Abstract:The present study evaluated the simultaneous correction of bilateral temporomandibular joint ankylosis with mandibular micrognathia using internal distraction osteogenesis (DO) with the help of a 3-dimensional craniomaxillofacial model technique.A total of 16 patients (age 18 to 43 years) with bilateral temporomandibular joint ankylosis and mandibular micrognathia were included in the present study. Obstructive sleep apnea and hypopnea syndrome was diagnosed in all patients preoperatively. Three-dimensional craniomaxillofacial models of the 16 patients were constructed using computed tomography and a rapid prototype technique. Simulation surgery and individual internal DO was performed on the models. The treatment included simultaneous DO of the mandibular body and transport DO for temporomandibular joint arthroplasty. The distraction was started on the seventh day after surgery. The distraction rate was 0.8 mm/day. The patients began active mouth opening postoperatively. Distracters were kept in place for 4 months after distraction completion and then removed. Polysomnography, cephalometry, and computed tomography were performed at 6 months postoperatively.The obstructive sleep apnea and hypopnea syndrome was cured, and the micrognathia was corrected in all patients. The average mouth opening increased from 4.6 mm preoperatively to 33.5 mm postoperatively. The average range of the sella-nasion-supramental angle increased from 68.7 degrees preoperatively to 77.6 degrees postoperatively. Bone formation in the distraction gaps was observed. The follow-up period was 29.7 months (range 6 to 52). No complications or recurrence of temporomandibular joint ankylosis or micrognathia occurred in any patient during the follow-up period.Bilateral temporomandibular joint ankylosis accompanied by mandibular micrognathia and obstructive sleep apnea and hypopnea syndrome can be corrected effectively by simultaneous internal DO. The application of preoperative simulation surgery using 3-dimensional craniomaxillofacial model has many advantages for planning the surgical method and precise operation. Our preliminary results have shown that it is a safe, effective, and feasible technique.
[Treatment of Distraction Osteogenesis in the Patients of Obstructive Sleep Apnea-Hypopnea Syndrome with Micrognathia].

Abstract:OBJECTIVE:To apply the treatment of distraction osteogenesis(DO) to obstructive sleep apnea-hypopnea syndrome(OSAHS) patients with croniomaxillofacial deformities.METHODS:All 46 OSAHS patients with micrognathia are had polysomnography(PSG) study and cephalometric analysis. Their age from 4 to 18 years old, the mean age is 11.4. The number of temporal mandibular joint (TMJ) ankylosis with micrognathia, micrognathia; 1st & 2nd bronchial arch syndrome and crouzon syndrome patients were 32, 9, 2 and 3 respectively. All were treated with DO. Maxilla or mandible was advanced from 5 to 35 micrometers; the mean advanced distance is 18.34 mm. They were all revaluated by PSG and cephalometric analysis postoperatively.RESULTS:All patients have good respond to the treatment. They have a better appearance and the narrow upper airway was enlarged remarkably, their AHI drop from 66.31 +/- 14.74 pre-operately to 3.16 +/- 1.70 pro-operately, and minimal posterior airway space(PAS) from (5.48 +/- 2.76) mm to (9.97 +/- 2.05) mm. There is remarkable difference (P < 0.001).CONCLUSION:DO is a good method for the patients of OSAHS with micrognathia.
Application of Distraction Osteogenesis in Treatment of Mandibular Micrognathia Accompanying with Obstructive Sleep Apnea Syndrome

Abstract:Objective: This paper evaluates the effect of distraction osteogenesis in treatment of mandibular micrognathia accompanying with obstructive sleep apnea hypopnea syndrome(OSAHS).Methods: 9 cases were treated by distraction osteogenesis.TMJ arthroplasty was carried out after 3 months.Every patient had been evaluated pre-and postoperatively with cephalometry and polysomnography.Results: After distraction process was finished,PAS and SaO_2 increased and AHI decreased.Subjective symptoms with OSAHS disappered or alleviated,and deformity of all patients was corrected appearently.Conclusion: DO has played important role in clinic treatment of mandibular deformity.

