Abstract:A 55-year-old female patient underwent surgery for her thyroid lumps 10 years ago and pathological examination confirmed the diagnosis of primary papillary carcinoma of the thyroid. The metastasis of neck lymph nodes was conformed 8 years after operation, and the patient received metastatic lymph nodes resection. During the 3-year follow-up the patient had recurrent tumor. The pathological results showed that the lesion changed from papillary carcinoma to papillary carcinoma accompanied with squamous differentiation and transformed to squamous cell carcinoma of the thyroid at last. Finally the patient died from deterioration of the disease rapidly. From the literature review, it has been shown that the squamous cell carcinoma of the thyroid might originate from adenocarcinoma or squamous metaplasia of papillary carcinoma. Once the transformation occurs, the progress of the disease may be accelerated. This transformation is rare and usually confers a poor prognosis.

Abstract:BackgroundAdrenal tumours are common in urology and endocrinology, and the diagnosis of adrenal tumours were mainly depends on imaging diagnosis. Howerver, misdiagnosis can still occur for some adrenal space-occupying lesions without specific manifestations or abnormal biochemical indexes. MethodsWe report the case of a 55-year-old patient with a soft-tissue mass in the left adrenal region, and have no specific manifestations or abnormalities in biochemical indexes. The patient had undergone open splenectomy 20 years ago for splenic rupture caused by traffic-accident trauma, and had a 10-year special history of hypertension. Because of the uncertain nature of the mass, surgical treatment was recommended. ResultsThe surgeon managed to remove the left adrenal region mass. During the surgery, the adrenal source was excluded. In the histological examination, the splenic corpuscle and splenic medullary structure were seen under the microscope, and an accessory spleen was diagnosed. ConclusionsThe accessory spleen was located in the adrenal region rarely, and can easily be misdiagnosed as an adrenal tumour. When the cases show abnormal adrenal space-occupying lesions in imaging examinations, non-adrenal diseases should be considered. we need to combine different imaging techniques for analysis, and think more about it, avoid misdiagnosis leading to unnecessary surgery.

Abstract:A 37-year-old woman presented with sudden onset of severe headache and vomiting. Cerebrospinal fluid examination was consistent with subarachnoid hemorrhage, but no venous sinus thrombosis or aneurysmal bleeding was found by magnetic resonance angiography or computed tomography angiography. Pituitary magnetic resonance imaging revealed a suprasellar mass with intratumoral hemorrhage. The patient's history revealed symptoms of hypothyroidism over the past 10 years. Primary hypothyroidism was confirmed by markedly decreased free T4 and T3 along with elevated thyroid-stimulating hormone levels. The patient was treated conservatively with mannitol to reduce intracerebral pressure, followed by hydrocortisone and levothyroxine. After 3 months of levothyroxine replacement, an magnetic resonance imaging revealed slight shrinkage of the pituitary gland.

Abstract:Objective To explore the differentiate points between hemorrhagic degeneration of nodular goiters and subacute thyroiditis.Method The clinical features,diagnosis and treatment of the hemorrhagic degeneration of nodular goiter in three patients were restrospectively analyzed,and the differential diagnosis with subacute thyroiditis was discussed.Resluts Patients demonstrated subacute thyroiditis-like symptoms including abrupt pain and swelling of the thyroid,fever,accelerated erythrocyte sedimentation rate,lower thyroidal radioactive iodine uptake and transient thyroid dysfunction.Through ultrasonography and fine needle aspiration biopsy patients were diagnosed as hemorrhagic degenerarion of nodular goiter.Two cases received operation procedure and one case received non-operation treatment,patients were recovery of thyroid dysfunction in follow up.Conclusion Acute hemorrhagic degeneration of nodular goiter is easily misdiagnosed as subacute thyroiditis.The differential diagnosis is necessary for avoiding misuse of glucocorticoids.

Abstract:Atypical Parathyroid Adenoma (APA) is a type of tumor that lies somewhere between parathyroid adenoma and parathyroid carcinoma. It often affects adults over the age of 60, and the clinical symptoms are consistent with those of hyperparathyroidism. This condition has a low occurrence, and its ultrasonographic signs are strikingly similar to thyroid malignant tumors, making it easily misdiagnosed. As a result, a case of APA ultrasonography misdiagnosis admitted to our hospital was recorded in order to serve as a reference point for APA diagnosis.

