Abstract:OBJECTIVE:Insensible Urinary Incontinence (IUI) is a situation when you complain of urinary incontinence but are unaware of how it occurred. Therefore, it is necessary to apply highly specific diagnostic methods to promote accuracy in the diagnosis of IUI, including pelvic floor ultrasound (PFU) and urodynamic studies (UDS).METHODS:A total of 41 women with IUI were retrospectively included. Patients were categorized into two groups: the urodynamic urinary incontinence group (UUI group, n=20) and the non-urodynamic urinary incontinence group (NUUI group, n=21), according to the urine leakage during UDS. The baseline clinical characteristics, UDS results, and PFU parameters were collected.RESULTS:Compared with the NUUI group, the UUI group had a smaller maximum cystometric capacity (P=0.008), lower maximum urethral closure pressure (P=0.005), shorter functional urethral length (FUL) (P=0.01), more bladder neck funneling (BNF) (P=0.02), greater BNF depth (P=0.04), and larger BNF area (P=0.01). The area and depth of BNF were negatively correlated with maximum urethral closure pressure (r=-0.42, P=0.01), FUL (r=-0.36, P=0.02 versus r=-0.39, P=0.01), and maximum cystometric capacity (r=-0.35, P=0.03), but positively correlated with maximum urinary flow rate (r=0.33, P=0.04 versus r=0.36, P=0.02). The canonical correlation analysis of the ultrasound parameters and UDS parameters shows that the first pair of canonical variables was statistically significant (r1=0.9, P<0.001).CONCLUSIONS:The PFU is associated with UDS in evaluating IUI. It has the advantages of low cost and high comfort, thus should be used as an auxiliary examination for IUI.
Influence of biofeedback therapy on the pelvic floor muscle and urodynamic state of patients with stress in-continence

Abstract:Objectives:To study and observe the influence of biofeedback therapy on the pelvic floor mus-cle and urodynamic state of patients with stress incontinence.Methods:60 patients with stress incontinence in our hospital from April 2011 to April 2014 were selected and randomly divided into control group (conventional pelvic floor muscle training group)and observation group (biofeedback therapy group),30 cases in each group.The mic-turition related indexes,pelvic floor muscle,urine pad test and ICS urinary incontinence quality of life question-naire,urodynamic indexes of two groups before the treatment and 4th and 8th week after the treatment were respec-tively detected and compared.Results:The micturition related indexes,pelvic floor muscle and urodynamic indexes of the observation group at 4th and 8th week after the treatment were all significantly better than those of control group,all P <0.05,with significant differences.Conclusion:The influence of biofeedback therapy for the pelvic floor muscle and urodynamic state of patients with stress incontinence is great,which is more helpful for the effective and quick improvement of disease.

Abstract:In order to make a correct diagnosis and suitable treatment for female patients with stress urinary incontinences, the preoperative urodynamic study is necessary besides detailed medical history, physical examination, and laboratory tests. With the help of the urodynamic study, the concomitant diseases of patients with stress urinary incontinence may be detected, the individualized treatment regimen can be developed, and more importantly, the inappropriate surgical decision can be avoided.

Abstract:AimTo assess the impact of preoperative urodynamic testing on the clinical management of urinary incontinence and pelvic organ prolapse. MethodsUrodynamic testing was performed prior to surgery in 307 patients with pelvic organ prolapse or urinary incontinence. The resulting changes to planned treatment were recorded and retrospectively assessed. ResultsResults of urodynamic testing prompted changes to the treatment plan in 136 cases (44.2%), including 86 cases of no or slight-stress urinary incontinence, 28 cases of bladder outlet obstruction, 12 cases of occult stress urinary incontinence and 10 cases of detrusor overactivity and urge urinary incontinence. Urodynamic testing excluded disadvantageous factors in 69 cases (22.4%), including 46 cases of overactive bladder and 23 cases of voiding dysfunction. The information provided to patients when seeking informed consent was augmented by the results of urodynamic testing in 63 cases (20.5%). Urodynamic testing was of no value in 39 cases (12.7%). ConclusionsPreoperative urodynamic testing for urinary incontinence and pelvic organ prolapse can result in changes to treatment plans, exclusion of disadvantageous factors, and augmentation of information provided to patients when seeking informed consent.
Analysis of Urodynamics in 80 Women with Pelvic Organ Prolapse

