Abstract:Objective To evaluate the visual cortex activation in patients with glaucoma by fMRI and demonstrate the correlation between visual field defect and visual cortex activation. Materials and Methods 30 patients with glaucoma and 30 healthy normal controls were included in this study. Block-design single-eye (left, right respectively) stimulated fMRI was performed on a 1.5 T MRI scanner. Black and white check board and a center silence white dot were used as stimulus and control respectively. 6 control and 5 stimulus blocks appeared alternately, every block lasted for 20s. The fMRI data were processed with SPM2 software. Results 16 data based on glaucoma and normal control were recruited for the final group analysis in left-eye and righted-eye stimulation respectively. (1) Single-eye stimulation resulted in bilateral visual cortex activation with right cortex prominence in patients with glaucoma. (2) Compared to normal control group, the extent and intensity of visual cortex was decreased in glaucoma group, which was more significant in right visual cortex. (3) 9 data from patients with severe visual defect in late phase of glaucoma demonstrated the visual cortex activation was difference with right visual cortex domination in left-eye stimulation and left visual cortex domination in right-eye stimulation. Conclusion fMRI can demonstrate the correlation between visual filed defect and visual cortex activation in patients with glaucoma and can deduce the severity of retinal lesion, which is helpful for the diagnosis and treatment planning of glaucoma.