Abstract:Cervical tracheal adenoid cystic carcinoma (TACC) invading the thyroid gland is very rare and easily misdiagnosed as thyroid tumor, this paper reports a TACC invading thyroid in a 33-year-old woman who had been diagnosed as a thyroid follicular tumor in right lobe of thyroid by sonography and ultrasound guided fine needle aspiration in other hospital. She accepted surgical treatment in our hospital and was diagnosed as TACC by pathology, locally involving the thyroid. This paper presents the patient's clinical data, imaging findings, pathological diagnosis, treatment process and reviews the literature of TACC mimicking a thyroid tumor.
Subacute Thyroiditis Presenting as a Painful Thyroid Nodule and Subclinical Hyperthyroidism: A Case Report

Abstract:This case report details the evaluation and management of a 55-year-old woman who presented to our endocrinology clinic due to low TSH and thyroid nodules previously evaluated by her ENT. The patient originally presented with anterior neck pain and dysphagia. Ultrasonography demonstrated thyroid nodules with suspicious features, prompting a fine needle aspiration (FNA). A biopsy showed a follicular lesion/atypia of undetermined significance (Bethesda III). Due to concerns for malignancy, a right lobectomy was recommended. Thyroid function tests showed subclinical hyperthyroidism. She presented to our endocrinology clinic and was diagnosed with subacute thyroiditis based on signs of symptoms, radioactive iodine scanning, and biochemical studies (elevated ESR). Within approximately seven months, thyroid function tests and inflammatory markers returned to baseline, and symptoms and physical findings resolved. The case highlights the importance of understanding the similarities and differences between subacute thyroiditis and malignant pathologies in order to avoid misdiagnosis and unnecessary procedures.

Abstract:Purpose Langerhans cell histiocytosis (LCH) is a rare clonal disorder of Langerhans antigen-presenting cells. However, thyroid LCH involvement is relatively rare. We present the first case of spontaneous thyroid hemorrhage due to LCH progression and discuss the clinical features, diagnosis, and treatments of thyroid LCH in a literature review. Methods Clinical data were collected. Previously published articles on thyroid LCH involvement were reviewed to assess the clinical features, diagnosis, and treatments for thyroid LCH. Results A 54-year-old female presented with a multi-system LCH, affecting the uterus, liver, pituitary gland, and thyroid gland. Clinical stability was achieved after systemic chemotherapy. After 7 years of regular follow up, the patient complained of a sudden painful neck swelling and progressive dyspnea. Computed Tomography revealed bilateral goiter with hematoma, and the patient was diagnosed with spontaneous thyroid bleeding based on her clinical symptoms and radiological findings. The patient was incubated to relieve airway compromise and partial thyroidectomy was performed for definitive treatment. Pathological evaluation further confirmed the diagnosis of thyroid LCH. The patient recovered well after surgery. Conclusion Spontaneous thyroid bleeding due to thyroid LCH progression is extremely rare. Treatments for LCH vary depending on the severity of the disease. We suggest that, for patients with multi-system LCH with thyroid lesion, long-term active surveillance of thyroid hormone concentrations, and thyroid gland volume is required. Physicians should be alert of the potentially life-threatening spontaneous thyroid hemorrhage when aggravated diffuse goiter and hypothyroidism appear. Further investigation is required to establish the guidelines for thyroid LCH treatment.
Adenomatous hyperplastic intratracheal ectopic thyroid tissue: a case report

Abstract:Intratracheal ectopic thyroid (ITET) is a rare disease, with limited cases reported in the literature. ITET is an unusual congenital abnormality and can be easily mistaken for a respiratory illness. We present a case of a 61-year-old man with a history of slight discontinuous hemoptysis for 2 years. A tracheal mass, which appeared to be connected to the left thyroid gland, was found by chest computed tomography scan. Ultrasound revealed one suspiciously malignant, solid and hypoechoic nodule in the left thyroid gland. After the thyroid origin of the mass was confirmed by bronchoscopic biopsy, the patient underwent segmental resection and anastomosis of the trachea, together with left thyroidectomy. Histopathology of the tracheal tumor showed adenomatous hyperplastic ITET, and the orthotopic left thyroid gland showed nodular goiter with atypical adenomatous hyperplasia. Clinical suspicion is warranted in patients presenting with a tracheal tumor seemingly connected to the thyroid gland, particularly in patients who have imaging features suggestive of a malignant tumor in the orthotopic thyroid but without confirmative histopathology of malignancy before surgery.

Abstract:Among the extensive variety of disorders that can cause acute abdominal pain are hepatocellular adenomas (HCAs), pathological entities that otherwise are asymptomatic. Here, we describe a 33-year-old female who presented in the emergency department with acute abdominal pain and a history of liver focal nodular hyperplasia (FNH) diagnosed 10 years ago. An abdominal magnetic resonance imaging (MRI) revealed that the cause of the pain was an intrahepatic hematoma. The mass was surgically removed, and the biopsy revealed inflammatory adenomas, a subtype of HCA. Hepatic adenoma diagnosis remains challenging by clinical and imaging techniques, and usually, a biopsy is the main diagnostic tool. HCA should be differentiated from hepatocellular carcinoma (HCC), FNH, hepatic angioleiomyoma, and hepatic hemangioma. In our case, HCA was misdiagnosed in the past as FNH. HCA rarely may present as acute right abdomen pain, and a potential catastrophic hemorrhage or rupture must be excluded.

