Abstract:Objective: To analyze the incidence,generation system and defect distribution of fetal anomalies and evaluate related factors of the cause of fetal anomalies to provide basis for clinical prevention and treatment.Methods: We have retrospectively analyzed 198 cases which were diagonosed of fetal anomalies in prenatal diagnosis and aborted in our hospital from January 2006 to December 2009.Results: The first 6 reasons of the abortion of 198 fetal anomalies are respectively as follows: congenital heart disease(32 cases,accounting for 16.6%),hydrocephalus(21 cases,accounting for 10.60%),lip/ palate cleft(20 cases,accounting for 10.10%),limb malformation,including short limbs,syndactyly(19 cases,accounting for 9.60%),neural tube defects(18 cases,accounting for 9.09%),digestive system malformations(15 cases,accounting for 7.58%).Conclusions: Occurrence of fetal anomalies has relation with maternal age,education,quality,environment,medication during pregnancy,pregnancy complications,and nutrition etc.In order to reduce the incidence of birth defects,we should focus on prevention,strengthen pre-pregnancy and prenatal counseling,avoid the influence of adverse factors during early pregnancy,detect the unusual early and timely implement therapeutic abortion in order to avoid burden to families and society.

Abstract:Pregnancy is a complicated and delicate process, the maternal body undergoes changes on hormones, immunity, and metabolism during pregnancy to support fetal development. Microbiomes in the human body mainly live in the intestine, and the human gut microbiomes are complex, which composed of more than 500 to 1500 different bacteria, archaea, fungi, and viruses. Studies have shown that these microbiomes are not only involved in the digestion and absorption of food but also indispensable in regulating host health. In recent years, there has been increasing evidence that microbiomes are important for pregnant women and fetuses. During pregnancy, there will be great changes in gut microbiomes. Regulating gut microbiomes is beneficial to the health of the mother and the fetus. In addition, many complications during pregnancy are related to gut microbiomes, such as gestational diabetes, obesity, preeclampsia, digestive disorders, and autoimmune diseases. Moreover, the microbiomes in mother's milk and vagina are closely related to the colonization of microbiomes in the early life of infants. In this review, we systematically review the role of maternal microbiomes in different gestational complications, and elucidate the function and mechanism of maternal microbiomes in the neural development and immune system of offspring. These will provide a clear knowledge framework or potential research direction for researchers in related fields.

Abstract:Medicine, Science and the Law, Ahead of Print. This article provides an analysis of the legal rights and protection accorded to fetuses under the Nepali law and the comparative common statues. It also analyses the abortion law in Nepal, which aims to balance the needs of women seeking abortion with limited protections for late-term fetuses. The article considers the case, "Lakshmi Dhikta v. the Government of Nepal," which held that access to abortion was a constitutionally formed right. However, barriers to this right still exist, especially among disadvantaged women. Globally, the debate continues between those who advocate for the rights of the fetus and those who put the autonomy of the women first. While some countries grant the fetus limited legal rights, others grant the fetus personhood rights. It is therefore pertinent to discuss the ethics of prenatal harm, sex-selective abortion, and the possible conflict between maternal and fetal interests. The multifaceted law should regulate maternal health, the interest of the fetus, and discrimination while ensuring feasible and affordable abortion.

Abstract:Background: Birth defects are the leading cause of mortality in newborn babies and children under five years old. In response, the Chinese government has implemented a three-tiered prevention strategy, which has brought ethical concerns about fetuses with birth defects. This study aims to explore the attitudes toward fetuses with birth defects among health professionals engaged in maternal and child health services. Methods: A qualitative study was conducted among 13 health professionals engaged in maternal and child health services in Hunan Province, China. The questions were designed to elicit the participants' work experience and attitudes toward fetuses with birth defects. The data were collected through in-depth semi-structured interviews, and NVivo 12 was used for data coding and analysis. A thematic analysis approach was employed following the SRQR checklist. Results: Five themes and 13 attributes were generated regarding health professionals' perspectives on fetuses with birth defects. The five themes included: (1) severity and curability of diseases (two attributes), (2) family relations (four attributes), (3) medical assessments (two attributes), (4) social situations (three attributes), (5) self-value orientations (three attributes). The findings showed that the majority of health professionals held the view that a fetus with a curable disease could be born, whereas a fetus with severe disability and teratogenesis should be terminated. Twelve out of the 13 health professionals believed that parents should be the decision-makers, while only one thought that the family should make a decision together. Conclusions: Attitudes toward birth defects were influenced by various factors, indicating the complexity of real-world cases identified in this study. The findings highlight the dilemmas faced by both families and health professionals regarding birth defects. Adequate medical knowledge and support from society are crucial to inform decision-making among family members. Additionally, standardized norms and policies for birth defects are needed. Establishing an ethics committee for prenatal diagnosis is necessary to address current ethical issues in this field.

