Therapeutic effect of neoadjuvant chemotherapy on local advanced cervical carcinoma

Abstract:Objective To explore the effect of neoadjuvant chemotherapy(NACT)for local advanced cervical carcinoma.Methods A total of 55 patients with cervical carcinoma at stage Ib2~IIb were treated with neoadjuvant chemotherapy followed by radical hysterectomy and pelvic lymphadenectomy or radiotheraphy after evaluation.We analyzed the factors influencing the short-term effect of this NACT group.Another 33 patients at the same stages of cervical carcinoma who had not undergone NACT were recruited as control.The operative blood loss,parametrial invasion,vaginal margin involvement and the lymph nodes metastasis were compared.All the patients were followed up and the effects of NACT on survival were analyzed.Results The overall response rate in NACT group is 94.6%,including complete pathological response in 5 patients.Fifty-two patients underwent surgery after NACT.The lymph node involvement in NACT group was not significantly different from the control group(36.5% vs 48.5%,P>0.05).However,the lymph node involvement in patients with stage Ⅱ in NACT group was less than the control group(35.3% vs 69.2%,P<0.05).The parametrial invasion in NACT were less than the control group(1.9% vs 18.2%,P<0.05).The blood loss during operation was more in NACT group than in control group [(874.0±675.6)ml vs(493.9±316.7)ml,P<0.05].Conclusions NACT shows short-term effect on local advanced cervical cancer,and provides more opportunity for operation.Postoperative chemotherapy may extend disease free survival of patients with local advanced cervical cancer.Postoperative chemotherapy and lymph node involvement are factors influencing disease free survival.NACT has no effect on disease free survival and overall survival.
