Abstract:Tissue engineered scaffolds have demonstrated values in fabricating corneal equivalence. Attempts in this area tend to develop functional scaffold with physiochemical properties and structure resembling the native corneal tissue. Herein, a novel recombinant human collagen (RHC) based scaffold with ordered microstructures is presented to induce the regeneration of corneal stroma. As the RHC is modified with methacrylate anhydride (MA) under optimized concentration and is tailored with hierarchically ordered microstructures of aligned microgrooves and inverse opal nano-scale pores, the derived collagen hydrogels (RHCMA) can possess physiochemical properties resembling native cornea. Diverse topological structures (culture dish, RHCMA hydrogel without patterns, MI-RHCMA hydrogel patch with different width of microgrooves) are determined to study their influence on the growth and differentiation of corneal limbal stromal stem cells (LSSCs), and results demonstrate that the novel MI-RHCMA hydrogel patch could induce LSSCs to grow orderly and differentiate into keratocyte in vitro. Rat intrastromal keratoplasty are performed to examine the tissue repair efficacy of the derived collagen hydrogels, and the results indicate that the novel MI-RHCMA hydrogel patch could induce the regeneration of damaged corneal stroma in vivo. These results indicate that our presented scaffolds can be an effective transplantation graft and will find important values for the damaged cornea in the clinic.

Abstract:The cornea serves as the main refractive component of the eye with the corneal stroma constituting the thickest component in a stratified layered system of epithelia, stroma, and endothelium. Current treatment options for patients suffering from corneal diseases are limited to transplantation of a human donor cornea (keratoplasty) or to implantation of an artificial cornea (keratoprosthesis). Nevertheless, donor shortage and failure of artificial corneas to integrate with local tissue constitute important problems that have not been yet circumvented. Recent advances in biofabrication have made great progress toward the manufacture of tailored biomaterial templates with the potential of guiding partially or totally the regeneration process of the native cornea. However, the role of the corneal stroma on current tissue engineering strategies is often neglected. Here, we achieved a tissue-engineered corneal stroma substitute culturing primary keratocytes on scaffolds prepared via melt electrowriting (MEW). Scaffolds were designed to contain highly organized micrometric fibers to ensure transparency and encourage primary human keratocytes to self-orchestrate their own extracellular matrix deposition and remodeling. Results demonstrated reliable cell attachment and growth over a period of 5 weeks and confirmed the formation of a dense and highly organized de novo tissue containing collagen I, V, and VI as well as Keratocan, which resembled very closely the native corneal stoma. In summary, MEW brings us closer to the biofabrication of a viable corneal stroma substitute.
Salvage prostatectomy post-definitive radiation therapy: The Vancouver experience.

Abstract:INTRODUCTION Prostate cancer recurrence following primary radiation is common. If the recurrence remains localized to the prostate gland, surgical removal may result in long-term local control or cure. Despite the well-established oncological outcomes, salvage prostatectomy is infrequently performed or reported. We present our experience with salvage prostatectomy at a Canadian centre. METHODS We identified all patients undergoing salvage prostatectomy at the Vancouver General Hospital between 1995 and 2010 from a prospectively recorded and maintained prostate cancer database. Details regarding initial presentation, delivery of radiotherapy, clinical features at the time of recurrence, as well as oncological and functional outcomes, were collected. Information regarding postoperative morbidity was collected prospectively and confirmed by retrospective chart review. RESULTS Over a 15-year period, salvage prostatectomy was successfully completed in 21 patients. With a median follow-up period of 68 months (range: 2-122), 9 (43%) patients experienced a biochemical recurrence, with most failing within the first 2 years of surgery. There were 3 deaths in the cohort, all from prostate cancer, giving a prostate cancer specific and overall survival of 86%. The main postoperative morbidity was bladder neck contracture, occurring in 40%. One patient each developed a recto-urethral fistula and osteitis pubis. Physician-recorded data regarding continence was available in 13 (62%). Of these 13 patients, 10 (85%) men were recorded as dry or using 1 pad per day. CONCLUSIONS This is the first Canadian centre to report that salvage prostatectomy can be performed with favourable oncological and functional outcomes.
Tissue-Engineered Corneal Stroma by Using Autologous Adipose Derived Stem Cell Tissue and Polylacticcocglycolic Acid

