Abstract:OBJECTIVE To investigate application of fetal echocardiography in diagnosis of fetal arrhythmia and its clinical significance. METHODS Fetal echocardiography was performed on 725 fetuses for evaluation of fetal heart structures and arrhythmias. Two-dimensional, M-mode, Color M-mode and pulsed Doppler echocardiography were used. RESULTS Ninety fetuses were documented with fetal arrhythmia, the commonest fetal arrhythmia encountered in 72 cases extrasystole was (65 atrial extrasystoles, 7 ventricular extrasystoles), followed by bradycardia in 9 cases, tachycardia in 6 cases, 2:1 atrioventricular block in 2 cases, atrial flutter in 1 case. There were 4 fetuses with arrhythmias and structural heart diseases: 2 fetuses were found died in uterus within two-week follow-up (1 with single ventricle and pulmonary stenosis, 1 with cardiac rhabdomyoma, fetal echocardiographic findings were confirmed at autopsy), another 2 cases (1 with mitral atresia and 1 with atrioventricular canal defect, autopsy confirmed the fetal echocardiographic diagnoses) received termination of pregnancy later. One 38-week fetus with atrial flutter underwent cesarean section, neonatal ECG confirmed the arrhythmia, and Cedilanid D induced successful conversion. All the other 85 fetuses were with intermittent arrhythmia, normal heart structures, and had no fetal hydrops. After receiving routine treatment, all of them had term deliveries and follow-up monitoring showed normal neonatal heart rhythms (auscultation). CONCLUSIONS Fetal echocardiography is the main diagnostic tool for prenatal evaluation of fetal arrhythmias. The outcomes of the vast majority of fetal arrhythmias are benign, especially for fetuses with extrasystoles. Their arrhythmias are always well tolerated and disappear during the perinatal period. Fetuses with intermittent arrhythmias without structural heart malformations, fetal hydrops or heart failure, can be followed up with routine prenatal care without the need for special intervention.

Abstract:Foetal arrhythmia is defined as any irregular foetal cardiac rhythm or regular rhythm at a rate outside the reference range of 120 to 160 beats/minute. Foetal echocardiography permits accurate determination of the structural characteristics and the nature of the arrhythmia in foetuses. It is particularly indicated for mothers from high-risk groups. Management of diagnosed heart disease, including foetal arrhythmia, leads either to the termination of pregnancy or to optimal postnatal care of baby and mother. This retrospective study examined the application of foetal echocardiography in the diagnosis of foetal arrhythmia and its clinical significance.

Abstract:Fetal heart rate monitoring is very important for improving the quality of perinatal fetus. At present, the common fetal heart rate monitoring is always based on the fetal heart sound, which is difficult for long time fetal heart monitoring, and fetal ECG contains more information about fetal health. Therefore, it is of great value for research on fetal ECG monitoring. Although there is a long time in the research on the fetal ECG monitoring. But, it has not yet reached the goal of mature application. In this article, the current research situation of fetal ECG in domestic and overseas was summarized, and the difficulties of the fetal heart monitoring were analyzed at the same time. Then several methods of fetal heart monitoring and their development were introduced briefly. Next, the advantages and disadvantages of various kinds of algorithms were analyzed.

