Abstract:Introduction: Mastitis is a common problem encountered by breastfeeding mothers. This study investigated the incidence and risk factors of lactation mastitis among Chinese women. Subjects and Methods: A prospective cohort study on infant feeding practices was conducted during 2010 and 2011 in Jiangyou, Sichuan Province, China. Poisson regression analysis was performed to determine factors influencing the incidence of mastitis within 6 months postpartum. Results: Of the 670 Chinese mothers who were breastfeeding at discharge, 42 women (6.3%) experienced at least one episode of mastitis during the first 6 months after delivery. The cumulative incidence of mastitis was 10.3%. Mothers with a cracked and sore nipple (incidence rate ratio 2.24; 95% confidence interval 1.38, 3.63) and those who felt stressed (incidence rate ratio 3.15; 95% confidence interval 1.56, 6.37) appeared to sustain more episodes of mastitis. Conclusions: The incidence of lactation mastitis was low among Chinese mothers. To further reduce the risk of mastitis, instructions on the correct positioning of the baby during breastfeeding should be emphasized. Providing new mothers with guidance on how to cope with stress may also prevent the recurrence of the condition.
Risk Factors and Control Countermeasures of Puerperal Hospital Infection

Abstract:OBJECTIVE To investigate the infection factors and countermeasures of puerperal hospital infections.METHODS The cases of puerperal hospital infection in recent 5 years were analyzed by retrospective survey.RESULTS The rate of infection was 1.02%,the top three infections sites were respiratory system(52.79%),surgical incision(21.32%) and digestive system(8.63%).The infection rate of cesarean section was higher than delivery vagina operation.Malnutrition,prolonged labor,premature rupture of membrane,multiple vagina checks,bad economic conditions,obesity and so on were the risk factors of puerperal hospital infection.CONCLUSION There are many relevant factors of puerperal hospital infection,scientific prenatal care,postnatal recovery,aseptic operation,proper procedure of the labor,strict infection management,and reasonable use of antibiotics are the key to prevent and control the puerperal hospital infections.
Application Research of Community Voluntary Nursing Service in Puerperal Women

Abstract:Objective To explore application effects of community voluntary nursing service in puerperal women. Methods A total of 128 natural labored puerperal women were selected as experimental group ( routine nursing education and voluntary nursing service) from January to June 2014, and another 128 natural labored puerperal women were selected as control group (routine nursing education) from January to June 2013. Eight weeks after interventions, we compared the puerperal mastery of nursing skills and nursing satisfaction, and incidence of complications. Results In the experimental group, puerperal women had higher mastery rate than that of the control group in nursing skills such as perineal care, feeding baby skills, postpartum setting-up exercises, copping with cracked nipple, bath, diaper change, and umbilical nursing (P<0. 05). The nursing satisfaction was 100. 0% in the experimental group compared with 89. 8% in the control group (χ2 =15. 236, P<0. 01). The incidence of complications in the experimental group was lower than that in the control group (P<0.01). Conclusions Community voluntary nursing service can enhance nursing skills of puerperal women, reduce the incidence of complications and enhance nursing satisfaction of puerperal women.

Abstract:Background: Early puerperal rehabilitation can interfere with a woman's ability to care for herself and her infant. Acupoint hot compress, with a combination of acupoints and natural physical agent heat, has significant potential to alleviate symptoms experienced during early puerperium. Current evidence regarding the effects of acupoint hot compress therapy on early puerperal rehabilitation is insufficient. The aim of this study is to address this with a multicenter design and large sample size. Methods: This is a prospective, multi-center, and randomized controlled clinical trial. A total of 1400 nulliparous women with a singleton pregnancy experiencing natural childbirth from 14 hospitals will be enrolled and randomly allocated to either an intervention group or a control group in a 1:1 ratio. Subjects in the control group will only receive routine postpartum care. In addition to routine postpartum care, the subjects in the intervention group will be administered a 4-hour acupoint hot compress with a constant temperature of 45 +/- 2 degrees C respectively within 30 minutes after delivery, 24 hours and 48 hours after delivery. The primary outcome will be the time elapsed from delivery to the first urination. The secondary outcomes will be postpartum uterine contraction pain intensity, the Edinburgh Postnatal Depression Scale for screening postpartum depression and the assessment of lactation including recording the lactation initiation time, postpartum diet, appetite, weight, neonatal weight. Discussion: These results will provide evidence for obstetricians and parturients on considering nonpharmacologic and noninvasive intervention in early puerperal rehabilitation.
Effect of Comprehensive Nursing Intervention Under Internet-Based WeChat Platform Education on Postoperative Recovery of Puerperae Undergoing Cesarean Section

