Abstract:Diseases of the cardiovascular system, especially myocardial infarction remains a leading cause of morbidity and mortality. Autologous adult stem cells transplantation bring new hope of treatment for myocardial infarction. Up to present , these cells include skeletal muscle satellite cell, marrow stem cell, and endothelial precursor cell. Autologous cell transplantation avoid immunologic rejection and ethics related issues. Recent studies revealed that transplanted stem cells can survive in cardiac scar tissue with subsequent improved cardiac function. Further studies are necessary in order to understand the intergradation of implanted cells into host myocardium and to elucidate optimal cell number of cells obtained by proliferation, the proper delivery technique, and finally the influence on global cardiac function.

Abstract:Stem cell-based therapy is emerging as a promising strategy to treat end-stage heart failure, a leading cause of morbidity and mortality. Stem cells can be isolated from a variety of sources and exhibit unique characteristics that impact their potential therapeutic utility. The adult heart contains small populations of committed, multipotent cardiac stem cells (CSC), which are adapted to the cardiac microenvironment and participate in postnatal physiological and pathological cardiac renewal or repair. These cells can be isolated, expanded in culture, and administered therapeutically to improve cardiac function in the setting of heart failure. CSC can be differentiated into three distinct cardiovascular lineages and exhibit enhanced paracrine factor production and engraftment as compared with other types of mesenchymal stem cells, which in turn may translate into improved therapeutic efficacy. The cell surface marker expression and phenotype of these CSC, however, depends on the method of isolation, selection and propagation, which likely explains the variable experimental results obtained to date. Moreover, invasive procedures are required to obtain CSC from humans. Early trials using autologous CSC in patients with ischemic cardiomyopathy have demonstrated feasibility and safety, along with variable degrees of therapeutic efficacy in terms of enhancing myocardial viability and cardiac function. Further studies are needed to optimize methods of CSC isolation, manipulation and delivery. If fully realized, the potential of CSC therapy could fundamentally change the approach to the treatment of end-stage heart failure.

Abstract:The mesenchymal stem cell(MSC) has a powerful differentiation potential to differentiate into cardiomyocytes and vascular endothelial cells.This makes it possible for the myocardial and blood vessels to regenerate,myocardial structural to remodel,the myocardial contractile function and ventricular compliance to be,improved.Moreover,angiogenesis is promoted,effective coronary collateral circulation is established to improve the cardiac function,the blood supplement is increased and the decreasing of the cardiomyocytes number is stopped which is the ultimate reason leading to heart failure.The source of autologous transplantation is unlimited,getting materials is easy,the trauma is small,the metabolic activity is high and the rate of renewal is low.The cardiomyocytes differentiated from the MSC have an effective electro-mechanical coupling with the peripheral receptor cells.Together with transgenic technology MSC makes the target gene express easily in vivo and in vitro for a long time.There are no immune rejection,gene mutation or other safety problems after MSC transplantation.So it is a new way to treat myocardial infarction with autologous MSC transplantation and it has bright prospects.In this review,the development of autologous MSC transplantation in the treatment of myocardial infarction is summarized.

