Endoscopic Features and Surgical Results of Gastric High-Grade Intraepithelial Neoplasia

Abstract:Objective To investigate the morphological feature of biopsy-confirmed gastric high-grade intraepithelial neoplasia(HGIEN)and its relationship with relevant surgical results.Methods Forty-four cases with gastric HGIEN confirmed by bioposy among 26 730 cases of upper GI endoscopy screening received radical surgeries.The endoscopic and macroscopic features of the lesions,biopsy quantities and histopathological results were studied.Results Radical surgeries were successfully performed on all the patients.An average of 4.25 biopsies was made per examination.Twenty-six superficial lesions were identified,among which a mere six(13.6%)were post-surgically confirmed as HGIEN while 16(36.4%)and 4(9.1%)were confirmed as early gastric cancer and advanced cancer mimicking superficial lesions(type 5 gastric cancer),respectively.The other 18 lesions with an appearance of various Borrmann types were all confirmed as advanced cancer.Conclusion Cautious differentiation of lesions with biopsy-suggested HGIEN is critical in clinical practice.Attention should be paid to the depth and accuracy of biopsies to increase the diagnostic yield of gastric cancer with assistance of chromoendoscopy and intrinsic fluorescence spectroscopy.Carcinoma is post-surgically confirmed in a large proportion of patients with biopsy-revealed HGIEN,suggesting the importance of surgical intervention to avoid delayed diagnosis and treatment.