Abstract:Abstract Objective: To investigate the safety and efficacy of internal electronic fetal heart rate (FHR) monitoring during labor. Methods: This was a retrospective case-control study, which was an analysis of monitoring FHR with a fetal scalp electrode or a Doppler probe (94 pregnant women per group). In the internal monitoring group, when the opening of the uterine orifice was ≥3 cm, the fetal scalp electrode was placed after natural or artificial rupture of the membrane. FHR was simultaneously monitored using a Doppler probe. In the external monitoring group, continuous FHR monitoring was performed using an ultrasound Doppler transducer fixed on the maternal abdomen. The toco transducer was used to record uterine contractions. Pathological examination of the placenta was performed prospectively in 49 and 48 cases in the internal electronic FHR monitoring group and control group, respectively. Maternal-infant outcomes (e.g. fever, puerperal infection, puerperal morbidity, delivery mode, Apgar score, and scalp injury) were recorded. Umbilical artery blood was extracted for blood gas analysis. Differences between the two groups were compared using the paired t-test, χ2 test, Yates corrected χ2 test or Fisher exact test. Results: Non-statistically significant differences between the internal and external monitoring groups were observed in the incidence of neonatal acidosis (1/94 (1.06%) vs. 3/94 (3.19%), respectively; χ2 = 0.255, P = 0.613), cesarean section/operative vaginal delivery (8/94 (8.51%) vs. 15/94 (15.96%), respectively; χ2 = 2.427, P = 0.181), fever during labor (18/94 (19.15%) vs. 15/94 (15.96%), respectively; χ2 = 0.331, P = 0.565), puerperal morbidity (2/94 (2.13%) vs. 3/94 (3.19%), respectively; χ2 = 0.000, P = 1.000), chorioamnionitis (9/49 (18.37%) vs. 7/48 (14.58%), respectively; χ2 = 0.252, P = 0.616), and neonatal asphyxia (0/94 (0.00%) vs. 1/94 (1.06%), respectively; χ2 = 0.000, P = 1.000). There were no puerperal infections, neonatal scalp injuries, or scalp abscesses found in either group. Using the internal monitoring value as reference, the incidence of FHR false deceleration in external FHR monitoring was 20.21% (19/94 women). Conclusion: Internal FHR monitoring during labor does not increase the incidence of adverse perinatal outcomes. External monitoring was associated with FHR false decelerations.

Abstract:Objective To evaluate the clinical validity of continuous electronic fetal monitoring(EFM) during the second stage of labor. Methods A total of 1 979 pregnant women with gestational age ≥37 weeks in our hospital in the period from Jan.2007 to Dec.2007(as monitoring group) received continuous electronic fetal monitoring during the second stage of labor.And 1 596 pregnant women with same gestational age(as control group) delivered in our hospital in the period between Jan.2006 and Dec.2006 were monitored by intermittent auscultation and selectivity monitoring during the second stage of labor for fetal heart examination. Results The pregnant women in monitoring group had a significantly lower neonatal asphyxia rate compared with those in control group(P0.05).Twenty-seven cases in monitoring group suffered from neonatal asphyxia(1.36%),in which 10 cases were mild and 17 cases were severe.While there were 45 cases of neonatal asphyxia(2.82%) including 26 mild ones(1.63%) and 19 severe ones(1.19%).There were no significant differences between the two groups as for forceps delivery(P0.05),which were 119(83.22%) in monitoring group and 79(72.48%) in control group.Meanwhile,there were significant differences(P0.01) in neonatal asphyxia between monitoring group and control group,which were 12(44.44%) and 38(84.44%),respectively. Conclusions Application of continuous electronic fetal monitoring during the second stage of labor slightly increases vaginal operative delivery rate.However,the neonatal asphyxia rate was decreased.

Abstract:Objective:To investigate the clinical value of fetal heart rate monitoring in forecasting fetal distress.Methods: Reviewing the wave-form of the electronic fetal heart rate monitoring of 2 653 pregnant women from Jan·2006 to Dec·2007 in our hospital.Results: In 2 653 cases pregnant women,243 cases(9.16%) had abnormal fetal heart rate curve,there were 181 casesa found postpartum bnormal factors(74.49%).49 cases new born distress(20.16%).Conclusion: Abnormal fetal heart rate curve has important value in diagnosis of fetal distress.