Abstract:OBJECTIVE:To investigate the effect of distraction osteogenesis (DO) in the treatment of severe mandibular micrognathia with severe obstructive sleep apnea and hypopnea syndrome (OSAHS).METHODS:19 cases of severe mandibular micrognathia with OSAHS were treated by DO. All the patients received PSG and MSCT examination before and after DO to evaluate the therapeutic effect and changes in the upper airway.RESULTS:According to the evaluation standard, 17 cases were cured and 2 cases improved markedly. The sagittal distance and area, transverse distance and area of the upper airway increased markedly after DO. The volume of upper airway increased from (15 572.03 +/- 3 370.11) mm3 to (21 182.69 +/- 4 533.15) mm3.The airway change happened mainly in velopharyngeal region and the lingopharyngeal region, but not in the laryngopharyngeal region.CONCLUSIONS:DO can treat severe mandibular micrognathia patients with OSAHS effectively by enlarging the volume of upper airway,especially in the velopharynx and glossopharyngeum region. The MSCT plays an effective and important role.

Abstract:Objective To explore the effect of distraction osteogenesis(DO)in the treatment of juvenile mandibular deformity.Method Five cases of juvenile patients with micrognathia deformity,which 1 case caused from hemifacial microsomia and 4 cases from ankylosis,were treated with distraction osteogenesis,6 distractors were fixed,the changes of facial appearance and relative symptoms were compared preoperatively and postoperatively.Result DO have been finished,the facial deformity of all patients were corrected,the average distraction distance on the affected side reached 23.4 mm(range 17.6 mm to 28.0 mm)after distraction,the relative symptoms were minimized.Expected appearance and quality of bone formation in the gaps achieved.Conclusion DO is a feasible method in the treatment of juvenile mandibular deformity,it is worthy of recommending widely.
Distraction Osteoporosis for the Treatment of Temporomandibular Joint Ankylosis

Abstract:Objective:To observe the curative effect of distraction osteoporosis on temporomandibular joint ankylosis.Methods:Six patients with temporomandibular joint ankylosis and obstructive sleep apnea(OSA)admitted by Dalian Hospital of Stomatology were selected from July 2006to 2011.All patients were treated by distraction osteoporosis and the curative effect was observed by the measurement of incision and distraction length.Meanwhile,the complications and the improvement in OSA and hypopnea syndrome(OSAHS)were observed.Results:All 6patients were included in the final analysis.The lowest distraction length was 30mm and the highest was 32mm with the average length of 31mm.The mouth opening was increased from pre-operative 7.5mm to post-operative 31mm.Follow-up 6month-2years,the mouth opening was 38mm.Bone formation was good,and the X-ray films and wound examination indicated the sclerotized was good.No obvious complication was found in all patients with infection,neutral injury,distractor defluxion or breakage.No relapse was found.OSAHS was improved significantly.Conclusion:The distraction osteoporosis in the treatment of temporomandibular joint ankylosis presents many advantages,for example the simple operation,the lower risk,the confessed curative effects and the lower relapse rate.At the same time,it can cure the OSAHS caused by maxillary deficiency.
Combined mandibular distraction osteogenesis and surgical-assisted maxillary rapid expansion to treat patients with severe skeletal Class Ⅱ malocclusion and narrow maxillary dental arch secondary to bilateral idiopathic condylar resorption

Abstract:PURPOSE:To introduce a treatment approach for severe skeletal Class Ⅱ malocclusion and narrow maxillary dental arch secondary to bilateral idiopathic condylar resorption.METHODS:Six cases with severe skeletal Class Ⅱ malocclusion and narrow maxillary dental arch secondary to bilateral ICR were included in this study,and they were treated with mandibular distraction osteogenesis and surgery-assisted rapid maxillary expansion.The path and magnitude of distraction were simulated using computer-aided surgical simulation technique.The outcome of distraction,the necessity of secondary surgery and stability were evaluated through 4 aspects:profile,occlusion,temporomandibular joint (TMJ) condition and sleep respiratory function at T0,T1,T2 and T3.CT,TMJ questionnaire,MRI of TMJ and PSG were acquired at this 4 time points.RESULTS:Three cases finished all treatments.The distraction magnitudes were 13.0mm,6.7mm and 8.1rm,respectively.Chin advanced in genioplasity for 7.2 mm,0 and 11 mm,respectively.Maxillary dental arch widened 5.3mm,7.3 mm and 4.9 mm,respectively.Polysomnography data revealed that severe obstructive sleep apnea hypopnea syndrome was cured.Skeletal malocclusion was effectively corrected in all 3 cases and there was no obvious relapse during remodeling phase.TMJ condition remained stable.CONCLUSIONS:Mandibular DO and SARME approach can effectively correct severe skeletal Class Ⅱ malocclusion and narrow maxillary dental arch secondary to bilateral ICR.Using CASS technology to simulate the magnitude and path of distraction,the outcomes are more stable and predictable.

