Abstract:Objective To evaluate the efficacy of amniotic membrane transplantation with continu- ous lock stitch suture for diseases of ocular surface. Method Amniotic membrane transplantation with continu- ous lock stitch suture was performed in 31 eyes of 26 patients for diseases of ocular surface (group 1), Amniotic membrane transplantation with interrupted sutrue of simple continuous suture was performed in 34 eyes of 31 patients for diseases of ocular surface (group 2). Results In group 1,Amniotic membrane desquamated in 3 of 31 eyes within 3 weeks post-operatively, and persisted in 19 of 22 eyes after 3 weeks post-operatively, in 17 of 28 eyes corneal epithelialization within 2 weeks post-operatively. In group 2, Amniotic membrane desquamated in 15 of 34 eyes within 3 weeks post-operatively, and persisted in 6 of 21 eyes after 3 weeks post-operatively, in 12 of 32 eyes corneal epithelialization within 2 weeks post-operatively. Conclusions Amniotic membrane transplantation with continuous lock stitch suture can prevent amniotic membrane from desquamated and absorption, promote the epithelialization, reduce pain and increase the efficacy of amniotic membrane transplantation.
Feasibility of adipose tissue-derived stem cell sheet fabricated ex vivo on temperature-responsive scaffolds as seed cells for ocular surface reconstruction

Abstract:Background Development of corneal tissue engineering creates a new therapeutic method for severe corneal diseases.However,ideal seed cells and scaffold for corneal surface reconstruction have not yet been investigated well.Adipose-derived stem cells (ADSCs) are varified to have a self-renewal ability and epithelioid features,and temperature-responsive scaffolds (TRSs) can offer technical support for stem cell sheet.Objective This study was to investigate the characteristics of ADSCs cultured on TRSs and compare these features to typical oral mucosal epithelial cells (OMECs),and therefore to explore the feasibility of reconstruction of ocular surface with ADSCs as seed cells.Methods Self-made TRSs were prepared by adding isopropyl alcohol dissolved poly-Nisopropylacrylamide (PNIPAAm) to each polystyrene tissue culture dish and then irradiating using an election beam.Subcutaneious fatty tissue of rabbit neck was obtained to culture ADSCs,and 4 pieces of oral cavity mucosal tissue were digested and cultured to obtain OMECs.Then the ADSCs and OMECs were incubated on TRSs,and cell morphology,growth rate,detached duration and survival counts were compared between ADSCs and OMECs.The ADSCs sheet and OMECs sheet were stained with hematoxylin and eosin for morphological examination.Immunochemistry was used to observe the expressions of stem-cell biomakers and epithelioid-cell biomakers in the cells.The ultrastructure of cell surface was observed under the scanning electron microscope.Results Self-made TRSs were similar to ordinary culture dish in the transparancy and smoothness.The water contact angle of 4 in 5 samples were ＞10° with the effective rate upto 80％.A DSCs showed the elongated fusiform in shape,while OMECs showed a cobblestone appearance.The growth cycle,detached duration and cell number of ADSCs were 12-14 days,(46.0 ±9.6) minutes and (7.9 ±1.1)×105/sheet,and those of OMECs were 14-16 days,(91.9 ±10.9) minutes and (45.8 ±26.5)×105/sheet,respectively,showing statistically significant differences in the detached duration and cell counts between ADSCs and OMECs (P=0.002,0.028).Hematoxylin and eosin staining showed that ADSCs sheet comprised only 1-3 layer cells,while OMECs showed 4-5 layer cells.ATP-binding cassette superfamily G member 2 (ABCG2),p63 and cytokeratin 12 (CK12) were positively expressed in both ADSCs sheet and OMECs sheet.Closely packed cells and typical eithelial microvilli in the cell surface were exhibited in both ADSCs sheet and OMECs sheet under the scanning electron microscope.Conclusions Self-made TRSs can be used as scaffold of ADSCs.The ADSCs sheet on the TRSs appears to have a good cell vitality and therefore is a new seed source of ocular surface reconstruction.

