The Relationship Between Local Myocardial Viability and Long Term Adverse Cardiovascular Events after Coronary Artery Bypass Graft

Abstract:Objective:To explore the relationship between local myocardial viability in left ventricle and long term adverse cardiovascular events after coronary artery bypass graft(CABG).Methods:A total of 709 patients with coronary heart disease who underwent CABG in our hospital from January 1999 to December 2005 were included in this study.All patients were examined with SPECT for the myocardial viability prior the operation,post operative follow up was conducted in all patients.The patients died in the hospital were excluded.The semi quantitative scores of myocardial perfusion and metabolism according to nuclear medicine prior the operation with 17 segments in left ventricle were calculated to obtain the quantified results.Those results were further used to demand the perfusion and survival in 5 different positions in left ventricle as anterior wall,lateral wall,partition,inferoposterior wall and cardiac apex.COX multivariate analysis was conducted to explore the relationship between the myocardial perfusion/survival and the long term adverse cardiovascular events.Results:The mean follow up time was(3.43±2.42)years.COX multivariate analysis indicated that the myocardial viability in left ventricular apex and lateral wall were the independent protective factors for long term cardiac death,the myocardial viability in left ventricular anterior and lateral wall were the independent protective factors for long term readmission,and the myocardial viability of anterior and lateral wall were the independent protective factors for combined end events.Conclusion:The myocardial viability in left ventricular anterior wall,cardiac apex and lateral wall were the important independent factors influencing the long term prognosis of CABG.Revascularization in anterior wall and in lateral wall were important for the patients with myocardial infarction in the anterior wall,this was meaningful for long term prognosis of CABG.
