Abstract:Abstract Background Understanding the anatomical morphology of the root canal is key for successful root canal treatment. The aims of this case presentation are to report a unique case of root canal treatment involving five root canals in the mandibular first premolar and to highlight the importance of variation in root canals of mandibular first premolars in clinical practice. Case presentation A 25-year-old male with intermittent pain in relation to the lower right posterior teeth over 3 weeks was diagnosed with symptomatic pulpitis in tooth #44. Four root canals were found, including mesiobuccal, distobuccal-1, distobuccal-2, and distolingual roots, and the Mtwo rotary system was used for root canal preparation. The four root canals were filled after 2 weeks, when a fifth canal was found, located in the buccal cavity. The fifth canal was confirmed to be the mesiolingual root canal by cone beam computed tomography (CBCT) and was found to be curved. After completion of the root canal filling, CBCT was performed, and a three-dimensional root canal image was reconstructed. After 1 week of observation, the tooth was repaired using composite resin filling. Conclusions This is the first case presentation of a fifth canal of the mandibular first premolar and advances our understanding of variations in the anatomy of the mandibular first premolar. This case report provides a reference for the treatment of mandibular first premolars.

Abstract:Endodontic therapy aims at disinfection of the root canal system. Efforts are made by root canal instrumentation and chemical disinfection to remove the infected pulp tissue and debris from the root canal space. However due to variations in canal anatomy, it is impossible to instrument all the parts of canal and that may lead to persistence or progression of infection. Thorough knowledge of tooth morphology is important to improve the success rate of endodontic treatment. Missed canals are considered to be an important factor for endodontic failure and about 4% cases treated by specialists also reported have shown to have missed canals. Hence use of all the available methods to detect the additional aberrant canals and disinfection of them during root canal treatment procedure is essential for favorable endodontic outcome.

Abstract:Maxillary molar with three roots and 3 to 4 canals is a common occurrence. However, in addition to common root canal anatomy, there may be significant differences in the number, distribution, and morphological structure of root canals. The success of root canal treatment is dependent on ensuring that all the intricate details associated with it are meticulously followed. Failure to locate all canals could have a negative effect on the treatment as it may lead to initiation or continuation of periapical pathology. Missed canals were the main reason for patients reporting back for nonsurgical root canal retreatment. Moreover, the bacteria residing in such canals could also result in persistence of symptoms. Root canal anatomy is complex, and the recognition of anatomic variations could be a challenge for clinicians. This article presents three cases of endodontic management of maxillary molars with atypical canal morphology. In the three cases of this study, the patients underwent cone beam computed tomographic (CBCT) examination before root canal treatment. The CBCT images revealed that the maxillary molars in case 1 and case 2 had 5 canals. Case 1: 2 mesiobuccal (MB, MB2), two distobuccal (DB and DB2), and one palatal canal. Case 2: 2 mesiobuccal (MB, MB2), one distobuccal (DB), and two palatal canals (MP and DP). In case 3, CBCT scan slices showed that the maxillary first molar presented as a C-shaped root canal with a rare tooth anomaly of taurodontism. Although C-shaped root canals were most frequently seen in the mandibular second molar, they might also appear in maxillary molars. A literature search revealed only a few case reports of C-shaped root canal systems in maxillary molars. Case 3 described the fusion between mesiobuccal and palatal roots of the maxillary first molar, forming a C-shaped mesiopalatal root canal. The above cases suggest that endodontists should always be aware of aberrancies in root canal system apart from the knowledge of normal root canal anatomy. CBCT as a means of diagnosis can be helpful for identifying and managing these complex root canal systems. This case series also highlights the importance of magnification and illumination. Through using an endodontic microscope, clinicians can identify root canals that are difficult to locate or overlooked with normal vision. A correct access opening is a most important step to locate and negotiate the orifices of root canals. The use of ultrasonic tips can refine the access cavity and allow controlled and delicate removal of calcifications and other interferences to the canal orifices, thereby helping to locate the extra orifices.

