Effects of Concurrent Chemoradiation and Neoadjuvant Chemotherapy on Local Advanced Cervical Cancer

Abstract:Objective:To determine the effects of concurrent chemoradiation and neoadjuvant chemotherapy before operation on local advanced cervical cancer.Methods:We retrospectively analyzed the clinical data of 42 female patients with stage ⅠB2~ⅡB cervical cancer from March 2002 to March 2005 in our hospital. According to preoperative treatments, the patients were divided into two groups, group 1 preoperatively treated by concurrent chemoradiation(the experimental group), group 2 treated by neoadjuvant chemotherapy (the control group).The schedule was the same in two groups. For squamous carcinoma: cisplatin(DDP) plus bleomycin were used, and for adenocarcinoma: cisplatin+5-fluorouracil(5-Fu)+mitomycin(MMC) were used. Group 1 was accompanied by intracavity brachytherapy. Operation was done after concurrent chemoradiation and neoadjuvant chemotherapy. The recent curative effects, the rate of radical hysterectomy and the condition of incision margin, adverse reaction and the long term effect (the one-year, three-year and the five-year survival rate) were observed in two groups.Results:There was no significant differences in the recent curative effect between the two groups(P0.05). The operative rate and the negative rate of incision margin were significantly higher in group 1 than in group 2(P0.01). No obvious difference of blood and gastrointestinal side effects were found between the two groups (P0.05). There was highly statistical difference in late complication about radiation proctitis and cystitis between two groups (P0.01). The 1 and 3 years survival rate in two groups was no statistical difference(P0.05), while the five-year survival rate was higher in group 1 than in group 2(P0.05).Conclusions:Preoperative concurrent chemoradiation is better than neoadjuvant chemotherapy in the treatment of local advanced cervical cancer.