Abstract:Purpose: To determine whether a polylactic-co-glycolic acid (PGLA) scaffold bearing the autologous adipose derived stem cells (ADSC) insert could repair the defect of rabbit corneal stroma without compromising tissue transparency. Methods: ADSC were isolated from rabbits and expanded by tissue culture. The fourth passage of cells were harvested and mixed with PLGA meterial to form cell-scaffold constructs. After 1 week of co-culturing, they were transplanted into the rabbit corneal stroma. Upon follow-up, slit lamp and confocal corneal scanning microscope were used to investigate the cornea. Nine weeks after transplantation, transmission electron microscopy and histological evaluation were performed. Transplantation of PLGA scaffold alone and autologous corneal stroma served as controls. Results: ADSC were successfully obtained and maintained which appeared fibroblast-like. ADSC attached and proliferated well either on the surface of or inside the scaffolds. After stroma transplantation in vivo, implants gradually became transparent without corneal neovascularization, while the corneal epithelium and endothelium remained normal. Regeneration of nerve fibers without inflammatory cell infiltration was observed in the corneal stroma. HE examination revealed that the corneal stroma gradually renovated. Transmission electron microscopy analysis showed no significant difference in the diameter of collagen fibrils among the groups but that the collagen fibers were aligned in a disordered fashion. Conclusions: The successful repair of corneal stroma defects with this approach supports the potential use of ADSC with PLGA scaffolds for cornea tissue engineering. The minimally invasive surgical procedure needed to obtain autologous ADSC and the low risk of rejection makes this a promising clinical approach.

Abstract:<p>Engineering substantia propria (or stroma of cornea) that mimics the function and anatomy of natural tissue is vital for in vitro modelling and in vivo regeneration. There are, however, few examples of bioengineered biomimetic corneal stroma. Here we describe the construction of an orthogonally oriented 3D corneal stroma model (3D-CSM) using pure electro-compacted collagen (EC). EC films comprise aligned collagen fibrils and support primary human corneal stromal cells (hCSCs). Cell-laden constructs are analogous to the anatomical structure of native human cornea. The hCSCs are guided by the topographical cues provided by the aligned collagen fibrils of the EC films. Importantly, the 3D-CSM are biodegradable, highly transparent, glucose-permeable and comprise quiescent hCSCs. Gene expression analysis indicated the presence of aligned collagen fibrils is strongly coupled to downregulation of active fibroblast/myofibroblast markers α-SMA and Thy-1, with a concomitant upregulation of the dormant keratocyte marker ALDH3. The 3D-CSM represents the first example of an optimally robust biomimetic engineered corneal stroma that is constructed from pure electro-compacted collagen for cell and tissue support. The 3D-CSM is a significant advance for synthetic corneal stroma engineering, with the potential to be used for full-thickness and functional cornea replacement, as well as informing <em>in vivo</em> tissue regeneration.</p>

Abstract:Corneal damage forms scar tissue and manifests as permanent corneal opacity, which is the main cause of visual impairment caused by corneal diseases. To treat these diseases, herein, we developed a novel approach based on the exosome derived from induced pluripotent stem cell-derived mesenchymal stem cells (iPSC-MSCs) combined with a thermosensitive hydrogel, which reduces scar formation and accelerates the healing process. We found that a thermosensitive chitosan-based hydrogels (CHI hydrogel) sustained-release iPSC-MSC exosomes can effectively promote the repair of damaged corneal epithelium and stromal layer, downregulating mRNA expression coding for the three most enriched collagens (collagen type I alpha 1, collagen type V alpha 1 and collagen type V alpha 2) in corneal stroma and reducing scar formation in vivo. Furthermore, iPSC-MSCs secrete exosomes that contain miR-432-5p, which suppresses translocation-associated membrane protein 2 (TRAM2), a vital modulator of the collagen biosynthesis in the corneal stromal stem cells to avert the deposition of extracellular matrix (ECM). Our findings indicate that iPSC-MSCs secrete miRNA-containing exosomes to promote corneal epithelium and stroma regeneration, and that miR-432-5p can prevent ECM deposition via a mechanism most probably linked to direct repression of its target gene TRAM2. Overall, our exosomes-based thermosensitive CHI hydrogel, is a promising technology for clinical therapy of various corneal diseases.

