Anterograde or Retrograde Change:Magnetic Resonance Imaging on Amyotrophic Lateral Sclerosis

Abstract:Objective To discuss water molecular diffusion motion and metabolite changes along cortical spinal tract(CST) in amyotrophic lateral sclerosis (ALS) with diffusion tensor imaging (DTI), magnetic resonance spectroscopic imaging (MRSI) and magnetic resonance imaging, and to observe if there is anterograde or retrograde pathologic changes. Materials and Methods DTI and MRSI were used in 12 ALS patients and 12 normal volunteers. Fractional anisotropy (FA) and mean diffusivity (MD), and the ratio of N acetyl asparte (NAA) to creatine (Cr) were measured and analyzed at 5 different sites along the CST, including the subcortical white matter (SWM), centrum semiovale (CS), periventricular white matter (PV), posterior limb of the internal capsule (PIC) and cerebral peduncle (CP). Results FA ratios in ALS showed significantly lower than those in controls within the CST (P<0.001), including the SWM, CS, PV and PIC (P<0.05). Although there was a trend towards elevated MD in ALS patients, it did not reach statistical significance. NAA/Cr ratios at SWM and PV were both decreased in ALS patients compared to normal controls (P<0.05). FA ratios at PV in the left side was much higher than those in the right side (P<0.05) and MD at CP had similar change. Conclusion Combined whole brain DTI and MRSI can in vivo detect axonal degeneration along CST in ALS. Left SWM is a possible early location of ALS onset, and the degeneration of CST in ALS is probably anterograde and asymmetric at different levels.
