Significance of Increased FDG Accumulation in the Anterior Mediastinum following Chemotherapy by Follow-up Study

Abstract:Objective:To assess whether the FDG uptake in the anterior mediastinum following chemotherapy is indeed a benign finding.Methods:Forty-nine patients with FDG uptake in anterior mediastinum on PET scans after chemotherapy were enrolled in our retrospective study.Among them,19 were males,30 were females,the age was 27.3 years old averagely.Twenty-six of them had PET follow-up,17 of them had CT follow up,6 of them had both PET and CT scans during follow-up.The average follow-up period was 11.3 months.Results:No malignancy was noted during the follow-up period in 48 of 49 patients(97.9%).In patients with PET follow-up,FDG uptake in the thymus region remained unchanged in 5 patients,decreased in intensity or returned to completely normal in 20 patients,mildly increased in only one patient.In patients with CT follow-up,9 patients had thymus hyperplasia after chemotherapy,7 patients had normal CT scan.In patients followed up by PET and CT scan,FDG uptake was normal in 3 patients,mildly decreased in 2 patients,mildly increased in one patient who was demonstrated without any pathologic finding by subsequent CT follow-up to 721days post PET scan.CT showed negative in 5 of 6 patients and thymus hyperplasia in 1 patient.Conclusion:Mild or moderate FDG uptake in thymus region on PET scan after chemotherapy represents benign process mostly.