Abstract:Purpose : Regeneration of corneal stroma is believed to be among the greatest challenges in corneal tissue engineering. The aim of this study is to investigate the effect of a 3D-printed nano-structural poly(e-caprolactone) (PCL) scaffold on the material-cellular interaction, and to evaluate the efficiency for the corneal stroma regeneration in vitro. Methods : Two nano-structural scaffolds of different design and fiber distribution were fabricated by melt electrospinning 3D printing method using PCL. Human keratocytes were isolated from collagenase-digested limbal stromal tissue and seeded on the 3D-printed PCL scaffold. Cell morphology on scaffolds was studied by confocal microscopy, and cell proliferation was analyzed by PrestoBlue assay. Extracellular matrix(ECM) produced by keratocytes on the scaffolds was characterized by scanning electron microscope(SEM) and two-photon fluorescent microscopy. Results : The 3D-printed PCL scaffolds exhibited different micro-structure under SEM. As revealed in Figure.1, scaffold on the left presented offset fiber distribution, while the other one showed a fiber distribution at regular intervals. SEM and two-photon fluorescent microscopy demonstrated that keratocytes tended to grow along the fibrous network, and spread well on both scaffolds. Collagenous ECM produced by cells on the regular-fibrous scaffold was more abundant than on the offset-fibrous scaffold, forming a relatively thicker cross-section (Figure 2). Compared to a collagen fibrillar network secreted on regular-fibrous scaffolds, a lamellar ultrastructural ECM composed of aligned collagen was remodeled on offset-fibrous scaffolds, which was similar to native cornea stroma. Immunofluorescence staining showed that regular-fibrous scaffolds induced less secretion of type I collagen (COLI) and type V collagen (COLV) than offset-fibrous scaffolds. Conclusions : Our study showed that the fiber composition and topography of 3D-printed nanofibrous PCL scaffolds have a significant effect on the cell behavior through the material-cellular interaction. The refined design of nano-structure by melt electrospinning method stimulates self-organization and self-adaption of ECM, which helps mimicking native tissue microenvironments and offering the potential as cornea stroma regeneration substitutes.
Wound dressing gel with resisted bacterial penetration and enhanced re-epithelization for corneal epithelial-stromal regeneration

Abstract:Corneal injury-induced inflammation may result in permanent vision loss. In situ forming adhesive hydrogels offer a promising tissue substitute for repair of partial-thickness corneal wounds, but the uncertainty of biodegradation rate potentially fails to form new tissues in vivo . To overcome this problem, a methacrylated collagen (ColMA) hydrogel is newly developed as a wound dressing to repair partial thickness corneal defects using a two-stage approach. Such a hydrogel can maintain the viability and adhesion of human corneal epithelial cells (HCECs) as well as promoting their migration in vitro . During surgery, we initially photopolymerized ColMA hydrogel to form a in situ coating at defects. The hydrogel is further designed to be dislodged off after implantation and thus avoid the potential risks from the uncontrollable gel biodegradation. In both rabbit and pig corneal defect models, this collagen hydrogel not only serves as a physical barrier to prevent bacterial penetration and corneal wound dehydration, but also produces nanogranules to highly promote re-epithelization, thereby contributing to the decrease of scar formation and improvement of corneal epithelial-stromal regeneration via reducing myofibroblast activation. This study presents a useful platform system that can be adapted and extended towards the repair of diverse tissues or organs through the addition of bioactive molecules, encouraging the ColMA hydrogel wound dressing to be translated into advanced regenerative therapies. Abstract:PURPOSE: To construct a scaffold using silk fibroin (SF) and chitosan (CS) that could replace the corneal stroma with the biological characteristics of the scaffold materials still intact.METHODS: To develop an organotypic corneal stroma, SF and CS were chosen to synthesise the tissue-engineered bioscaffold. We cultured primary rabbit corneal epithelial cells and corneal stromal cells in vitro. Keratocytes were used to assess cytotoxicity on SF-CS (SFCS) blends, which was determined by a Cell Counting Kit-8 assay. The corneal lamellar scaffolds were developed with sequential culture techniques to form cell-scaffold constructs. Implantation was tested in 15 New Zealand White rabbits. The corneal substitutes were analysed by light and electron microscopy.RESULTS: The reconstructed lamellar cornea was comparable to native tissue, with high levels of K3/12 expression in the corneal epithelial cells and vimentin in the stromal cells; moreover, the morphology and the position of the cells could be distinguished by histological methods. There was no sign of any immune reaction in or around the transplanted discs 12 weeks after implantation.CONCLUSION: A SFCS scaffold might be a suitable blend for corneal tissue engineering.

