Abstract:According to literature, the incidence of three-canal maxillary first premolar is between 0.5% and 7.5%. The two types of three-canal maxillary premolars are two-rooted and three-rooted three-canal maxillary premolars. Most case reports in Chinese literature focus on three-rooted three-canal maxillary premolars. To our knowledge, no domestic case report on two-rooted three-canal maxillary premolar has been published. In this study, we describe the root canal treatment of a two-rooted three-canal maxillary premolar.

Abstract:Abstract Background Understanding the anatomical morphology of the root canal is key for successful root canal treatment. The aims of this case presentation are to report a unique case of root canal treatment involving five root canals in the mandibular first premolar and to highlight the importance of variation in root canals of mandibular first premolars in clinical practice. Case presentation A 25-year-old male with intermittent pain in relation to the lower right posterior teeth over 3 weeks was diagnosed with symptomatic pulpitis in tooth #44. Four root canals were found, including mesiobuccal, distobuccal-1, distobuccal-2, and distolingual roots, and the Mtwo rotary system was used for root canal preparation. The four root canals were filled after 2 weeks, when a fifth canal was found, located in the buccal cavity. The fifth canal was confirmed to be the mesiolingual root canal by cone beam computed tomography (CBCT) and was found to be curved. After completion of the root canal filling, CBCT was performed, and a three-dimensional root canal image was reconstructed. After 1 week of observation, the tooth was repaired using composite resin filling. Conclusions This is the first case presentation of a fifth canal of the mandibular first premolar and advances our understanding of variations in the anatomy of the mandibular first premolar. This case report provides a reference for the treatment of mandibular first premolars.

Abstract:The objective of dentists is to preserve teeth affected by either caries, trauma, or any other pathological condition. Teeth with deep carious lesions often require endodontic treatment to preserve their form and function. Sound knowledge of root canal anatomy, endodontic pathology, and proper treatment protocol are vital in endodontic treatment success. Currently, there has been an ongoing trend of case reports that highlight the presence of extra canals, which in turn, cautions the clinician to be more prudent. Any missed canal during root canal therapy is the common cause of treatment failure. This case report emphasizes the fact that proper understanding of pulp anatomy combined with the use of modern diagnostic aids and proper treatment protocol is essential for treatment success. This case report describes successful management of three rooted mandibular first molar with extra canals.

Abstract:The mandibular first molars normally have three or four root canals and rarely have five or more root canals. The present study reported a rare anatomical configuration with six root canals in the mandibular right first molar diagnosed during endodontic treatment using a dental operating microscope and confirmed with the help of cone-beam computed tomography (CBCT) images. The present case report revealed that there is an increasing possibility of detecting additional canals through the magnification of the microscope and the improvement of CBCT diagnostic technology. As more abnormal morphologies in root canals are reported, dentists need to understand this diversity in anatomical structure and improve treatment techniques.

Abstract:Unusual anatomical configurations must be considered in the radiographic and clinical evaluation during the endodontic treatment. The canal anatomy maxillary first and second premolar has been studied extensively and the presence of three canals is relatively rare especially in the maxillary second premolar. The occurrence of three canals with separate foramina in both the first and second premolar is shown to be in extremely low percentages and has been documented as practically non-existent in Asian populations. This article describes a case of a maxillary first and second premolar with three canals each.

Abstract:The most common configuration of the mandibular first molar is the presence of two roots and three root canals. The objective of this work is to present two rare anatomic configurations with six root canals on two mandibular left first molars diagnosed during endodontic therapy. Root canal therapy was performed using a dental operating microscope. Ultrasonic troughing in the grooves in between the mesial root canals and in between the distal root canals was able to show the middle root canals. Large samples population characterization researches and systematic reviews were unable to detect a single case of six root canals configuration in a mandibular first molar in their investigations. Although it is a rare configuration, a six root canal configuration is possible to be found in the mandibular first molar. Three different pulp chamber configurations are possible to be found. Two or three roots may be present and the root configuration more common in the mesial root is the Type 8 and Type 12 for the distal root. Some concepts about the required technique to approach these cases are also debated.

