Abstract:Ectopic thyroid occurs when it is not located on the normal thyroid compartment. While 90% of the ectopic thyroids were located at the base of the tongue, only 1% were lingual thyroid carcinoma (LTC). Only 56 LTC cases have been reported so far. Here we reported two cases of LTC. Patient 1 was a 47-year-old female with LTC and co-current sub-hyoid ectopic thyroid. She experienced major hematemesis and dyspnea requiring emergent tracheotomy. Patient 2 was a 61-year old female who was presented with LTC with multiple lymph node metastasis and bilateral lung metastasis. Both of the patients’ lingual masses were removed via trans-submaxillary excisions. Pathology revealed ectopic papillary thyroid carcinoma. Then they were treated with radio-active iodine (RAI). These patients had full recovery and there were no complications. A review of literature was also presented.

Abstract:BACKGROUND Ectopic thyroid is defined as a rare developmental anomaly where thyroid tissues are atypically found in locations other than its normal anatomical position: Anterolateral to the second, third, and fourth tracheal cartilages. An intemperate descent or a migration failure of the thyroid anlage results in sub-diaphragmatic thyroid ectopia, a sparse clinical entity. CASE SUMMARY This case portrays a 63-year-old female patient presenting with chronic abdominal discomfort at a local hospital whereby a computed tomography (CT) scan revealed a well-defined mass in the hepatic entrance. For further examination, the patient underwent a CT scan with contrast, magnetic resonance imaging (MRI), and CT-angiography (CTA) at our department. The CT scan showed a well-defined and high attenuated mass measuring 43 mm × 38 mm in the hepatic entrance with calcification. The CTA revealed an additional finding: Blood supply to the mass from the right hepatic artery. MRI of the upper abdomen demonstrated a mass with mixed signal intensity on T1 and T2 weighted images in the hepatic entrance. The patient underwent surgery with resection of the mass which was sent for histopathology. Ectopic thyroid at the level of porta hepatis with nodules was the definitive diagnosis since histopathological report revealed presence of thyroid tissue in the resected liver mass. CONCLUSION This case delivers a rare insight of pre-operative radiological imaging of an ectopic thyroid located in the liver. These findings can aid in narrowing down potential differential diagnosis when managing a patient with those subsequent findings.

Abstract:Objective To explore the clinical features, diagnosis and treatment of ectopic thyroid lesions. Methods Retrospectively reviewed the clinical data of 15 patients with ectopic thyroid tissue from Oct. 2002 to Jun 2017 in Beijing Tongren Hospital, Capital Medical University and Beijing Anzhen Hospital, Capital Medical University, and figured out the clinical features, therapy and prognosis. Among the 15 patients, there were 2 males and 13 females. There were 8 cases of lingual ectopic thyroid, 3 cases of endotracheal ectopic thyroid, 3 cases of thyroglossal duct ectopic thyroid, and 1 case of retropharyngeal ectopic thyroid. Follow-up were conducted during outpatient review or by telephone. Results Twelve patients received surgical treatment and the lesions were pathologically confirmed as ectopic thyroid tissue, including 2 cases of papillary thyroid carcinoma. These patients were followed up for 1 to 6 years. Their main symptoms were alleviated and the malignant patients had no recurrence or metastasis. Three patients with lingual ectopic thyroid selected observation and follow-up without any treatment and no progress was observed. Conclusions The incidence of ectopic thyroid is low and the forms are diverse. The clinician should take the possibility of ectopic thyroid and its associated pathological changes into account to avoid the complete removal of the only functional thyroid tissue without preparation. Squamous cell carcinoma of thyroid transformed from papillary carcinoma:One case report and literature review

Abstract:A 55-year-old female patient underwent surgery for her thyroid lumps 10 years ago and pathological examination confirmed the diagnosis of primary papillary carcinoma of the thyroid. The metastasis of neck lymph nodes was conformed 8 years after operation, and the patient received metastatic lymph nodes resection. During the 3-year follow-up the patient had recurrent tumor. The pathological results showed that the lesion changed from papillary carcinoma to papillary carcinoma accompanied with squamous differentiation and transformed to squamous cell carcinoma of the thyroid at last. Finally the patient died from deterioration of the disease rapidly. From the literature review, it has been shown that the squamous cell carcinoma of the thyroid might originate from adenocarcinoma or squamous metaplasia of papillary carcinoma. Once the transformation occurs, the progress of the disease may be accelerated. This transformation is rare and usually confers a poor prognosis.