Abstract:Objective:Urodynamics and clinical significance were explored in women with pelvic organ prolapse.Methods:80 patients with pelvic organ prolapse were recruited from July,2005 to March,2009,the urodynamics were examined after operation.Among these patients,38 had urinary incontinence symptoms,20 had urinary irritation symptoms,14 had urinate,8 were asymptomatic.Results:The coincidence rates after urodynamic examination for urinary incontinence,urinary irritation,and urinate were respectively 73.7%(28/38),55.0%(11/20)and 71.4%(10/14).The incontinence diagnostic accuracy by urodynamics was 66.7% when Aa was +1 ~-1 cm according to the symptoms and urodynamic results,80 patients had operation and medication treatment with good results.Conclusions:Urodynamics plays a very important role in the diagnosis and treatment for female pelvic organ prolapse complicated with urination problems.When Aa≥-1cm,no matter what the symptoms are,urodynamics should be applied to clarify whether there is stress urinary incontinence.
Efficacy of Pelvic Floor Ultrasound for Assessing Transobturator Tape Sling Procedure.

Abstract:Importance Complications after midurethral sling, which require a high degree of clinical suspicion for timely diagnosis, accurate evaluation, and proper intervention, are closely associated with surgical success or failure. Objectives This study aimed to evaluate the efficacy and complications of tension-free midurethral sling for stress urinary incontinence (SUI) using pelvic floor ultrasound. Study Design This retrospective study included 152 female patients with SUI admitted to the Jinhua Central Hospital between January 2020 and December 2021 who were selected. All patients underwent midurethral transobturator tape sling procedures and were divided into success, voiding dysfunction, overactive bladder, and failure groups according to postoperative efficacy and complications. The pelvic floor ultrasound examination was performed before and after the surgery. Results The posterior vesicourethral angle difference (P < 0.01) was significantly lower after the surgery than before the surgery. The rate of bladder neck funneling (P < 0.01) and the area of bladder neck funneling (P < 0.01) were smaller after the surgery than before the surgery. The tape-longitudinal smooth muscle distance, the tape-symphysis pubis distance, the sling angle, and the tape-bladder neck/urethra increased successively in the voiding dysfunction, overactive bladder, success, and failure groups. Conclusions Pelvic floor ultrasound can accurately evaluate the postoperative efficacy and complications of transobturator tape sling procedures for SUI and can reasonably guide the management of complications. Therefore, it is an effective imaging method for postoperative follow-up after tension-free midurethral tape suspension.
Diagnostic value of pelvic floor ultrasonography for diagnosis of pelvic organ prolapse: a systematic review

Abstract:Introduction and hypothesis: Pelvic organ prolapse (POP) is a common medical condition universally. In addition to physical examination, experts have increasingly turned their attention to ultrasound in diagnosing POP for its low cost and dynamic imaging. The aim of this paper is to provide an overview of the methods of pelvic floor ultrasound in diagnosing POP, which has been lacking up till now. Methods: We included original papers comparing the outcome of the Pelvic Organ Prolapse Quantification system and ultrasound, published from 2008 to present in English, using electronic databases (MEDLINE, EMBASE, CENTRAL, PUBMED). All stages of the review were conducted in parallel by two reviewers. Results: Fifteen papers were included. We found that current methods have advantages and limitations. The main methods are to measure levator hiatus-related parameters and distances between the lowest point of the pelvic organs and reference lines during Valsalva maneuver, contraction, and at rest. Conclusions: Pelvic floor ultrasound is valuable in diagnosing POP, yet suffers from a weakness in precision compared with physical examination. From the existing research, we found that the differences in baseline data such as weight, height, ethnicity, etc., may affect the cutoffs of the above-mentioned parameters. Further research is required to find one appropriate cutoff for each parameter, even if it is necessary to set group values for every parameter according to varying situations.
Urodynamic Characteristics of Women with Pelvic Organ Prolapse in Prolapse and Reset Condition