Abstract:The scarcity of corneal tissue to treat deep corneal defects and corneal perforations remains a challenge. Currently, small incision lenticule extraction (SMILE)-derived lenticules appear to be a promising alternative for the treatment of these conditions. However, the thickness and toughness of a single piece of lenticule are limited. To overcome these limitations, we constructed a corneal stromal equivalent with SMILE-derived lenticules and fibrin glue. In vitro cell culture revealed that the corneal stromal equivalent could provide a suitable scaffold for the survival and proliferation of corneal epithelial cells, which formed a continuous pluristratified epithelium with the expression of characteristic markers. Finally, anterior lamellar keratoplasty in rabbits demonstrated that the corneal stromal equivalent with decellularized lenticules and fibrin glue could repair the anterior region of the stroma, leading to re-epithelialization and recovery of both transparency and ultrastructural organization. Corneal neovascularization, graft degradation, and corneal rejection were not observed within 3 months. Taken together, the corneal stromal equivalent with SMILE-derived lenticules and fibrin glue appears to be a safe and effective alternative for the repair of damage to the anterior cornea, which may provide new avenues in the treatment of deep corneal defects or corneal perforations.

Abstract:Millions of individuals across the world suffer from corneal stromal diseases that impair vision. Fortunately, three-dimensional (3D) bioprinting technology which has revolutionized the field of regenerative tissue engineering makes it feasible to create personalized corneas. In this study, an artificial cornea with a high degree of precision, smoothness, and programmable curvature was prepared by using digital light processing (DLP) 3D bioprinting in one piece with no support structure, and the construct was then confirmed by optical coherence tomography (OCT). On the basis of this approach, we developed a novel corneal decellularized extracellular matrix/gelatin methacryloyl (CECM-GelMA) bioink that can produce complex microenvironments with highly tunable mechanical properties while retaining high optical transmittance. Furthermore, the composite hydrogel was loaded with human corneal fibroblasts (hCFs), and in vitro experiments showed that the hydrogel maintained high cell viability and expressed core proteins. In vivo tests revealed that the hydrogel might promote epithelial regeneration, keep the matrix aligned, and restore clarity. This demonstrates how crucial a role CECM plays in establishing a favorable environment that encourages the transformation of cell function. Therefore, artificial corneas that can be rapidly customized have a huge potential in the development of in vitro corneal matrix analogs.

Abstract:Abstract Biomaterials designed to replace the diseased cornea could be used to treat corneal blindness where human donor tissue is in short supply, but challenges are the integration of biomaterials with host tissue and cells, avoiding a rapid material degradation and maintaining corneal transparency. Additionally, implantation surgery often triggers an aggressive wound healing response that can lead to corneal thinning and opacity. Here, we report a collagen-based hydrogel with transparency and mechanical properties suitable for replacing a substantial portion of a damaged or diseased corneal stroma. The porous hydrogel permitted migration and population by host cells while maintaining transparency and thickness six months after surgical implantation in an in vivo model of human corneal surgery. With a novel hybrid surgical implantation technique inspired by LASIK refractive surgery, rapid wound healing occurred around implants to maintain biomaterial integrity, transparency and function. Host stromal cell repopulation and regeneration of host epithelium and nerves were observed, as necessary steps towards corneal regeneration. Finally, as a proof-of-principle, the hydrogel loaded with a neuroregenerative drug achieved sustained slow-release drug delivery in vitro. The proposed hydrogel and novel implantation technique together represent a therapeutic approach with translational potential for replacing and regenerating diseased corneal stromal tissue.

