Abstract:This paper introduced professor DING Xue-ping's clinical experience in the treatment of obesity with hyperinsulinemia from the traditional Chinese medicine.Professor DING pointed out that obesity disease was related to spleen and kidney.The occurrence of obesity with hypednsulinemia was closely related to qi deficiency and blood stasis and phlegm stagnation.At the beginning of the disease,we should focus on removing the pathological factors.With the development of disease,we should obey the rules of reducing the excess and reinforcing the deficiency.Last but not least,it was very important to promote blood circulation for removing blood stasis and eliminate dampness and phlegm.Professor DING pay attention to the change of dampness,phlegm,turbidity,water and drink.Meanwhile he attached importance to the dial up triple burner and the lung regulating visceral activities.Professor DING emphasized that the treatment based on syndrome differentiation should be paid attention to cold,hot,weak and strong,and be combined with multiple therapies.

Abstract:Objective:To analyze the "disease-syndrome-treatment-medicine" experience of professor DING for diabetic peripheral neuropathy by using "Traditional Chinese Medicine(TCM) inheritance support system(V2.5)".By analyzing the data,we summarized the Professor DING's academic experience.Methods:Through collecting the standard clinical cases,combined with the software analysis system,we analyzed the composing principles by using datamining methods such as frequency statistics,revised mutual information,association rules analysis,cluster analysis,improvement of mutual information,complex system entropy clustering,unsupervised hierarchical clustering entropy and so on.Results:Characteristics of diabetic peripheral neuropathy:Based on the analysis of 31 cases of diabetic peripheral neuropathy internal wind syndrome.The highest-frequency used insect drugs were Scorpion,bombyx batryticatus,cicada slough.The highest frequency used drug combinations were folium mori and the root bark of white mulberry,scorpion and bombyx batryticatus,rhizoma coptidis,folium mori and the root bark of white mulberry,radices rehmanniae,bombyx batryticatus and scorpion and so on.Conclusion:Treating diabetic peripheral neuropathy focuses on wind.Extinguishing wind and dredging collaterals is important.

Abstract:In recent decades,with the prevalence of diabetes,more researches were carried out on Type 2 Diabetes Mellitus(T2DM) in China as well as the world.Modern Western medicine opened up a new era of the therapy of T2DM with high tech aiding,in which is developed from the cellular ultra structure of human body to the imperceptible changes of molecular biology.One of the milestone on the diabetic research is using of GLP-1 analogues in the treatment of diabetes.Traditional Chinese medicine(TCM) has a long history on the research of diabetes,at least 3500 years from the written records,revealing the process of the T2DM and the differentiation of symptoms and signs.With the developing of research of TCM on syndrome types of T2DM,we have found that there is a certain relationship between the diabetes TCM syndrome types and the lab data,particularly GLP-1 level.We believe that the physiological and pathogenic roles of GLP-1 are strongly associated to the TCM syndrome of damp heat of spleen and stomach of diabetes.

Abstract:Professor PENG Wannian is good at using classical prescriptions in the diagnosis and treatment of diabetic and miscellaneous problems under the six meridians syndrome differentiation system. The professor attached great importance to strengthening the spleen and kidney, suggesting that blood stasis changes throughout diabetes. If the diabetic "Sanxiao" symptoms are typical, professor PENG often uses Baihu Rensheng decoction. Abstract:Diabetes is a major medical problem that imperils public health. Over two thousand years ago, Traditional Chinese Medicine (TCM) called diabetes-related symptoms "Xiaoke" disease. In ancient China, TCM and Chinese herbal medicines were used widely in treating Xiaoke and abundant experience has been accumulated. This article discusses the TCM theory on diabetes and its achievements in the prevention and treatment of diabetes in the past. Using Chinese herbal medicine, recent progress in diabetes therapeutics, including data from clinical trials, are presented. Mechanistic studies from basic research are discussed. Yin-yang balance and a holistic approach of TCM may complement diabetes treatment in Western medicine. With continuous efforts, TCM could play a more important role in fighting this disease.

Abstract:The mechanism of type 2 diabetes on traditional Chinese herbal therapy is unclear recently.The recent concept of the mechanism for diabetes indicates that it is a multiple-organ disease,which involves pathogenesis of the pancreas,adipose tissue,liver,gastrointestinal hormones,muscle and nervous system.The nervous system plays a key role on symphony of glucose metabolism of other organs,and controls the neuroendocrine system.Our recent studies indicate that some of Chinese herbs(eg.Huangqi)could affect the neuronal activity in the nervous system and modulate glucose metabolism via neuroencrine system.The study of the role of Chinese herbs on the nervous system may provide an interface between the modern diabetic research and traditional Chinese herbal therapy.