Abstract:OBJECTIVE:To study the urodynamic characteristics of women with pelvic organ prolapse in prolapse and reset condition.METHODS:From Jan. 2010 to Apr. 2010, 30 patients with pelvic organ prolapse (POP), including 1 case in grade I, 6 cases in grade II, 21 cases in grade III and 2 cases in grade IV, treated in Peking University People's Hospital were analyzed their urodynamical changes at status of prolapse and reset with pessary.RESULTS:(1) The symptoms: in the prolapse condition, there were 11 patients with bladder outlet obstruction (BOO), 7 patients with residual volume more than 100 ml, and 4 patients with stress urinary incontinence (SUI). In the reset condition, there were 5 patients with BOO, 5 patients with residual volume more than 100 ml, and 4 SUI patients. (2) Characteristics of urodynamic: there were significant difference in first desire to void (FD, P = 0.047), normal desire to void (ND, P = 0.007), strong desire to void (SD, P = 0.001), maximum cystometric capacity (MCC, P = 0.001), maximum flow rate (Q(max), P = 0.001), average flow rate (Q(ave), P = 0.001), and residual volume (RV, P = 0.025) between prolapse and reset condition among all patients. In patients with grade I to II prolapse, Q(max) were (11 ± 6) ml/s in prolapse condition and (15 ± 4) ml/s in reset condition (P = 0.014), Q(ave) were (6 ± 4) ml/s in prolapse condition and (7 ± 3) ml/s in reset condition (P = 0.237), RV were (29 ± 46) ml in prolapse condition and (15 ± 25) ml in reset condition (P = 0.157). Among patients with grade III to IV prolapse, Q(max) were (11 ± 8) ml/s in prolapse condition and (17 ± 10) ml/s in reset condition (P = 0.001), Q(ave) were (5 ± 4) ml/s in prolapse condition and (7 ± 4) ml/s in reset condition (P = 0.002), RV were (105 ± 169) ml in prolapse condition and (47 ± 92) ml in reset condition (P = 0.037).CONCLUSIONS:Patients with pelvic organ prolapse III-IV may present pseudo BOO, recessive stress urinary incontinence and residual volume more than 100 ml. There were significant improvement in cystometric sensation, capacity, flow rate, and residual volume after reset. The urodynamic test for patients with pelvic organ prolapse in the reset condition might be much better in evaluating lower urinary tract function.

Abstract:Objective To summary our experiences of 2330 consecutive urodynamic examinations.Methods From Nov 2005 to Dec 2010,there were all 2330 urodynamic examinations.The age of patients ranged from 9 to 88 years old,an average of 67.5 years old.Results Disease diagnosis:dysuria 1449 cases,62.2%;urinary incontinence 378 cases,16.2%;operation evaluation 87 cases,3.7%;frequency and urgency 416 cases,17.9%.Urodynamic diagnosis:bladder outlet obstruction 1389 cases,59.6%;weak detrusor contraction 269 cases,11.5%;stress urinary incontinence 187 cases,8.0%;normal bladder function 103 cases,4.4%;detrusor areflexia 287 cases,12.3%;detrusor hyperreflexia 56 cases,2.4%;overactive bladder 39 cases,1.7%.Complications of urodynamic examination:43 cases,1.8%.Conclusions Avoid unnecessary urodynamic examination can reduce complications and alleviate pain for patients.
Factors Affecting Female Patients with Non-Neurogenic Lower Urinary Tract Symptoms