Abstract:Corneal epithelial defects and excessive wound healing might lead to severe complications. As stem cells can self-renew infinitely, they are a promising solution for regenerating the corneal epithelium and treating severe corneal epithelial injury. The chemical and biophysical properties of biological scaffolds, such as the amniotic membrane, fibrin, and hydrogels, can provide the necessary signals for stem cell proliferation and differentiation. Multiple researchers have conducted investigations on these scaffolds and evaluated them as potential therapeutic interventions for corneal disorders. These studies have identified various inherent benefits and drawbacks associated with these scaffolds. In this study, we provided a comprehensive overview of the history and use of various stem cells in corneal repair. We mainly discussed biological scaffolds that are used in stem cell transplantation and innovative materials that are under investigation.

Abstract:Tissue engineering cornea has shown great clinical potential for cornea reconstruction, but efficient recovery of natural structure and physiological function remains great challenges. In this study, the acellular porcine corneal stroma (APCS) was prepared by a phospholipase A2 decellularization method and further crosslinked with aspartic acid (Asp). The modified APCS-Asp scaffold showed significant increase of hydration degree, ultrastructure regularity, corneal viscoelasticity and anti-degradation ability compared to APCS. Autologous rabbit limbal tissue was pre-treated by tumor necrosis factor-alpha (TNF-α) and collagenase IV, and the pretreated primary limbal stem cells (LSCs) were cultured with embryonic stem cells conditioned medium (ESCM), and LSCs showed 3D cell sphere structure and improved stem cell properties compared to the control group. The auto-tissue engineering lamellar cornea (ATELC) was quickly reconstructed by using peptide hydrogel with a dynamic culture system. With intact and functional epithelial cell layer, the reconstructed ATELC quickly recovered natural optical characteristics 1 week post transplantation in the rabbit lamellar keratoplasty model and satisfying neural regrowth as well as favorable stromal repopulation were observed in the transplanted eyes in the 6 months following up post-surgery. In summary, this study provides a comprehensive optimized reconstruction strategy for ATELC, which maybe similar medical application to natural cornea.

A stable nonfluorescent derivative of resorufin for the fluorometric determination of trace hydrogen peroxide: applications in detecting the activity of phagocyte NADPH oxidase and other oxidases.

Abstract:The enzymatic determination of hydrogen peroxide can be accomplished with high sensitivity and specificity using N-acetyl-3, 7-dihydroxyphenoxazine (Amplex Red), a highly sensitive and chemically stable fluorogenic probe for the enzymatic determination of H2O2. Enzyme-catalyzed oxidation of Amplex Red, which is a colorless and nonfluorescent derivative of dihydroresorufin, produces highly fluorescent resorufin, which has an excitation maximum at 563 nm and emission maximum at 587 nm. The reaction stoichiometry of Amplex Red and H2O2 was determined to be 1:1. This probe allows detection of 5 pmol H2O2 in a 96-well fluorescence microplate assay. When applied to the measurement of NADPH oxidase activation, the Amplex Red assay can detect H2O2 release from as few as 2000 phorbol myristate acetate-stimulated neutrophils with a sensitivity 5- to 20-fold greater than that attained in the scopoletin assay under the same experimental conditions. Furthermore, the oxidase-catalyzed assay using Amplex Red results in an increase in fluorescence on oxidation rather than a decrease in fluorescence as in the scopoletin assay. In comparison with other fluorometric and spectrophotometric assays for the detection of monoamine oxidase and glucose oxidase, this probe is also found to be more sensitive. Given its high sensitivity and specificity, Amplex Red should have a broad application for the measurement of H2O2 in a variety of oxidase-mediated reactions and very low levels of H2O2 in food, environmental waters, and consumer products.

