Abstract:This paper introduced professor DING Xue-ping's clinical experience in the treatment of obesity with hyperinsulinemia from the traditional Chinese medicine.Professor DING pointed out that obesity disease was related to spleen and kidney.The occurrence of obesity with hypednsulinemia was closely related to qi deficiency and blood stasis and phlegm stagnation.At the beginning of the disease,we should focus on removing the pathological factors.With the development of disease,we should obey the rules of reducing the excess and reinforcing the deficiency.Last but not least,it was very important to promote blood circulation for removing blood stasis and eliminate dampness and phlegm.Professor DING pay attention to the change of dampness,phlegm,turbidity,water and drink.Meanwhile he attached importance to the dial up triple burner and the lung regulating visceral activities.Professor DING emphasized that the treatment based on syndrome differentiation should be paid attention to cold,hot,weak and strong,and be combined with multiple therapies.

Abstract:Objective:To analyze the "disease-syndrome-treatment-medicine" experience of professor DING for diabetic peripheral neuropathy by using "Traditional Chinese Medicine(TCM) inheritance support system(V2.5)".By analyzing the data,we summarized the Professor DING's academic experience.Methods:Through collecting the standard clinical cases,combined with the software analysis system,we analyzed the composing principles by using datamining methods such as frequency statistics,revised mutual information,association rules analysis,cluster analysis,improvement of mutual information,complex system entropy clustering,unsupervised hierarchical clustering entropy and so on.Results:Characteristics of diabetic peripheral neuropathy:Based on the analysis of 31 cases of diabetic peripheral neuropathy internal wind syndrome.The highest-frequency used insect drugs were Scorpion,bombyx batryticatus,cicada slough.The highest frequency used drug combinations were folium mori and the root bark of white mulberry,scorpion and bombyx batryticatus,rhizoma coptidis,folium mori and the root bark of white mulberry,radices rehmanniae,bombyx batryticatus and scorpion and so on.Conclusion:Treating diabetic peripheral neuropathy focuses on wind.Extinguishing wind and dredging collaterals is important.

Abstract:In recent decades,with the prevalence of diabetes,more researches were carried out on Type 2 Diabetes Mellitus(T2DM) in China as well as the world.Modern Western medicine opened up a new era of the therapy of T2DM with high tech aiding,in which is developed from the cellular ultra structure of human body to the imperceptible changes of molecular biology.One of the milestone on the diabetic research is using of GLP-1 analogues in the treatment of diabetes.Traditional Chinese medicine(TCM) has a long history on the research of diabetes,at least 3500 years from the written records,revealing the process of the T2DM and the differentiation of symptoms and signs.With the developing of research of TCM on syndrome types of T2DM,we have found that there is a certain relationship between the diabetes TCM syndrome types and the lab data,particularly GLP-1 level.We believe that the physiological and pathogenic roles of GLP-1 are strongly associated to the TCM syndrome of damp heat of spleen and stomach of diabetes.

Abstract:Urinary tract infections(UTI) is one of the frequent complications in diabetics.After retrieving the pertinent literature of recent years,we focused on the pathologic mechanism and pathologic characteristics,therapeutic principles and combined treatment of traditional Chinese medicine(TCM) and western medicine.We expounded the advantages of TCM on treating urinary tract infection in diabetics,emphasizing on the simple or empirical recipes of Chinese medicine,Chinese patent medicine and combined treatment of Chinese medicine and western medicine.TCM has significant curative effects on UTI,can prevent the side effect caused by western medicine and improve the immunity of patients.It has broad prospects for development.

Abstract:Professor PENG Wannian is good at using classical prescriptions in the diagnosis and treatment of diabetic and miscellaneous problems under the six meridians syndrome differentiation system. The professor attached great importance to strengthening the spleen and kidney, suggesting that blood stasis changes throughout diabetes. If the diabetic "Sanxiao" symptoms are typical, professor PENG often uses Baihu Rensheng decoction. In addition, Professor likes to use the Xuanfu Daizhe decoction and Guizhi Shaoyao decoction to treat diabetic patients with gastrointestinal diseases, uses Xiaochaihu decoction to regulate mood and sleep disorders in diabetic patients, uses Danggui Sini decoction to treat diabetic peripheral neuropathy,and uses Shenqi Pill and Zhenwu decoction in treating diabetic nephropathy, Tianqi and Sini powder often through each period of treatment of diabetes.