Abstract:Study design, materials and methods The medical records of total 7,200 patients with urodynamic studies were reviewed at our center between January 2002 and March 2012. Among them, 2195 belonged to the population of female patients with non-neurogenic LUTS .Patients with conditions including a prior continence procedure, advanced pelvic prolapse, hysterectomy or neurologic deficits were excluded from our study. Comprehensive medical histories, physical examinations, bladder diaries and results of multi-channel urodynamic testing were analyzed. The urodynamic investigations included maximum flow rate (free Qmax), filling cystometrography (CMG), voiding pressure-flow study (PFS), external anal sphincter electromyography (EAS EMG) and urethral pressure profilometry (UPP) according to previously described techniques. Methods, definitions and units were employed according to the standards recommended by the ICS (International Continence Society), except where specifically noted [3]. During CMG and PFS, vesical pressure (Pves) and abdominal pressure (Pabd), and then detrusor pressure (Pdet=Pves-Pabd) were measured by using a two-lumen 8 French catheter and a rectal balloon catheter, respectively. EAS EMG was also simultaneously monitored using two needle-guided-wire electrodes inserted at 3, and 9 o’clock of the anus aperture with lateral distance of 0.5 cm. Filling CMG was performed with a rate of 50 to 70 mL/min of saline and the compliance was recorded continuously. The infusion was stopped to initiate voiding with changing position from supine to sitting when maximum cystometric capacity (MCC) was reached. The urinary flow rates, Pdet, detrusor contraction fashion and EAS EMG were measured during the voiding phase when the patient was instructed to void. Finally, the UPP was obtained and the maximum urethral closure pressure (MUCP) and the functional profile length (FPL) were recorded [3]. The distribution of various functional disorders of detrusor and sphincter was calculated and measured according to mode EAU used for NLUTS.
Diagnosis of Lower Urinary Tract Voiding Dysfunction with Video-Urodynamic Studies

Abstract:Objective To evaluate the role of video-urodynamics (VUD) in the diagnosis of lower urinary tract voiding dysfunction. Methods From December 2008 to March 2010, 115 patients with lower urinary tract voiding dysfunction were included in our study. All patients underwent VUD studies. Results Neurogenic bladder was found in 37 patients, including 25 male patients and 12 female patients. Among these patients, 18 patients were detrusor areflexia( DA ), 2 patients were overactive bladder( OAB), 10 patients were low compliance bladder with hydronephrosis and 7 patients were detrusor-external sphincter dyssynergia. Non-neurogenic voiding dysfunction was found in 59 patients, including 34 male patients and 25 female patients. Among these patients, bladder outlet obstruction was found in 33 patients, OAB in 4 patients, urethra stricture in 4 patients and sphincterismus in 3 patients. Seven patients receiving augmentation of bladder took second VUD examination, including 4 male patients and 3 female patients. One patient receiving Indiana pouch and one patient receiving ureter reimplantation all took VUD examination.Ten patients had basic normal bladder urethra function from VUD examination, including 6 male patients and 4 female patients. Conclusion From combination of pressure-flow figure and real time image, VUD examination provides precise evidence of diagnosis and treatment for lower urinary tract voiding dysfunction.

Abstract:The role of urodynamic study (UDS) in the management of female stress urinary incontinence (SUI) is one of the most controversial and debated topic in urogynecology. Here, we aimed to systematically assess the most relevant available evidence on urodynamics' value in the management of women with stress urinary incontinence.A systematic review according to the Preferred Reporting Items for Systematic Reviews and Meta‐Analyses (PRISMA) Statement was performed in May 2018. Only randomized clinical trials, prospective studies, or retrospective studies articles evaluating the use of urodynamic studies in women with stress urinary incontinence were included.After screening a total of 3055 records, 39 studies published from 1996 to 2018 were included.In an uncomplicated population of women with SUI, it is not demonstrated that preoperative urodynamic evaluation can improve the outcome of continence surgery; however, UDS provides additional information regarding lower urinary tract function that could guide the physician to make the right therapeutic choice. UDS should be considered mandatory before surgery in complicated patients, but its use should also be evaluated in index patients when the results may help counseling and management of these women.

The value of transperineal ultrasound as compared to urodynamic test in assessment of stress urinary incontinence