Abstract:Corneal transplantation is the most effective clinical treatment for corneal defects, but it requires precise size of donor corneas, surgical sutures, and overcoming other technical challenges. Postoperative patients may suffer graft rejection and complications caused by sutures. Ophthalmic glues that can long-term integrate with the corneal tissue and effectively repair the focal corneal damage are highly desirable. Herein, a hybrid hydrogel consisting of porcine decellularized corneal stroma matrix (pDCSM) and methacrylated hyaluronic acid (HAMA) was developed through a non-competitive dual-crosslinking process. It can be directly filled into corneal defects with various shapes. More importantly, through formation of interpenetrating network and stable amide bonds between the hydrogel and adjacent tissue, the hydrogel manifested excellent adhesion properties to achieve suture-free repair. Meanwhile, the hybrid hydrogel not only preserved bioactive components from pDCSM, but also exhibited cornea-matching transparency, low swelling ratio, slow degradation, and enhanced mechanical properties, which was capable of withstanding superhigh intraocular pressure. The combinatorial hydrogel greatly improved the poor cell adhesion performance of HAMA, supported the viability, proliferation of corneal cells, and preservation of keratocyte phenotype. In a rabbit corneal stromal defect model, the experimental eyes treated with the hybrid hydrogel remained transparent and adhered intimately to the stroma bed with long-term retention, accelerated corneal re-epithelialization and wound healing. Giving the advantages of high bioactivity, low-cost, and good practicality, the dual-crosslinked hybrid hydrogel served effectively for long-term suture-free treatment and tissue regeneration after corneal defect.

Abstract:Corneal transplantation remains the gold standard for treating corneal blindness; however, it is hampered globally by donor shortages and the complexity of suture-dependent procedures. Tissue-engineered corneas have demonstrated potential as corneal equivalents. Nevertheless, the development of adhesive corneal patches and full-thickness corneal substitutes remains challenging. In this study, a multifunctional hydrogel corneal patch (MHCP) is constructed by integrating a dual-crosslinked hybrid hydrogel with temperature and light responsiveness with a natural extracellular matrix scaffold. When applied to the ocular surface, MHCP spontaneously releases adhesives at body temperature and forms a stable adhesion with the recipient cornea through photocuring. In addition to its inherent mechanical, optical, and ultrastructural characteristics, which are similar to those of the natural stroma, MHCP demonstrates excellent suture resistance, anti-swelling, and anti-degradation properties after curing. MHCP promotes the proliferation and migration of corneal epithelial cells in vitro and maintains the phenotype of corneal stromal cells. In vivo, MHCP maintains graft hydration and restores corneal structural integrity and transparency during penetrating keratoplasty of various sizes and sutureless lamellar keratoplasty. Collectively, given the advantages of native stroma-like characteristics, operation-facilitating multiple functions, and convenient preparation, MHCP is a promising corneal substitute for clinical applications.

Abstract:Corneal defects can lead to stromal scarring and vision loss, which is currently only treatable with a cadaveric corneal transplant. Although in situ-forming hydrogels have been shown to foster regeneration of the cornea in the setting of stromal defects, the cross-linking, biomechanical, and compositional parameters that optimize healing have not yet been established. This, Corneal defects are also almost universally inflamed, and their rapid closure without fibrosis are critical to preserving vision. Here, an in situ forming, bioorthogonally cross-linked, nanocluster (NC)-reinforced collagen and hyaluronic acid hydrogel (NCColHA hydrogel) with enhanced structural integrity and both pro-regenerative and anti-inflammatory effects was developed and tested within a corneal defect model in vivo. The NCs serve as bioorthogonal nanocross-linkers, providing higher cross-linking density than polymer-based alternatives. The NCs also serve as delivery vehicles for prednisolone (PRD) and the hepatocyte growth factor (HGF). NCColHA hydrogels rapidly gel within a few minutes upon administration and exhibit robust rheological properties, excellent transparency, and negligible swelling/deswelling behavior. The hydrogel's biocompatibility and capacity to support cell growth were assessed using primary human corneal epithelial cells. Re-epithelialization on the NCColHA hydrogel was clearly observed in rabbit eyes, both ex vivo and in vivo, with expression of normal epithelial biomarkers, including CD44, CK12, CK14, α-SMA, Tuj-1, and ZO-1, and stratified, multilayered morphology. The applied hydrogel maintained its structural integrity for at least 14 days and remodeled into a transparent stroma by 56 days.