Abstract:Diabetes is a widespread chronic disease that occurs mainly in the elderly population. Due to the difference in pathophysiology between elderly and young patients, the current clinical practice to treat elderly patients with anti-diabetes medications still faces some challenges and dilemmas, such as the urgent need for early diagnosis and prevention, and an imbalance between restricted dietary intake and the risk of undernutrition. Traditional Chinese medicine (TCM) offers various treatment regimens that are actively utilized in the field of diabetes management. Through multiple targets and multiple pathways, TCM formulas, medicinal herbs, and active natural products enhance the efficacy of diabetes prevention and diabetes control measures, simplify complex medication management, and improve common symptoms and common diabetic complications in elderly people. Historically, natural products have played a key role in material composition analysis of TCM and mechanism interpretation to enable drug discovery. However, there have been few conclusions on this topic. This review summarizes the development of TCM for the prevention and management of diabetes in elderly people, existing evidence-based clinical practices, and prospects for future development.

Abstract:Diabetes mellitus (DM) is a chronic metabolic disorder characterized by hyperglycemia resulting from insulin secretion defects or insulin resistance. The global incidence of DM has been gradually increasing due to improvements in living standards and changes in dietary habits, making it a major non-communicable disease that poses a significant threat to human health and life. The pathogenesis of DM remains incompletely understood till now, and current pharmacotherapeutic interventions are largely inadequate, resulting in relapses and severe adverse reactions. Although DM is not explicitly mentioned in traditional Chinese medicine (TCM) theory and clinical practice, it is often classified as "Xiaoke" due to similarities in etiology, pathogenesis, and symptoms. With its overall regulation, multiple targets, and personalized medication approach, TCM treatment can effectively alleviate the clinical manifestations of DM and prevent or treat its complications. Furthermore, TCM exhibits desirable therapeutic effects with minimal side effects and a favorable safety profile. This paper provides a comprehensive comparison and contrast of Xiaoke and DM by examining the involvement of TCM in their etiology, pathogenesis, treatment guidelines, and other relevant aspects based on classical literature and research reports. The current TCM experimental research on the treatment of DM by lowering blood glucose levels also be generalized. This innovative focus not only illuminates the role of TCM in DM treatment, but also underscores the potential of TCM in DM management.

Abstract:Diabetes mellitus has become a major public health issue globally, putting an enormous burden on global health systems and people. Among all diseased groups, a considerable part of patients are elderly, while their clinical features, pathogenic processes, and medication regimens are different from patients of other ages. Despite the availability of multiple therapies and techniques, there are still numerous elderly diabetes patients suffering from poor blood glucose control, severe complications, and drug adverse effects, which negatively affect the quality of life in their golden years. Traditional Chinese Medicine (TCM) has been widely used in the treatment of diabetes for several decades, and its relevant clinical practice has confirmed that it has a satisfactory effect on alleviating clinical symptoms and mitigating the progression of complications. Chinese herbal medicine and its active components were used widely with obvious clinical advantages by multiple targets and signaling pathways. However, due to the particular features of elderly diabetes, few studies were conducted to explore Traditional Chinese Medicine intervention on elderly diabetic patients. This study reviews the research on clinical features, pathogenic processes, treatment principles, and TCM treatments, hoping to provide fresh perspectives on the prevention and management strategies for elderly diabetes.

Abstract:Type 2 diabetes mellitus (T2DM) occurs in 95% of the diabetic populations. Management of T2DM is a challenge. Traditional Chinese medicines (TCM) are usually served as adjuvants used to improve diabetic syndromes in combination of routine antidiabetic drugs. For single-herb prescriptions, Ginseng, Bitter melon, Golden Thread, Fenugreek, Garlic, and Cinnamon might have antidiabetic effects in T2DM patients. Among 30 antidiabetic formulas approved by the State Food and Drugs Administrator of China, top 10 of the most frequently prescribed herbs are Membranous Milkvetch Root, Rehmannia Root, Mongolian Snakegourd Root, Ginseng, Chinese Magnoliavine Fruit, Kudzuvine Root, Dwarf Lilyturf Tuber, Common Anemarrhena Rhizome, Barbary Wolfberry Fruit, and India Bread, which mainly guided by the theory of TCM. Their action mechanisms are related to improve insulin sensitivity, stimulate insulin secretion, protect pancreatic islets, and even inhibit intake of intestinal carbohydrates. However, it is very difficult to determine antihyperglycemic components of TCM. Nevertheless, TCM are becoming popular complementary and alternative medicine in treatment of syndromes of T2DM. In the future, it requires further validation of phytochemical, pharmacological, and clinical natures of TCM in T2DM in the future studies, especially for those herbs with a high prescription frequency.