Abstract:The existing fetal monitor is prone to false detection in the case of high maternal interference in the second stage of labor. With this background, the article designs and implements a combined maternal-fetal fetal heart monitoring system. The system obtains the Doppler signal of the abdominal fetal heart and the blood oxygen signal of the mother's finger, and estimates the maternal interference degree in the fetal heart rate Doppler signal according to the maximum correlation value between the maternal finger blood oxygen signal and the abdominal fetal heart Doppler signal, and switches the fetal heart rate extraction algorithm between the autocorrelation method suitable for lower interference and improved template method suitable for higher interference according to the maternal interference degree. The accuracy of our method is 9.2% which is higher than that of the improved template matching method and 6.1% higher than that of the autocorrelation method.

Abstract:Continuous monitoring of fetal and maternal vital signs, particularly during labor, can be critical for the child and mother's health. We present a novel wearable electronic system that measures, in real-time, maternal heart rate using phonocardiography (PCG) and Electrocardiography (ECG). Uterine contractions using electromyography (EMG). When in later stages we employed ECG technique for maternal heart rate monitoring. The heart rate is determined using moving average filters to remove noises in the signal and ACF(Autocorrelation Function) for determining periodicity. For UC monitoring we stick to the same EMG technique. We also tried employing EMG technique to monitor the Fetal Heart Rate(FHR). But, in later stages of this design, this idea was aborted as we concluded that it needs further research on pregnancy stages and would require more intricate sensor integration that might not be in our reach at the moment. The system is accurate, low-cost, and portable, so it can be deployed at primary healthcare centers in low-income countries. The system can also be used by women in the comfort of their homes. At the same time, the data collected is transferred to their doctor for analysis and diagnosis, which can bring a revolutionary change in the continuous monitoring of fetal wellbeing during labor.

[Clinical Use of Central Electronic Monitoring System].

Abstract:OBJECTIVE:To evaluate the effects of central electronic monitoring system (CEMS). METHODS:A total of 1,216 patients with > or = 37 weeks of gestation assigned as monitoring group, were performed central electronic monitoring during labor from Nov. 1997 to Mar. 1998. A total of 1,137 patients with same gestational age assigned as control group, were monitored by using intermittent auscultation during labor from Nov. 1996 to Mar. 1997. The rate of fetal distress, neonatal asphyxia, cesarean section, and using of forceps or vacuum extractor in the 2 groups were compared. RESULTS:Patients in the monitoring group had a higher fetal distress rate (12.8%) than that of the control group (9.8%, P < 0.05), but a significantly lower neonatal asphyxia rate (2.3%) than that of the control group (4.8%, P < 0.01). There were no significant differences in the overall cesarean rate, cesarean rate for fetal distress, overall use rate of forceps or vacuum extractor delivery, and vaginal operative delivery rate for suspected fetal distress between the two groups (P > 0.05). CONCLUSION:Quality for obstetric care was significantly improved by application of central electronic monitoring for labor management, it didn't increase the cesarean section rate and vaginal operative delivery rate, but decreased the neonatal asphyxia rate.

Abstract:The fetal heart rate is vital for monitoring fetal well-being. Fetal heart rate monitoring based on acoustic techniques is passive and noninvasive. In this work, a fetal heart rate monitoring system based on phonocardiographic method is proposed. A portable low-power stethoscope is customized which meets the need for sensitivity in the monitoring. A noise cancellation method and adaptive matching method are applied to extract the fetal heart rate effectively. Clinical trials are carried out on pregnant women, and the comparison of fetal heart rates given by the proposed system with those given by the Doppler monitor is given to show the accuracy.

