Clinical features of twenty-four cases of bladder cancer combined with incidental prostate cancer

Abstract:Objective To analyze the prognostic features of incidental prostate cancer in patients with bladder cancer, so as to improve the diagnosis and treatment for the disease. Methods The clinical data of 24 patients with bladder cancer and incidental prostate cancer who were treated in Renji Hospital from January 2004 to December 2010 were retrospectively analyzed. Results All the 24 patients underwent radical cystoprostatectomy. Postoperative pathologic results showed that there were 6 low grade and 17 high grade cases of urothelial carcinoma and 1 case of squamous cell carcinoma. The tumor stages were as follows: T1N0M0 (n=8), T2N0M0 (n=10), T2N1M0 (n=2), T3N0M0 (n=3) and T3N1M0 (n=1). The Gleason score ranged from 3 to 8 (mean [5.9±1.2]); Gleason scores of 18 patients were≤6, while those of the other 6 were≥7. The mean duration of follow up was (50±21.8) months (ranged 9-89 months). Two patients were lost during the follow up. One died of cardiovascular accident, four died of distant metastasis of bladder caner, two with biochemical recurrence received endocrine therapy, and the other 15 patients survived without neoplasm. Conclusion Radical cystoprostatectomy is an important prognostic factor for bladder cancer patients with incidental prostate cancer. Prompt surveillance may be feasible for the patients with Gleason score≤6, and antiandrogen therapy is suitable for the patients with Gleason score≥7 instead of maximum androgen blockage.