The benefit shift of family planning policy should not be the change from interest allocation focused on punishment to the equal emphasis on punishment and reward, but the fundamental change from restriction to encouragement. Abstract:This study examines the legal dilemma of surrogacy in the novel Frog by the Nobel prize winner Mo Yan, which was written in the context of China's past one-child policy. This study offers a tri-tier exploration of how surrogacy led to conflicts and tensions in Frog: first, it traces how the intended parents' demand for surrogacy developed under the one-child policy in China while clarifying the application of "the principle of public order"; second, it explores the predicament of the surrogate mother Chen Mei regarding the violation of her rights and dignity, and delves into the interpretation of "the principle of human dignity"; third, it focuses on the custody dispute between the surrogate mother and the intended parents to explore the controversies over the concept of parentage in surrogacy and consider the principle of "best interest of the child." This study concludes that despite all the legal and ethical controversies, the spirit of law demands a response to the growing practice of surrogacy and an extension of our exploration of legal principles.

Abstract:Abstract The obstetric issues and management styles in China are different from that in Western countries. Chinese medical education, residency training, obstetric care structure, and management of common obstetric complications are briefly reviewed and compared to the United States. Maternal-fetal medicine (MFM) is rapidly developing in China, but the development of MFM may not follow the same trajectory as in the West. Understanding the difference between China and the West may facilitate communication and foster mutual development.

Abstract:Obstetric ultrasound is an important tool in managing pregnancies and its use is increasing globally. However, the status of the pregnant woman and the fetus may vary in terms of clinical management, views in the community and legislation. To investigate the views and experiences of Vietnamese health professionals on maternal and fetal health interests, priority setting and potential conflicts, we conducted a cross-sectional study using a structured questionnaire. Obstetricians/gynecologists, midwives and sonographers who manage pregnant women in maternity wards were invited to participate. We purposively chose public health facilities in the Hanoi region of Vietnam to obtain a representative sample. The final sample included 882 health professionals, of which 32.7% (n = 289) were obstetricians/gynecologists, 60.7% (n = 535) midwives and 6.6% (n = 58) sonographers. The majority of participants (60.3%) agreed that "The fetus is a person from the time of conception" and that maternal health interests should always be prioritised over fetal health interests in care provided (54.4%). 19.7% agreed that the fetus is never a patient, only the pregnant woman can be the patient, while 60.5% disagreed. Participants who performed ultrasounds were more likely to agree that fetal health interests are being given more weight in decision-making the further the gestation advances compared to those who did not perform ultrasounds (cOR 2.47, CI 1.27-4.79: n = 811). A significant proportion of health professionals in Vietnam assign the fetus the status of being a person, where personhood gradually evolves during pregnancy. While the fetus is often considered a patient with its own health interests, a majority of participants did give priority to maternal health interests. Health professionals appear to favour increased legal protection of the fetus. Strengthening the legal status of the fetus might have adverse implications for maternal autonomy. Measures to restrict maternal autonomy might require close observation to ensure that maternal reproductive rights are protected.

Abstract:International society has paid continuous attention to the human rights protection in the Family Planning field of China.This article analyzes the Family Planning articles in China's Constitution,outlines the current state and tendency of our legal system in the Family Planning field,and describes the achievement that the work of human rights protection has attained.The legal system shows a good tendency,but some shortcomings still need to be improved.A certain disparity exists between our legal system and international criteria of human rights,which reminds us the importance of enhancing mutual understanding and rethinking and improving our human rights protection.

Abstract:The determination of parentage of foreign surrogacy directly involves the protection of the rights and interests of the surrogate child, the commissioning parents and the surrogate mother, and is also a prerequisite for the determination of guardianship and custody of the surrogate child. At present, China is faced with the dilemma that the criteria for determining "foreign surrogacy" are not in line with international trends, and that the current Law on the Application of Laws lacks clear and appropriate conflict rules for the determination of "foreign surrogacy parent-child relationship". Therefore, it is necessary for China to reconstruct the rules of law applicable to the determination of parent-child relationships involving foreign surrogacy in the light of the best interests of the child.