Abstract:Fetal heart sound is an important part of fetal monitoring and has attracted extensive research and attention from scholars at home and abroad in recent years. The fetal heart rate, extracted from the fetal heart sound signal, is one of the important indicators that reflect the health of the fetus in the womb. In this study, a maternal-fetal Holter monitor based on f ECG technology was used to collect maternal heart rate, fetal heart rate, and uterine contractions signals, isolate the fetal heart rate, and design an algorithm to extract the fetal heart rate baseline, acceleration, variation, wake-up cycle, and nonlinear parameters. Using statistical methods to analyze the average value and range of various characteristic parameters of fetal heart rate under continuous long-term monitoring, the results show that the baseline has a downward trend from 10 o'clock in the night to 4 o'clock in the morning and is the lowest around 2 o'clock in the morning. The area and acceleration time were significantly higher than those in the suspicious group. However, there was no significant difference in the number of acceleration values between the two groups; the proportion of small mutations in the normal group was lower than that of the suspicious group and the proportion of medium mutations was higher than that of the suspicious group. There is no statistically significant difference in maternal age, gestational age at childbirth, pregnancy comorbidities, and complication rates in the five-level interpretation system of ACOG (2009), RCOG (2007), SOGC (2007), and the United States (2007). The difference of pregnancy and parity in various images was statistically significant, P < 0.05. The second type of fetal heart rate monitoring images appeared in the highest among the diagnostic standards, and the difference in the second type of fetal heart rate monitoring images between the various diagnostic standards was statistically significant, P <= 0.001.

Abstract:Fetal movement (FM) is an important indicator of fetal health. However, the current methods of FM detection are unsuitable for ambulatory or long-term observation. This paper proposes a non-contact method for monitoring FM. We recorded abdominal videos from pregnant women and then detected the maternal abdominal region within each frame. FM signals were acquired by optical flow color-coding, ensemble empirical mode decomposition, energy ratio, and correlation analysis. FM spikes, indicating the occurrence of FMs, were recognized using the differential threshold method. FM parameters including number, interval, duration, and percentage were calculated, and good agreement was found with the manual labeling performed by the professionals, achieving true detection rate, positive predictive value, sensitivity, accuracy, and F1_score of 95.75%, 95.26%, 95.75%, 91.40%, and 95.50%, respectively. The changes in FM parameters with gestational week were consistent with pregnancy progress. In general, this study provides a novel contactless FM monitoring technology for use at home.

Abstract:Objective:To study relationship between Short-Term variation(STV) of fetal monitoring and pregnant outcome.Methods:208 cases of normal delivery in our hospital from Jun.2002 to Dec.2004 were investigated.All the cases were examined by Nonstress test(NST)and vibtratory acoustic stimulation test(VAS-T) that be analyzed with Sonicaid system 8002 at the same time.80 cases with STV>5.0 ms and 128 cases with STV<5.0 ms were chosed and divided into STV<2.9 ms,3.0～3.9 ms,4.0～4.9 ms,and >5.0 ms group.The association between STV variation and poor neonatal prognosis,abnormal NST,fetal appurtenances was analyzed.Results:The rate of abnormal NST,the poor prenatal prognosis,and abnormal fetal appurtenances in STV between <2.9 ms and 3.0～3.9 ms groups were higher than those in STV between 4.0～4.9 ms and STV> 5.0 ms groups(P<0.05).Conclusions:There is a better relationship between STV<4.0 ms and fetal distress or intrauterine deaths.STV is not only a new index of fetal heart rate variability and a efficient supplement to estimate CTG by eyeball,but also a impersonal index to foretell the prenatal prognosis and to assistant diagnose abnormal fetal appurtenances.

Abstract:Despite broad application during labor and delivery, there remains considerable debate about the value of electronic fetal monitoring (EFM). EFM includes the surveillance of fetal heart rate (FHR) patterns in conjunction with the mother's uterine contractions, providing a wealth of data about fetal behavior and the threat of diminished oxygenation and cerebral perfusion. Adverse outcomes universally associate a fetal injury with the failure to timely respond to FHR pattern information. Historically, the EFM data, stored digitally, are available only as rasterized pdf images for contemporary or historical discussion and examination. In reality, however, they are rarely reviewed systematically or purposefully. Using a unique archive of EFM collected over 50 years of practice in conjunction with adverse outcomes, we present a deep learning framework for training and detection of incipient or past fetal injury. We report 94% accuracy in identifying early, preventable fetal injury intrapartum. This framework is suited for automating an early warning and decision support system for maintaining fetal well-being during the stresses of labor. Ultimately, such a system could enable obstetrical care providers to timely respond during labor and prevent both urgent intervention and adverse outcomes. When adverse outcomes cannot be avoided, they can provide guidance to the early neuroprotective treatment of the newborn.