Abstract:Introduction and importance: Postoperative neck hematoma (PNH), a rare complication following thyroidectomy, occurs in only 1.1-3.15% of cases and can lead to life-threatening outcomes. More rarely, delayed PNHs with atypical clinical manifestations and positions have not yet been reported. Early identification and immediate medical intervention are of utmost importance in such cases. Case presentation: The authors represented a patient with thyroid cancer adherent to the trachea, who underwent post-thyroidectomy, experienced delayed PNH in the retrosternal region and was infected by respiratory pathogens. Meanwhile, the patient developed recurrent laryngeal nerve (RLN) paralysis after surgery. PNH was not identified in the clinical manifestations; instead, it was detected only through successive cervical ultrasound examinations. Clinical discussion: Although rare, PNH can lead to serious complications, especially delayed complications or those in atypical positions, without neck swelling. When simultaneously with RLN paralysis, the hematoma may be neglected. Therefore, early diagnosis and treatment are crucial. Conclusion: Clinicians should be vigilant of atypical PNH because neck swelling may be absent. Cervical ultrasonography is essential for diagnosis and can be performed multiple times. Cervical CT scans should be part of the routine procedure, while contrast-enhanced ultrasound can help detect active bleeding. Early postoperative antibiotics are recommended if the tumor is closely attached to the trachea.
Case Analysis of Characteristics of Thyroid Nodules Commonly Misdiagnosed by Ultrasound

Abstract:Objective To analysis the characteristics of the thyroid nodules which were inclined to be misdiagnosed as malignancies by ultrasound, so as to raise the awareness and reduce misdiagnosis rate. Methods A retrospective analysis of a total of 61 cases with highly suspicious thyroid malignancies in sonography, which were consistent with neither final pathological findings nor further examination results, were conducted. Both ultrasonic and pathological results were analyzed and summarized. The strategies and methods to facilitate the ability of differential diagnosis were further discussed. Results The misdiagnosed cases included 9 types: parathyroid lesion, esophageal diverticula, medullary thyroid carcinoma, metastatic thyroid carcinoma, cervical neoplasm with thyroid gland invading, thyroid lymphoma, follicular thyroid carcinoma, absorption of thyroid adenoma with cystic degeneration and subacute thyroiditis. Four types of misdiagnosed cases were summarized: papillary thyroid carcinoma (72.1%), thyroid adenoma (19.7%), anaplastic thyroid carcinoma (3.3%) and Hashimoto′s thyroiditis (4.9%). By combining with laboratory examinations, ultrasound guided fine-needle aspiration cytology examinations, MIBI and other relative examinations, corresponding countermeasures were made. Conclusion Even if the sonograph of thyroid nodules has some typical features of malignancies, it should be further identified by combining the necessary auxiliary examinations or coordinate with scanning skills to reduce the misdiagnosis. Abstract:Adenoid cystic carcinoma usually occurs in the salivary glands of the head and neck. It is a malignant tumor with a high degree of malignancy, resistance to radiotherapy and chemotherapy and poor prognosis. The clinical course of adenoid cystic carcinoma is slow and easy to be misdiagnosed. The main diagnosis and treatment means are individualized and precise treatment under the multi-disciplinary consultation mode, that is, surgical treatment and radiotherapy and chemotherapy. Adenoid cystic carcinoma is prone to relapse and hematologic metastasis, and the traditional radiotherapy and chemotherapy based therapies have not achieved satisfactory efficacy in the past three decades. How to detect, diagnose and treat early is an urgent task faced by clinicians.
Ectopic Thyrotropin Secreting Pituitary Adenoma Concomitant with Papillary Thyroid Carcinoma Case Report