Abstract:BACKGROUND In recent years, the fertility issue in China has received more and more widespread attention. OBJECTIVE This study aims to detect attitudes toward giving birth to a baby among young users on Douban - a very popular Chinese social media platform. METHODS 3,403 messages under the topic of “What does childbirth mean to a woman” on Douban were crawled for analysis. We employed both qualitative and quantitative descriptive analyses methods to understand user’s concept of childbirth. RESULTS Findings show that majority of the users expressed generally neutral (40.24%) or negative (39.90%) attitudes toward childbirth, only around one fifth of the users expressed positive (19.86%) attitudes. In the sampled messages, ‘pain’ (1520 times), ‘difficult’ (843 times), ‘suffering’ (832 times), ‘cry’ (648 times), ‘fear’ (595 times), and ‘tired’ (297 times) are the top six frequently used words to express negative experiences; while ‘joy’ (464 times), ‘happiness’ (359 times), ‘laugh’ (328 times), ‘cheerful’ (198 times), ‘cute’ (164times) and ‘thankful’ (157 times) are the six most frequently used words to express positive emotions. Messages with negative attitudes received more follows and likes, and the proportion of posts expressing negative attitudes increased with time in fluctuation. Childbirth cost is the most frequently mentioned health risk (12.87%), while subjective happiness or meet of mental needs are expressed as the major childbirth benefits. Only 10 comments (0.4%) talked about the traditional objective benefits of bringing up children for the purpose of being looked after in old age. Qualitative analysis suggested that discussions about fertility on social media may lead to exaggeration of women's perception of health risks, and many women lack knowledge about childbirth that to some extent due to traditional cultural bias that communications about these knowledges had long been neglected and avoided among individuals. In addition, people take for granted to consider that women should sacrifice themselves for childbirth and childcare because of the deification of female figure. Hence women may hesitate to have a baby due to the possibility of losing their subjectivity. CONCLUSIONS The findings showed that modern Chinese women's concept of childbirth had changed with time because of their rising social status and the needs of self-realization. Strategies including public education of health risks of pregnancy and delivery, protecting women’s rights and providing a supportive environment for mothers may increase women’s childbirth willingness. CLINICALTRIAL The study’s ethics code is H15009, and it was approved by the IRB at the Institute of Psychology at the Chinese Academy of Sciences.

Abstract:Objectives: This study explores the attitudes of Chinese maternal and child health professionals toward the termination of pregnancy for fetal anomaly (TOPFA) based on four case scenarios and further identifies the factors that influence their attitudes. Methods: This cross-sectional study, conducted from February 14-21, 2022, aimed to explore the attitudes of maternal and child health professionals toward TOPFA in Hunan Province. We targeted health service institutions across 14 prefecture-level cities and the autonomous prefecture. A questionnaire was made available online and shared via the instant communication platform, WeChat. Participants were recruited through the same platform and completed the survey online. Descriptive statistics were used to analyze the data, and binary logistic regression was performed to determine factors affecting the health professionals' attitudes toward TOPFA, expressed as the odds ratio (OR) and 95% confidence intervals (CI). Results: The study found that 63.5% of health professionals approved of the birth of a fetus with cleft lip and palate, while 36.5% opposed it. Similarly, 39.7% approved of the birth of a fetus with phenylketonuria, while 60.3% opposed it. The percentages of those in favor of and against the birth of a fetus with precocious heart disease were 45.5 and 54.5%, respectively, and those for and against the birth of a fetus with missing fingers were 50.8 and 49.2%, respectively. The top three factors considered by health professionals when agreeing on TOPFA were "the impact of fetal disease on fetal function and growth," "the severity of fetal disease," and "the assessment of indications for fetal disease by professionals and related professional advice." The majority of health professionals (75-78%) preferred joint decision-making by parents regarding the right to decide TOPFA. Conclusion: Our study indicates that the attitudes of health professionals toward TOPFA can differ significantly depending on the specific birth defect under consideration. Notably, the majority of health professionals prioritized "the impact of fetal abnormalities on fetal function and development" when deciding their support for TOPFA, advocating for the decision to be a joint one between the parents. Additionally, factors such as religious beliefs, professional training, age, and job title appeared to influence these attitudes toward TOPFA. Our findings could serve as a reference point in the development of guidelines for the prevention and management of birth defects.

Abstract:Scientific progress has seen wide applications of modern artificial birth technology. Artificial insemination, the test-tube baby, the artificial uterus, and asexual reproduction (cloning) have not only changed the human concept of birth, but also brought value conflicts that pose an unprecedented challenge to law in its traditional sense. The fundamental principle of human morality should be a starting point for law to examine from a broader perspective the social consequences that birth technology may bring. Greater concern and regulation should be exercised over blood relations of artificial birth, eugenic legislation and possible thorny issues concerning civil and criminal laws. The compensatory benefit of artificial birth technology for the function of marriage and birth should be recognized, but the use of artificial birth technology should be given proper guidance and control.