Investigation on the correlation between the adrenomedulin level in retroplacental blood and contraction intensity of uterus

Abstract:Objective: To explore the value of fetal monitor in the monitoring of force at" labor, the level of adrenomedulin in retro-placental blood and the relationship between adrenomodulin level and contraction intensity of uterus. Methods: 78 pregnant women were di-vided into 3 groups according to delivery modes: vaginal delivery group, vaginal delivery and oxytocin group, midwifery and caesarean sec-tion group. Fetal monitor was used in the study, the contraction intensity of uterus was calculated by Montevideo unit (MU), the level of ad-renomedulin was detected by radioimmunonssay. Results: There was no significant difference in sMU of latent phase among three groups,sMU of active phase in midwifery and caesarean section group was lower than that in vaginal delivery group, but the level of adrenomedulin in retreplaeental blood reversed. Conclusion: Fetal monitor can evaluate the change of force of labor, but the contraction intensity of uterus varies in different women, thus the dynamic monitoring of force of labor conduces to the early diagnoeis of abnormal conditions; adrenomedu-lin repress the contraction intensity of uterus, but the mechanisms is unknown.

Abstract:OBJECTIVE: To observe the prognostic value of intrapartum continuous fetal heart monitoring(FHM). METHODS: A review of 150 patients who had intrapartum FHM was performed. The relation between fetal heart rate and amniotic fluid condition, newborn Apgar scores, and delivery method was determined. RESULTS: Among the 150 cases,normal fetal heart rates were recorded in 108 cases. The abnormalities in the other 42 cases with abnormal FHM were as follows: cloudy amniotic fluid 27 cases(64.3%)compared with 17/108 (15.7%) in the normal FHM group (P<0.005); 10 cases (23.8%) had Apgar scores <8 compared with only 2 (1.85%) in the normal FHM group (P<0.005). In the abnormal FHM group,there were 15 cases (35.7%) of spontaneous labor,10 cases (23.8%) of forceps delivery and 17 cases (40.5%) of C-section compared with 81, 3, and 24 (75%,2.8%, and 22.2%) respectively in the normal FHM group (P<0.005). Spontaneous delivery was more common in the normal FHM (75%)compared with abnormal FHM group(35.7%). C-section rate was higher in the abnormal FHM group (40.5%) (P<0.005). CONCLUSION: Intrapartum fetal heart monitoring detects early fetal distress and thus may lead to earlier intervention such as C-section during the perinatal period.

Abstract:Fetal electrocardiography (ECG) monitoring during pregnancy can provide crucial information for assessing the fetus's health status and making timely decisions. This paper proposes a portable ECG monitoring system to record the abdominal ECG (AECG) of the pregnant woman, comprising both maternal ECG (MECG) and fetal ECG (FECG), which could be applied to fetal heart rate (FHR) monitoring at the home setting. The ECG monitoring system is based on data acquisition circuits, data transmission module, and signal analysis platform, which consists of low input-referred noise, high input impedance, and high resolution. The combination of the adaptive dual threshold (ADT) and the independent component analysis (ICA) algorithm is employed to extract the FECG from the AECG signals. To validate the performance of the proposed system, AECG is recorded and analyzed of pregnant women in three different postures (supine, seated, and standing). The result shows that the proposed system can record the AECG in different postures with good signal quality and high accuracy in fetal ECG and heart rate information. Sensitivity (Se), positive predictive accuracy (PPV), accuracy (ACC), and their harmonic mean (F1) are utilized as the metrics to evaluate the performance of the fetal QRS (fQRS) complexes extraction. The average Se, PPV, ACC, and F1 score are 99.62%, 97.90%, 97.40%, and 98.66% for the fQRS complexes extraction,, respectively. This paper shows the proposed system has a promising application in fetal health monitoring.

