Detection of Occult Carcinomatous Diffusion in Lymph Nodes from Laryngeal Squamous Cell Carcinoma

Abstract:Objective To evaluate the micrometastases in cervical lymph nodes from laryngeal squamous cell carcinoma based on real-time quantitative reverse transcriptional polymerase chain reaction (real-time qRT-PCR) assay,and to find sensitive and reliable tumor markers.MethodsA total of 563 lymph nodes (77 lymph node levels) from 21 patients were prospectively evaluated by hematoxylin-eosin (HE) staining,immunohistochemistry (IHC) and real-time qRT-PCR assay. Amplification of cytokeratin 19 (CK19),squamous cell carcinoma antigen (SCCA),pemphigus vulgaris antigen (PVA) mRNA transcripts using real-time qRT-PCR was used to quantify the cervical micrometastatic burden. ResultsThe cervical lymph node metastatic rates determined by three markers were significantly higher than those by routine HE staining (P0.01). The expressions of the three markers in HE positively stained specimens were significantly higher than those in HE negatively stained ones (P0.01). There were no significant distinctions between the results determined by IHC and routine HE staining (P0.05). ConclusionsIHC has a similar efficiency in detection of micrometastases with HE staining. Detection of CK19,SCCA and PVA using real-time qRT-PCR in cervical lymph nodes from laryngeal squamous cell carcinoma can raise the sensibility and specificity of micrometastases detection.