Abstract:Background: The value of imaging modalities especially transperineal ultrasound in assessment and categorization of pelvic floor disorders in women remains ambiguous. The aim of this study was to compare the diagnostic value of transperineal ultrasound with urodynamic test in assessment of stress urinary incontinence. Methods: This cross-sectional study was conducted on women referred to pelvic floor clinic at Mahdieh hospital with the complaint of stress urinary incontinence. Along with physical examination, urodynamic test and pelvic floor ultrasound were requested for all participants. Results: The patients were divided into two groups with pelvic organ prolapse (n=25) and without pelvic organ prolapse (n=18). There was no difference between the two groups with and without pelvic organ prolapse in β angle greater than 120 degrees during rest and maneuvering Valsalva, and the rate of bladder neck funneling. In the groups with and without prolapsed, the sensitivity and specificity of β angle greater than 120 degrees measuring by ultrasound at rest to determine stress urinary incontinence were 80.0% and 60.0% in the group with prolapsed and 70.0% and 50.0% in those without prolapse. The pointed values for β angle greater than 120 degrees during Valsalva maneuver were 93.3% and 40.0% in those without prolapsed and 80.0% and 12.5% in those without prolapse respectively. The evaluation of bladder neck funneling by ultrasound could diagnose stress urinary incontinence with a sensitivity of 10.0% and a specificity of 62.5% in the group without pelvic organ prolapse and with a sensitivity of 53.8% and a specificity of 80.0% in those with pelvic organ prolapse respectively. Conclusion: The use of ultrasound alone is not sufficient to diagnose stress urinary incontinence, especially in cases where there is no evidence of pelvic organ prolapse, and thus comprehensive use of physical examinations and imaging techniques for such a diagnosis is recommended.
[Perspective Study of Urodynamic Factors with Pre- and Post-Operation Overactive Bladder Symptoms for Anterior Vaginal Prolapse].

Abstract:OBJECTIVE:To identify pre-operative demographic and urodynamic (UD) parameters related to overactive bladder (OAB) symptoms of pre-operation and persistence of after pelvic organ prolapse (POP) repair.METHODS:From Jan 1, 2010 to Oct 31, 2012, this perspective study examined demographic and UD data of 175 patients undergoing POP surgery. Pre- and post- operative urinary distress inventory 6 (UDI- 6) scores for frequency and urge urinary incontinence (UUI) were analyzed, and correlations between scores and pre-operative UD data were also analyzed.RESULTS:(1)Surgery resulted in a improvement of frequency (71.0% , 71/100) and UUI (69.2% , 63/91) at 12-24 months follow-up. (2)Pre-operative UD parameters: first desire of bladder <100 ml, 6.3% (11/175); detrusor overactivity, 14.9% (26/175); post-void residual ≥50 ml, 11.4% (20/175); maximal flow rate (Qmax), (16±7) ml/s. (3)Comparison of pre- operative UD datas between patients with OAB symptoms and without, with OAB symptoms group had more patients of type III stress urinary incontinence [5% (5/91) versus 0 (0/84), P = 0.022]. (4)Qmax was higher in improvement in UUI group than that in persistent UUI after POP repair [(17±7) ml/s versus (13±5) ml/s, P < 0.01].CONCLUSIONS:POP repair significantly reduces OAB symptoms; however, there had not exact predictor for symptoms of persistent frequency, UUI after POP repair. Persistent UUI symptom may be related to Qmax of pre-operative UD.
Identification of Lower Urinary Tract Voiding Dysfunction in Females by Using Video-Urodynamic Study

Abstract:OBJECTIVE:To assess the value of video-urodynamic study (VUD) in the identification of lower urinary tract voiding dysfunction in female.METHODS:A total of 126 female patients with sign and symptoms of lower urinary tract voiding dysfunction underwent VUD from December 2008 to January 2011 in Beijing Chaoyang Hospital. The causes of voiding dysfunction were analyzed based on VUD findings.RESULTS:Neurogenic voiding dysfunction was found in 30 patients (23.8%), non-neurogenic voiding dysfunction was found in 96 patients (76.2%). The 72 patients suffered from recurrent urinary tract infection (57.1%) and 23 patients suffered from hydronephrosis (18.3%). Based on special characteristics of video-urodynamic study, a total of 126 patients were classified as: (1) Bladder outlet obstruction (BOO) was found in 65 patients. Of them, bladder neck obstruction in 40 patients (61.5%), their VUD showed "high pressure-low flow" obstructive curve and synchronic image showed bladder neck did not open in a funnel shape and no contrast was found in urethra. Distal urethral stricture in 22 patients (33.8%), VUD showed "high pressure-low flow" obstructive curve and synchronic image showed bladder neck open in a funnel shape, proximal urethra dilated and no contrast was found in distal urethral. Urethral sphincter obstruction in 3 patients (including detrusor-sphincter dyssynergia in 1 and sphincterismus in 2 patients), VUD showed "high pressure-low flow" obstructive curve and synchronic image showed bladder neck open in a funnel shape, sphincteric urethra did not open, proximal urethra dilated and no contrast was found in distal urethral. (2) Detrusor areflexia (DA) was found in 39 patients (2 patients with hydronephrosis), the VUD finding was: no voluntary detrusor contraction in voiding phase, and abdominal pressure voiding pattern. (3) OAB was found in 3 patients, which VUD findings was frequent involuntary detrusor contraction at storage period with or without urine leakage. (4) Low compliance bladder was found in 17 patients (13.5%), 16 patients with hydronephrosis, the VUD showed that increased bladder storage pressure with significantly decreased bladder safe capacity and compliance, appearance of the bladder as "Christmas tree", with or without ureteral reflux. (5) And stress urinary incontinence (SUI) was in 2.CONCLUSIONS:The main causes of female BOO may be non-neurogenic conditions or organic obstruction. VUD would offer valuable information for confirmed diagnosis of voiding dysfunction in female patients.
Urodynamic Studies Characteristics of Patient with Cerebral Palsy and Its Clinical Significance