Abstract:BACKGROUND: Currently, interventional therapy can ameliorate myocardial ischemia, but necrotic myocardium cannot actively repair. Recently, stem cell transplantation has been the hot research in the treatment of ischemic heart diseases. Mesenchymal stem cells have been reconstructed with the characteristics of easy material collection, low immunologic rejection, multi-directional differentiation and no ethics problems. OBJECTIVE: To summarize the research progress of bone marrow mesenchymal stem cell transplantation for myocardial infarction. RETRIEVE STRATEGY: The Pubmed Database was undertaken to identify the relevant articles on bone marrow mesenchymal stem cell transplantation for myocardial infarction published from January 1990 to December 2007 with the key words of "Mesenchymal stem cells, transplantation,myocardial infarction" in English. Simultaneously, the Wanfang Database was also searched for relevant articles published from March 2002 to March 2007 with the same key words in Chinese. The data were primarily searched. Inclusive criteria included ①articles on bone marrow mesenchymal stem cell transplantation for myocardial infarction, and ②articles in the same circle published in the authoritative journals or recently published. Exclusive criteria included articles on repetitive studies. LITERATURE EVALUATION: The articles were randomized controlled trials on bone marrow mesenchymal stem cell transplantation for myocardial infarction. Of the 31 articles, 7 were review, and others were clinical or basic experimental studies. DATA SYNTHESIS: ①Bone marrow stem cells have higher plasticity, can move to the injured myocardium after myocardial infarction, differentiate into cardiomyocytes and vascular endothelial cells, and participate in the regeneration of necrotic myocardium. ②Clinical studies verified that bone marrow stem cell transplantation could improve heart function in patients with myocardial infarction. ③Studies on bone marrow mesenchymal stem cells in the repair of cartilage have made great progresses. But there are many problems, for example, the measurement of transplanted cell survival rate; the method of elevating survival rate; how large the infarcted area is to have an effective cellular transplantation; a technique that can label transplanted cells for a long term; the condition of successfully transplanted cells; the determination of optimal transplanted time and transplanted amount; improved transplanted methods, and so on. CONCLUSION: Studies on bone marrow mesenchymal stem cells improving heart function in patients with myocardial infarction, and cell transplantation methods have had great progresses, but many problems are still existed.

Abstract:OBJECTIVE:Some studies have indicated that stem cells transplantation therapy can replace necrotic myocardial cells,increase the number of functional myocardial cells,and build new blood vessels to improve blood supply and heart function.In this paper,we summarized the domestic development and achievement in this field in recent years.DATA SOURCES:Computer-based online search of Medline,CBM and CNKI databases was undertaken to identify related clinical(regardless of age and sex of subjects) and basic researches either in vivo or vitro dated from January 2001 to November 2006 with the Keywords of "stem cell,bone marrow,mesenchymal,transplantation" in English and "stem cell,transplantation,clinical research,myocardial infarction,heart failure" in Chinese.STUDY SELECTION:820 literatures were collected.All titles and most abstracts of articles were read through.Only articles ①related to stem cells transplantation therapy,②related to treatment of angiocardiopathy,and ③related to clinical treatment were selected.Review,repeated study and Meta analytical papers were excluded.DATA EXTRACTION:Totally 225 articles matched the inclusive criteria,and 595 were excluded.Of 225 articles,30 were analyzed involved 13 in English,and 19 in Chinese.One Chinese article was searched manually from supplement.DATA SYNTHESIS:①Since 2002,domestic researchers began to explore how to use stem cells transplantation to treat angiocardiopathy.②Nowadays,the majority of stem cells used in clinical practice are bone marrow mononuclear cells.Other kinds of stem cells including bone marrow mesenchymal stem cells,skeletal muscle stem cells,and peripheral blood stem cells are also applied in clinic.③Intracoronary injection is the main way in stem cell injection.It is mainly used to treat some clinical diseases such as acute and olden myocardial infarction and chronic cardiac insufficiency.CONCLUSION:Stem cells transplantation can replace necrotic myocardial cells,increase the number of functional myocardial cells,and build new blood vessels to improve the blood supply and heart function.Clinical researches have showed that it is one of the methods to deal with angiocardiopathy due to its safety and efficacy.