Abstract:BACKGROUND:Corneal disease is the second most common cause of blindness in China. Clinically, treatment options for corneal diseases with limbal stem cell deficiency (LSCD) are limited due to a shortage of organ donors and inevitable immune rejection. This study aims to determine the efficacy of reconstructing the ocular surface using autologous cultivated adipose tissue-derived stem cells (ADSCs) and to develop a new clinical treatment for patients with LSCD.METHODS:A rabbit LSCD model was first established. Two weeks later, the animals were divided into three groups, including the sham group, the amniotic membrane transplantation group, and the ADSC combined with amniotic membrane transplantation group, and underwent surgery. The efficacy of reconstructing the ocular surface using ADSCs was evaluated using immunofluorescent staining, confocal microscopy (CM) observation, H&E staining, immunohistochemical staining, and scanning transmission electron microscopy observation one, two and four weeks after surgery.RESULTS:Evaluations of immunofluorescent staining of the cornea pre- and post-surgery yielded significantly lower scores for the corneas in the ADSCs transplantation group than for those in the sham group (F=-7, P=0.002, <0.05) and the amniotic membrane transplantation group (F=-4.67, P=0.018, <0.05) two weeks after surgery. Four weeks after surgery, the corneas of the ADSC combined with amniotic membrane transplantation group were scored significantly lower than those in the sham group (F=-8, P=0.007, <0.05) and the amniotic membrane transplantation group (F=-5.33, P=0.046, <0.05). The data suggest that the use of ADSCs to treat LSCD showed greater efficacy than the other treatment methods. The growth of ADSCs on the corneal surface was examined using confocal and electron microscopes. K3/K12 expression in the corneal epithelium, which was reconstructed by ADSCs, was negative, as shown by immunohistochemical staining.CONCLUSIONS:Ocular surface reconstruction can be improved by using ADSCs as seed cells and the amniotic membrane as a carrier, thus providing a new therapeutic strategy for patients with LSCD.

Abstract:Stem cell-based therapy is a novel technique used to reconstruct the ocular surface of patients with limbal stem cell deficiency (LSCD). However, the reported stem cell therapies for LSCD are limited to two-dimensional (2D) culture systems, which cannot mimic the native microenvironment in which stem cells reside in vivo . Herein, an injectable photocrosslinkable hydrogel was developed based on porous gelatin methacryloyl (GelMA)/silk fibroin glycidyl methacrylate (SilMA), which encapsulated limbal stem cells (LSCs) to reconstruct ocular surfaces. The porous GelMA/SilMA hydrogels demonstrated excellent transparency and good adhesion. More importantly, this hydrogel maintained viability, supported the adhesion of encapsulated rabbit LSCs and promoted their migration in vitro in three-dimensional (3D) culture environments. Proteomic analysis confirmed that the porous GelMA/SilMA hydrogel 3D cell culture system maintained both the self-renewal and differentiation of the LSCs. Furthermore, histological analyses demonstrated that the porous GelMA/SilMA hydrogel encapsulating LSCs effectively promoted the re-epithelization of LSCD. Porous hydrogels were subjected to simplified serial cell culture operations to construct an ocular surface with a uniform cell distribution. These findings provide valuable insights into stem cell therapies and regenerative medicine for ocular surface reconstruction.

Abstract:Amniotic membrane (AM) has been widely used as a temporary or permanent graft in the treatment of various ocular surface diseases. In this study, we compared the epithelial wound healing and tissue remodeling after ocular surface reconstruction with intact amniotic membrane (iAM) or denuded amniotic membrane (dAM). Partial limbal and bulbar conjunctival removal was performed on New Zealand rabbits followed by transplantation of cryo-preserved human iAM or dAM. In vivo observation showed that the epithelial ingrowth was faster on dAM compared to iAM after AM transplantation. Histological observation showed prominent epithelial stratification and increased goblet cell number on dAM after 2 weeks of follow up. Collagen VII degraded in dAM within 2 weeks, while remained in iAM even after 3 weeks. The number of macrophages and α-SMA positive cells in the stroma of remodelized conjunctiva in the dAM transplantation group was considerably less. In conclusion, dAM facilitates epithelial repopulation and goblet cell differentiation, further reduces inflammation and scar formation during conjunctival and corneal limbal reconstruction.