Abstract:To evaluate the urodynamic studies characteristics of children with cerebral palsy and its clinical significance.Methods:We reviewed urodynamic studies in 78 cases of cerebral palsy who underwent selective posterior rhizotomy within one week preoperatively.Result:25 out of 78 (32.05%) children with cerebral palsy presented with lower urinary tract symptoms and of the patients 20 cases had evidence of a pure upper motor neuron injury in urodynamic studies,3 cases a mixed motor neuron injury,1 case a pure lower motor neuron injury and 1 case normal;Out of 53 (67.95%) cases without dysfunctional voiding symptoms,35 (66.04%) cases manifested the decrease of total bladder capacity and bladder compliance,as well as the increase of bladder filling pressure.Conclusion:Urodynamic studies are effective methods to identify neuropathic etiology of lower urinary tract symptom in childern with cerebral palsy.According to urodynamic studies,at least half of the children with cerebral palsy,if fit for SPR,who need SPR management as soon as possible,have clinically silent bladder dysfunction.
Value of pelvic examination in women with pelvic organ prolapse: A systematic review

Abstract:Background Recent recommendations from the French High Authority of Health on pelvic organ prolapse (POP) management underline the value of a pelvic examination. Objectives The aim of this paper was to analyze the literature and identify the best evidence available regarding pelvic examination for women presenting prolapse‐associated symptoms in terms of diagnosis and predictability of treatment success. Search Strategy The databases were queried similarly using Medical Subject Headings (MeSH) and non‐MeSH terms broadly related to pelvic examination and POP management. Selection Criteria We included studies assessing the diagnostic contribution of pelvic examination (correlation with symptoms) and its value for assessing the risk of pessary failure or recurrence after reconstructive surgery. Data Collection and Analysis We assessed peer‐reviewed articles on PubMed, Embase, and Cochrane database up to May 2023. The methodological quality of all the included studies was assessed using the ROBINS‐E or RoB2 tools. Main Results In all, 67 studies were retained for the review. Prolapse‐associated symptoms are poorly correlated with POP diagnosis. The symptom that is best correlated with the POP stage is the presence of a vaginal bulge (moderate to good correlation). The factors most strongly associated with the risk of recurrence after surgery or pessary failure are clinical: essentially a higher POP stage before surgery, levator ani muscle avulsion, and vaginal and genital measurements. Conclusions In women complaining of prolapse‐associated symptoms, a pelvic examination (vaginal speculum and digital vaginal examination) can confirm the presence of POP and identify risk factors for treatment failure or recurrence after surgical management or pessary placement. A higher stage of POP and levator ani muscle avulsion—discernible on pelvic examination—are major risk factors for POP recurrence or treatment failure. These features must be taken into account in the treatment choice and discussed with the patient.