This study summarized the progress of stem cell transplantation in treating cardiovascular diseasesDATA SOURCES: A computer-based online search of Pubmed database was undertaken to identify English articles published from January 2000 to December 2005, the keywords were "stem cell transplantation, cardiovascular diseases".STUDY SELECTION: The articles were primarily checked, and the references of each literature were looked up. Inclusive criteria: articles correlated with the classification and the transplanted pathways of stem cells, the present status,problems and prospect of its basic and clinical application in treating cardiovascular diseases. Exclusive criteria: repetitive study or Meta analysis.DATA EXTRACTION: Totally 50 relative literatures were collected, and 31 of them accorded with the inclusive criteria,and the other 19 were excluded due to the old or repetitive contents. Of the 31 involved literatures, 19 were about the classification of stem cells used in cardiovascular system, 1 about the transplanted pathways of stem cells, 8 about the application of stem cells in treating cardiovascular diseases, and 3 about the existing problems and prospect.DATA SYNTHESIS: With the continuous improvement of living level and aging, cardiovascular diseases have become the main causes for death and disability in modern society. Stem cells have the potentials of multi-directional differentiation, and have become the main seed cells for cell replacement therapy, and provided important investigative and applicating field for the treatment of cardiovascular diseases. At present, there are many stem cells to be used in cardiovascular system, including embryonic stem cells, umbilical cord blood stem cells, peripheral blood stem cells, bone marrow stem cells, skeletal muscle satellite cells, myocardial stem cells, adipose tissue-derived stem cells, etc. The common pathways for stem cell transplantation are direct injection into myocardium, intracoronary artery injection and intravenous injection. Stem cell transplantation can treat cardiovascular diseases by promoting angiogenesis and myocardial regeneration, decreasing myocardial apoptosis and reducing myocardial collagen, then ameliorate the cardiac function, and breakthrough and progress have been made in both animal models and clinical studies. However, problems sill exist to be solved, and there is agreat distance to the routine clinical application.CONCLUSION: Stem cells are the important fields of the basic research and clinical application for preventing and treating cardiovascular diseases, thus it will provide powerful theoretical and practical supports for the more effective prevention and treatment of cardiovascular diseases to solving the key points and difficulties in cardiovascular diseases.

Abstract:Background: A variety of adult stem cells have been transplanted into the infarcted heart to cure myocardial infarction(MI), however, comparative studies are lacking to show more suitable source of cells for transplantation. Mesenchymal stem cells hold promise for myocardial regeneration therapy. Derivation of these cells from the endometrium tissue might be easier compared to bone marrow and adipose tissue. However,the in vivo fate of endometrium stem cells (EnSCs) in the infarcted heart has never been compared directly to mesenchymal cells derived from bone marrow(BMMSCs) and adipose tissue(AdMSCs). Methods: EnSCs, AdMSCs and BMMSCs were isolated from healthy donors were characterized using flow cytometry for surface markers identification and microscopy for cell morphology. They were characterized with β-actin promoter driving firefly luciferase and green fluorescent protein (Fluc-GFP) double fusion reporter gene, and were characterized using flow cytometry, bioluminescence imaging (BLI) and luminometry. Cell proliferation was tested by CCK-8 kit, colony forming unit(CFU) was stained by crystal violet staining and apoptosis ratio were detected by TUNEL assay. Rat (n=8/group) underwent myocardial infarction followed by intramyocardial injection of 5х105 EnSCs, AdMSCs and BMMSCs, or saline (negative control). Cell survival was measured using BLI for 6 weeks and cardiac function was monitored by echocardiography and hemodynamics analysis. Ventricular morphology was assessed using histology. Results: EnSCs, AdMSCs and BMMSCs were CD29+, CD90+, CD105+, shared similar morphology, but EnSCs had best proliferation, colony-forming and anti-apoptosis activity of 3 types of MSCs. Cells expressed Fluc reporter genes in a number-dependent fashion, as confirmed by luminometry. After cardiac transplantation, transplantation of EnSCs was better capable of preserving ventricular function and dimensions than others, as confirmed by echo test, PV-loops and histology. Conclusions: This is the first study comparing the in in vitro and in vivo behavior of 3 types of MSCs in the infarcted heart. AdMSCs and BMMSCs do not tolerate well in the cardiac environment, resulting in more cell death andworse cardiac function than EnSCs groups.