Abstract:Rationale: Ectopic thyrotropin (TSH)-secreting pituitary adenomas are exceedingly rare. To date, there are only 6 cases reported. Here, we describe an even rarer ectopic TSH-secreting pituitary adenoma (TSH-oma) concomitant with papillary thyroid carcinoma. Patient concerns: A 27-year-old female was admitted to the hospital in 2002 for neck enlargement and palpitation. Thyroid function test showed increased thyroid hormones and unrepressed TSH. Thyroid ultrasound examination displayed diffuse goiter. The patient was presumptively diagnosed as primary hyperthyroidism and treated with anti-thyroid drugs. Her condition was then improved, but the serum TSH was persistently unrepressed. Therefore, central hyperthyroidism due to TSH-oma or pituitary resistance to thyroid hormone (PRTH) was suspected. Pituitary magnetic resonance imaging (MRI) examination was deservedly performed to rule out TSH-oma, which turned out to be normal. In addition, T3 suppression test was negative. Thus, PRTH, as an uncommon cause of inappropriate TSH secretion, was regarded as the working diagnosis. Triiodothyroacetic acid, which was reported to be effective for PRTH, was then administrated. But it did not work well. To control the symptoms completely and normalize the level of thyroid hormones, radioiodine therapy was carried out in 2007, followed by levothyroxine replacement therapy. Consequently, the symptoms were relieved, whereas serum TSH remained at high levels even with adequate levothyroxine. Unexpected, thyroid papillary carcinoma and a neoplasm in her nasopharynx were successively detected in 2012, which were then removed by surgery. Somewhat interestingly, the serum TSH declined to normal after the operation. Diagnoses: The patient was ultimately diagnosed as an ectopic TSH-secreting pituitary adenoma concomitant with papillary thyroid carcinoma. Interventions: Thyroidectomy and removal of the ectopic TSH-secreting pituitary adenoma by surgery were carried out, followed by levothyroxine replacement therapy. Outcome: Three years after the surgery, the patient felt well with levothyroxine 125ug daily. Serum thyroid hormones and TSH kept in normal and no signs of neoplasm recurrence. Lessons: Although extremely rare, ectopic TSH-secreting pituitary adenoma, as an uncommon cause of thyrotoxicosis, should be taken into consideration among those who have a longstanding hyperthyroidism with unsuppressed TSH.
A case of primary parahyperthyroidism with chief complaint of upper gastrointestinal bleeding

Abstract:SUMMARY Hyperparathyroidism is the clinical syndrome that results from elevated circulating parathyroid hormones,leading to multiple systems manifestations,and is divided into three kinds: primary, secondary and ternary.This patient’s main chief complaint was intermittent melena for 3 days,with the diagnosis of upper gastrointestinal hemorrhage according to endoscopy. Hypercalcemia, hypophosphatemia and high level of circulating parathyroid hormone and chronic renal failure were also found during hospitalization. Furthermore, MIBI showed a parathyroid adenoma located near the isthmus in the lower part of the left lobe. The patient had undergone nephrectomy because of kidney carcinoma 2 years ago. Primary hyperparathyroidism usually has the common gastrointestinal syndrome , but upper gastrointestinal hemorrhage reported as the chief complaint is rare and we had never seen similar reports before.

Abstract:Ectopic thyroid-stimulating hormone (TSH)-secreting tumors are extremely rare, with only 15 reported cases in the literature. Herein, we described a 60-year-old female patient with thyrotoxicosis and elevated or unsuppressed levels of TSH. Family history and laboratory and genetic tests did not support a diagnosis of resistance to thyroid hormone (RTH). Given the unsuppressed TSH, TSH-secreting tumor was suspected, and magnetic resonance imaging (MRI) of the pituitary gland was performed. Surprisingly, the MRI scans revealed a nodule in the nasopharynx rather than a pituitary tumor in the sella region. Further evaluation using Gallium-68 DOTATATE positron emission tomography/computed tomography ( 68 Ga-DOTATATE PET/CT) demonstrated increased DOTATATE uptake in the nasopharyngeal nodule. Additionally, an octreotide suppression test (OST) revealed an obvious reduction in TSH levels, further supporting the suspicion of the nasopharyngeal mass as the cause of inappropriate TSH secretion. To prepare for surgery, the patient received preoperative administration of octreotide, resulting in the normalization of TSH and thyroid hormone levels. The patient subsequently underwent successful surgical removal of the nasopharyngeal mass. Following the procedure, the patient experienced complete resolution of hyperthyroidism symptoms, with TSH declined and thyroid hormone levels returned to normal. Histochemistry analysis of the tumor revealed positive staining for TSH, growth hormone (GH), prolactin (PRL), luteinizing hormone (LH), and somatostatin receptor 2 (SSTR2). We discussed differential diagnosis of hyperthyroidism due to inappropriate TSH secretion, with a particular emphasis on the importance of 68 Ga-DOTATATE PET/CT in combination with OST for identifying ectopic pituitary tumors.

Abstract:Objective To investigate clinical characteristics and the causes of misdiagnosis of primary hyperparathyroidism caused by adenoma.Methods The clinical data of 9 patients who were diagnosed with primary hyperparathyroidism caused by parathyroid adenoma was analyzed retrospectively.Results There were 4 males and 5 females in this group.The period for misdiagnosis was from 8 months to 11years.All of them suffered symptoms and signs of skeleton system.There were 8 patients and 7 patients suffering urological and digestive symptoms respectively.In laboratory investigation,all had hypophosphatemia and increased serum alkaline phosphatase level,eight of them had hypercalcemia and increased excretion of urine calcium and seven of them had obviously increased serum PTH level. 99m Tc-MIBI image functions best in localization of the adenoma.Conclusion It is very important to get full medical history and detailed physical examination and laboratory investigation in diagnosing parathyroid adenoma.