Abstract:Objective. To explore the effect of comprehensive nursing intervention under internet-based WeChat platform education on postoperative recovery of puerperae undergoing cesarean section (C-section). Methods. A total of 96 patients undergoing C-section in our hospital from June 2018 to October 2021 were selected for the retrospective analysis and grouped according to the nursing intervention modes as follows: patients who received a routine nursing intervention were included in the control group, and patients who received comprehensive nursing intervention under internet-based WeChat platform education were included in the study group, with 48 cases each. After the intervention, the postoperative recovery of puerperae undergoing C-section and obstetrics nursing quality were scientifically assessed. Results. No statistical differences in general data between the two groups were observed ( P > 0.05 ). Compared with the control group, patients in the study group had obviously earlier initial time of lactation ( P < 0.05 ), significantly higher breast milk amount within 48 h after delivery ( P < 0.05 ), significantly earlier spontaneous urination time, anal exhaust time and off-bed activity time ( P < 0.05 ), obviously better mastery of health knowledge ( P < 0.05 ), and, in terms of SCL-90, remarkably lower total score, average total positive score, and the number of positive items and scores in dimensions, such as somatization, obsessive-compulsive, depression, anxiety, interpersonal sensibility, anger-hostility, phobic-anxiety, and paranoid ideation ( P < 0.05 ). At discharge, 3 months, and 6 months of follow-up, patients in the study group obtained significantly higher scores on quality of life than the control group ( P < 0.05 ). Conclusion. In the nursing of puerperae undergoing C-section, comprehensive nursing intervention measures based on the internet-based WeChat platform education can effectively improve the early lactation, mastery of health knowledge, and adverse emotions of puerpera and promote their postoperative recovery.

Dietary habit and lifestyle of postpartum women in South China: a mixed methods research

Abstract:Objective To investigate the dietary and hygiene behaviours and influencing factors of women during the puerperium in South China. Design A cross-sectional study. Setting Three tertiary hospitals in Guangdong Province, China Participants From November 2022 to March 2023, 327 women with puerperal experience were enrolled using the purposive sampling method, with 323 valid questionnaires collected for data analysis. Semistructured interviews were conducted with six puerperium women and five primary caregivers. Primary and secondary outcome measures This study used a questionnaire survey to collect and analyse the current postpartum dietary and hygiene behaviours among women in the South China region, along with their influencing factors. Results Multinomial logistics regression revealed women residing in urban-rural fringe were more likely to use ventilated rooms (OR 4.496, 95% CI 2.363 to 8.552) and avoid hair washing (OR 0.345, 95% CI 0.159 to 0.749) compared with urban residents. Additionally, women who practiced Buddhism were more likely to wash their hair (OR 11.070, 95% CI 2.339 to 52.379) and take baths (OR 6.856, 95% CI 2.057 to 22.855) compared with those with no religious affiliation. Lastly, those whose primary caregiver was their husband were more likely to consume watermelon (OR 2.235, 95% CI 1.119 to 4.463), persimmon (OR 4.395, 95% CI 1.886 to 10.242) and longan (OR 2.612, 95% CI 1.362 to 5.010). The qualitative study identified five themes: dietary practices, hygiene habits, personal attitude, sources of information and support. Conclusion The dietary and hygiene behaviours of puerperium women in South China are significantly influenced by the residen, primary caregivers and religious beliefs. Medical care providers should adopt a precision postpartum care strategy to improve the quality of care.

Abstract:Objective To evaluate current practice and develop strategies in nipple pain and trauma management and promote continuous improvement in breast feeding practice. Methods Using JBI-PACES Program,the project was implemented and the quality was assessed before and after. The project was conducted in a 20-bed maternity ward of a large women hospital in China,from September 2012 to January 2013. Results The compliance rates in the four criteria ranged from 23% to 67% at baseline. After implementation of the project,significant improvement was achieved and the compliance rates of all criteria achieved 100%. Moreover,nurses' knowledge,patients' knowledge and skills and breast feeding support system had been strengthened. Conclusion This project is successful in improving clinical changes in the management of nipple pain and trauma in breastfeeding mothers. It is suggested to further integrate resources and improve the quality of nursing continuously.