Abstract:BACKGROUND: Numerous studies have demonstrated that mesenchymal stem cells exhibit great potential in treatment of myocardial infarction. OBJECTIVE: To review the strategies of mesenchymal stem cell transplantation for treatment of myocardial infarction and evaluate the therapeutic effects after cell transplantation. METHODS: A computer-based online retrieval of PubMed, CNKI, CQVIP and FMJS databases was performed to search papers regarding therapeutic effects of mesenchymal stem cell transplantation with key words"mesenchymal stem cell; function, angiogenesis or survival" in Chinese and English. Papers describing different transplantation methods to enhance heart function recovery, angiogenesis and survivalrate after mesenchymal stem cell transplantation were searched. In the same research field, papers published recently or in high-impact journals were selected. A total of 220 papers were acquired in the initial retrieval. According to inclusion criteria, 20 papers were suitable for final analysis. RESULTS AND CONCLUSION: Stem cell in vitro expansion and differentiation followed by transplantation have emerged as a promising alternative therapeutic approach for cardiovascular disease, but existing strategies are restricted by low cell engraftment, differentiation and survival. In addition, it is also important that reasonable methods should be adopted to accurately assess transplantation efficacy for determining later transplantation strategies. Heart function reconstruction, myocardial fibrosis inhibition and angiogenesis are mainly used as evaluation criteria. From the global to the local function and from the right single imaging to "multimodality noninvasive imaging" are two new developments. How to choose appropriate transplantation strategies and efficacy evaluation method to enhance the efficacy of mesenchymal stem cell therapy needs further studies.

Abstract:Heart failure is still the main complication affecting the prognosis of acute myocardial infarction (AMI), and mesenchymal stem cells (MSCs) are an effective treatment to replace necrotic myocardium and improve cardiac functioning. However, the transplant survival rate of MSCs still presents challenges. In this review, the biological characteristics of MSCs, the progress of mechanism research in the treatment of myocardial infarction, and the advances in improving the transplant survival rate of MSCs in the replacement of necrotic myocardial infarction are systematically described. From a basic to advanced clinical research, MSC transplants have evolved from a pure injection, an exosome injection, the genetic modification of MSCs prior to injection to the cardiac tissue engineering of MSC patch grafting. This study shows that MSCs have wide clinical applications in the treatment of AMI, suggesting improved myocardial tissue creation. A broader clinical application prospect will be explored and developed to improve the survival rate of MSC transplants and myocardial vascularization.

Abstract:Heart failure (HF) after myocardial infarction (MI) represents a worldwide public health issue with gradually increasing incidence. HF may eventually affect all organs and tissues and is associated with significant morbidity and mortality. Current therapeutic methods to treat HF stay at only delaying the disease progression without repair and regeneration. Recently, cell transplantation has gradually become the most promising treatment for the damaged myocardial tissue. Meanwhile, the bone marrow-derived mesenchymal stem cells (MSCs) are the most used cells for the treatment of HF. In the present review, we summarize the current literature on MSCs and their potential use to treat HF after MI. So far, MSCs have been demonstrated beneficial competence in animal studies and clinical trials, which may open novel avenues for further clinical studies.

Abstract:<p>Cardiovascular disease (CVD) is one of the leading causes of death worldwide. Currently, many methods have been proposed by researchers for the prevention and treatment of CVD; among them, stem cell-based therapies are the most promising. As the cells of origin for various mature cells, stem cells have the ability to self-renew and differentiate. Stem cells have a powerful ability to regenerate biologically, self-repair, and enhance damaged functional tissues or organs. Allogeneic stem cells and somatic stem cells are two types of cells that can be used for cardiac repair. Theoretically, dilated cardiomyopathy and acute myocardial infarction can be treated with such cells. In addition, stem cell transplantation procedures, including intravenous, epicardial, cardiac, and endocardial injections, have been reported to provide significant benefits in clinical practice; however, there are still a number of issues that need further study and consideration, such as the form and quantity of transplanted cells and post-transplantation health. The goal of this analysis was to summarize the recent advances in stem cell-based therapies and their efficacy in cardiovascular regenerative medicine.</p>