Abstract:Objective: To compare the treatment effect of distraction osteogenesis (DO) and maxillomandibular advancement (MMA) for severe obstructive sleep apnea hypopnea syndrome (OSAHS) patients and to guide clinical decisions about treatment of OSAHS. Methods: Thirty-seven OSAHS patients which accepted maxillomandibular advancement (MMA) or distraction osteogenesis (DO) in Stomatological Hospital of the Department of Maxillofacial Trauma and Orthognathic Surgery, School of Stomatology, The Forth Military Medical University from June 2017 to June 2019 were collected. Their preoperative and postoperative data of cephalometry, polysomnography (PSG), Pittsburgh sleep quality index (PSQI) and Epworth sleepiness scale (ESS) scores were collected and analyzed. With propensity score matching method, the treatment effect of MMA and DO was analyzed and compared. Results: According to the statistics of MMA group, only AHI was correlated with operative successful rate and cure rate. With the increase of AHI, the treatment effect of MMA on OSAHS patients gradually decreased. The cut-off point of AHI as a predictor of MMA treatment failure was 78.2 n/h. All the matched cases were severe OSAHS patients. Statistical analysis showed that the mandibular elongation of DO patients[(24.00±4.39) mm] was significantly more than that of MMA group [(11.20±1.37) mm] (t=-6.11, P<0.001), the improvement of PSG index [including lowest oxygen saturation (LSpO2), longest apnea (LA) and longest hypopnea (LH)] in DO group [LSpO2=(93.40±1.82)%; LA=(18.28±8.32) s; LH=(61.84±32.94) s] was significantly higher than that in the MMA group [LSpO2=(86.00±4.06)%, LA=(64.08±21.78) s, LH=(172.40±30.70) s](t=-3.72, P=0.005; t=4.39, P=0.003; t=5.49, P=0.004). The PSQI and the ESS scores of DO group (PSQI=4.20±0.83; ESS=3.40±1.52) were also significantly better than that of MMA group (PSQI=8.80±2.39, ESS=9.40±2.88)(t=4.07, P=0.001; t=4.12, P=0.002). Conclusions: For severe OSAHS patients, the objective and subjective indicators of DO treatment group showed a better therapeutic effect than that of MMA.

Abstract:Severe obstructive sleep apnea syndrome (OSAS) threatens patients' lives. To solve ventilation problem, snoring, and avoid another orthognathic surgery for mandibular advancement, bone-anchored rapid maxillary expansion and bilateral interoral mandibular distraction osteogenesis were tried on a 20-year-old Chinese male patient with severe skeletal class II malocclusion and OSAS. The patient had polysomnography (apnea-hypopnea index 54.2), body mass index measurement (19.7 kg/m(2)), and cephalometry before the treatment. Bone-anchored rapid maxillary expansion was performed for the correction of maxillary transverse and minor sagittal deficiency and the improvement of nasal airflow by decreasing nasal resistance. Bilateral interoral mandibular distraction osteogenesis was operated to lengthen the small, retruded mandible by 15 mm. Orthodontic treatment after the maxillary expansion and mandibular distraction osteogenesis can help obtain stable occlusion. The Epworth Sleepiness Scale, a questionnaire for temporomandibular joint, cephalometric analysis, polysomnography, acoustic rhinometry, and multislice spiral computed tomography were performed to evaluate changes from the treatment. All the results showed that the patient had a significantly alleviated OSAS. In addition, an acceptable occlusion was also obtained.
[Distraction Osteogenesis of the Mandibular Ramus in Hemifacial Microsomia].