Abstract:Introduction Breastfeeding is crucial in infant nutrition and bonding, recommended for at least 6 months and up to 2 years postpartum, providing health benefits for both infants and mothers. However, nipple trauma is common among lactating mothers. Moisturising therapies, such as hydrogel application, aim to promote the healing of nipple wounds. Despite various interventions proposed, comprehensive reviews evaluating the efficacy of moisturising therapies for nipple trauma are lacking. This review aims to appraise the effectiveness of moisturising therapy, particularly tailored for nipple trauma in lactating women. Methods and analysis This systematic review protocol adheres to the Preferred Reporting Items for Systematic Review and Meta-Analysis Protocols guidelines and is registered in the International Prospective Register of Systematic Reviews. The focus will be on both randomised and non-randomised controlled trials addressing the treatment of nipple trauma in lactating women. In this context, 'moisturising therapy' is categorised as a non-pharmacological intervention applied directly to the nipple, excluding treatments for bacterial or fungal infections. The primary outcomes will include measures of the healing efficiency of nipple trauma and reduction in nipple pain. Searches will be conducted without date restrictions across multiple databases, including PubMed, CINAHL, the Cochrane Library, Web of Science, CiNii Research and Ichushi-Web, in addition to exploration of grey literature. Two independent reviewers will manage all stages of the review process, under the supervision of a third reviewer. Data extraction will encompass participant demographics, study methodologies, sample specifics and measured outcomes. Quality assessment will be conducted using the Joanna Briggs Institute Appraisal Checklist Tools. Ethics and dissemination Ethics approval is not required for this study. The findings of this review will be disseminated through academic publications, detailed reports and presentations at relevant conferences. PROSPERO registration number CRD42023481761.

The “zuòyuèzi” Practices During Puerperium and Its Association with Health Status Among Postpartum Women in Southern China: A Cross Sectional Study

Abstract:Background: Maternal health problems during postpartum period are highly prevalent worldwide, which may be related to unhealthy dietary and behavioral practices of postpartum women. Accordingly, this study aimed to explore postpartum dietary and behavioral practices, and their relationship with maternal health in Guangzhou, China.Methods: We conducted a cross-sectional study among postpartum women in urban and suburban area in Guangzhou China. Data about postpartum dietary and behavioral practices and health conditions were collected by a standardized questionnaire. Logistic regression analysis was used to identify the factors related to women’s health. Results: A total of 2040 women were recruited, 2013 of which complete the questionnaire, including 1007 from urban area and 1006 from suburban area. The consumption of animal food was triple the recommended intake, while the intakes of fruits, milk and seafood were deficient; and most women were lack of physical activities. 75.5% of women reported at least one postpartum disease, and the most common problems were prolonged uterine bleeding (70.0%) and backache (43.0%), followed by constipation (23.6%), insufficient milk secretion (19.2%), breast swelling (18.5%) and hemorrhoids (13.8%). Median of postpartum body weight retention was 3.5 kg. Logistic regression analysis revealed that 12-18 h/d of bed rest time, breastfeeding, doing postpartum exercise, basking, getting out of bed within 2 days after delivery, intake of animal food (250-350 g/d), vegetables (>500 g/d), grain and potato (250-450 g/d) were protective factors for at least one out of these health problems or weight retention (P<0.05). Bed rest time for more than 18 h/d or less than 12 h/d, ginger vinegar intake, doing housework, cesarean section, and excessive intake of grain and potato (>450 g/d) had adverse association (P<0.05). Conclusion: Postpartum low physical activity and adherence to the diet with high intake of animal food and inadequate intake of fruits, vegetables and milk were very common in Guangzhou. Further studies are needed to assess whether postpartum diet and behavioral intervention improve maternal health during postpartum period.
Effect of care intervention with a health education form for breastfeeding on breast distension, pain, and lactation in postpartum mothers

