Abstract:Closed reduction is applied in most cases of isolated zygomatic arch fracture. Open reduction and internal rigid fixation through a coronal incision have to be performed in other comminuted arch fractures. The authors introduced an endoscopic-assisted approach via a small preauricular incision to achieve reposition and osteosynthesis of isolated zygomatic arch fractures. The endoscopic-assisted surgical technique was performed in 7 patients with unilateral isolated zygomatic arch fractures. Endoscopically controlled management of the isolated zygomatic arch fracture was feasible in all 7 patients. The follow-up period was 1 year. All preauricular scar and facial lateral contour were aesthetically satisfactory. The endoscope-assisted approach via a small preauricular incision can achieve in situ reduction and fixation in zygomatic arch fracture and it should become an integral part of isolated zygomatic fracture repair, assuming the development of specialized training programs and improvements in endoscopes.
Endoscopic-assisted open treatment of displaced subcondylar mandibular fracture in adults

Abstract:PURPOSE: To evaluate the feasibility of the endoscopy assisted transoral approach for open reduction of subcondylar mandible fractures with limited incisions in order to reduce the risk of facial nerve damage and extensive visible scars. METHODS: From April 2003 to December 2005, the minimally invasive transoral approach for endoscopically assisted reduction and osteosynthesis of 11 displaced subcondylar mandible fractures (with/ without mandibular symphysis fracture and zygomatic maxillary complex fracture) was performed in 11 consecutive patients. By means of limited transoral incision, the endoscopically assisted reduction and fixation of condylar fractures was performed using 30-degree angled endoscopes. All the 11 fractures were subcondylar. The condylar neck of the proximal fragment was displaced medially in 4 fractures and laterally in 7 fractures. RESULTS: Using angled endoscopes, good visibility of the fracture site was obtained, which allowed for precise anatomical reduction in 9 out of 11 patients. A transbuccal drill and screwdriver facilitated miniplate fixation by a 3mm-in-length skin incision. Postoperatively,all patients showed limited swelling, quick recovery to pre-injury occlusion and short in-hospital time. No facial nerve injury and visible scars were observed. Normal temporomandibular joint function was noted 6 months after surgery in all patients. CONCLUSIONS: The transoral endoscopically assisted treatment using an transbuccal drill and screwdriver is the method of choice for surgical management of displaced subcondylar fractures. Facial nerve injury and visible scars can be avoided by using the transoral approach.
Endoscopic-assisted repair of subcondylar fractures

Abstract:Objective: To evaluate outcomes of a series of mandibular subcondylar fractures repaired with endoscopic reduction and fixation. Study design Six consecutive subcondylar fractures were treated endoscopically. Intermaxillary fixation was used intraoperatively to aid in fracture reduction. A modified Risdon incision was used to gain access to the lateral ramus, and a modified retractor and endoscope were used for retraction and visualization. Fracture fixation was achieved with a 2-mm titanium plate and screws. Patients were evaluated clinically and radiographically for 6 months and functional, radiographic, and esthetic parameters were assessed at each time period (1, 2, 4, 12, and 24 weeks). Results: All patients demonstrated a stable occlusion in the postoperative period and anatomic alignment of the condyle radiographically. By 1 month, maximum interincisal opening was 42.2 +/- 5.7 mm. There was no joint noise or temporomandibular joint (TMJ) pain postoperatively. Radiographs at each follow-up visit indicated the ramus height was maintained in most cases. There was minimal transient facial nerve paresis following surgery. Scar perception was considered acceptable by all patients. Operative times were acceptable as well. Conclusion: Endoscopic-assisted repair of subcondylar fractures is an additional tool for management of subcondylar fractures, however there is a steep learning curve based on this study. The technique allows good visualization of the fracture site for reduction through an incision with an acceptable cosmetic result.
Treatment of Mandibular Condyle Fractures Using A Modified Transparotid Approach Via the Parotid Mini-Incision: Experience with 31 Cases