Abstract:[Summary] From February 2008 to June 2008, two cases of old ectopic pregnancy were misdiagnosed: Case 1 was misdiagnosed as gestational trophoblastic disease (GTD), and Case 2 was misdiagnosed as cornual pregnancy.In order to further confirm the diagnosis, two cases were all given laparoscopic exploration.Surgery confirmed interstitial chronic ectopic pregnancy, leading to a resection of the fallopian tubes and mass lesions.Embryo tissue was pathologically confirmed postoperatively.The diagnosis of old ectopic pregnancy is difficult.Serum hCG determination is often found no positive, and ultrasound examination has no specificity.The clinical images are varied.Detailed medical history inquiry and attention to differential diagnosis are key for the diagnosis of old ectopic pregnancy.

Abstract:OBJECTIVE:To evaluate clinical-pathological features, diagnosis and therapy of gestational trophoblastic tumor (GTT) misdiagnosed as ectopic pregnancy.METHODS:From 1999 to 2003, a total of 13 patients with GTT misdiagnosed as ectopic pregnancy were retrospectively analyzed.RESULTS:The main symptoms were amenorrhea, abdominal pain, irregular vaginal bleeding. Serum beta-human chorionic gonadotrop in (hCG) was measured in 10 patients. Eight had hCG values above 10,000 IU/L; 3 had hCG values above 50,000 IU/L. The lesions of GTT misdiagnosed as ectopic pregnancy were fallopian tube, horn of uterus, peritoneal cavity, greater omentum, recto-uterine pouch. According to standards of the International Federation of Gynecology and Obstetrics (FIGO) the 13 patients were categorized as 6 of stage I, 2 of stage II, 3 of stage III and 5 of stage IV. Histologically they included 10 cases of choriocarcinoma and 3 of invasise mole. All patients were treated by complete surgical resection combined with subsequent adjuvant chemotherapy.CONCLUSIONS:Misdiagnosis leads to delay in therapy with resultant increased morbidity of GTT. Analysis on serial hCG is helpful to differential diagnosis between ectopic pregnancy and GTT.

Abstract:Ectopic thyroid-stimulating hormone (TSH)-secreting tumors are extremely rare, with only 15 reported cases in the literature. Herein, we described a 60-year-old female patient with thyrotoxicosis and elevated or unsuppressed levels of TSH. Family history and laboratory and genetic tests did not support a diagnosis of resistance to thyroid hormone (RTH). Given the unsuppressed TSH, TSH-secreting tumor was suspected, and magnetic resonance imaging (MRI) of the pituitary gland was performed. Surprisingly, the MRI scans revealed a nodule in the nasopharynx rather than a pituitary tumor in the sella region. Further evaluation using Gallium-68 DOTATATE positron emission tomography/computed tomography ( 68 Ga-DOTATATE PET/CT) demonstrated increased DOTATATE uptake in the nasopharyngeal nodule. Additionally, an octreotide suppression test (OST) revealed an obvious reduction in TSH levels, further supporting the suspicion of the nasopharyngeal mass as the cause of inappropriate TSH secretion. To prepare for surgery, the patient received preoperative administration of octreotide, resulting in the normalization of TSH and thyroid hormone levels. The patient subsequently underwent successful surgical removal of the nasopharyngeal mass. Following the procedure, the patient experienced complete resolution of hyperthyroidism symptoms, with TSH declined and thyroid hormone levels returned to normal. Histochemistry analysis of the tumor revealed positive staining for TSH, growth hormone (GH), prolactin (PRL), luteinizing hormone (LH), and somatostatin receptor 2 (SSTR2). We discussed differential diagnosis of hyperthyroidism due to inappropriate TSH secretion, with a particular emphasis on the importance of 68 Ga-DOTATATE PET/CT in combination with OST for identifying ectopic pituitary tumors.

