Abstract:This paper introduced professor DING Xue-ping's clinical experience in the treatment of obesity with hyperinsulinemia from the traditional Chinese medicine.Professor DING pointed out that obesity disease was related to spleen and kidney.The occurrence of obesity with hypednsulinemia was closely related to qi deficiency and blood stasis and phlegm stagnation.At the beginning of the disease,we should focus on removing the pathological factors.With the development of disease,we should obey the rules of reducing the excess and reinforcing the deficiency.Last but not least,it was very important to promote blood circulation for removing blood stasis and eliminate dampness and phlegm.Professor DING pay attention to the change of dampness,phlegm,turbidity,water and drink.Meanwhile he attached importance to the dial up triple burner and the lung regulating visceral activities.Professor DING emphasized that the treatment based on syndrome differentiation should be paid attention to cold,hot,weak and strong,and be combined with multiple therapies.

Abstract:Objective:To analyze the "disease-syndrome-treatment-medicine" experience of professor DING for diabetic peripheral neuropathy by using "Traditional Chinese Medicine(TCM) inheritance support system(V2.5)".By analyzing the data,we summarized the Professor DING's academic experience.Methods:Through collecting the standard clinical cases,combined with the software analysis system,we analyzed the composing principles by using datamining methods such as frequency statistics,revised mutual information,association rules analysis,cluster analysis,improvement of mutual information,complex system entropy clustering,unsupervised hierarchical clustering entropy and so on.Results:Characteristics of diabetic peripheral neuropathy:Based on the analysis of 31 cases of diabetic peripheral neuropathy internal wind syndrome.The highest-frequency used insect drugs were Scorpion,bombyx batryticatus,cicada slough.The highest frequency used drug combinations were folium mori and the root bark of white mulberry,scorpion and bombyx batryticatus,rhizoma coptidis,folium mori and the root bark of white mulberry,radices rehmanniae,bombyx batryticatus and scorpion and so on.Conclusion:Treating diabetic peripheral neuropathy focuses on wind.Extinguishing wind and dredging collaterals is important.

Abstract:In recent decades,with the prevalence of diabetes,more researches were carried out on Type 2 Diabetes Mellitus(T2DM) in China as well as the world.Modern Western medicine opened up a new era of the therapy of T2DM with high tech aiding,in which is developed from the cellular ultra structure of human body to the imperceptible changes of molecular biology.One of the milestone on the diabetic research is using of GLP-1 analogues in the treatment of diabetes.Traditional Chinese medicine(TCM) has a long history on the research of diabetes,at least 3500 years from the written records,revealing the process of the T2DM and the differentiation of symptoms and signs.With the developing of research of TCM on syndrome types of T2DM,we have found that there is a certain relationship between the diabetes TCM syndrome types and the lab data,particularly GLP-1 level.We believe that the physiological and pathogenic roles of GLP-1 are strongly associated to the TCM syndrome of damp heat of spleen and stomach of diabetes.

Abstract:Professor PENG Wannian is good at using classical prescriptions in the diagnosis and treatment of diabetic and miscellaneous problems under the six meridians syndrome differentiation system. The professor attached great importance to strengthening the spleen and kidney, suggesting that blood stasis changes throughout diabetes. If the diabetic "Sanxiao" symptoms are typical, professor PENG often uses Baihu Rensheng decoction. In addition, Professor likes to use the Xuanfu Daizhe decoction and Guizhi Shaoyao decoction to treat diabetic patients with gastrointestinal diseases, uses Xiaochaihu decoction to regulate mood and sleep disorders in diabetic patients, uses Danggui Sini decoction to treat diabetic peripheral neuropathy,and uses Shenqi Pill and Zhenwu decoction in treating diabetic nephropathy, Tianqi and Sini powder often through each period of treatment of diabetes.