Abstract:IntroductionIn order to introduce the diabetology history in Traditional Chinese Medicine (TCM), including nomenclature, understanding, treatment, prevention, etc., our research specifically focused on ancient medical literature used in the evaluation of diabetes in TCM or wasting-thirst (xiao-ke).MethodologyThe history of wasting-thirst was classified into four periods: (1) the nomenclature of wasting-thirst; (2) the monograph of wasting-thirst; (3) the understandings of sweet urine and its extensive development; and (4) the integration of wasting-thirst and diabetes.ResultsBy mining and reviewing the literature in the 2000-year history of wasting-thirst, it is believed that TCM has a profound understanding of, and effective management on, diabetes and its complications. For period one, wasting-thirst was first recorded in The Yellow Emperor's Classic of Internal Medicine. For period two, it was elaborated in Treatise on Febrile and Miscellenous Disease, and its “pathology” was summarized as “three wasting-thirsts (upper, middle, lower)” from the perspectives of spatial locations of organs, which serves as a guide in clinical practice. For period three, a self-contained system, including diagnosis, treatment, prevention, nursing, and education, had been established. For period four, wasting-thirst was intimately integrated into diabetes in The Integration of Traditional Chinese Medicine and Western Medicine.DiscussionTraditional Chinese Medicine harbors a long history of treating wasting-thirst/diabetes. With its unique therapeutic approaches, TCM is an effective system of understanding and managing diabetes as proven by medical literatures, clinical benefits and basic research studies.

Abstract:Introduction: Diabetes is a group of metabolic disorders characterized by high blood glucose in patients. In spite of numerous progresses have been achieved in diabetes research and care, it is still challenging to cure diabetes. The objective of this study was to explore a new approach to treat diabetes with Traditional Chinese Medicine. Materials and Method: The Traditional Chinese Medicine used in this study was a natural bio-product(s) (NBP) which has been used for humans for several hundred years. This NBP was dried and grinded into powders and capsulated (patent pending). Based on the degree of severity determined by OGTT data analysis, patients took 4.5g - 30g of this NBP orally per day for 3 to 24 months, and might take prescribed medicines (Gliclazide MR, Metformin, and/or Saxagliptin) at the initial stage of therapy. Starting on the first day of therapy, the patients discontinued the insulin injection if they had relied on it. Results: The patients ranged from 34 yo to 73 yo (mean=55.6 yo, n=18). Before therapy, six of 18 patients were on a regular insulin injection with various dosages, and the mean HbA1c was 7.65 (SD=1.97, n=18). After therapy (without any insulin injection) for an average of 231 days (97-748 days), the mean HbA1c was 6.61 (SD=0.86, n=18). The difference was statistically different (t-test, p=0.008). Ten of 18 patients were on western medicines for an average of 77 days while the rest took NBP only. The insulin resistance (estimated by HOMA-IR) decreased and the disposition index (estimated at 2-h OGTT) increased in 12 of the 18 patients. Discussion and Conclusion: This NBP can effectively control plasma glucose level by reducing the insulin resistance and enhancing the beta cell functions. However, its exact mechanism is not clear, partially because this NBP is a compound which may affect multiple systems simultaneously. This method might not be suitable for all patients with T2DM. He has unique experience in the treatment of refractory angina pectoris. This paper comprehensively explains from the aspects of etiology and pathogenesis. Abstract:In recent years, with the improvement of people's living standards, the incidence of DM has increased year by year in China. DM is a common metabolic syndrome characterized by hyperglycemia caused by genetic, environmental and other factors. At the same time, long-term suffering from DM will also have an impact on the heart, blood vessels, eyes, kidneys and nerves, and associated serious diseases. The human body has a large and complex gut microbiota, which has a significant impact on the body's metabolism. Research shows that the occurrence and development of DM and its complications are closely related to intestinal microbiota. At present, western medicine generally treats DM with drugs. The hypoglycemic effect is fast and strong, but it can have a series of side effects on the human body. Compared with western medicine, Chinese medicine has its unique views and methods in treating DM. TCM can improve symptoms and treat complications by improving the imbalance of microbiota in patients with DM. Its characteristics of health, safety, and reliability are widely accepted by the general public. This article reviews the relationship between intestinal microbiota and DM, as well as the mechanism of TCM intervention in DM by regulating intestinal microbiota.