Abstract:OBJECTIVE:To study the application of distraction osteogenesis (DO) of the mandibular ramus in hemifacial microsomia.METHODS:From 1999 to 2006, 15 patients with hemifacial microsomia were treated with DO of the ramus, including 12 cases of children and juveniles, 3 cases of grown-ups. Maxillary Le Fort I osteotomy, unilateral sagittal split ramus osteotomy, genioplasty and free scapular flaps were selected to perform on the first or secondary stage of treatment.RESULTS:Successful distraction was achieved in all patients with an average distraction distance of 21.25 mm. The distractor was removed after consolidation proved by clinical examination and X-ray. Facial appearance and and occlusal relationship were improved greatly. The occlusal plane was almost normal. There was no infection, malunion or permanent injury of inferior alveolar nerve.CONCLUSIONS:DO of the ramus is very effective for the correction of severe hemifacial microsomia. It has much better therapeutic effect than traditional orthognathic procedures.
Treatment of Postoperative Midfacial Deformity of Chilopalatognathus by Distraction Osteogenesis with A Self-Constructed Tooth-Borne Distraction Device

Abstract:Objective: Distraction osteogenesis has recently evolved a challenging technique to overcome major drawbacks of the traditional orthodontic treatment modalities. The aim of this study was to evaluate the therapeutic efficacy of patients with chilopalatognathus who have premaxillary deficiency through distraction osteogenesis using a self-constructed tooth-borne distraction device.Material and Methods: Individual tooth-borne distraction devices were used for advancement of the maxillary anterior segment. Distraction was performed for 26 patients in accordance with the specific requirements of each individual. Cephalometric radiographs were taken before treatment (T1), after distraction (T2), and after consolidation for 8 weeks (T3).Results: Cephalometric analysis revealed that the premaxilla was moved forward and that the length of palatal plane increased. In 2 cases, the distractor did not work during distraction and was removed.Conclusions: Distraction osteogenesis using individual tooth-borne distraction devices in patients with chilopalatognathus could effectively resolve soft tissue insufficiencies and hypoplasia of the maxilla.
[Management of Micrognathia Deformity Associated with Obstructive Sleep Apnea Syndrome by Distraction Osteogenesis].

Abstract:OBJECTIVE:To review the early results of lengthening of the mandible by distraction osteogenesis for the correction of micrognathia deformity and treatment of the obstructive sleep apnea syndrome (OSAS) simultaneously.METHODS:5 cases with micrognathia deformity associated with OSAS resulted from unilateral/bilateral temporomandibular joint ankylosis had undergone mandibular distraction osteogenesis. In all of the patients intraoral device was employed. Cephalometric radiographs and polysomnograms were obtained preoperatively and postoperatively.RESULTS:The average distraction distance was 15.28 mm (range 8.5 mm to 24.4 mm). The micrognathia deformity was corrected effectively, at the mean time the incapacious pharyngeal airway was ameliorated. The patients' Apnea Index (AI) had been decreasing, SaO(2) during sleep had got to rise.CONCLUSIONS:Compared with the traditional surgery for the micrognathia associated with OSAS, the technique of mandibular distraction osteogenesis has the advantages of avoiding bone grafting and donor side morbidity, expanding the surrounding soft tissue concurrently, reducing the extent of operation and stability of the treatment effects.
[Distraction Osteogenesis for Treatment of Unilateral Temporomandibular Joint Ankylosis and Secondary OSAHS in Children: Report of 4 Cases].

Abstract:PURPOSE:To evaluate the efficiency of distraction osteogenensis for treatment of unilateral temporomandibular joint ankylosis and secondary OSAHS in children.METHODS:4 children with temporomandibular joint ankylosis and secondary OSAHS were retrospectively reviewed for their treatment. There were 2 males and 2 females, ranging from 5 to 13 years in age with the median of 6.5 years. Based on the history of the disease, the clinical manifestations, CT scan and PSG findings, the diagnosis was confirmed to be unilateral temporomandibular joint ankylosis and secondary OSAHS. All the patients were treated by gap arthroplasty to restore mouth opening, combined with mandibular body distraction osteogenesis for treatment of OSAHS. One-stage operation was performed on 3 patients, and two-stage operation on 1 patient. Unilateral and bilateral mandibular distraction osteogenesis was undergone on 2 patients individually. At the end of consolidation period of 3 months, the PSG examination was performed again and the distractor was removed.RESULTS:OSAHS was cured, the average AHI was lowered from 42.7 to 4.9, the average lowest saturation of blood oxygen rose from 74.3% to 89.8%, the average incisor distance reached 25.5mm compared to 6.5mm before surgery and facial deformity was corrected satisfactorily. Following up 38.1 months (13-58 months), no relapse appeared.CONCLUSION:Mandibular body distraction osteogenesis, combined with gap arthroplasty, could be the potential treatment modality for children with unilateral temporomandibular joint ankylosis, secondary OSAHS and facial asymmetry; it is recommended that both procedures be performed at the same time.
[Preliminary Study on Orthodontic Treatment Before and after Distraction Osteogenesis].