Abstract:Diabetes is a major medical problem that imperils public health. Over two thousand years ago, Traditional Chinese Medicine (TCM) called diabetes-related symptoms "Xiaoke" disease. In ancient China, TCM and Chinese herbal medicines were used widely in treating Xiaoke and abundant experience has been accumulated. This article discusses the TCM theory on diabetes and its achievements in the prevention and treatment of diabetes in the past. Using Chinese herbal medicine, recent progress in diabetes therapeutics, including data from clinical trials, are presented. Mechanistic studies from basic research are discussed. Yin-yang balance and a holistic approach of TCM may complement diabetes treatment in Western medicine. With continuous efforts, TCM could play a more important role in fighting this disease.

Abstract:Diabetes is a widespread chronic disease that occurs mainly in the elderly population. Due to the difference in pathophysiology between elderly and young patients, the current clinical practice to treat elderly patients with anti-diabetes medications still faces some challenges and dilemmas, such as the urgent need for early diagnosis and prevention, and an imbalance between restricted dietary intake and the risk of undernutrition. Traditional Chinese medicine (TCM) offers various treatment regimens that are actively utilized in the field of diabetes management. Through multiple targets and multiple pathways, TCM formulas, medicinal herbs, and active natural products enhance the efficacy of diabetes prevention and diabetes control measures, simplify complex medication management, and improve common symptoms and common diabetic complications in elderly people. Historically, natural products have played a key role in material composition analysis of TCM and mechanism interpretation to enable drug discovery. However, there have been few conclusions on this topic. This review summarizes the development of TCM for the prevention and management of diabetes in elderly people, existing evidence-based clinical practices, and prospects for future development.

Abstract:Diabetes mellitus (DM) is a chronic metabolic disorder characterized by hyperglycemia resulting from insulin secretion defects or insulin resistance. The global incidence of DM has been gradually increasing due to improvements in living standards and changes in dietary habits, making it a major non-communicable disease that poses a significant threat to human health and life. The pathogenesis of DM remains incompletely understood till now, and current pharmacotherapeutic interventions are largely inadequate, resulting in relapses and severe adverse reactions. Although DM is not explicitly mentioned in traditional Chinese medicine (TCM) theory and clinical practice, it is often classified as "Xiaoke" due to similarities in etiology, pathogenesis, and symptoms. With its overall regulation, multiple targets, and personalized medication approach, TCM treatment can effectively alleviate the clinical manifestations of DM and prevent or treat its complications. Furthermore, TCM exhibits desirable therapeutic effects with minimal side effects and a favorable safety profile. This paper provides a comprehensive comparison and contrast of Xiaoke and DM by examining the involvement of TCM in their etiology, pathogenesis, treatment guidelines, and other relevant aspects based on classical literature and research reports. The current TCM experimental research on the treatment of DM by lowering blood glucose levels also be generalized. This innovative focus not only illuminates the role of TCM in DM treatment, but also underscores the potential of TCM in DM management.

Abstract:Diabetes is manifested as polydipsia,polyphagia and urorrhagia.It is termed as the up-per,middle and lower Jiao in TCM.The disease can be combined with loss of weight,turbid and sweet-flavor urine.It is believed in TCM that poor emotions lead to fire accumulation and consumption of yin,improper di-et results in heat accumulation and consumption of body fluids,congenital insufficiency results in weakness of five zang organs,sexual indulgence leads to consumption of kidney essence,and the indulgence of pungent and dry food consumes and damages yin.Prof.Duan Guijin,the national master of TCM,thinks that the treat-ment of diabetes should focus on the disease itself and its complications.The holistic view and pathogenesis of the disease are the principles.The main syndromes include qi and yin deficiency,dry heat in the lung and stomach and yin and yang deficiency,of which,qi and yin deficiency is the most common.The satisfactory effects are only obtained by the individual differentiation and corresponding treatment of the disorders.

Abstract:Diabetes mellitus has become a major public health issue globally, putting an enormous burden on global health systems and people. Among all diseased groups, a considerable part of patients are elderly, while their clinical features, pathogenic processes, and medication regimens are different from patients of other ages. Despite the availability of multiple therapies and techniques, there are still numerous elderly diabetes patients suffering from poor blood glucose control, severe complications, and drug adverse effects, which negatively affect the quality of life in their golden years. Traditional Chinese Medicine (TCM) has been widely used in the treatment of diabetes for several decades, and its relevant clinical practice has confirmed that it has a satisfactory effect on alleviating clinical symptoms and mitigating the progression of complications. Chinese herbal medicine and its active components were used widely with obvious clinical advantages by multiple targets and signaling pathways. However, due to the particular features of elderly diabetes, few studies were conducted to explore Traditional Chinese Medicine intervention on elderly diabetic patients. This study reviews the research on clinical features, pathogenic processes, treatment principles, and TCM treatments, hoping to provide fresh perspectives on the prevention and management strategies for elderly diabetes.