Abstract:Objective To study the distribution and drug-resistance of the pathogen in diabetic with urinary tract infection, and direct rational use of antibiotic in clinical.Method 229 in-patients were chosen in recent two years,which were accompanied with diabetes and urinary tract infection.120 cases were cultivated with pathogen.K-B method was used to test the drug-resistant strain and do drug-sensitivity test.Result The former three bacteria in diabetic with urinary tract infection were escherichia coli (55.0%), dung enterococcus(9.2%),and microzyme(7.5%).The last five resistant antibiotics were chloromycetin,amikacin,ceftazidime,nitrofurantoin,and Imipenem,were all below 28.8%.The last four antibiotics which were not sensitive to G~+ cocci,were chloromycetin,nitrofurantoin,Imipenem,and vancomycin,were all below 17.4%.The drug-resistance rate of pathogen to antifungal,such as fluconazole and itraconazole,was 10.5%~26.3%.Conclusion The result of urine cultivation and drug sensitivity are useful to clinic.It is important to choose antibiotic under the direction of it.

Abstract:BACKGROUND:Urinary tract infections (UTIs) occur more frequently in diabetic patients. This study was conducted to investigate the prevalence, risk factors and microorganisms of UTIs in Chinese patients with type 2 diabetes (T2D).PATIENTS AND METHODS:A total of 3,652 Chinese inpatients with T2D were reviewed and data on their clinical characteristics, symptoms of UTIs, random blood glucose, HbA1c, glutamic acid decarboxylase antibody, insulin autoantibody, albumin excretion rate in 24-hour urine, urine culture and susceptibility to antibiotics, and so on were collected. Binary logistic analysis was performed to look for risk factors of UTIs.RESULTS:There were 409 (11.2%) patients suffering from UTIs. Gender, age, random blood glucose, insulin autoantibody and albumin excretion rate in 24-hour urine were the risk factors of UTIs in diabetic patients. The percentage of positive urine cultures was higher in the asymptomatic bacteriuria patients than in symptomatic patients (P<0.001). The incidence of septicemia was considerable in the UTIs and asymptomatic bacteriuria groups. Escherichia coli was the most common pathogenic microorganism isolated in diabetic patients with UTIs, and one-half of the E. coli infections were multidrug resistant. Furthermore, meropenem was the most effective antibiotic on E. coli.CONCLUSION:We suggest that a routine urine analysis or urine culture should be conducted in patients with T2D diabetes who have the identified risk factors. The UTIs might affect the islet function or blood glucose control in patients with T2D. Before a doctor decides to prescribe antibiotics to a diabetic patient with UTIs, the drug sensitivity test should be performed.

Abstract:Foot ulceration is one of the most serious complications of diabetes mellitus and may lead to amputation of the lower extremity. Timely prophylaxis and treatment of diabetic foot ulceration are important to maintain a good quality of life. This article reports a complicated diabetic patient with severe limb-threatening necrotizing infection. We successfully applied endovascular stent insertion, digit amputation, negative pressure wound therapy, and advanced dressings in different wound phases to achieve definitive wound healing after 12 months of treatment. Based on this case report, we would like to emphasize the importance of combined multiple therapies and patient compliance for severe diabetic foot ulcers.
Effects of Electroacupuncture on Bladder Dysfunction and the Expression of PACAP38 in a Diabetic Rat Model.

Abstract:Objective: To explore the effects and the possible mechanism of electroacupuncture (EA) on diabetic bladder dysfunction (DBD) in streptozotocin-high fat diet (STZ-HFD) induced type 2 diabetes mellitus (T2DM) rats.Methods: The experiment was divided into Control, diabetic bladder dysfunction, electroacupuncture, and Sham electroacupuncture group. After 8 weeks of electroacupuncture intervention, the body mass, 24 h urine volume, intraperitoneal glucose tolerance test (IPGTT), and urodynamics were detected. After the wet weight of the bladder was detected, the hematoxylin-eosin (HE), Masson’s trichrome, and TUNEL were used to analyze histological changes. The PACAP38 expressions in the bladder were detected by Real-time PCR and Western blot.Results: Compared to the Control group, the bladder wet weight, 24 h urine volume, blood glucose, maximum bladder capacity, bladder compliance, bladder wall thickness, the smooth muscle/collagen ratio, and apoptosis rate of the diabetic bladder dysfunction group were significantly increased. Moreover, the body mass and leak point pressure were significantly reduced. Compared with the Sham electroacupuncture group, the bladder wet weight, maximum bladder capacity, bladder compliance, bladder wall thickness, and apoptosis rate of the electroacupuncture group were significantly reduced. In contrast, the leak point pressure was increased. The PACAP38 mRNA and PACAP38 protein expression of the diabetic bladder dysfunction group were significantly lower than the Control group, while electroacupuncture treatment could upregulate PACAP38 mRNA levels and PACAP38 protein expression of diabetic bladder dysfunction model rats.Conclusion: electroacupuncture could ameliorate bladder dysfunction in the diabetic bladder dysfunction model rats by reversing bladder remodeling, which might be mainly mediated by regulating the PACAP38 level.

