Arthroscopic Release of Primary Frozen Shoulder

Abstract:Objective To investigate the feasibility and effectiveness of the arthroscopic release of primary frozen shoulder. Methods Forty three patients with primary frozen shoulder from August 2005 to March 2007,including 25 males and 18 females with an average age of 61±6.3 years(range from 48 to 72 years) were enrolled in this study.Cases with cervical spondylopathy,thoracic outlet syndrome,and tumor,as well as the secondary shoulder stiffness following shoulder injury,surgery or external fixation of shoulder were excluded.Arthroscopic release procedure was performed under general anesthesia.All cases started active and passive postoperative rehabilitation from 2nd day after arthroscopic release and were followed up for average of 10.9 months(7~15 months).Range of motion(ROM) of shoulder joint and Constant score were compared before and after the operation. Results ① ROM of shoulder improved significantly 6~8 weeks after surgery.Abduction,anterior flexion,and external rotation with adduction of shoulder increased to 162°±14.34°,165°±13.56°,and 75°±12.31°,respectively.The level of internal rotation went up to T12~T6.② The VAS of the shoulder pain improved from 5.7±1.57 before surgery to 0.8±0.75 after surgery.③ There were no significant differences in the strength of shoulder abduction,external rotation,and internal rotation between two shoulders 6~8 weeks after surgery.④ Secondary shoulder instability was not found in all cases after surgery.⑤ Postoperative Constant score was 92.3±8.54 by the 16th week of operation. Conclusion Results showed that arthroscopic release was an effective,minimal invasive management for primary frozen shouder with significantly improving ROM and releasing pain of the shoulder.
