaD) was 63.93% (39/61). The prevalence of VCI ⁃ ND and VaD were 36.07% (22/61) and 27.87% (17/61), respectively. 2) There were significant differences of age and educational years among NCI, VCI⁃ND and VaD groups (P = 0.032, 0.019). Paired comparison showed that VaD patients were older and less educated than NCI patients (P = 0.029, 0.022). The age difference between NCI and VCI groups nearly approached statistical significance (t = 3.740, P = 0.058), while the year of education of VCI was significantly lower than that of NCI (t = 7.888, P = 0.007). 3) There were significant differences of diabetes mellitus (DM) cases among NCI, VCI ⁃ ND and VaD groups (P = 0.037). There were more DM cases in VCI group than those in NCI group (P = 0.012), but no significant statistical difference was found among other vascular risk factors. 4) There was no difference of lacunar infarcts in different site of subcortical grey matter among these groups, respectively (P > 0.05, for all). Significant differences of subcortical white matter infarcts, WML scores and medial temporal atrophy (MTA) scores were found in patients in NCI, VCI⁃ND and VaD group (P = 0.020, 0.001, 0.000). The number of lacunar infarcts within white matter in VaD group was higher than that in NCI group (P = 0.027). The WML scores of VaD was much higher than that of VCI⁃ND and NCI (P = 0.001, 0.017). The MTA scores of VaD patients was much higher than that of VCI⁃ND and NCI patients (P = 0.001, 0.000). The number of white matter lacunar infarcts, the WML scores and the MTA scores of VCI was much higher than that of NCI (t = ⁃ 2.661, P = 0.003; t = ⁃ 2.953, P = 0.005; t = ⁃ 4.712, P = 0.000). 5) Logistic regression showed that DM, the number of white matter infarcts and MTA scores were independent risk factors for SVCI. Study of Followup CT Scans from Stroke to Vascular Dementia

Abstract:Objective: To study the pathogenesis of vascular dementia. Methods: Followup CT scans and IQ measurement in 167 first stroke patients had been performed for five years. All patients were devided into two groups-VD group which included 95 cases who evolved vascular dementia (VD) and control group which included 72 stroke patients without dementia. The side, location, volume and amount changes of the lesions, the area of the white matter lesions (leuco-araiosis) and the quantitative indexes of cerebral atrophy between the two groups were compared. Results: 56.89% of the patients evolved dementia and 50.17% of them occurred in two years after first onset. More lesions and larger volumes especially in subcortical lesions were observed in VD group. Most of the VD patients with unilateral lesions were changed to bilateral lesions. Subcortical cerebral atrophy was more serious in VD group. There was not significant difference statistically on leuco-araiosis area changes between the two groups. Conclusion: Vascular dementia after stroke occurs largely in two years after the first onset. The occurrence of VD correlates with the relapse of stroke, the lesion volume, amount and location changes from unilateral to bilateral or left lesion enlargement. Rapid subcortical cerebral atrophy is the secondary factor of VD development. Leuco-araiosis seems to be not related to it. The effective treatment of stroke in two years after the first onset will be conducive to reduce the occurrence of VD.
Multi-model MRI Study of Patients with Subcortical Vascular Mild Cognitive Impairment