Abstract:Therapy with mesenchymal stem cells is one of the promising tools to improve outcomes after myocardial infarction. Adipose-derived stem cells (ASCs) are an ideal source of mesenchymal stem cells due to their abundance and ease of preparation. Studies in animal models of myocardial infarction have demonstrated the ability of injected ASCs to engraft and differentiate into cardiomyocytes and vasculature cells. ASCs secrete a wide array of angiogenic and anti-apoptotic paracrine factors such as vascular endothelial growth factor, hepatocyte growth factor, and insulin-like growth factor 1. ASCs are capable of enhancing heart function, reducing myocardial infarction, promoting vascularization, and reversing remodeling in the ischemically injured hearts. Furthermore, several ongoing clinical trials using ASCs are producing promising results for heart diseases. This article reviews the isolation, differentiation, immunoregulatory properties, mechanisms of action, animal models, and ongoing clinical trials of ASCs for cardiac disease.
Progress on Embryonic Stem Cells for the Treatment of Myocardial Infarction

Abstract:Embryonic stem cells are pluripotent cells derived from blastocyst-stage embryos. They are characterized by their capacity for self-renewal and by the ability to differentiate into a variety of cell types. In vitro, ES cells can spontaneously aggregate into the formation of embryoid bodies (EBs) comprised of a range of differentiated cells. When injected into severe combined immunodeficient (SCID) mice, ES cells will induce the generation of teratomas, which include differentiated cells from three embryonic germ layers. We can induce the differentiation of ES cells into cardiocytes by treatment with several growth factors or co-culture with certain other cells. The pure differentiated cardiocytes from ES cells are then selected and implanted into the infarcted hearts. The hearts' function will be improved. ES cells implantation is a novel and potential method to treat infarcted hearts.

Abstract:Ischemic heart diseases (IHDs) remain major public health problems with high rates of morbidity and mortality worldwide. Despite significant advances, current therapeutic approaches are unable to rescue the extensive and irreversible loss of cardiomyocytes caused by severe ischemia. Over the past 16 years, stem cell-based therapy has been recognized as an innovative strategy for cardiac repair/regeneration and functional recovery after IHDs. Although substantial preclinical animal studies using a variety of stem/progenitor cells have shown promising results, there is a tremendous degree of skepticism in the clinical community as many stem cell trials do not confer any beneficial effects. How to accelerate stem cell-based therapy toward successful clinical application attracts considerate attention. However, many important issues need to be fully addressed. In this Review, we have described and compared the effects of different types of stem cells with their dose, delivery routes, and timing that have been routinely tested in recent preclinical and clinical findings. We have also discussed the potential mechanisms of action of stem cells, and explored the role and underlying regulatory components of stem cell-derived secretomes/exosomes in myocardial repair. Furthermore, we have critically reviewed the different strategies for optimizing both donor stem cells and the target cardiac microenvironments to enhance the engraftment and efficacy of stem cells, highlighting their clinical translatability and potential limitation.

Abstract:Cardiovascular disease remains the leading cause of death and disability in advanced countries. We describe (a) the current status of clinical trials of human pluripotent stem cells (hPSCs) compared with clinical trials of human adult or fetal stem cells, (b) the gap between fundamental research and application of human stem cells, (c) the use of biomaterials in clinical and pre-clinical studies of stem cells, and finally (d) trends in bioengineering to promote stem cell therapies for patients with myocardial infarction. We explain why the number of clinical trials using hPSCs is so limited compared with clinical trials using human adult and fetal stem cells such as bone marrow-derived stem cells.

Abstract:With the discovery of cardiac stem cell,the conception of the heart considered to be a terminally differentiated organ was changed.Cardiac stem cells possess the common characteristics of self-renew,clone formation and differentiating into cardiomyocyte,smooth muscle cell,and endothelial cell.Because of the properties of tissue specificity and lineage commitment,cardiac stem cells are considered to have great advantages over other stem cells in the treatment of cardiovascular disease.However,the low rate of engraftment still remains a problem to be solved.In recent years,people attempted to combine stem cell therapy with other ways,such as tissue engineering,gene therapy,exosome therapy,to cure cardiovascular diseases,aiming at finding better ways to treat the cardiovascular disease.This article is mainly for the reviewing of the mechanisms underlying the stem cell therapy and the combinatory use of new technology emerged these years.