Abstract:Rationale: Thyroglossal duct carcinoma, a rare clinical condition characterized by ectopic thyroid adenocarcinoma within thyroglossal duct cysts (TGDCs), typically confirmed through intraoperative rapid pathology, this condition generally has a favorable prognosis. Nevertheless, comprehensive treatment guidelines across all disease stages are lacking, the purpose of this study is to report 1 case of the disease and propose the treatment plan for each stage of the disease. Patient concerns: A patient presented with thyroid swelling, classified as C-TIRADS 4A following a physical examination. Preoperative thyroid puncture identified papillary thyroid carcinoma, and genetic testing revealed a BRAF gene exon 15-point mutation. Ancillary tests showed a slightly decreased thyroid stimulating hormone (TSH) level (0.172) with no other significant abnormalities. Diagnoses: Preoperative fine-needle aspiration cytology (FNAC) confirmed right-side thyroid cancer. Intraoperative exploration uncovered a TGDC and intraoperative rapid pathology confirmed thyroglossal duct carcinoma. Interventions: A Sistrunk operation and ipsilateral thyroidectomy were performed. Outcomes: Postoperative recovery was satisfactory. Lessons: Thyroglossal duct carcinoma is a rare disease affecting the neck. Due to limited clinical cases and the favorable prognosis associated with this condition, there is currently no established set of diagnostic and treatment guidelines. According to tumor size, lymph node metastasis, thyroid status and other factors, the corresponding treatment methods were established for each stage of thyroglossal duct cancer, which laid the foundation for the subsequent treatment development of this disease.

Mucoepidermoid Carcinoma of the Thyroglossal Duct: Two Rare Entities in a Unique Case Report

Abstract:Background: Mucoepidermoid Carcinoma (MEC) of the thyroid represents less than 0.5% of all thyroid neoplasms. Thyroglossal duct cyst carcinoma is a rare condition with only approximately 300 cases reported. Case report: A 34-year-old pregnant woman at 37 weeks gestation presented to an endocrinological center for primary autoimmune hypothyroidism. The thyroid ultrasound revealed a pseudonodular pattern. The patient was followed up after two years. She reported a full-term delivery without complications. A new thyroid ultrasound was performed, showing a cystic lesion in the median suprathyroid area, measuring 6 x 9 x 10 mm, not previously reported. After 4 months, the suprathyroid cystic lesion was confirmed by thyroid ultrasound, measuring 6 x 11 x 12 mm. The patient was referred for fine-needle aspiration cytology. Cytological examination showed lymphocytes, red blood cells, and some epithelial aggregates with large cytoplasm and nuclear polymetrism with oxyphilic aspects. The patient underwent the Sistrunk procedure for the suprathyroid lesion. The histological examination revealed lymphocytic thyroiditis in heterotopic thyroid tissue with solid cell nest, epidermoid epithelium, and mucus-secreting cells suggestive of low-grade mucoepidermoid carcinoma. The immunohistochemistry study was positive, exhibiting thyroid transcription factor 1 and cytokeratin-19. No positivity was observed for thyroglobulin, calcitonin, galectin-3, and Hector Battifora mesothelial antigen 1. The recent follow-up examination, 13 months after the surgery, has been found negative for disease recurrence. Conclusion: This is the first case of an MEC occurring within a thyroglossal duct. Considering the age of the patient, the histological diagnosis, and the absence of thyroid nodules and metastasis, we decided on the Sistrunk procedure without total thyroidectomy.