Abstract:Surgery for mandibular condyle fractures must allow direct vision of the fracture, reduce surgical trauma and achieve reduction and fixation while avoiding facial nerve injury. This prospective study was conducted to introduce a new surgical approach for open reduction and internal fixation of mandibular condyle fractures using a modified transparotid approach via the parotid mini-incision, and surgical outcomes were evaluated. The modified transparotid approach via the parotid mini-incision was applied and rigid internal fixation using a small titanium plate was carried out for 36 mandibular condyle fractures in 31 cases. Postoperative follow-up of patients ranged from 3 to 26 months; in the first 3 months after surgery, outcomes for all patients were analyzed by evaluating the degree of mouth opening, occlusal relationship, facial nerve function and results of imaging studies. The occlusal relationships were excellent in all patients and none had symptoms of intraoperative ipsilateral facial nerve injury. The mean degree of mouth opening was 4.0 (maximum 4.8 cm, minimum 3.0 cm). No mandibular deviations were noted in any patient during mouth opening. CT showed complete anatomical reduction of the mandibular condyle fracture in all patients. The modified transparotid approach via the smaller, easily concealed parotid mini-incision is minimally invasive and achieves anatomical reduction and rigid internal fixation with a simplified procedure that directly exposes the fracture site. Study results showed that this procedure is safe and feasible for treating mandibular condyle fracture, and offers a short operative path, protection of the facial nerve and satisfactory aesthetic outcomes.

Abstract:Objective: To introduce a novel modified periauricular approach and rigid internal fixation in treating condylar fractures.Methods: 36 condylar fractures were retrospectively reviewed.Occlusion,maximum mouth opening,facial nerve function and 3D-CT were used in evaluating the postoperative results.Results: All patients showed good occlusion and maximum mouth opening.No symptom of facial nerve injury occurred.CT scans revealed that all condyles were anatomically reducted.Conclusion: Compared with traditional approach,modified periodic transparotid approach has minimum invasion.The modified incision is concealed and feasible for facial nerve protection.The condyle can also be anatomical reducted with rigid internal fixation.
Clinical outcomes of endoscopic vs retromandibular approach for the treatment of condylar fractures-a randomized clinical trial

Abstract:Objective: Condylar fractures remain the most controversial topic in maxillofacial trauma. Open reduction and internal fixation (ORIF) with use of an extraoral approach has certain benefits over the nonsurgical treatment. Concerns, however, remain about the risk of facial nerve injury, postoperative facial nerve weakness, and facial scarring when operating in this region despite the various extraoral approaches that have been developed. The recently popularized endoscope-assisted open reduction and internal fixation (EAORIF) is claimed to provide better results because it is minimally invasive, provides excellent visibility, and eliminates surgical scarring and the risk of facial nerve injury. This study, therefore, aims to compare the retromandibular (extraoral) approach and EAORIF for the treatment of condylar fractures. Study design: A prospective analysis of 32 cases of condylar fractures that reported to the SDM Craniofacial Centre (Dharwad, India) was carried out. Sixteen patients had been treated with the retromandibular (RM) approach, and 16 had undergone EAORIF in the period from 2012-2017. Patients were evaluated for clinical parameters, such as fracture site, displacement, and surgical duration, as well as for functional parameters, such as occlusion, maximum interincisal opening, deviation of mouth on lateral movements, temporomandibular joint (TMJ) pain and clicking, and facial nerve weakness. Statistical significance was elicited with P < .05. Results: The patients subjected to either approach had suffered fractures of the low condylar neck and the subcondyle. Maximum interincisal opening, mandibular deviation, occlusion, and TMJ function at postoperative month 6 were comparable between the groups. Although no permanent facial nerve injury was seen in this study, the incidence of transient facial nerve weakness was higher in the RM group (56.25%) compared with the EAORIF group (6.25%) (P = .036). The surgical time was longer in the EAORIF group (RM: 107+/- 19.7 minutes and EAORIF: 155 +/- 18.2 minutes) (P = .04). The rest of the parameters were comparable between both groups. Conclusions: Although there is consensus on closed reduction in pediatric and adult condylar head fractures, the role of a surgical approach to treatment of displaced condylar neck and subcondylar fractures remains controversial. In our study, both surgical approaches were found to be suitable for the treatment of these fractures.
Transoral Endoscopic-Assisted Reduction and Internal Fixation of Mandibular Condylar Fractures in Children