Abstract:OBJECTIVE:To get some guidelines for the orthodontic treatment before and after distraction osteogenesis (DO).METHODS:DO was performed in 4 patients maxilla and two for mandible. The patients were aged from 19 to 25 (average 21.5). We try to sum up the procedures and clinical experiences of pre- and post-surgery orthodontics.RESULTS:The procedures and experiences obtained are the followings: 1. Pre-surgery orthodontics for DO is simplified as alignment of dental arch in a short period. 2. Post-surgery orthodontics for DO is more complicated and peculiar. The main task is to make the opened posterior teeth occlude as soon as possible by light force vertical elastics. At the same time, some cross elastics may be needed for correction of posterior occlusal relationship. 3. Expansion of upper arch may be needed in some patients because of constricted dental arch. 4. Class II or III elastics may be used for prevention of relapse after DO.CONCLUSIONS:DO is a new technique in orthognathic surgery. Post-surgery orthodontics is more complicated and important in the treatment course.
Distractor with Supporting Computer Program for the Reconstruction of Arc Defects in Mandible by Distraction Osteogenesis

Abstract:Distraction osteogenesis as a technique seems to be a promising method in craniofacial bone lengthening and reconstruction through the process of bone and soft-tissue regeneration under stimulation of gradual tensile stress. Distractor plays a very important role especially in the reconstruction of large, irregular bone defects. We designed a new-type distractor and supporting computer program for the reconstruction of arc defects in the mandible by distraction osteogenesis. This distractor was applied for the reconstruction of the monkeys' artificial mandibular median defects. Owing to computer program, perfect distracting plan was made to finish this task. X-ray files showed that the mandibular defects in all animals were repaired successfully, and the anterior mandibular contours were similar to the normal ones. The success of distraction osteogenesis for repair of mandibular median defects in monkeys suggests that this distractor and supporting computer program met the needs of the reconstruction of arc defects in the mandible by distraction osteogenesis.

Abstract:Condylar defects and ramus hypoplasia usually result from trauma,temporomandibular joint ankylosis,congenital and acquired deformities,which may lead to disturbed masticatory function and compromised facial aesthetics.This common clinical disease seriously impairs patient's physical and mental health.Without immune reaction and donor site injury,distraction osteogenesis is an ideal modality in the reconstruction of condylar defect.Condyle-like structure and morphology can be obtained by precisely adjusting distraction vector and distance.The deficiency of soft tissue can also be corrected in the distraction process.This paper summarizes the application of distraction osteogenesis to the condylar defect reconstruction based on author's experience and relevant literature,emphasizing the time of treatment,preoperative design and simulation,as well as discussing the intraoperative and post-operative key technical points.
Distraction Osteotomy Combined With Orthodontics: An Effective Way to Decrease Short-term Recurrence in Treating Child Craniofacial Microsomia

Abstract:Background: This study aims to investigate strategies for reducing early relapse following mandibular distraction osteotomy (MDO) in craniofacial microsomia (CFM) patients. Methods: Forty CFM patients were divided into 2 groups: 20 underwent MDO alone, while 20 received MDO combined with orthodontic treatment (MDO-O). Preoperative, prefixation, and postfixation CT data in DICOM format were collected and reconstructed using Proplan software. Spatial geometric analysis was conducted to assess the inclination angle of the occlusal plane (OP), and OP deviation was directly measured using the software. Results: Patients in the MDO-O group demonstrated a statistically significant improvement in OP inclination at the end of the fixation period compared with those who underwent MDO alone ( P =0.003). Conclusions: Combining MDO with a bite plate and bite glue sticks in CFM patients during the mixed dentition period enhances OP outcomes, effectively reducing short-term recurrence.