Abstract:Objective To investigate the clinicopathologic features and treatment of thyroid microcarcinoma (TMC). Methods From January 1997 to December 2006,311 patients who underwent surgery and defined as TMC(tumor size≤1 cm)were enrolled. Results TMC was identified incidentally by frozen pathologic examination on thyroidectomy specimens in tentative benign goiters in 181 patients; another 130 patients with clinically detectable primary tumors or suspected nodal metastases were grouped to as clinically overt TMC. The clinically overt TMC had a higher incidence of bilateral multifocal tumors (18.5%vs.9.4%,P=0.03),and cervical lymph node metastases(27.7%vs.10.5%,P=0.000)than that in clinically occult TMC group. Conclusion TMC may vary considerably in clinical and biologic behaviors between these two subtypes: clinically overt and occult. Lobectomy for single lesion, total or near total thyroidectomy for multifocal with central compartment nodal dissection should be performed, lateral nodal dissection was not carried out unless US or physical examination detected nodal metastases. Lobetomy, subtotal or more limited thyroidectomy for occult TMC, diagnosed incidentally following thyroid surgery for initially tentative benign thyroid disease, could all be treatment of choice depending on the preference of surgeons.
Individual treatment strategy of thyroglossal duct cyst carcinoma: a case report

Abstract:Background: Neoplasia of ectopic thyroid components is relatively rare in thyroglossal duct cysts. We report a case of histopathologically confirmed papillary thyroid carcinoma in a thyroglossal duct cyst, discuss its clinical characteristics of, and provide reference for diagnosis and treatment.Case Description: We presented a 25-year-old female went to hospital because of "a tumor in her neck". She was preoperatively diagnosed with thyroglossal duct cyst by cervical ultrasound, and enhanced computed tomography (CT). However, the solid component of the mass suggested intracystic neoplasia. She underwent Sistrunk surgical resection, and postoperative histopathology showed thyroglossal duct cyst, and papillary thyroid carcinoma in the cyst wall. The patient had no high-risk factors and had a low risk of recurrence. After full disclosure, the patient chose close follow-up, and to date there has been no recurrence.Conclusions: There are controversies regarding the origin of thyroglossal duct cyst carcinoma and the extent of surgery required, and a lack of unified treatment guidelines. We recommend tailoring individualized treatment based on individual risk stratification. By reporting this case, we hope to inform surgeons of the various abnormalities that may occur in ectopic thyroid tissue.

Intra-thyroid esophageal duplication cyst: A case report

Abstract:BACKGROUND Esophageal cysts are relatively rare in clinical practice, with most of the literature comprising case reports. Esophageal cysts protruding into the thyroid gland are easily misdiagnosed as thyroid tumors. No such cases have been reported so far. CASE SUMMARY This article reports the case of a 31-year-old adult male diagnosed with thyroid nodules before admission. The patient underwent left thyroidectomy and isthmusectomy. During the surgery, esophageal cysts were identified in the esophageal muscle and thyroid glands. The pathology results confirmed a nodular goiter combined with esophageal cysts. Postoperatively, the patient developed a neck infection and underwent another operation and broad-spectrum antibiotic treatment for recovery. CONCLUSION We report the first clinical case of an esophageal cyst located within the thyroid gland that was successfully treated surgically. Esophageal cyst located within the thyroid gland cause difficulties in diagnosis. In the present study, the contents of the esophageal cysts were calcified foci, and a small amount of fluid mixture, which were easily misdiagnosed as thyroid nodules and misled the surgical methods.