Abstract:Objective To investigate the sensitive magnetic resonance imaging (MRI) indices for early cognitive impairment in patients with subcortical ischemic vascular disease (SIVD). Methods Thirty two SIVD patients were thoughtfully interviewed and evaluated by a set of neuropsychological tests. The patients were divided into no cognitive impairment (NCI), vascular mild cognition impairment (VaMCI) groups. The correlations between MRI indices and cognitive characteristics of these patients were assessed. Results The mean diffusivity (MD) values within whole brain white matter (WBWM) were (0.84±0.03)×10-9·m2·s-1 and (0.89±0.05) ×10-9·m2·s-1, while the MD values within normal appearing white matter (NAWM) were (0.87±0.04)×10-9·m2·s-1 and (0.92±0.05)×10-9·m2·s-1 respectively. There were significant differences between two groups (all P=0.003). The fractional anisotropy (FA) values within the WBWM were (0.37±0.02) and (0.35±0.02), while the FA within the NAWM were (0.35±0.02) and (0.33±0.02) respectively. There were also significant differences between two groups (P=0.002, P=0.001). The correlation coefficients of WBWM MD, NAWM MD and NAWM FA with attention-executive function were-0.401 (P=0.031),-0.422 (P=0.023) and 0.409 (P=0.027) respectively, while the correlation coefifcient of NAWM MD with memory function was -0.377 (P=0.044). Multi-factor stepwise regression analysis showed that the NAWM MD correlated negatively with attention-executive (β=-0.453, P=0.009), memory (β=-0.414, P=0.019) and general cognition (β=-0.410, P=0.02) independently. Conclusion Diffusion tensor imaging (DTI) can be regarded as an effective method for identiifcation of early cognitive impairment among SIVD patients. The MD value within NAWM decreased signiifcantly during early phase of cognitive impairment, and it correlated independently with manifestations of different cognitive domains, which suggested it may be regarded as a sensitive measurement of DTI.

Altered Functional Connectivity in Patients with Subcortical Vascular Cognitive Impairment-A Resting-State Functional Magnetic Resonance Imaging Study

Abstract:Recent neuroimaging studies have shown that people with subcortical vascular cognitive impairment (sVCI) have structural and functional abnormalities in the frontal lobe and subcortical brain sites. In this study, we used seed-based resting-state functional connectivity (rsFC) analysis and voxel-mirrored homotopic connectivity (VMHC) techniques to investigate the alteration of rsFC in patients with sVCI. rsFC and structural magnetic resonance images were acquired for 51 patients with subcortical cerebrovascular disease. All patients were subdivided based on cognitive status into 29 with sVCI and 22 controls; patient characteristics were matched. rsFC of the posterior cingulate cortex (PCC) and VMHC were calculated separately, and rsFC of the PCC and VMHC between the two groups were compared. The regions showing abnormal rsFC of the PCC or VMHC in sVCI patients were adopted as regions of interest for correlation analyses. Our results are as follows: The patients with sVCI exhibited increases in rsFC in the left middle temporal lobe, right inferior temporal lobe and left superior frontal gyrus, and significant decreases in rsFC of the left thalamus with the PCC. sVCI patients showed a significant deficit in VMHC between the bilateral lingual gyrus, putamen, and precentral gyrus. Additionally, the z-memory score was significantly positively associated with connectivity between the left thalamus and the PCC (r = 0.41, p = 0.03, uncorrected) in the sVCI group. Our findings suggest that the frontal lobe and subcortical brain sites play an important role in the pathogenesis of sVCI. Furthermore, rsFC between the left thalamus and the PCC might indicate the severity of sVCI.
The Neuropsychological Characteristics of Subcortical Ischemic Vascular Disease

Abstract:Objective To investigate cognitive and emotional charcteristics of subcortical ischemic vascular disease(SIVD).Methods Detailed clinical interviews,neuropsychological and psychiatric assessments were administrated on ischemic stroke patients who fulfilled the criteria of SIVD.Results Mean age of 50 patients was 70.3±7.7 years old,78% were male,20 patients(40%) were with mild SIVD while the rest 30 were with moderate to severe SIVD.Among 33 patients whom were diagnosed as vascular cognitive impairment(VCI),24(72.7%) were diagnosed as vascular cognitive impairment no dementia(V-CIND),11(33.3%)were with single cognitive domain deficit,and 22(66.7%)were with multi-domain deficits.Executive function and memory were the most prominently impaired domains.Neither language nor spatial function was impaired separately among them.13 VCI patients were depressed(39.3%) while 2 out of 17 SIVD patients with normal cognition were depressed(P=0.043).Delusions,hallucination,elation/euphoria and nighttime behavior disorders were only seen in vascular dementia patients.The proportion of cognitive impairment was significantly higher in patients with moderate to severe SIVD than those with mild SIVD(P=0.002).There was a tendency for more depression among patients with moderate to severe SIVD than those with mild SIVD(P=0.059).Conclusion Cognitive impairments,especially the V-CIND,were common in SIVD patients.Executive function and memory were the most vulnerable cognitive domains.Cognition impairment paralleled with depression among SIVD patients indicates that frontal cortical-subcortical circuit injury might be the mechanism underneath.

