Vancomycin concentration in CSF and serum in different administration approaches and comparison of the effect

Abstract:Objective To investigate the effect and safety of vancomycin intrathecal administration for therapy gram-positive bacterial meningitis. Methods In our study, twenty six adult patients with gram-positive bacterial meningitis were randomly divided into two groups:therapy group(13 cases)and controlled group(13 cases). Vancomycin was administered intravenously and intrathecally in therapy group and only intravenously in controlled group. The samples of serum and CSF were collected, the concentration of vancomycin in them was determined by fluorescence polarization immunoassay (FPIA) method. Results In therapy group, the concentration of vancomycin in CSF was higher than controlled group and the treatment time was shorter. There was significant difference between the two groups(P<0.01, P<0.01), and no adverse effect except in one patient with "Red man syndrome" in the period of administration and following up 3 to 12 months. Conclusions The results of our study demonstrate that intrathecal vancomycin application is possible to be a safe and efficacious treatment modality in gram-positive bacterial meningitis, with several times vancomycin levels than Minimum Inhibitory Concentration(MIC), with much shorter treatment time and lower costs.