Abstract:Intracardiac ectopic thyroid tissue is a rare lesion. We present a case of successful excision of an ectopic thyroid adenoma originated from the right ventricular aspect of the interventricular septum and extending to the right ventricular outflow tract. The 12-month follow- up shows normal thyroid function and good clinical status of the patient. In our estimation, there was no case with an intracardiac ectopic thyroid adenoma reported before in China. 2009 Published by European Association for Cardio-Thoracic Surgery. All rights reserved. Ectopic thyroid tissue is a congenital disease caused by abnormal migration of thyroid in the embryonic stage. Ninety percent of the reported cases of ectopic thyroid are found in the base of the tongue, while only 10% lie in the anterior aspect of the neck superficial to the hyoid bone w1x. Ectopic thyroid tissue located in the heart is extremely rare with only -30 cases reported worldwide since 1941 w2-7x. Here we present a case with an intracardiac ectopic thyroid adenoma that successfully received surgical treatment.

Abstract:Ectopic thyroid tissue is a rare condition manifested as the appearance of thyroid tissue outside the thyroid gland. Here, we report a case of ectopic thyroid tissue in the breast. A 48-year-old Chinese woman who was diagnosed with breast cancer received modified radical mastectomy. A thyroid tissue was found on subsequent pathological examination. The ectopic thyroid tissue was confirmed by immunohistochemistry staining of thyroid biomarkers, including thyroglobulin, thyroid transcription factor-1, and thyroid peroxidase. Currently, abnormal thyroid anlage descent is the main theory to explain ectopic thyroid tissue, especially lingual thyroid. However, it is far-fetched to explain the pathogenesis of ectopic thyroid tissues existed in organs or tissues far from thyroid such as iris, cardiac, pulmonary, duodenal, adrenal, and vertebral. Here, we reviewed the previous cases of ectopic thyroid tissue in breast and proposed a "entoderm migration" theory to explain distant ectopic thyroid tissues based on embryonic development perspective.

Abstract:Ectopic thyroid-stimulating hormone (TSH)–secreting pituitary adenomas are rare and can often be misdiagnosed as primary hyperthyroidism. We present a case of an ectopic suprasellar TSH-secreting pituitary adenoma. A literature review of previously reported ectopic TSH-secreting and suprasellar pituitary adenomas is included to illustrate the clinical characteristics of this disease entity and the diversity of operative approaches to treating ectopic suprasellar pituitary adenomas. A 46-year-old man presented with typical clinical signs of hyperthyroidism and a history of progressive visual field impairment and vision loss. Laboratory investigations revealed that the patient had elevated levels of free thyroxine and free triiodothyronine and a normal level of TSH. Neuro-ophthalmologic examination showed right eye/left eye = 1.0/0.6 and left temporal hemianopia. Magnetic resonance imaging revealed a mass located in the suprasellar space. The patient underwent preoperative short-term octreotide treatment followed by gross total resection of the tumor via the extended endoscopic endonasal transtuberculum sellar approach. At 6-month follow-up evaluation, the patient's endocrinologic function tests met the criteria for cure, and magnetic resonance imaging revealed a normal pituitary gland and stalk with no tumor recurrence. Histologic diagnosis confirmed the presence of a TSH-secreting pituitary adenoma. To the best of our knowledge, this is the first reported case of an ectopic suprasellar TSH-secreting pituitary adenoma. Preoperative preparation and complete resection are the keys to a cure. The extended endoscopic endonasal transtuberculum sellar approach is an alternative minimally invasive method for the removal of an ectopic suprasellar pituitary adenoma.
Ectopic Thyroid Tissue in the Adrenal Gland: A Case Report

Abstract:Background. Ectopic thyroid tissue is a rare abnormality resulting from a developmental defect during the early stages of thyroid gland embryogenesis. It is usually found anywhere along the embryonic pathway of the thyroid anlage in the midline of the ventral pharynx, from the tongue to the trachea. It is difficult to explain ectopic thyroid tissue beneath the diaphragm from the perspective of embryonic evolution. A few cases of ectopic lesion have been reported in abdominal organs including adrenal glands. The current study describes a case of ectopic thyroid tissue in the adrenal gland. Case Presentation. We reported a case of ectopic thyroid tissue in the adrenal gland. A 48-year-old male, with history of hyperthyroidism many years ago who was in euthyroid state after treatment. He was diagnosed as having a right adrenal tumor about 4 cm in size during a health check-up. Routine laboratory studies including TSH, free T4, aldosterone, and renin showed normal results. The patient received laparoscopic adrenalectomy. Microscopically, the tumor was composed of normal adrenal tissue with a cystic lesion lined by a single layer of follicular cells which were thyroid transcription factor-1 positive. A diagnosis of ectopic thyroid tissue was reached. Conclusion. Ectopic thyroid tissue in the adrenal gland is rare and the pathogenesis remains unknown. Nevertheless, it should be taken into consideration in a non-functional adrenal mass.