Abstract:In light of the tremendous number of patients with vascular dementia in China, it is of great significance for the treatment of this disease to summarize related research focuses. In this study, articles on the treatment of vascular dementia, which were included in CNKI and Web of Science from January 1, 2012 to December 31, 2021, were analyzed. Specifically, CiteSpace 5.7.R2 was employed to visualize nationalities of authors, author affiliations, authors, keywords, and journals, and dissect the status quo and trend of research on the treatment of this disease. On this basis, the research focuses and evolution were elucidated. The findings are expected to serve as reference for the future research. Finally, 2 579 Chinese articles and 453 English articles were included. The annual number of published articles showed an upward trend. Authors from China published most papers and England had the highest centrality value. HU Yue-qiang and LIU Cun-zhi respectively published the most Chinese and English articles. Guangxi University of Chinese Medicine and Capital Medical University respectively topped the author affiliations in the number of published Chinese and English articles. Among the English journals, Anal Biochem and Stroke separately boasted the highest centrality value and the highest cited frequency. The analysis of keywords in the Chinese articles suggested that most studies on the treatment of vascular dementia focused on the observation of patients' mobility after treatment. Moreover, as for the therapeutic method, western medicine, as well as the Chinese medicine and acupuncture frequently attracted the attention of scholars. Basic research highlighted the oxidative stress, angiogenesis, and apoptosis. According to the analysis result of keywords in English articles on treatment of vascular dementia, the focus was the improvement of the memory function of patients with vascular dementia. As to the therapeutic method, drug therapy was frequently studied compared with other methods. The basic research focused on autophagy, nerve regeneration, and oxidative stress. This study concludes that the future research trend might be the combination of Chinese and western medicine in the treatment of vascular dementia.

It is found that it is also present in the elderly with normal aging. Therefore, more and more attention has been paid to it. Selective vulnerability of hippocampal sub-regions in patients with subcortical vascular mild cognitive impairment

Abstract:Early diagnosis of subcortical vascular mild cognitive impairment (svMCI) is clinically essential because it is the most reversible subtype of all cognitive impairments. Since structural alterations of hippocampal sub-regions have been well studied in neurodegenerative diseases with pathophysiological cognitive impairments, we were eager to determine whether there is a selective vulnerability of hippocampal sub-fields in patients with svMCI. Our study included 34 svMCI patients and 34 normal controls (NCs), with analysis of T1 images and Montreal Cognitive Assessment (MoCA) scores. Gray matter volume (GMV) of hippocampal sub-regions was quantified and compared between the groups, adjusting for age, sex, and education. Additionally, we explored correlations between altered GMV in hippocampal sub-fields and MoCA scores in svMCI patients. Patients with svMCI exhibited selectively reduced GMV in several left hippocampal sub-regions, such as the hippocampal tail, hippocampal fissure, CA1 head, ML-HP head, CA4 head, and CA3 head, as well as decreased GMV in the right hippocampal tail. Specifically, GMV in the left CA3 head was inversely correlated with MoCA scores in svMCI patients. Our findings indicate that the atrophy pattern of patients with svMCI was predominantly located in the left hippocampal sub-regions. The left CA3 might be a crucial area underlying the distinct pathophysiological mechanisms of cognitive impairments with subcortical vascular origins.
P1‐424: Diffusion Tensor Imaging Correlates with Cognition Better Than Conventional MRI in Patients with Subcortical Ischemic Vascular Disease