Abstract:Purpose: Extraoral approaches for open reduction and fixation of condylar fractures in children are associated with serious risks of complications, including facial nerve injury, facial scarring, parotid fistula, and auriculotemporal nerve injury. The purpose of this study was to retrospectively evaluate the outcomes of transoral endoscopic-assisted open reduction and internal fixation of condylar fractures and hardware removal in pediatric patients. Material and methods: This study was designed as a retrospective case series. The study included pediatric patients admitted with condylar fractures that were indicated for treatment with open reduction and internal fixation. The patients were clinically and radiographically evaluated with regard to occlusion, mouth opening, lateral and protrusive movement of the mandible, pain, chewing and speech difficulties, and bone healing at the fracture site. Computed tomography images were used to assess the reduction of the fractured segment, the stability of fixation and progress of healing of the condylar fracture at follow-up visits. The same surgical treatment approach was applied to all patients. The data from the study were analyzed for a single group without any comparison to other groups. Results: The technique was used for the treatment of 14 condylar fractures in 12 patients between the ages of 3 to 11 years. A total of 28 transoral endoscopic-assisted approaches to the condylar region either for reduction and internal fixation or hardware removal were applied. The mean operating time was 53.1 (±11.3) minutes for the fracture repair and 20 (±2.6) minutes for hardware removal, respectively. The mean follow-up time of the patients was 17.8 (±2.7) months (median: 18) months. All patients regained stable occlusion, satisfactory mandibular motion, stable fixation, and complete bone healing at the fracture site at the end of their follow-up period. There was no transient of permanent facial nerve or trigeminal nerve injury in any of the patients. Conclusions: Endoscopically assisted transoral approach is a reliable technique for reduction and internal fixation of condylar fracture and hardware removal in pediatric patients. The serious risks of extraoral approaches including facial nerve injury, facial scar, and parotid fistula can be eliminated by using this technique.
Modified anterior approach for treatment of ulnar coronoid process fracture

Abstract:Objective To investigate the clinical effect of modified anterior approach to manage fracture of the ulnar coronoid process via the space of brachial artery and vein with median nerve.Methods From June 2012 to January 2013,11 patients with ulnar coronoid fracture were fixed via the modified anterior approach.The operation time,intraoperative blood loss and postoperative complications were recorded.Flexion and rotation range of motion about the injured and normal elbow were observed during postoperative follow-up period.Function of elbow joint was evaluated by mayo elbow performance index (MEPI).Results There was approximate 8 cm in length and 5 cm in width between the brachial vessels and median nerve.Operated angle from radial to ulnar side was fifty degrees and from proximal to distal end was sixty degrees.All the patients were available for follow-up.The fracture healed,that is the elbow flexion restored [(130.7 ±5.0) °] was 96.6％ of the unaffected elbow,elbow extension restored [(7.6 ± 8.1) °] was 84.0％ of the unaffected elbow,pronation restored [(86.9 ± 3.8) °] was 98.2％ of the unaffected side,and supination restored [(85.6 ± 6.0) °] was 96.7％ of the unaffected side.MEPI of the elbow joint was over 75 points.Conclusion Modified anterior approach is relatively safe and simple in operation and results in satisfactory function recovery of the elbow joint,providing a new surgical approach for treatment of coronoid process fracture.

Abstract:Background: The traditional preauricular incision allows adequate exposure, but risks facial nerve injury and results in visible facial scars. The ideal approach for intracapsular condylar fractures should allow an excellent view of the surgical field, facilitate handling of the fracture, while minimize the risk to the facial nerve and leavet he scar in a barely noticeable region.

Abstract:OBJECTIVE:This is a preliminary study of the endoscopic technique on open surgery. The purpose of this study is to introduce the use of endoscopic technique for the reconstruction of the condyle with costochondral graft through a preauricular incision.METHODS:33 patients of temporomandibular joint diseases (n=49) underwent reconstruction of mandibular condyle with costochondral graft. After preparation of the recipient site through a preauricular incision, maxillo-mandibular fixation, and preparation of the costochondral graft, the graft was fixed to the lateral side of the mandible ramus under the supervision of arthroscope.RESULTS:All the patients had successful reconstruction of the condyle with costochondral graft without any severe bleeding or craniocerebral injury.CONCLUSION:This case series demonstrates the feasibility of endoscopic technique for the reconstruction of the condyle through a preauricular incision. It has the advantage of high efficiency, minimal postoperative morbidity, great patient comfort, and little appearance impairment. The endoscopic technique has a promising future.
Le Fort I Osteotomy Combined with Endoscopic Assistance for Treatment of Compound Fracture of Maxilla, Zygoma, and Orbital Floor.