Abstract:Ectopic thyroid is not a common disease, and a double ectopic thyroid is extremely rare. We report a recent case of dual ectopic thyroid in an 11-year-old girl mimicking mono-ectopic lingual thyroid on the anterior view of the thyroid scan. The dual ectopic thyroid was shown on the lateral view, with foci in the lingual and anterior hyoid regions, but no thyroid tissue in the pre-tracheal region. She had originally been scheduled for surgery with a diagnosis of neck schwannoma, however, pre-operative thyroid scan and CT scan of the neck revealed the presence of dual ectopic thyroid tissue: Thyroid scan, and either CT, or ultrasonography should be performed routinely in cases presenting with a neck mass to avoid unnecessary misdiagnosis. Parathyroid Adenoma Carcinogenesis:A Rarely Misdiagnosed Case Report and Literature Review

Abstract:Objective To summarize the misdiagnosis cause and therapy of parathyroid adenoma carcinogenesis. Methods A retrospective analysis was conducted on a misdiagnosed case with parathyroid adenoma carcinogenesis in the General Hospital of PLA . Results The patient suffered from the pain of the right tibia and sternal rib for over 7 months. He had been misdiagnosed as having giant cell tumor of bone in another hospital. The pain was not relieved after surgery. Moreo-ver, he had the obvious polydipsia, polyuria and frequency of micturition before he was admitted to our hospital. Laboratory analyses included hypercalcemia and high parathyroid hormone levels. The thyroid ultrasound confirmed nodules in the rear lower pole of the right lobe of the thyroid, and then it was diagnosed as parathyroid adenoma by the puncture biopsy guided by ultrasound. The lesion underwent complete surgical resection, and the patient was diagnosed as having parathyroid adenoma carcinogenesis by routine pathology examination and immunohistochemistry. Postoperative therapy was mainly infused calcium and phosphorus. The patient had no symptoms of pain sine then, and no recurrence during a 9-month follow up. Conclusion The symptoms, signs, imaging and laboratory of parathyroid carcinoma all lack specificity, therefore it can be easily misdi-agnosed in preoperative diagnosis. It can be conformed by histopathology and immunohistochemistry.
Incidental Finding of Triple Ectopic Thyroid: Case Report and Review of the Literature

Abstract:Ectopic thyroid tissue is a rare congenital anomaly, with the presence of three simultaneous ectopic foci being exceedingly rare. We describe a case of a totally asymptomatic 26-year-old male discovered to have triple ectopic thyroid following incidental elevated thyroid-stimulating hormone (TSH) levels. Subsequent ultrasonography of the neck showed an absent thyroid gland in its conventional location. A Technetium-99m pertechnetate (Tc-99m) thyroid scan showed three distinct foci of radiotracer uptake in the upper cervical, lingual, and sublingual regions, corresponding to triple ectopic thyroid. An extensive review of the literature was conducted to provide a broader understanding and deeper insights into this uncommon condition. This case underscores the pivotal role of Technetium-99m thyroid scanning in diagnosing triple ectopic thyroid, particularly in instances where the orthotopic thyroid gland is absent. A comprehensive understanding of this rare entity is indispensable for radiologists and clinicians, enabling accurate diagnosis and informed decision-making regarding the appropriate therapeutic strategies.