Abstract:Tissue engineered scaffolds have demonstrated values in fabricating corneal equivalence. Attempts in this area tend to develop functional scaffold with physiochemical properties and structure resembling the native corneal tissue. Herein, a novel recombinant human collagen (RHC) based scaffold with ordered microstructures is presented to induce the regeneration of corneal stroma. As the RHC is modified with methacrylate anhydride (MA) under optimized concentration and is tailored with hierarchically ordered microstructures of aligned microgrooves and inverse opal nano-scale pores, the derived collagen hydrogels (RHCMA) can possess physiochemical properties resembling native cornea. Diverse topological structures (culture dish, RHCMA hydrogel without patterns, MI-RHCMA hydrogel patch with different width of microgrooves) are determined to study their influence on the growth and differentiation of corneal limbal stromal stem cells (LSSCs), and results demonstrate that the novel MI-RHCMA hydrogel patch could induce LSSCs to grow orderly and differentiate into keratocyte in vitro. Rat intrastromal keratoplasty are performed to examine the tissue repair efficacy of the derived collagen hydrogels, and the results indicate that the novel MI-RHCMA hydrogel patch could induce the regeneration of damaged corneal stroma in vivo. These results indicate that our presented scaffolds can be an effective transplantation graft and will find important values for the damaged cornea in the clinic.

Abstract:· AIM: To investigate the clinical effect and mechanism of preserved human amniotic membrane transplantation on ocular surface reconstruction. · METHODS: 144 patients (154 eyes) received preserved human amniotic membranes transplantation for ocular surface diseases resulted from various causes. All cases were followed-up for 4-12mo postoperatively. · RESULTS: Conjunctival congestion and irritation sign were relieved after the operation. Edema of transplanted tissue disappeared within 1-2wk. 111 eyes(72.1%)had improved visual acuity postoperatively; 147 eyes(95.5%)had satisfactory outcome by one treatment. · CONCLUSION: Preserved human amniotic membrane transplantation can safely and effectively reconstruct ocular surface in the cases with ocular surface disorder. The technique is valuable to be applied extensively.·

Abstract:The integrity and transparency of the cornea plays a key role in preserving vision. This paper reports a procedure to create an artificial sheet of corneal epithelium from cryopreserved limbal stem cells (LSCs) and to use this for corneal transplantation. Corneal LSCs were isolated from biopsy specimens of rabbit limbal lamellar and cryopreserved in liquid nitrogen at 2–4 passages. The cells were grown in culture medium for 12–14days on top of a cell-free human amniotic membrane framed on a nitrocellulose sheet. The corneal epithelium generated was transplanted into the right eyes of 14 LSC deficient (LSCD) rabbits (seven experimental animals, seven controls) with corneal damage. The seven LSCD rabbits in the experimental group were transplanted with a corneal epithelial sheet generated from the cryopreserved corneal LSCs. Four LSCD rabbits were used as the vehicle control and were transplanted with a cell-free amniotic membrane, and the remaining three LSCD rabbits were negative controls without transplantation. Over a 2-month recovery period, 2/7 animals in the experimental group recovered completely, four recovered partially and one did not respond. In the control groups, three negative controls and three vehicle controls lost their vision completely, and one of the vehicle controls partially recovered transparency of the cornea Following treatment, corneal transparency of the experimental rabbits was significantly improved compared to controls (P<0.05). The results indicated that cryopreserved corneal LSCs can repair damaged rabbit cornea, suggesting a possible new clinical approach to reconstruction of corneal epithelium.

Abstract:Engineered tissue derived from ocular surface stem cells (SCs) are a cutting edge biotechnology for repair and restoration of severely damaged eyes as a result of ocular surface dysfunction because of SC failure. Ex-vivo SC expansion techniques have advanced significantly since the first patients were treated in the late 1990s. The techniques and clinical reports reviewed here highlight the evolution and successes of these techniques, while also revealing gaps in our understanding of ocular surface and SC biology that drives further research and development in this field. Although hurdles still remain before stem-cell-based therapies are more widely available for patients with devastating ocular surface disease, recent discoveries in the field of mesenchymal SCs and the potential of induced pluripotent SCs heralds a promising future for clinicians and our patients.