Abstract:PURPOSE:The traditional surgery to reconstruct the compound fracture of the zygoma, maxilla, and orbital floor was usually open reduction and internal fixation using miniplate, and surgeons now can perform the endoscopic repair of relatively simple zygoma or orbital blowout fracture. In this study, we try to reconstruct midfacial complex fracture by combined application of intraoral approach and endoscopic-assisted minimally invasive method.METHODS:Six patients with traumatic midfacial fracture, including maxilla, zygoma, and orbital floor fracture, were selected. Intraoral Le Fort I osteotomy approach and endoscopic-assisted minimally invasive method were combined and applied to treat this complex midfacial fracture.RESULTS:The intraoral incision combined with endoscope offered suitable approach for reduction and fixation of fractured zygoma. The Le Fort I osteotomy could help to effectively reduce the fractured maxilla and offered useful operative approach for endoscope. The endoscope combined with a balloon catheter could successfully reconstruct the orbital floor fracture, and no intraoperative complications were encountered. The balloon catheter was removed 4 to 8 weeks after operation and did not lead to infection and obviously disrupt the healing of bone segments. The postoperative eye and occlusion function, evaluated by clinical examination, was satisfactory at 3 months.CONCLUSIONS:Although there are some limitations and strict indications, the advantages of this method may offer alternative choice for reconstruction of compound midfacial fracture.
Simplified Method for Reduction of Displaced Condylar Head Fracture Using a Monocortical Screw

Abstract:Background: Condylar head fractures is difficult to treat because of medial displacement caused by muscle pull and limited access to visualize the proximal segment and secure them back to anatomic position. A novel technique is proposed to overcome the difficulties and ease the task of a surgeon. Purpose: The main goal in Open reduction and internal fixation is to achieve anatomical position and establish occlusion, a task possible only after securing the segment back into place. Over decades, many authors have described various techniques to ease the task yet surgeons are forced to treat cases using closed technique which adds limitation to the rehabilitation of stomatognathic system for a prolonged time. Methods and materials: Self holding screw driver, 1.5x10mm monocortical screws and drill bit. Using any preauricular access incision, dissection is proceeded anteromedially and the medial segment is identified. A drill hole is made with 1.2 mm diameter drill with counter using periosteal retractor, monocortical screw fixed and segment secured using a self-holding driver and lateralized after increasing the joint space thus achieving the original retromandibular height. Results: The treatment outcomes are better achieved with ease to surgeon and with minimal armamentarium without any special needs per se. Conclusion: The technique proposed is novel and can help achieve desired outcomes even in hands of budding surgeon with minimal time and efforts invested.
Open reduction of subcondylar fractures using a new retractor

Abstract:Many operative approaches have been described for the open reduction of subcondylar fractures and rigid fixation. However, fracture portions are deep and embedded among facial nerves so that visual surgery in this region is extremely limited. Once the operative field is exposed, the displacement of the condylar head is often dislocated by the anteromedial pull of the lateral pterygoid muscle and the fracture end of the condylar process is pulled up to the mandibular fossa by contraction of the masseter muscle. We made a new retractor to achieve a better field of view. It is possible to pull down the condylar process by opening the tips of the retractor using the specially made wrench system without special effort and keep the condylar process in the same position during reduction. In using this retractor, the fracture stumps were clearly exposed and more easily reposited.

Abstract:Objective:To introduce the endoscopic-assisted vertical ramus osteotomy (VRO) for the treatment of condylar osteochondroma and discuss its indications and effectiveness. Methods: 5 cases with unilateral osteochondroma of mandibular condyle but without severe occlusal deviation were treated with endoscopic-assisted VRO in our department from April 2013 to May 2015. CT imagings were taken for diagnosis and treatment planning. Occlusion, facial contour and maximum mouth opening (MMO) were recorded and compared before and after operation to evaluate the effectiveness of the therapy. Results: 5 patients were treated with endoscopic-assisted VRO and rigid fixation. 1 patients accepted an additional mandibular contour shaping. The pathologic diagnosis were all confirmed as osteochondroma. A mean of 28.6 months follow-up for all 5 cases showed stable post-operative occlusion and facial situation. No functional disturbance were found according to the MMO. Conclusion: For osteochondrama patients without severe malocclusal, endoscopic-assisted VRO is a good option while orthognathic surgery might not be carry out due to patients' situation. This operation achieves two targets, tumor resection and occlusion rehabilitation, as well as reduces the trauma. It is a powerful supplement to the traditional surgical procedures.
Clinical Analysis of Condylar Neck Fracture Using Preuricular Approach

Abstract:Objective To evaluate the treatment of a surgical approach for open reduction and internal fixation of condylar neck fracture.Methods A retrospective study was conducted in 17 cases with condylar neck fractures for open reduction and internal fixation using preauricular approach.Results During the follow-up conducted over 1 to 2 years,X-ray and CT examinations revealed good fracture healing.The occlusal relationship and degree and type of mouth opening improved significantly over those before operation,no case of ankylosis and/or salivary fistula was observed.Conclusion Compared with other traditional surgical incision approach,this method can completely expose operative field to facilitate reduction and fixation.In additions,it substantially reduces the risk to the facial nerve and salivary fistula.
Repositioning Complex Midface Fractures via Coronal Incision Combined with Subciliary Incision Approach