Abstract:Background: Breastfeeding not only meets the nutritional needs of newborn growth and development but also promotes uterine contraction and discharge of lochia, which helps in maternal recovery. However, some mothers experience abnormal lactation and breast swelling due to a lack of breastfeeding knowledge, painful cesarean incisions, anesthesia, negative emotions, and other factors, resulting in a reduced breastfeeding rate, which adversely affects neonatal and maternal health. Aim: To explore the effects of care intervention with a health education form for breastfeeding on breastfeeding-related conditions. Methods: In this study, 207 mothers with postpartum breast pain and difficulty lactating were selected and divided into intervention and control groups using a random number table. Both groups of mothers were provided with basic nursing and related treatment measures after delivery. The intervention group additionally received care intervention with a health education form for breastfeeding. The scores of lactation volume, breast distension and pain, breastfeeding rate, breastfeeding self-efficacy, treatment effect, and complication rate of the two groups were compared. Results: After treatment, the breast pain score of the intervention group was significantly lower than that of the control group, while the lactation score, score of Breastfeeding Self-Efficacy Scale Short Form scale, parent-child communication score, maternal-infant interaction score, total score of maternal-infant communication, and breastfeeding rate of the intervention group were significantly higher than those of the control group. After intervention, the overall therapeutic effect of the intervention group was better than that of the control group, and the complication rate of the intervention group was lower than that of the control group. Conclusion: Breastfeeding health education and nursing intervention combined with basic clinical treatment have good clinical effects in managing postpartum breast distension and pain and increasing lactation yield.

Abstract:Objective To evaluate the impact of the comprehensive assessment system application on decision of perineum protection methods during childbirth and the outcomes of perineum.Methods A mixed method of qualitative and quantitative research was uti lized,and 223 single birth primiparas with normal fetal position and underwent vaginal delivery were selected as study participants,and 18 midwives who grasped different perineum protection methods assessed the puerperas using a comprehensive assessment system.Then the perineum protection methods,rate of perineal laceration and episiotomy among puerperas delivered by midwives with different seniority were compared,and 10 midwives were interviewed deeply to understand strength and weakness of the com prehensive assessment system.Results There was significant difference in episiotomy rate among puerperas delivered by midwives with different seniority (P＜0.05),while there were no significant differences in rate of perineal laceration and episiotomy among puerperas (P＞0.05 for both),and there was no significant difference between the real perineum protection methods and recommended method (P＞0.05).And two themes were extracted based on interview data:the comprehensive assessment system during childbirth could guide clinical practice to some extent,while some limitations existed.Conclusion The comprehensive evaluation system during childbirth has some clinical guiding significance for choosing suitable perineum protection methods,but it has some shortcomings in item setting,which result in limitations in clinic application.It is necessary to modify the system according to existing research evidence and clinical practice to meet the needs of midwives better.

Abstract:Objective To investigate the countermeasures of nursing chronic dermal ulcer of the low extremity.Methods Retrospective analysis was carried out in 20 patients who had hospitalized in department of orthopedics in 2000 to 2004.Results The reasons of heal-delayed factors included those of patients themselves(psychological barrieer) and those underling disease which impeded to the healing process,such as infection,venous hypertension,diabetes,wound eczema.Conclusion Controlling the underlying conditions and improvement wound environment are necessary for wound healing.
The Effects of A Self-Prepared Medicinal Solution of Huangbai and Kushen Combined with Psychological Care on Postpartum Perineal Incisions

Abstract:Medicinal solution steam washing is a common clinical treatment for the management of various skin diseases and wound infections. According to traditional Chinese medicine, the lack of healing of infected incisions is often attributed to excessive intraoperative blood loss, resulting in an imbalance of Qi and Blood, meridian obstruction, blood stasis, and the accumulation of dryness, heat, and turbid Qi at the affected area. Therefore, detoxification washing of the affected area with medicinal solutions promotes healing and is clinically recommended. Based on this premise, a self-prepared medicinal solution using cold-natured Chinese medicinal herbs, such as Huangbai, Kushen, and Pugongying, was used to promote the healing of postpartum perineal incisions in women. The aim of this study was to determine the effects of a self-prepared medicinal solution combined with psychological care on the healing of postpartum perineal incisions. Eighty parturient women who underwent spontaneous delivery with a mediolateral episiotomy at our hospital from August 2020 to August 2021 were enrolled in the study. The parturient women were divided into control and observation groups using a random number table, with 40 patients per group. The control group received conventional treatment and nursing intervention. The observation group received intervention with self-prepared medicinal liquid fumigation using Huangbai and Kushen, combined with psychological care. Pain perception, incision healing time, and the psychological status of the two groups were compared. After 3 days of nursing, the visual analog scale (VAS) scores in the observation group were significantly lower than the scores in the control group ( P <0.05). The VAS scores in the observation group were also significantly lower than the scores in the control group after nursing ( P <0.05). The healing time of the incision in the observation group was significantly shorter than the healing time in the control group ( P <0.05). After nursing, the self-rating anxiety scores and self-rating depression scores in the observation group were significantly lower than the scores in the control group ( P <0.05). Compared to routine treatment and nursing interventions, the combined application of self-prepared medicinal solution steam washing and psychological care promotes the healing of postpartum perineal incisions, alleviates pain in the incision area, and improves negative emotional states in parturient women.