Abstract:Conjunctival restoration is indispensable to help maintain the ocular surface microenvironment by secreting lubricative mucins. However, conventional amniotic membrane transplantation therapy has many limitations in its application due to risks involved with disease transmission and alloreactivity. As decellularized tissues have been frequently used for tissue engineering and adipose mesenchymal stem cells (ADMSCs) have been acknowledged for their low immunogenicity, we fabricated a decellularized matrix of adipose-derived mesenchymal stromal cells (DMA) to study the therapeutic potential of DMA in healing conjunctival defects. In the present study, the fabricated DMA, with certain thickness, exhibited transplantation operability in vivo. When applied in conjunctival defect rabbit models, the sheet of DMA played a substantial role in providing structural support without causing cosmetic difference. Moreover, DMA displayed great superiority in promoting faster wound closure with better stratified epithelium containing more goblet cells than the amniotic membranes (AMs). Mechanistically, compared with the tissue culture plates (TCPs) and TCPs coated with collagen or fibronectin (two of the main components of DMA), DMA exhibited its unique property in maintaining the stem cells of CjECs in an undifferentiated state, which is highly essential for long-term conjunctival reconstruction. In addition, DMA effectively enhanced the proliferation of CjECs by activating stronger phosphorylation of the Akt signaling pathway, the results of which were further verified in the in vivo experiment via the histological examination of p-Akt levels in reconstructed conjunctival epithelium by DMA. Thus, the decellularized matrix of ADMSCs depicts a promising conjunctival substitute in ocular reconstruction.

Abstract:Conjunctival defects severely impair ocular surface homeostasis and vision. However, the existing conjunctival reconstruction methods are still unable to achieve satisfactory functional replacement of conjunctiva. Scaffold materials that provide a more stem-cell-friendly microenvironment are in urgent need of improvement. Herein we reported a functional conjunctival reconstruction scaffold constructed from decellularized matrix derived from rabbit subconjunctival fibroblasts (DM-SCF) loaded with conjunctival epithelial stem cells (CjESCs) for ocular surface repair. The DM-SCF had a thickness and mechanical strength close to that of normal conjunctiva and showed excellent biocompatibility. CjESCs inoculated on DM-SCF maintained good stem cell properties, which showed long-term maintenance of low differentiation and good proliferation activity. DM-SCF transplantation with CjESCs showed satisfactory results in rabbit models of a large conjunctival defect. The conjunctiva was reconstructed with abundant goblet cell expression and regular fiber arrangement. Protein mass spectrometry revealed the active extracellular matrix components of DM-SCF, regulating the activation of Wnt/β-catenin and Notch pathways for stem cell maintenance. Overall, our study developed a promising new strategy for ocular surface reconstruction by optimizing bioactive scaffold and providing functional stem cell niche, so as to achieve long-standing and superior functional repair.
Reconstruction of Damaged Cornea by Autologous Transplantation of Epidermal Adult Stem Cells

Abstract:Purpose: It is crucial for the treatment of severe ocular surface diseases such as Stevens-Johnson syndrome (SJS) and ocular cicatricial pemphigoid (OCP) to find strategies that avoid the risks of allograft rejection and immunosuppression. Here, we report a new strategy for reconstructing the damaged corneal surface in a goat model of total limbal stem cell deficiency (LSCD) by autologous transplantation of epidermal adult stem cells (EpiASC).Methods: EpiASC derived from adult goat ear skin by explant culture were purified by selecting single cell-derived clones. These EpiASC were cultivated on denuded human amniotic membrane (HAM) and transplanted onto goat eyes with total LSCD. The characteristics of both EpiASC and reconstructed corneal epithelium were identified by histology and immunohistochemistry. The clinical characteristic of reconstructed corneal surface was observed by digital camera.Results: Ten LSCD goats (10 eyes) were treated with EpiASC transplantation, leading to the restoration of corneal transparency and improvement of postoperative visual acuity to varying degrees in 80.00% (8/10) of the experimental eyes. The corneal epithelium of control groups either with HAM transplantation only or without any transplantation showed irregular surfaces, diffuse vascularization, and pannus on the entire cornea. The reconstructed corneal epithelium (RCE) expressed CK3, CK12, and PAX-6 and had the function of secreting glycocalyx-like material (AB-PAS positive). During the follow-up period, all corneal surfaces remained transparent and there were no serious complications. We also observed that the REC expressed CK1/10 weakly at six months after operation but not at 12 months after operation, suggesting that the REC was derived from grafted EpiASC.Conclusions: Our results showed that EpiASC repaired the damaged cornea of goats with total LSCD and demonstrated that EpiASC can be induced to differentiate into corneal epithelial cell types in vivo, which at least in part correlated with down-regulation of CK1/10 and upregulation of PAX-6.
Corneal Reconstruction with EGFP-Labelled Limbal Mesenchymal Stem Cells in a Rabbit Model of Limbal Stem Cell Deficiency