Abstract:Background: Root canal anatomy is a complex entity. The main objective of root canal treatment is to get rid of the infection and have a good apical and coronal seal with an appropriate filling. Inability to achieve thorough cleaning and shaping followed by three-dimensional obturation of the root canal system usually causes root canal treatment failure. For this reason, clinicians should be aware of these anatomical variations to achieve successful treatment. Aim: The aim of this article is to report on the successful treatment and follow-up of mandibular first molar with additional middle mesial (MM) and middle distal (MD) canals. Case report: A 29-year-old white male patient reported with a complaint of pain in relation with tooth #19. On clinical examination, diagnosis of symptomatic irreversible pulpitis with symptomatic apical periodontitis and condensing osteitis was made and nonsurgical root canal treatment was planned. Initially, two mesial and two distal canals were located, and the patient was planned for the obturation in the second visit. The complaint of mild persistent symptoms gave a possibility of additional canals. Under the dental operating microscope and selective troughing on the floor of the pulp chamber with ultrasonic tips, additional canals were located as MM and MD canals. Conclusion: Leaving some area of the root canal system untreated is found to be one of the main reasons for root canal treatment failure. Dentists should take advantage of new tools, such as dental operating microscope and ultrasonic tips to be able to locate and treat the hidden and unusual anatomy. Clinical significance: Mandibular first molar with six canals is very rare to encounter. Clinician should have a thorough knowledge of these unusual anatomy to avoid treatment failure due to incomplete disinfection of the root canal system.

Abstract:The mandibular first premolar, which is the anteriormost tooth in the mandibular arch, differs from other teeth in that it typically has two roots and two to four canals. The current case involves treating a mandibular first premolar with two canals through endodontic therapy. The 42-year-old male patient's left mandibular first premolar was diagnosed with irreversible pulpitis. The lateral view X-ray depicted two canals. In the dental procedure, endodontic treatment involves working under an operating microscope and using magnification to treat the affected tooth. After canal shaping the tooth was treated with the crown after it had been cleaned and then adjusted for the bite. As aforementioned, no signs of periapical disease were observed when the tooth was rediscovered a year later. However, this case study indicates that any shift in the anatomy of the canal in premolars should be well monitored and controlled to improve endodontic treatment outcomes of such teeth as the mandibular first premolars.

Abstract:Maxillary molar with three roots and 3 to 4 canals is a common occurrence. However, in addition to common root canal anatomy, there may be significant differences in the number, distribution, and morphological structure of root canals. The success of root canal treatment is dependent on ensuring that all the intricate details associated with it are meticulously followed. Failure to locate all canals could have a negative effect on the treatment as it may lead to initiation or continuation of periapical pathology. Missed canals were the main reason for patients reporting back for nonsurgical root canal retreatment. Moreover, the bacteria residing in such canals could also result in persistence of symptoms. Root canal anatomy is complex, and the recognition of anatomic variations could be a challenge for clinicians. This article presents three cases of endodontic management of maxillary molars with atypical canal morphology. In the three cases of this study, the patients underwent cone beam computed tomographic (CBCT) examination before root canal treatment. The CBCT images revealed that the maxillary molars in case 1 and case 2 had 5 canals. Case 1: 2 mesiobuccal (MB, MB2), two distobuccal (DB and DB2), and one palatal canal. Case 2: 2 mesiobuccal (MB, MB2), one distobuccal (DB), and two palatal canals (MP and DP). In case 3, CBCT scan slices showed that the maxillary first molar presented as a C-shaped root canal with a rare tooth anomaly of taurodontism. Although C-shaped root canals were most frequently seen in the mandibular second molar, they might also appear in maxillary molars. A literature search revealed only a few case reports of C-shaped root canal systems in maxillary molars. Case 3 described the fusion between mesiobuccal and palatal roots of the maxillary first molar, forming a C-shaped mesiopalatal root canal. The above cases suggest that endodontists should always be aware of aberrancies in root canal system apart from the knowledge of normal root canal anatomy. CBCT as a means of diagnosis can be helpful for identifying and managing these complex root canal systems. This case series also highlights the importance of magnification and illumination. Through using an endodontic microscope, clinicians can identify root canals that are difficult to locate or overlooked with normal vision. A correct access opening is a most important step to locate and negotiate the orifices of root canals. The use of ultrasonic tips can refine the access cavity and allow controlled and delicate removal of calcifications and other interferences to the canal orifices, thereby helping to locate the extra orifices.

Abstract:The purpose of this article is to report a successful endodontic treatment of a mandibular first molar with six canals; three root canals in the mesial root, two root canals in the distal root, and one radix entomolaris. The incidence of two anatomical variations presented in this report, middle mesial canal and radix entomolaris, is generally low. Knowledge of the internal anatomy of the root canals and their possible variations, and use of magnification, such as the operating microscope or loupes, can increase the chances of finding additional canals, contributing to the success of endodontic treatment.