Abstract:According to WHO, breastfeeding is the best for premature babies. Breastfeeding are beneficial for health of both mothers and their premature infatns and as well as reduction of medical and health expenditure. However, the breastfeeding rate in premature infants is still unsatisfactory so far, especially in those premature infants admitted to NICU in the early life. This article review promoting and hindering factors of breastfeeding in hospitalized premature infants from the following aspects:the characteristics of premature infants, the demographic and sociological characteristics, knowledge, beliefs, and psychological status of mothers, hospital policies and support, and family support.

Abstract:Background: In China, the prevalence of exclusive breastfeeding at 6 months was only 20.8%. In promoting breastfeeding for newborns, a number of strategies have been initiated by Chinese government. These actions facilitated a high breastfeeding initiation of 77 to 99.9% in different regions. However, the exclusive breastfeeding rates remained low at 6 months resulting from a high rate of perceived insufficient breast milk and complementary feeding during the early days after childbirth. The aim of this study was to understand the experiences of women in Shenzhen with regard to breastfeeding in the first 6 weeks after giving birth, to identify the facilitators and barriers impacting their breastfeeding decisions and to identify their perceived support needs that might facilitate breastfeeding in the future. Methods: This was a qualitative exploratory study. Data were collected in November 2018 through semi-structured, face-to-face, in-depth interviews. A purposive sample of early postpartum women was recruited from a postpartum clinic of a tertiary maternal hospital in Shenzhen, China. The dataset was analysed using inductive content analysis. Results: A total of 22women were interviewed within the first 6 weeks after delivery. Three themes related to breastfeeding were identified from the transcribed interviews: "breastfeeding facilitators," "breastfeeding barriers," and "recommendations for breastfeeding promotion." Conclusions: Women experienced both joy and suffering in their journey of breastfeeding. Insufficient knowledge of breastfeeding, discomfort, intergenerational disagreements regarding nutritional supplements, and a lack of professional support contributed to difficulties and the threat of discontinuation. A supportive environment for breastfeeding is crucial for women's decision on exclusive breastfeeding and the psychological wellbeing of breastfeeding women. Interventions that target to promote exclusive breastfeeding should include both new mothers and significant family members. Future studies could test the effectiveness of breastfeeding training for home visit nurses to promote exclusive breastfeeding in the early postpartum.

Abstract:Background and Objectives: This study aimed to explore the associations of postpartum dietary quality and behavioral practices with maternal health in Guangzhou China. Methods and Study Design: We conducted a cross-sectional study among puerperal women in urban and suburban areas in Guangzhou, China (n=2013). Data for postpartum dietary and behavioral practices and health conditions were collected by a standardized questionnaire. Dietary balance index (DBI) was calculated to assess an individual's dietary quality. Logistic regression analysis was used to identify the factors related to women's health. Results: 75.5% of women reported at least one postpartum disease, and the most common problems were prolonged duration of lochia (70.0%) and backache (43.0%), followed by constipation (23.6%), insufficient milk secretion (19.2%), breast swelling (18.5%) and hemorrhoids (13.8%). Average postpartum weight retention was 3.5 kg. Logistic regression analysis revealed that 12-18 h/d of bed rest time, breastfeeding, doing postpartum exercise, basking, getting out of bed within 2 days after delivery, higher intake of fish and shrimp, fruits, vegetable, milk were protective factors for at least one out of these health problems or weight retention (p<0.05). Bed rest time for more than 18 h/d or less than 12 h/d, ginger vinegar intake, doing housework, cesarean section, and excessive and inadequate intake of cereals had an adverse association (p<0.05). Conclusions: Some features of a traditional Chinese postpartum diet and behaviour are related to maternal morbidity during the puerperium. Further studies are needed to assess whether postpartum diet and behavioral intervention improve maternal health during the postpartum period.