Abstract:Ocular surface reconstruction is essential for treating corneal epithelial defects and vision recovery. Stem cell-based therapy demonstrates promising results but requires further research to elucidate stem cell survival, growth, and differentiation after transplantation in vivo. This study examined the corneal reconstruction promoted by EGFP-labeled limbal mesenchymal stem cells (L-MSCs-EGFP) and their fate after transplantation. EGFP labeling allowed us to evaluate the migration and survival rates of the transferred cells. L-MSCs-EGFP seeded onto decellularized human amniotic membrane (dHAM) were transplanted into rabbits with a modeled limbal stem cell deficiency. The localization and viability of the transplanted cells in animal tissue were analyzed using histology, immunohistochemistry, and confocal microscopy up to 3 months after transplantation. EGFP-labeled cells remained viable for the first 14 days after transplantation. By the 90th day, epithelialization of the rabbit corneas reached 90%, but the presence of viable labeled cells was not observed within the newly formed epithelium. Although labeled cells demonstrated low survivability in host tissue, the squamous corneal-like epithelium was partially restored by the 30th day after transplantation of the tissue-engineered graft. Overall, this study paves the way for further optimization of transplantation conditions and studying the mechanisms of corneal tissue restoration.

Methods: The experiment was carried out in the Central Laboratory and Laboratory of Endocrinology of the First Clinical Medical College of Nanjing Medical University between January 2000 and December 2003. Fifteen New Zealand white rabbits of both genders were made into models of unilateral limbal dysfunction by surgery, which were the recipient corneas to receive the transplantation experiment. The animals were divided into three groups randomly: Group A (tissue engineering cornea epithelium autograft transplantation), Group B (amniotic membrane transplantation), Group C (model control). Group A: With the aid of a surgical microscope, 3-by-3-mm biopsy samples were taken from the rabbit's limbus and transported to the laboratory for culture. The growth characteristics of the cultured cells were observed. The cells were stained positive to cytokeratin by AE1 monoclonal antibody and negative for the PAS. The characteristics of the cultured cells were also observed under transmission electron microscope and scanning electron microscopy. Tissue-engineered epithelial-cell sheets were obtained ex vivo by culturing the cells on amniotic membrane with the air-liquid interface culture for two weeks. Samples were fixed and processed with use of standard histologic procedures. The cell sheets on the amniotic membrane were examined by both light and electron microscopy. At 4 weeks after the ocular surface injury, the surfaces of the recipient conjunctivalized cornea of the rabbit were surgically reconstructed by transplanting tissue-engineered epithelial-cell sheets. Group B: 5 eyes underwent amniotic membrane transplanting to reconstruct the corneal surfaces. Group C: 5 eyes obtained no treatment being used as a model control. Corneal epithelial fluorescein staining with Slit lamp examination and anterior eye segment photography was done in 7,10,20,30 days after transplantation experiment. The tissue of the cornea was taken to observe under light microscopy by histological analysis. Results: 1In primary culture, most of the cells attached to the bottom and began to proliferate after being cultured for 48 hours. In 5 days, the cells continued proliferation rapidly, and in about 10 days, the cells were confluent into film gradually with a very smooth and regular perimeter. In subculture, the cells attached the bottom during the first 24-hour and were confluent in 10 days. 2 The cells ranged mosaic structure and localized together with clear rim. The cells had large nucleus and distinct nucleoli. Most of them were regularly of same size as polygonal appearance. Some cells stained positive to cytokeratin by AE1 monoclonal antibody and negative for the PAS. 3Transmission electron microscope showed lots of microvilli on the surface of the cells and tight junctions between them. Desmosomal cell-cell contact was evident by scanning electron microscopy. 4 Most of the cells attached to the amniotic membrane in 24 hours. A confluent culture of limbal corneal epithelial cells was established on acellular human amniotic membrane after 7 days. A multi-layered epithelium was obtained with the air-liquid interface culture for more than 2 weeks, and more desmosomal junctions between the cells were found. 5The cultivated epithelial sheets had 5 or 6 layers of stratified, well-differentiated cells. Histological examination revealed that the cultivated epithelial cells were similar in appearance to those of in vivo normal corneal epithelium. 6Corneas that were grafted with the cultivated corneal epithelial cells on an AM carder were clear and were epithelialized 20 and 30 days after surgery. Conclusion: We have generated cultures of rabbit corneal epithelial cells from limbal tissues on AM. We have successfully carried out autologous transplantation of these cultivated corneal epithelial cells onto the ocular surfaces of rabbit eyes. We believe that autologous transplantation of cultivated corneal epithelium is a feasible method for ocular surface reconstruction.

