Abstract:Objective To investigate the clinical value of contrast-enhanced ultrasonography and conventional ultrasound in the diagnosis of peripheral intrahepatic cholangiocarcinoma .Methods 34 cases with intrahepatic peripheral cholangiocarcinoma confirmed by pathology in our hospital from March 2015 to September 2016 were selected as the research object .The general in-formation and imaging data of patients were collected .The image representation of conventional ultrasound and CEUS in peripheral intrahepatic cholangiocarcinoma were analyzed .The rates of detection of peripheral intrahepatic cholangiocarcinoma were compared between the contrast-enhanced ultrasonography and conventional ultrasound .Results There were 30 cases with single lesion (88.23%) and 4 cases with multiple lesions (11.76%) in 34 patients.The diameter of lesion was 2.14~12.36 cm with aver-age diameter (5.36 ±3.12) cm.There were 24 lesions located in the left lobe (70.58%) and 10 cases located in hepatic right lobe (29.41%).27 cases (79.41%) were irregular lesions by conventional ultrasound , 26 cases (76.47%) with fuzzy bounda-ry and 24 cases (70.58%) with inhomogeneous low echo .The result of color Doppler showed that 28 cases with lesions of the pe-ripheral or inner flow (82.35%).The results of contrast-enhanced ultrasonography ultrasound showed that 13 cases (38.23%) with ring enhancement surrounding lesions in the arterial phase , 12 cases (35.29%) with inhomogeneous high enhanced , 3 cases (8.82%) with heterogeneous low enhancement in portal venous phase , 2 cases (5.88%) with presented enhanced lesions , 32 cases (94.11%) with lesions present of low enhancement and 33 patients (97.05%) with low enhancement in the delayed phase.The detection rate of conventional ultrasound was significantly lower than that of contrast -enhanced ultrasonography (44.11%vs 82.35%) ( P <0.001).Conclusion Compared with the conventional ultrasound , CEUS is more accurate in the diagnosis of peripheral intrahepatic cholangiocarcinoma .It can directly reflect the internal blood supply of the lesions , and can provide imaging data for the clinical treatment .