Abstract:Diabetic retinopathy(DR) is one of the most common and serious complications of diabetes.Through the study of ancient medicals and based on many years of clinical experiences,Professor GAO Yan-bin infers that DR is particularly closely related to the liver and kidney.Using these two organs as the theoretical foundation,the root of eye diseases can be grasped.The theory of collateral disease coincides and is responsible throughout the whole process of DR.Using syndrome differentiation,he holds the idea that with deficiency as the basic pattern and excess as the syndrome,the main reason for DR is qi,blood,yin and yang deficiency,while blood stagnation in the eye meridian is pathogen,The rules of treatment are tonifying qi and enriching yin,supplementing and nourishing liver and kidney,removing blood stasis and promoting circulation.By combining overall symptoms with fundus changes,together with dynamic differentiation and flexible treatment,satisfactory results have been obtained in clinical practice.

Abstract:Diabetes is a widespread chronic disease that occurs mainly in the elderly population. Due to the difference in pathophysiology between elderly and young patients, the current clinical practice to treat elderly patients with anti-diabetes medications still faces some challenges and dilemmas, such as the urgent need for early diagnosis and prevention, and an imbalance between restricted dietary intake and the risk of undernutrition. Traditional Chinese medicine (TCM) offers various treatment regimens that are actively utilized in the field of diabetes management. Through multiple targets and multiple pathways, TCM formulas, medicinal herbs, and active natural products enhance the efficacy of diabetes prevention and diabetes control measures, simplify complex medication management, and improve common symptoms and common diabetic complications in elderly people. Historically, natural products have played a key role in material composition analysis of TCM and mechanism interpretation to enable drug discovery. However, there have been few conclusions on this topic. This review summarizes the development of TCM for the prevention and management of diabetes in elderly people, existing evidence-based clinical practices, and prospects for future development.

Abstract:Objective: To explore the prevention and treatment of diabetic vascular diseases based on "Yingwei theory". Methods: Referring to some literatures of CNKI from 2001 to 2021, related researches on diabetic vascular diseases were carried out through "Ying Wei Theory", and the key words were "diabetes; Yingwei theory; Compound Chinese medicine; Single Chinese medicine; Vascular disease; Arteriosclerosis;" , 21 literatures were selected. Results: Through the understanding of the influence of diabetes on cardiovascular disease, the relationship between Yingwei theory and vascular disease, the relationship between Yingwei theory and atherosclerosis of diabetes. Yingwei theory in vascular disease and diabetes vascular disease related basic research, clinical research. The results showed that the "Yingwei theory" could relieve vascular endothelial function, regulate autonomic nerve remodeling, resist inflammation, reverse fibrosis, inhibit oxidative stress and improve cardiac function. Clinical application can improve unstable angina pectoris and other cardiovascular diseases. Conclusion: Ying Methodism can be used to treat diabetes mellitus and its complicated vascular diseases.

Abstract:Diabetes is a major medical problem that imperils public health. Over two thousand years ago, Traditional Chinese Medicine (TCM) called diabetes-related symptoms "Xiaoke" disease. In ancient China, TCM and Chinese herbal medicines were used widely in treating Xiaoke and abundant experience has been accumulated. This article discusses the TCM theory on diabetes and its achievements in the prevention and treatment of diabetes in the past. Using Chinese herbal medicine, recent progress in diabetes therapeutics, including data from clinical trials, are presented. Mechanistic studies from basic research are discussed. Yin-yang balance and a holistic approach of TCM may complement diabetes treatment in Western medicine. With continuous efforts, TCM could play a more important role in fighting this disease.