Abstract:Total face skin and soft-tissue defects remain one of the biggest challenges in reconstructive surgery. Reconstruction of the entire face with uniform coverage and delicate features is difficult to achieve. To avoid the patchwork result seen in multiple flaps and skin grafts, 1 monoblock flap that has similar color, texture, and thickness might be an ideal option to minimize the incisional scars and several surgical procedures but is unavailable with current approaches because of the lack of sufficient matched tissue and the unreliable blood supply for such a large flap. To acquire a monoblock flap for full face reconstruction, we combine the prefabricated flaps, skin overexpansion, and bone marrow mononuclear stem cell transplantation for total facial resurfacing. In this article, we present our experience from our case series that provides universally matched skin and near-normal facial contour. It is a reliable and an excellent reconstructive option for massive facial skin defect.
Reconstruction of Corneal Epithelium with Cryopreserved Corneal Limbal Stem Cells in a Goat Model

Abstract:We describe a procedure to construct an artificial corneal epithelium from cryopreserved limbal stem cells (LSCs) for corneal transplantation. The LSCs were separated from limbal tissue of male goats. The primary LSCs were identified by flow cytometry and were expanded. They were examined for stem cell-relevant properties and cryopreserved in liquid nitrogen. Cryopreserved LSCs were thawed and then transplanted onto human amniotic membrane, framed on a nitrocellulose sheet, to construct corneal epithelium sheets. The artificial corneal epithelium was transplanted into the right eye of pathological models of total limbal stem cell deficiency (LSCD). Then, the effects of reconstruction were evaluated by clinical observation and histological examination. Polymerase chain reaction analysis was used to detect the SRY gene. The data showed that transplantation of cryopreserved LSCs, like fresh LSCs, successfully reconstructed damaged goat corneal surface gradually, but the SRY gene expression from male goat cells could only be detected in the first 2 months after transplantation. The therapeutic effect of the transplantation may be associated with the inhibition of inflammation-related angiogenesis after transplantation of cryopreserved LSCs. This study provides the first line of evidence that cryopreserved LSCs can be used for reconstruction of damaged corneas, presenting a remarkable potential source for transplantation in the treatment of corneal disorders.

Abstract:Objective: Autologous osteochondral transplantation (OCT) is an option to treat articular cartilage defects. However, poor graft integration at the cartilage defect/graft interface and secondary graft degeneration limit the use of autologous OCT. Cell sheet engineering offers hope in regenerative medicine. The purpose of this study was to evaluate the effect of synovium derived mesenchymal stem cell (SMSC) sheet on autologous OCT for (1) lateral integration and (2) graft degeneration. Method: Full-thickness osteochondral defects (3.5 mm in diameter, 3.0 mm in depth) were created at both femoral grooves of 12 rabbits. The right knees were used as the cell sheet group and the left knees were used as the control group. Osteochondral plugs were harvested when the defects were created and then were placed into their original locations with (cell sheet group) or without (control group) three-layered SMSC sheets. The animals were sacrificed at 4 and 8 weeks postoperatively (6 animals at each time point). Histological and immunohistochemical staining were used to evaluate the result. Results: Histological and immunohistochemical findings at the interface between the grafts and the original cartilage showed better integration in the cell sheet group. In addition, less graft and original cartilage degeneration were found 4 and 8 weeks after operation. The mean histological score was significantly higher in the cell sheet group. Conclusion: The animal model showed that the SMSC sheet enhanced the quality of autologous OCT by improving the integration of grafts to native cartilage and decreasing cartilage degeneration. Thus, the application of SMSC sheet can represent a therapeutic option for autologous OCT.