Abstract:As a common cause of vascular cognitive impairment (VCI), subcortical ischemic vascular disease (SIVD) is defined primarily by lacunes and white matter lesions (WMLs) detected by conventional neuroimaging techniques. Diffusion tensor imaging (DTI), which made possible the in vivo assessment of white matter (WM) integrity, offers a powerful tool to study WMLs in diseased brains. The objectives of the study is to determine whether DTI is more sensitive on detecting cognitive deterioration in SIVD patients than conventional MRI and the neuropsychological correlations of these imaging indices respectively. 42 SIVD patients received a baseline evaluation included a complete socio-demographic and clinical (cognitive, behavioral, neurological, functional, and physical) data collection. They were also diagnosed as no cognitive impairment (NCI), VCI-no dementia (VCIND) and vascular dementia (VaD). Whole brain DTI and routine MRI was performed on these participants. After covarying for age and sex, there were significant differences of MD and FA in either whole brain WM or in normal appearance WM (NAWM) of cognitive impaired patients (VCI-ND, p < 0.01; VaD, p < 0.01) compared with NCI patients. After covarying for age, sex and education, there were significant correlations between DTI indices within whole brain WM and either of the attention/executive function or memory in SIVD subjects (p < 0.01). There were also significant correlations between these two domains and the DTI indices within NAWM (p < 0.01). Lacune numbers and T2 lesion volumes only correlated to attention/executive function (p < 0.05) while hippocampal volumes only correlated weakly to memory function (p = 0.05). But visual rating scores of WM lesions showed no correlation with either of the cognitive domains. After VaD patients were excluded from the analysis, correlations remained significant between these two cognition domains and most of the DTI measures. The DTI correlations with the cognitive measures remained significant even after covarying for T2 lesion volumes and hippocampal volumes. DTI correlates with cognition better than conventional MRI in patients with SIVD. Whole brain DTI histogram may be a reliable and sensitive technique for early detection of cognitive impairment in SIVD patients and may be also useful in monitoring disease progression in these patients.

Abstract:Flavonoids have been shown to improve cognitive function and delay the dementia progression. However, the underlying mechanisms remain elusive. In the present study, we examined the effect of Scutellaria baicalensis stem-leaf total flavonoids (SSTFs) extracted from S. baicalensis Georgi on spatial learning and memory in a vascular dementia (VaD) rat model and explored its molecular mechanisms. The VaD rats were developed by permanent bilateral occlusion of the common carotid artery. Seven days after recovery, the VaD rats were treated with either 50 or 100 mg/kg of SSTF for 60 days. The spatial learning and memory was evaluated in the Morris water maze (MWM) test. The tau hyperphosphorylation and the levels of the related protein kinases or phosphatases were examined by western blot analysis. In VaD rats, SSTF treatment at 100 mg/kg significantly reduced the escape latency in training trial in MWM test. In the probe trial, SSTF treatment increased the searching time and travel distance in the target quadrant. SSTF treatment inhibited the tau phosphorylation in both cortex and hippocampus in VaD rats. Meanwhile, SSTF reduced the activity of glycogen synthase kinase 3β and cyclin-dependent kinase 5 in VaD rats. In contrast, SSTF treatment increased the level of the protein phosphatase 2A subunit B in VaD rats. SSTF treatment significantly improved the spatial cognition in VaD rats. Our results suggest that SSTF may alleviate tau-hyperphosphorylation-induced neurotoxicity through coordinating the activity of kinases and phosphatase after a stroke. SSTF may be developed into promising novel therapeutics for VaD.

Abstract:ABSTRACT:Vascular cognitive impairment (VCI) encompasses a wide spectrum of cognitive disorders, ranging from mild cognitive impairment to vascular dementia. Its diagnosis relies on thorough clinical evaluations and neuroimaging. VCI predominately arises from vascular risk factors (VRFs) and cerebrovascular disease, either independently or in conjunction with neurodegeneration. Growing evidence underscores the prevalence of VRFs, highlighting their potential for early prediction of cognitive impairment and dementia in later life. The precise mechanisms linking vascular pathologies to cognitive deficits remain elusive. Chronic cerebrovascular pathology is the most common neuropathological feature of VCI, often interacting synergistically with neurodegenerative processes. Current research efforts are focused on developing and validating reliable biomarkers to unravel the etiology of vascular brain changes in VCI. The collaborative integration of these biomarkers into clinical practice, alongside routine incorporation into neuropathological assessments, presents a promising strategy for predicting and stratifying VCI. The cornerstone of VCI prevention remains the control of VRFs, which includes multi-domain lifestyle modifications. Identifying appropriate pharmacological approaches is also of paramount importance. In this review, we synthesize recent advancements in the field of VCI, including its definition, determinants of vascular risk, pathophysiology, neuroimaging and fluid-correlated biomarkers, predictive methodologies, and current intervention strategies. Increasingly evident is the notion that more rigorous research for VCI, which arises from a complex interplay of physiological events, is still needed to pave the way for better clinical outcomes and enhanced quality of life for affected individuals.