Abstract:Rationale: There is an increasing tendency for case reports to reveal anatomical aberrances in mandibular first molars, such as the lateral and accessory canals. Thus, clinicians should pay special attention to anatomic variances when dealing with mandibular first molars requiring endodontic treatment to prevent reinfection within the root canal system, which is responsible for the failure of root canal treatment. Patient concerns: This article presents the clinical report and successful root canal treatment of a 24-year-old healthy female patient with an extensive periapical lesion in a 6-canal first mandibular molar. The patient was admitted to the endodontic department because of a periapical abscess found 1 month ago in her left mandibular first molar. Diagnosis: Chronic apical periodontitis was diagnosed based on clinical examination coupled with radiographic and cone-beam computed tomography images. Interventions: The treatment plan was to first perform root canal therapy and then perform clinical observation. Outcomes: During 1-year follow-up period, the treated tooth was asymptomatic, and complete resolution of the extensive apical lesion was eventually achieved, as shown in the postoperative cone-beam computed tomography images and clinical examination. Lessons: The present case emphasizes the importance of a comprehensive understanding of root canal morphology, especially rare anatomical variations, to ensure successful root canal treatment. Additionally, the case report adds to the library of previously reported cases of extensive periapical lesions with a direct connection to the root canal system, which demonstrates the potential clinical advantages of root canal therapy as a conservative nonsurgical approach in these cases.

Abstract:Presented here is a case where 8 canals were located in a mandibular first molar. A patient with continuing pain in mandibular left first molar even after completion of root canal treatment was referred by a dentist. Following basic laws of the pulp chamber floor anatomy and under magnification, 8 canals were located with 3 in mesial root and 5 canals in distal root. At 1 month follow-up patient was asymptomatic .The story emphasizes how understanding the rules of pulp chamber anatomy and following them when looking for extra canals might help to avoid missing canals. Abstract:The objective of dentists is to preserve teeth affected by either caries, trauma, or any other pathological condition. Teeth with deep carious lesions often require endodontic treatment to preserve their form and function. Sound knowledge of root canal anatomy, endodontic pathology, and proper treatment protocol are vital in endodontic treatment success. Currently, there has been an ongoing trend of case reports that highlight the presence of extra canals, which in turn, cautions the clinician to be more prudent. Any missed canal during root canal therapy is the common cause of treatment failure. This case report emphasizes the fact that proper understanding of pulp anatomy combined with the use of modern diagnostic aids and proper treatment protocol is essential for treatment success. Abstract:Locating all the root canals, cleaning and shaping them and then sealing them completely with a root canal filling material is very important for the root canal therapy to be successful. Missed canals lead to failure of the root canal treatment rendering the patient in pain. A dentist should be well-versed with the variations in the root canal system. The given case report presents a case of lower first molar with six canals, 4 distal and 2 mesial. All the canals were located using the champagne bubble test and the bleeding points were identified and the area was scouted to locate the canals. The clinician should evaluate all the cases clinically as well as radiographically for the presence of anomalies. Usually, the clinicians try to locate only the predominant canals and do not scout the chamber for any abnormal variations. This failure to locate a canal will lead to remnants of pulp in the tooth which is the major reason for re-infection and failure of the treatment. Also, in some cases it can be a reason for persistent pain for the patient. So, it’s important to locate all the canals and clean them for a successful root canal treatment.

Abstract:The purpose of this article is to report a successful endodontic treatment of a mandibular first molar with six canals; three root canals in the mesial root, two root canals in the distal root, and one radix entomolaris. The incidence of two anatomical variations presented in this report, middle mesial canal and radix entomolaris, is generally low. Knowledge of the internal anatomy of the root canals and their possible variations, and use of magnification, such as the operating microscope or loupes, can increase the chances of finding additional canals, contributing to the success of endodontic treatment.

Abstract:The mandibular first molars normally have three or four root canals and rarely have five or more root canals. The present study reported a rare anatomical configuration with six root canals in the mandibular right first molar diagnosed during endodontic treatment using a dental operating microscope and confirmed with the help of cone-beam computed tomography (CBCT) images. The present case report revealed that there is an increasing possibility of detecting additional canals through the magnification of the microscope and the improvement of CBCT diagnostic technology. As more abnormal morphologies in root canals are reported, dentists need to understand this diversity in anatomical structure and improve treatment techniques.

Abstract:The most common configuration of the mandibular first molar is the presence of two roots and three root canals. The objective of this work is to present two rare anatomic configurations with six root canals on two mandibular left first molars diagnosed during endodontic therapy. Root canal therapy was performed using a dental operating microscope. Ultrasonic troughing in the grooves in between the mesial root canals and in between the distal root canals was able to show the middle root canals. Large samples population characterization researches and systematic reviews were unable to detect a single case of six root canals configuration in a mandibular first molar in their investigations. Although it is a rare configuration, a six root canal configuration is possible to be found in the mandibular first molar. Three different pulp chamber configurations are possible to be found. Two or three roots may be present and the root configuration more common in the mesial root is the Type 8 and Type 12 for the distal root. Some concepts about the required technique to approach these cases are also debated.

