The second surgical management of papillary thyroid cancer after inappropriate local treatment

Abstract:Background and purpose:It is frequent for papillary thyroid cancer to be treated improperly at the first time, so second surgical management is necessary for these patients.We investigated how to treat these patients in this study.The principle of primary site treatment and surgical fashion was also recommended at the same time. Methods:132 patients who received first surgical treatment in other hospitals and second treatment in our hospital were retrospectively analyzed. To eliminate the confusion of tumor local recurrence, the patients who received first therapy at 6 months ago were excluded. The complications were compared .The clinical data of local primary site remnants and level Ⅵ lymph nodes metastasis were analyzed.Results:As a major complication, voice hoarseness accounted for 3.79%(5/132) because of the first treatment and 1.57%(2/127) because of the second treatment respectively. 88 patients had tumor remnants in the primary sites (66.7%), and 65 patients had level Ⅵ lymph nodes metastasis at the same time(73.9%) . Among 44 patients without cancer remnants,34 patients had level Ⅵ lymph nodes metastasis(77.3%) .122 patients manifested local cancer remnants of primary site and level Ⅵ lymph nodes metastasis ,and accounted for 92.4%(122/132).Conclusions:For the treatment of single malignant thyroid node, our recommendation is: if the thyroid node is confirmed to be malignant by frozen section, ipsilateral complete thyroid lobectomy plus isthmusectomy with resection of level Ⅵ lymph nodes should be performed. Even though there are no frozen section or the results fail to differentiate malignancy and benignancy, this kind of procedure is also recommended. During the resection of level Ⅵ lymph nodes, recurrent laryngeal nerves should be exposed routinely to ensure complete resection of level Ⅵ lymph nodes.