Abstract:Distraction osteogenesis has been successfully used to restore the deformities of craniomaxillofacial surgery these years,while it has provided various advantages over conventional methods and has a widely used in future.However,to obtain the optimal results from distraction osteogenesis,one must be take enough attention to potential complications or unexpected events and how best to minimize or avoid these.In this article,we will make a review about the above issues.

Abstract:OBJECTIVE:To evaluate the effect of distraction osteogenesis in correction of severe undeveloped maxilla secondary to cleft palate and to evaluate the selection of distraction modality.METHODS:Distraction osteogenesis was performed upon 8 patients, 6 males and 2 females, aged 11 to 25 years, one with incomplete cleft palate, 4 with unilateral complete left palate, and 3 with bilateral complete cleft palate, all accompanied with severe maxillary hypoplasia. The 7 adult patients were treated by modified high-stepped Le Fort I osteotomy, the other patient, a child, was treated by Le Fort I osteotomy followed by rigid external distraction. X-ray films of skull in lateral, frontal, or panoramic positions, and X-ray film of temporomandibular joint in Schiller's position were taken pre- and postoperatively. In model surgery the accurate distraction distance was measured and the direction of distraction designed. Orthodontic therapy was started immediately after the distraction was completed and the distractor was removed after a period of 4 months' consolidation.RESULTS:The designed distraction was achieved in a11 8 patients. Except in one case with a bone defect 1 cm x 0.5 cm at the osteotomy line in the lateral wall of the right maxillary sinus, dense new bone was formed in the area of distraction in the other cases. The postoperative occlusal relationship was good and stable. No infection and other complication was found. During an average of 20 months' follow-up, the maxilla and occlusion were stable, and no relapse was found. The distance of forward distraction of the maxilla reached an average of l2 mm (5 approximately 15 mm). The SNA angle increased from 71 degree on average preoperatively to an average of 79 degree postoperatively. Both the facial appearance and occlusal relationship returned to normal in a11 the patients.CONCLUSION:Without need of bone grafting and with a stable effect and little influence on platatopharyngeal competence, distraction osteogenesis is an effective method of correcting maxillary hypoplasia secondary to cleft palate and is worthy of spreading.

Abstract:To evaluate the effect of distraction osteogenesis in correction of micrognathia accompanying obstructive sleep apnea syndrome, a total of 28 patients with different severities of obstructive sleep apnea syndrome underwent mandibular distraction osteogenesis. A total of 51 distraction devices were placed for bilateral distraction in 23 patients and for unilateral distraction in five patients. The mean age of patients was 21.2 years (range, 3 to 60 years). Eleven patients had micrognathia accompanying obstructive sleep apnea syndrome secondary to bilateral temporomandibular joint ankylosis, and 10 patients had micrognathia accompanying obstructive sleep apnea syndrome secondary to unilateral temporomandibular joint ankylosis. Three patients had developmental micrognathia accompanying obstructive sleep apnea syndrome. The other four patients had micrognathia and concomitant obstructive sleep apnea syndrome induced by trauma, infection, or tumor resection. Each patient had been evaluated preoperatively and postoperatively with cephalometry and polysomnography. Mandible advancement ranged from 9 to 30 mm (average, 20.4 mm) and was successfully achieved after distraction. Fine new])one formed in the distraction gap when the distraction devices were removed 3 to 4 months after distraction was completed. No infection or other complications occurred in any patients. Complete curative effects were achieved in nine severe, six moderate, and eight mild obstructive sleep apnea syndrome patients after distraction, and the other five patients had been improved to the mild level. After distraction was completed, the posterior airway space was increased on average from 4.6 mm to 12.5 mm and the sella-nasion-point B angle was increased on average from 66 degrees to 75 degrees on cephalometric studies. The polysomnographic examination showed that the apnea hypopnea index was lowered on average from 58.0 to 3.15, and the lowest oxygen saturation was increased on average from 77 percent to 90.3 percent after distraction was completed. The follow-up period was 3 to 61 months (average, 18.1 months). The curative effect was stable and no relapse occurred. Therefore, the authors conclude that mandibular distraction osteogenesis is an effective method for correcting micrognathia accompanying obstructive sleep apnea syndrome. Compared with other current routine surgical procedures, it has many advantages, such as low risk, simple manipulation, high curative rate, low relapse rate, and stable result. It is presently the most effective method for the treatment of this difficult and complicated disorder.