Abstract:We propose a stochastic mathematical program for finding the optimal location and compensation level of thyristor-controlled series compensators (TCSCs) and the optimal location of line switches in a transmission network. The combined uncertainty of the load and generation is captured by scenarios. The proposed problem has polynomial constraints, and we use the general linearization technique to linearize those constraints. The model becomes decomposable into a primal master and subprograms solvable by a branch-and-price procedure with the aid of column generation. The numerical results of implementing the model for the IEEE 118-bus test system show that combined investment in TCSCs and switches decreased the cost of energy and increased the penetration of wind energy more than individual investment of TCSCs or switches.

Abstract:Dental anatomy exhibits considerable variation with the presence of additional canals being a common occurrence. The upper second molar typically presents with three canals and three roots; however, variations such as the existence of an extra canal or a root can pose challenges during endodontic treatment. Maxillary molar is characterized by an additional canal located within the palatal root, often exhibiting complex configurations and variations in morphology. Access refinement is critical to gaining adequate visibility and facilitating instrumentation. Meticulous exploration of the pulp chamber floor and careful examination of radiographs from different angles are essential for accurate diagnosis. Careful negotiation and cleaning of the extra canal with appropriate files and irrigants are essential to remove pulp tissue and debris effectively. Furthermore, obturation of the canal space with biocompatible materials is crucial to ensure a three-dimensional seal and prevent bacterial ingress. Clinically, the inability to detect and treat the extra palatal canal can lead to persistent infection, incomplete debridement, and compromised treatment outcomes. This case report delves into the significance of this anatomical variation, diagnostic modalities, and effective management strategies.

Abstract:The objective of root canal treatment is to perform complete debridement of the root canals and subsequent obturation to facilitate healing of periapical pathosis. However, this process becomes complicated with the presence of additional root canal systems. The purpose of the present article is to report successful non-surgical retreatment of a mandibular first molar with five canals. This case report discusses the clinical management of a previously root filled mandibular firstmolar with two missed canal systems; distolingual and an additional mesial canal known as the middle mesial canal. The post-treatment radiographs show successful obturation to length in all canals. The middle mesial canal was found to be associated with mesiolingual canal and categorised as confluent. The configuration of canals in the mesial root was type XV, based on the classification given by Sert and Bayirli. This case report highlights the importance of knowledge and its application in the management of abnormal anatomic variants which play a crucial role in the success of endodontic retreatment.

Abstract:The occurrence of morphological deviations in root canal anatomy is challenging and can influence treatment outcomes. While mandibular second molars typically have 2 roots and 3 canals, a unique case emerged in a 23-year-old Indian male with a symptomatic permanent left mandibular second molar showing three distal, two mesial canals, and two roots. The tooth presented as non-mobile but tender on percussion. The patient was diagnosed with symptomatic irreversible pulpitis with symptomatic apical periodontitis. The presence of mid distal canal was radiographically and visually confirmed with the aid of limited FOV cone-beam computed tomography and the dental operating microscope. Successful nonsurgical root canal treatment with 6-month follow-up has been documented in this case report.

Abstract:BACKGROUND As the complex anatomy of maxillary first molars is one of the major challenges in endodontic therapy, knowledge of the complicated root canal anatomy and configuration is crucial to ensure the success of endodontic treatment and prognosis. This article presents an endodontically managed left maxillary first molar with an unusual palatal root morphology. The available literature on the anatomic variation of maxillary first molars is also reviewed. CASE SUMMARY A 36-year-old man with no medical history presented to the Stomatology Department of Peking University Third Hospital complaining of a toothache during mastication in the maxillary left posterior region for approximately 3 mo. He had a spontaneous and intermittent toothache that had worsened, particularly at night. The diagnosis based on clinical examination, X-ray imaging, and cone beam computed tomography (CBCT) was symptomatic irreversible pulpitis. Nonsurgical endodontic therapy was performed for the left maxillary first molar. Five root canals revealed by CBCT included a special palatal root canal (1-2-1-shaped), two mesiobuccal root canals, and one distobuccal root canal. Evaluation of the CBCT images confirmed the root canal morphology and the clinician performed more effective cleaning, obturation, and therapy. Finally, the tooth was restored using composite resin, and the patient was satisfied with the result. CONCLUSION CBCT and a complete review of the literature may be beneficial for investigating the root canal system to achieve a biological and functional therapeutic effect.