Abstract:Objective To explore the clinical significance of urodynamic examination for patients with benign prostatic hyperplasia and diabetes. Methods 76 cases with benign prostatic hyperplasia complicated by diabetes and 76 cases with benign prostatic hy-perplasia admitted in our hospital from October 2012 to March 2014 were selected as the observation group and the control group for study, respectively. Both groups underwent the urodynamic examination. After the examination, the detection pressure-uroflowmetry, filling cystometry of the two groups were compared and the bladder compliance (BC), the voiding maximal detrusor pressure(Pmax), post voiding residual urine volume (PVR), maximum cystometric capacity (MCC) were recorded. Results The Pmax (kPa) of the observation group was obviously lower than that of the control group, the difference was statistically significant (P<0.01), PVR (mL), BC (mL/cmH2O), MCC (mL), FDV (mL) and other indicators were significantly higher than those of the control group, the differences were statistically significant (P<0.01). The incidence of bladder outlet obstruction (BOO) of the observation group was significantly less than that of control group (P<0.05), while the incidences of increased compliance(BC), bladder sensory dysfunction, hypersensitivity, and unstable bladder were significantly higher than those of the control group (P<0.05). Conclusion The urodynamic examination of patients with benign prostatic hyperplasia and diabetes is mainly characterized by significant de-crease in the detrusor contraction function and bladder sensory function, increase in post voiding residual urine volume and blad-der capacity. The patients need urodynamics examination as well as positively controlling the blood glucose before the operation.
Analysis of Urodynamic Study of Female Outpatients with Lower Urinary Tract Symptoms and Follow-Up of the Patients with Detrusor Underactive

Abstract:OBJECTIVE To find out the prevalence of lower urinary tract symptoms (LUTS) and analyze the characteristics of the urodynamics diagnosis in female outpatients. To study the natural history of detrusor underactive (DU) by the followed up. METHODS A retrospective study of the female LUTS outpatients in Peking University People's Hospital from Jan. 2005 to Dec. 2015 was performed. The basic information and the urodynamic results of a total of 656 female outpatients were collected. The distribution of storage symptoms, voiding symptoms, complications and urodynamic diagnosis were analyzed. The symptoms and urodynamic results were compared among the groups, which were divided according to age, urodynamic diagnosis and diabetes mellitus. A follow-up of 163 female DU outpatients was performed, including the treatments and the American Urological Association symptoms scores (AUAss). RESULTS Frequency (25.03%) is the most common symptom in female outpatients, followed by stress urinary incontinence (20.04%), urgency (19.97%), and difficulty of voiding (17.32%). Stress urinary incontinence (SUI) accounted for the first (36.04%) of the whole outpatients, followed by the DU (24.08%), and bladder outlet obstruction (17.58%). The patients aged 51-60 years occupied the peak of almost all the diagnosis. There was a higher proportion of the young female patients than that of the middle and old patients diagnosed with no abnormal after the urodynamic study. The first, strong, urge and maximum bladder capacity were significantly larger in DU patients with diabetes than without diabetes. Follow-up results of the DU patients showed there was no significantly difference of the AUAss scores in both the two groups before and after the follow-up, but the quality of life decreased significantly. CONCLUSION Female LUTS outpatients showed a main complaint of storage symptoms. SUI ranked the first in female patients with LUTS. With the increase of age, bladder sensation and detrusor function decrease. In elderly patients, DU became the first ranked disease instead of SUI. Diabetes can affect the sensory function of bladder in patients with DU, and then increase the difficulty of voiding. The patients with DU, absent from treatment, experienced a lower quality of life.

Abstract:Female stress urinary incontinence(SUI)is the most common incontinence of female urinary incontinence.Female SUI is also an incontinence which could be cured with surgery.In recent years,with upgrading of technology of surgery for female SUI,especially with the widespread using of mid-urethra sling for female SUI,more patients with female SUI had been cured with this popular technology,and also more urologists and gynecologists have been interested in this disease.Since shortage of understanding on the anatomy and function of female bladder and urethra for most urologists and gynecologist who newly work in this field,many complications could occurred in clinical practice on this disease.The current paper would introduce the evaluation of female SUI and its advances in recent years,and try to clarify the mechanism of female SUI and related risk for surgery so that the clinical practice on female SUI would promote to a higher level.