Abstract:Aneurysmal subarachnoid hemorrhage (aSAH) is a common disease causing vascular dementia. Survivors often suffer from cognitive impairment especially working memory deficit. Currently, lack of theoretical support limits the improvement of cognitive intervention or rehabilitation. It is unclear how the large-scale network differs and to what extent is the brain network affected? Our study aims to provide novel information about the topological characteristics of brain organization, especially “small-world” property. A total of 62 aSAH patients are enrolled in this study. They are divided into two groups according to the syndrome of working memory deficit. Their working memory function is evaluated by TMT-B and AVLT (Chinese version). Functional MRI scan is also performed for detecting resting-state cortical plasticity. We utilized ICA to extract functional sub-networks including working memory network from imaging data. And then we establish binarized network and calculate the small-worldness property as well as local and global efficiency of networks. aSAH group with working memory deficit shows no significant difference of clustering coefficient with control group. Our study discovered significant decrease of characteristic path length indicating an increase of overall routing efficiency. We reason that patients with working memory deficit have to recruit more neuronal resources and thus develops higher overall routing efficiency of local network. This study provides novel information about the neural alterations of aSAH patients with working memory deficit. It might contribute to the understanding of neural mechanism and the improvement of current intervention for vascular dementia.

Abstract:Vascular dementia(VD) is a common and frequently-occurring disease,which is influencing seriously the life quality of aged people.The authors analyzed and summarized the etiology and pathogenesis of VD and therapeutic progress in clinic based on scientific research achievements and literature about treatment of VD with Chinese medicine.They thought that the important factors in the development of VD included deficiency of zang-fu organs,disorder of qi and blood and phlegm-stasis obstructing orifices,and the key points included accumulation of phlegm-stasis,production of turbid-toxin and injury of collaterals and marrow,the therapeutic period was divided into early,middle and late stages or platform,wave and glide stages,TCM therapies for VD included tonifying kidney,activating blood circulation and resolving phlegm,tonifying kidney,tonifying qi and activating blood circulation,activating blood circulation and freeing calleterals,and activating blood circulation and resolving phlegm,and acupuncture therapies for VD included body acupuncture,integration of acupuncture and medicinal,eye acupuncture,ear acupoint pressing combining with moxibustion,and acupoint application with Chinese medicinal.In the future,the pathogenesis and effective therapies for vascular cognitive impairment no dementia(V-CIND) should be explored for preventing,diagnosing and intervening VD early besides improving the study on the diagnosis and treatment of VD.

Abstract:Vascular cognitive impairment (VCI) defines an entire spectrum of neurologic disorders from mild cognitive impairment to dementia caused by cerebral vascular disease. The pathogenesis of VCI includes ischemic factors (e.g., large vessel occlusion and small vessel dysfunction); hemorrhagic factors (e.g., intracerebral hemorrhage and subarachnoid hemorrhage); and other factors (combined with Alzheimer’s disease). Clinical evaluations of VCI mainly refer to neuropsychological testing and imaging assessments, including structural and functional neuroimaging, with different advantages. At present, the main treatment for VCI focuses on neurological protection, cerebral blood flow reconstruction, and neurological rehabilitation, such as pharmacological treatment, revascularization, and cognitive training. In this review, we discuss the pathogenesis, neuroimaging evaluation, and treatment of VCI.