Abstract:Background: Animal and human studies have shown that fetal hypoxaemia during labour can alter the shape of the fetal electrocardiogram (ECG) waveform, notably (1) the relation of the PR to RR intervals and (2) elevation or depression of the ST segment. Technical systems have therefore been developed to monitor the fetal ECG during labour as an adjunct to continuous electronic fetal heart rate monitoring with the aim of improving fetal outcome and minimising unnecessary obstetric interference. Objectives: To compare the effects of analysis of fetal ECG waveforms during labour with alternative methods of fetal monitoring. Search strategy: The Cochrane Pregnancy and Childbirth Group trials register was searched (September 2002). Selection criteria: Randomised trials comparing fetal ECG waveform analysis with alternative methods of fetal monitoring during labour. Data collection and analysis: Trial quality assessment and data extraction were performed by the reviewer, without blinding. Main results: Three trials including a total of 8357 pregnant women were included. The trials were of sound methodological quality. All three trials assessed the use of the fetal ECG as an adjunct to continuous electronic fetal heart rate monitoring during labour. One study assessed PR intervals; two assessed the ST segment. The use of ST waveform analysis (7400 women) was associated with fewer babies with severe metabolic acidosis at birth (cord pH less than 7.05 and base deficit greater than 12 mmol/L) (relative risk (RR) 0.44, 95% confidence interval (CI) 0.26 to 0.75, data from 6672 babies). This was achieved along with fewer fetal scalp samples during labour (RR 0.86, 95% CI 0.76 to 0.98) and fewer operative deliveries (RR 0.89, 95% CI 0.82 to 0.97). Apart from a trend (that did not achieve statistical significance) towards fewer operative deliveries (RR 0.87, 95% CI 0.76 to 1.01), there was little evidence that monitoring by PR interval analysis conveyed any benefit. This may reflect limitations of the technique or, alternatively, the smaller numbers available for analysis from the single trial (957 women). Reviewer's conclusions: These findings support the use of fetal ST waveform analysis when a decision has been made to undertake continuous electronic fetal heart rate monitoring during labour. However, in most labours, technically satisfactory cardiotocographic traces can be obtained by external ultrasound monitors which are less invasive than internal scalp electrodes (which are required for electrocardiographic (ECG) analysis). A better approach might be to restrict fetal ST waveform analysis to those fetuses demonstrating disquieting features on cardiotocography.

Abstract:A central fetus monitoring system is introduced. This system is composed of a central workstation, mother/fetus monitoring inside hospital and family used fetus monitoring. Only one work station is needed for the whole system, including the sector inside the hospital and the external sector for family use and other small hospitals. Pregnant women in obstetrics hospitals and homes can be monitored in real time and all the time at the central workstation. The sector inside the hospital is based on RS 485 bus, while the external sector for family use is based on public telephone network. Unique techniques were adopted in the fetus monitoring. Part of the key circuits was built in the fetal heart sensor for noise reduction and reliability, and unique algorithm for fetal heart rate detection was designed. Besides, the monitors can operate independently or communicate with the central workstation through RS 485 bus. A remote or family used monitoring system was designed. The family used fetus monitor is portable and stable, which can detect doppler sound of the fetus, display the FHR values with its LCD and mark the fetal movements. It can be connected to the central workstation in the hospital throuth telephone line if needed. Thus, the information acquired at family side can be monitored in real time at workstation located in the hospital as if they were in the hosptal. Pregnant women and doctors at hospital can talk with each other while monitoring is going on. Built in chip technology is used and it is an effective solution for remote monitoring of fetus. Computer assistant diagnosis software was designed. Its functions include automatic evaluation of fetal monitoring curves, chart for delivery process, etc.