Abstract:Rationale: There is an increasing tendency for case reports to reveal anatomical aberrances in mandibular first molars, such as the lateral and accessory canals. Thus, clinicians should pay special attention to anatomic variances when dealing with mandibular first molars requiring endodontic treatment to prevent reinfection within the root canal system, which is responsible for the failure of root canal treatment. Patient concerns: This article presents the clinical report and successful root canal treatment of a 24-year-old healthy female patient with an extensive periapical lesion in a 6-canal first mandibular molar. The patient was admitted to the endodontic department because of a periapical abscess found 1 month ago in her left mandibular first molar. Diagnosis: Chronic apical periodontitis was diagnosed based on clinical examination coupled with radiographic and cone-beam computed tomography images. Interventions: The treatment plan was to first perform root canal therapy and then perform clinical observation. Outcomes: During 1-year follow-up period, the treated tooth was asymptomatic, and complete resolution of the extensive apical lesion was eventually achieved, as shown in the postoperative cone-beam computed tomography images and clinical examination. Lessons: The present case emphasizes the importance of a comprehensive understanding of root canal morphology, especially rare anatomical variations, to ensure successful root canal treatment. Additionally, the case report adds to the library of previously reported cases of extensive periapical lesions with a direct connection to the root canal system, which demonstrates the potential clinical advantages of root canal therapy as a conservative nonsurgical approach in these cases.

Abstract:Mandibular premolar can present a complex pulp anatomy. Apart from the usual single root and single canal a many other variations may be seen. Hence not only a thorough knowledge of the root canal anatomy but also it’s variations are indispensable pre requisites for the success of any endodontic treatment. Good magnification and CBCT scans will definitely be beneficial for successful endodontic treatment. The present case provides evidence that mandibular premolars may have more than one canal.

Abstract:This case report describes a unique C-shaped mandibular second premolar with four canals and three apical foramina and its endodontic management with the aid of cone-beam computer tomography (CBCT). C-shaped root canal morphology with four canals was identified under a dental operating microscope. A CBCT scan was taken to evaluate the aberrant root canal anatomy and devise a better instrumentation strategy based on the anatomy. All canals were instrumented to have a 0.05 taper using 1.0 mm step-back filing with appropriate apical sizes determined from the CBCT scan images and filled using a warm vertical compaction technique. A C-shaped mandibular second premolar with multiple canals is an anatomically rare case for clinicians, yet its endodontic treatment may require a careful instrumentation strategy due to the difficulty in disinfecting the canals in the thin root area without compromising the root structure.

Abstract:The roots and canals of maxillary second premolar have different typical anatomical features, as well as a number of anatomical variations and extra canals. The present cases describe the successful endodontic therapy of two maxillary second premolars with three canals of healthy individuals, with the help of cone-beam computed tomography and dental operating microscope. The first case was diagnosed as symptomatic irreversible pulpitis with asymptomatic apical periodontitis, and the second case was diagnosed as necrotic pulp with normal periapical area. Follow-up visits reported no symptoms of both patients. The awareness toward atypical anatomy can be a critical factor in determining the success of endodontic treatment.

Abstract:Effective and successful endodontic treatment requires dentists to have adequate information on the clinical variations in root canal anatomy. The maxillary first premolars usually have two roots; however, rarely, the occurrence of three root canals has been reported. Existence of such variations should be considered and noted, to be managed well and not missed. Herein, we illustrate the diagnosis and clinical management of four clinical cases presented with three root-canaled maxillary first premolars.

Abstract:Each tooth in the permanent dentition presents with extensive variation in number of roots and root canals. Inability to locate all the canals leads to incomplete debridement and obturation of the root canal space eventually leading to endodontic failure. Mandibular premolars have been reputed to have an aberrant anatomy and are thus termed as an Endodontist's enigma. Anatomically, they are described as teeth with single root and single root canal but there are increased chances of additional roots and root canals being present. The occurrence of three canals with three separate foramina in mandibular second premolars is very rare. The present case report describes root canal treatment of three canals with separate foramina. Dental loupes were used for magnication to locate the canal orices. After biomechanical preparation and obturation, post endodontic restoration was done using composite resin. The present report emphasizes on the importance of nding additional canals present to avoid endodontic failure.