Abstract:Objective:To discuss the application of coronal and subciliary incision approach in the treatment of complex mid-face fracture.Methods:28 cases were selected and treated with approach of coronal and subciliary incision.Fractures were reducted and fixed with titanium-miniplates and microplates.Results:All cases had been restored with norma1 face profile and good occlusion.No facial paralysis had been found.Conclusion:Rigid internal fixation has advantage over conventional intermaxillary fixation.It improves the stability of fracture fixation,promotes fracture healing and guarantees the rehabilitation of mid-face.

Abstract:Ear, Nose &Throat Journal, Ahead of Print. Condylar fractures are the most common mandibular fractures, and a variety of open and closed approaches are used for repair, each with its own inherent risks and benefits. A 27-year-old man had a severely displaced and angulated high subcondylar fracture with anterior open bite deformity. The patient underwent open reduction and internal fixation of the condyle via a combined preauricular and intraoral approach as well as maxillomandibular fixation to repair his fracture. This combined approach allows the surgeon to maximize exposure while limiting risks of permanent injury from surgery.

Simple Way of Facilitating Reduction the Fragment for Fractures with Condyle

Abstract:To describe a technique that uses a fixation screw with a steel wire ring which is implanted into the mandibular to facilitate the condylar fracture reduction. This technique has been used on 382 patients over the last 8 years from the Department of Oral and Cranio-Maxillofacial Science of Shanghai Ninth People's Hospital, School of Medicine, Shanghai Jiao Tong University, Shanghai, China. No complication has been encountered. Applying this approach, the displaced proximal condylar fragment can be found quickly, and then the fragment can be effectively prevented from being broken again in the reduction process.

The inclusion criteria were 1) a medical record of surgical treatment of a subcondylar fracture by RMA; 2) preoperative and postoperative radiographs; 3) mental status permitting an adequate neuromotor examination; 4) absence of a post-injury or pretreatment functional facial nerve deficit; and 5) regular postoperative follow-up longer than 6 months with documentation of complications, functional results, and fixation stability. The predictive variables were age, gender, fracture site, fracture pattern, concomitant fractures, etiology, and plate types. The outcome variable was FNP. Univariate, bivariate, and multiple logistic regression statistics were computed.RESULTS: Fifty patients with 55 displaced mandibular subcondylar fractures (35 men, 15 women; mean age, 44.5 yr; range, 17 to 87 yr) met the inclusion criteria. The condylar fracture involved the neck in 35 patients (63.6%) and the base in 20 patients (36.4%). The fracture pattern was deviation in 11 patients (20.0%), displacement in 23 (41.8%), and dislocation in 21 (38.2%). Precise ORIF with double-buttress fixation resulted in immediate functional recovery in all patients. Seven fractures (12.7%) were associated with FNP that resolved completely within 6 months. Further statistical analysis showed that dislocated and displaced condylar neck fractures were significant risk factors for postoperative FNP (P < .05). Other postoperative complications were minimal.CONCLUSION: The RMA for subcondylar fractures is feasible and safe. Dislocated condylar neck fractures are associated with a highly increased risk of temporary postoperative FNP as a surgical complication.

Computer-Aided Autogenous Coronoid Process Graft Combined With Median and Unilateral Sagittal Split Osteotomy for Late Reconstruction of Condylar Fracture and Occlusion after Trauma

Abstract:Purpose: This study proposes a high-precision surgical technique that combines both computer-aided design/computer-aided manufacturing (CAD/CAM) and the screw-track transfer technique for condylar and occlusal reconstruction. Materials and methods: A 43-year-old male patient with facial asymmetry, limited mouth opening, and malocclusion underwent condylar and occlusal reconstruction surgery. An autogenous cor-onoid process graft was performed by combining CAD/CAM and the screw-track transfer technique. Results: The surgical plan was transformed successfully from preoperative virtual surgical planning to actual surgery; this was confirmed by merging the postoperative computed tomography images with the preoperative three-dimensional surgical design. The patient recovered well and had better occlusion and facial symmetry, as well as an increased degree of mouth opening post-surgery. No complications were observed. Conclusions: CAD/CAM combined with the screw-track transfer technique is a precise and feasible method that can be applied to autogenous coronoid process grafts. This approach can be used to reconstruct the condylar process and achieve a good occlusal relationship.