Abstract:Background . Diabetes mellitus (DM) belongs to the progressive and irreversible disease. With the development of the disease, the function of beta cells declines significantly. Current treatments cannot reverse the course of the disease. The role of traditional Chinese medicine (TCM) in the DM treatment has been recognized widely, while there are few long-term observation reports. In this study, we introduced a case of DM treated by classical prescription alone for 10 years, which would provide the reference for clinical practice. Case Presentation . A 64-year-old male complained of a 2-month history of dry mouth, frequent urination, and blurred vision and found increased blood glucose for 3 days. The patient’s glycated hemoglobin was 14.2%, fasting plasma glucose fluctuated at 12-15 mmol/L, and urinary albumin excretion rate (UAER) was 32.9 μ g/min. The male was treated with 10 years of continuous classical prescription alone. After 3 months of TCM treatment, the patient’s blood glucose level decreased significantly and blurred vision symptoms improved. With continued TCM treatment, his UAER normalized. Subsequently, he continued outpatient consultation, and his TCM prescription was adjusted according to clinical symptoms. After 10 years of continuous TCM treatment, his blood glucose remained stable, urinary microalbumin quantitation showed no abnormalities, and blurred vision disappeared. Conclusions . This case provides specific treatment plans and effective references for long-term control of blood glucose, prevention and treatment of diabetes complications, delay of disease progression, and protection of impaired islet function in the treatment of diabetes with TCM. TCM may become a meaningful alternative DM treatment in the future.

Abstract:Diabetic nephropathy is a common complication of diabetes, Chinese medicine treatment has its advantages, the authors distinguish the diabetic nephropathy which due to type 1 diabetes or type 2 diabetes, respectively spleen hate nephropathy and consumer hate nephropathy. And the pathogenesis of them in the development process is different, the spleen hate nephropathy dues to stagnation, hot, and weakness which lead to vascular injury; the consumer hate nephropathy results from deficiency of qi and yin. In the early days, both of them have different pathogenesis and performance, so they should have different treatment, however, to the late stage they have the same pathogenesis. In short, diabetic nephropathy, the core pathogenesis is deficiency, stasis, damage; principles of treatment are both specimens and virtual treatment, treatments are complement deficiency, activating blood circulation, wenyang xiezhuo; prescriptions are from Huangqijianzhong Decoction, Dangguibuxue Decoction, Didang Decoction, Zhenwu Decoction and so on.

Abstract:The prevalence of diabetes is reaching epidemic proportions worldwide.Elevated glucose levels and associated increases in blood pressure and dyslipidemia lead to long-term complications including cardiovascular disease,diabetic retinopathy,kidney disease,and nervous system damage.These complications are largely responsible for the great increase in morbidity and mortality seen in diabetes.Unfortunately,once established,diabetes is very diffi cult to treat,and it is currently exerting tremendous pressure on healthcare systems worldwide.For these reasons,diabetes prevention has become a key target for clinicians,patients and policymakers,and a number of potential strategies are under study.The purpose of this article is fi rst to discuss the defi ciency of current preventing strategies,and the potential role and the advantages of Traditional Chinese Medicine(TCM) to prevent diabetes.Following that,based on observational studies as well as on prospective trials,and with respect to the prevention and treatment of diabetes,the authors,therefore,suggest that the best way to approach prevention is through lifestyle modification guided by TCM theory.And those interventions should include not only herbs,but acupuncture,massage,food therapy and mind-body exercise,and they must examine cost-effectiveness as well.Future studies should also be designed with using such interventions and should be of suff icient duration to differentiate between genuine diabetes prevention as opposed to simple delay or masking of the disease.
