CT Diagnosis of Primary Pulmonary Sarcomatoid Carcinoma

Abstract:Purpose To investigate the CT characteristics of primary pulmonary sarcomatoid carcinoma,and to improve the knowledge of this disease.Materials and Methods The plain chest CT,enhanced CT scans of nine patients with pathologically confirmed primary pulmonary sarcomatoid carcinoma were analyzed retrospectively, and compared with pathological results.Results All nine lesions of nine patients were peripheral.Of the nine lesions,four were in right upper lobe,two in right lower lobe,two in left upper lobe,and one in right middle lobe.All of them had no clear boundaries,no calcification,and no air bronchogram.The maximum diameter of lesions ranged from 1.6 to 10.5 cm(mean 6.2 cm).Of the nine patients,two had lobulation,one had vacuole, two had pleura invasion and five had enlarged mediastinal lymph nodes.The lesions showed nonuniform plain chest CT density with a mean value of 33 Hu.On enhanced CT images,lesions exhibited a low to moderate degree of nonuniform enhancement,with an insignificantly enhanced central core and enhanced periphery in either irregular shape or thick rim.The mean CT value was 38 Hu.The enhancement ratio was 1.15 and the average no enhancement area ratio was approximately 63%.The pathological results revealed that six were spindle cell carcinoma and three were pleomorphic carcinoma.Conclusion No specific CT sign is found for primary pulmonary sarcomatoid carcinoma.While a large tumor size with either thick rim or irregular enhanced periphery on CT images may be used as a surrogate indicator,histopathological assessment is still the most reliable way to make the definitive diagnosis.
